Background:Platinum chemotherapy(CT)remains the backbone of systemic therapy for patients with smallcell lung cancer(SCLC).The nucleotide excision repair(NER)pathway plays a central role in the repair of the DNA damage exerted by platinum *** in this repair mechanism may affect patients’*** and Methods:We conducted a retrospective analysis of data from 38 patients with extensive disease(ED)-SCLC who underwent platinum-CT at the Clinical Oncology unit,Careggi University Hospital,Florence(Italy),from 2015 to *** expression analysis and single nucleotide polymorphism(SNP)characterization of three NER pathway genes—namely ERCC1,ERCC2,and ERCC5—were performed on patient tumor ***:Overall,elevated expression of ERCC genes was observed in SCLC patients compared to healthy *** with low ERCC1 and ERCC5 expression levels exhibited a better median progression-free survival(mPFS=7.1 vs.4.9 months,p=0.39 for ERCC1 and mPFS=6.9 vs.4.8 months,p=0.093 for ERCC5)and overall survival(mOS=8.7 vs.6.0 months,p=0.4 for ERCC1 and mOS=7.2 vs.6.2 months,p=0.13 for ERCC5).Genotyping analysis of five SNPs of ERCC genes showed a longer survival in patients harboring the wild-type genotype or the heterozygous variant of the ERCC1 rs11615 SNP(p=0.24 for PFS and p=0.14 for OS)and of the rs13181 and rs1799793 ERCC2 SNPs(p=0.43 and p=0.26 for PFS and p=0.21 and p=0.16 for OS,respectively)compared to patients with homozygous mutant ***:The comprehensive analysis of ERCC gene expression and SNP variants appears to identify patients who derive greater survival benefits from platinum-CT.

Background： Video-assisted thoracic surgery （VATS） has been shown to be a safe alternative to conventional thoracotomy for patients with non-small cell lung cancer （NSCLC）. However, popularization of this relatively novel technique has been slow, partly due to concerns about its long-term outcomes. The present study aimed to evaluate the long-term survival outcomes of patients with NSCLC after VATS, and to determine the significant prognostic factors on overall survival. Methods： Consecutive patients diagnosed with NSCLC referred to one institution for VATS were identified from a central database. Patients were treated by either complete-VATS or assisted-VATS, as described in previous studies. A number of baseline patient characteristics, clinicopathologic data and treatment-related factors were analyzed as potential prognostic factors on overall survival. Results： Between January 2000 and December 2007, 1,139 patients with NSCLC who underwent VATS and fulfilled a set of predetermined inclusion criteria were included for analysis. The median age of the entire group was 60 years, with 791 male patients （69%）. The median 5-year overall survival for Stage Ⅰ, Ⅱ, Ⅲ and Ⅳ disease according to the recently updated TNM classification system were 72.2%, 47.5%, 29.8% and 28.6%, respectively. Female gender, TNM stage, pT status, and type of resection were found to be significant prognostic factors on multivariate analysis. Conclusions： VATS offers a viable alternative to conventional open thoracotomy for selected patients with clinically resectableNSCLC

Lobectomy with partial removal of the pulmonary artery in video-assisted thoracic surgery （VATS） currently remains a challenge for thoracic surgeons. We were interested in introducing pulmonary vessel blocking techniques in open thoracic surgery into video-assisted thoracic surgery （VATS） procedures. In this study, we reported a surgical technique simultaneously blocking the pulmonary artery and the pulmonary vein for partial removal of the pulmonary artery under VATS. Seven patients with non-small-cell lung cancer （NSCLC） received lobectomy with partial removal of the pulmonary artery using the technique between December 2007 and March 2012. Briefly, rather than using a small clamp on the distal pulmonary artery to the area of invading cancer, we replaced a vascular clamp with a ribbon and Hem-o-lock clip to block the preserved pulmonary veins so as to prevent back bleeding and yield a better view for surgeons. The mean occlusion time of the pulmonary artery and pulmonary veins were 44.0±10.0 and 41.3±9.7 minutes, respectively. The mean repair time of the pulmonary artery was 25.3±13.7 minutes. No complications occurred. No patients showed abnormal blood flow through the reconstructed vessel. There were no local recurrences on the pulmonary artery. In conclusion, the technique for blocking the pulmonary artery and veins is feasible and safe in VATS and reduces the risk of abrupt intraoperative bleeding and the chance of converting to open thoracotomy, and extends the indications of VATS lobectomy.

Dear Editor,We would like to present a novel system for monitoring double-lumen tube(DTL)positions in thoracic surgery by respiratory sounds at the trachea and the bronchi,based on our previously described methodology,algorithms,and patented technology[1-2].Most thoracic surgeries require one-lung ventilation(OLV).A DLT or a bronchial blocker(BB)is the most common means for managing *** DLT or BB malposition is a critical issue that needs to be accurately and promptly detected to prevent the incidence of hypoxemia and unintended re-expansion of operated lung during OLV[3-4].A multi-center study of 2127 patients requiring OLV during thoracic surgery revealed that DLTs were used in 95%of patients with a malposition rate of 14%,and BBs were used in 5%of patients with a malposition rate of 33%[5].

The outbreak of corona virus disease 2019(COVID-19)imposes a major challenge in managing patients undergoing surgical *** this study,we analyzed clinical and transmission features of 25 cases of COVID-19 from a single thoracic department,including 13 patients and 12 health care *** were 13 males and 12 *** median age of the patients was 61(range:51 to 69)*** median age of the health care staff was 35(range:22 to 51)*** the end of follow-up date(Mar.3,2020),there were 16 non-severe cases(64%)and 9 severe cases(36%),5 cases were dead(20%).Nineteen(76%)of the infected cases were confirmed by SARS-CoV-2 nucleic acid test,the rest were clinically diagnosed as suspected COVID-19 cases,and 19(76%)of the infected cases had positive exposure *** found that COPD was significantly associated with severity and death(P-0.040,and P=0.038,respectively),and chest operation was significantly associated with death for COVID-19 patients(P-0.039).A potential“super spreader"may be the source of the transmission before the implementation of quarantine and comprehensive *** was concluded that COVID-19 is associated with poor prognosis for patients undergoing thoracic operation,especially for those with *** of comprehensive protective measures is important to control nosocomial infection.

Introduction: World Health Organization announced on April 2009 a public health emergency of international concern caused by swine-origin influenza A(H1N1) virus. Acute respiratory distress syndrome(ARDS) has been reported to be the most devastating complications of this pathogen. Extracorporeal membrane oxygenator(ECMO) therapy for patients with H1N1 related ARDS has been described once all other therapeutic options have been exhausted. Here, we report the case of a child(German, male) with H1N1-associated fulminate respiratory and secondary hemodynamic deterioration who was rescued by initial emergent ECMO established through a dialysis catheter and subsequent switch to central cannulation following median sternotomy. This report highlights several important issues. First, it describes a successful use of a dialysis catheter for the establishment of a veno-venous ECMO in an emergency case by child. Second, it highlights the importance of a closely monitoring of clotting parameters during ECMOtherapy and third, if severe respiratory failure is complicated by cardiogenic shock, veno-atrial ECMO support via median sternotomy should be considered as a viable treatment option without further delay.

Introduction: Esophageal foreign bodies are important and serious cause of morbidity and mortality in both children and adults, usually as a result of serious complications, such as perforation, necrosis, mediastinitis, and fistulation. Therefore, rapid and accurate diagnosis with subsequent removal is very important. Clinical Case: We present and discuss the presentation, diagnosis and surgical management of a 6-year-old with a 2-year history of ingested coin locked in the oesophagus. Discussion: Diagnosis was established by upper gastrointestinal endoscopy, barium swallow and chest x-ray. The coin was removed by open surgery via oesophagotomy through right thoracotomy after failed retrieval via rigid oesophagoscopy. Conclusion: Surgical treatment in the form of esophagotomy via thoracotomy may be necessary in some cases of impacted foreign bodies in the esophagus where endoscopic removal is unsuccessful.
