＂Qi＂ and ＂blood＂ are two essential concepts in Chinese medicine （CM）. As qi is intangible, the concept of qi is still controversial between CM and Western medicine. However, the endogenous hydrogen sulfide （H2S） and other gaseous signaling molecules provides a new approach for understanding the essence of qi in CM. Blood stasis syndrome is a common syndrome in CM. According to the CM theory, the incidence of blood stasis syndrome is closely correlated to the reckless movement of qi, as qi and blood are inseparable in regulating physiological functions. In recent years, more and more evidences suggest a close correlation between blood stasis syndrome and microcirculation dysfunction. In this paper, we discuss the relationship between endogenous H2S and blood stasis syndrome based on qi-blood theory of CM. We found that endogenous H2S maybe a material basis in concept of qi in CM, while dysfunctional microcirculation is the pathological basis of the blood stasis syndrome. As qi is closely associated with incidence and progression of blood stasis syndrome, endogenous H2S may play an important role in preventing and treating the blood stasis syndrome by improving the function of microcimulation.

Objective： To study, at the cytological level, the basic concept of Chinese medicine that “the Kidney （Shen） controls the bone”. Methods： Kaempferol was isolated form Rhizoma Drynariae （Gu Sui Bu, GSB） and at several concentrations was incubated with opossum kidney （OK） cells, osteoblasts （MC3T3 El） and human fibroblasts （HF） at cell concentrations of 2 x 104/mL. Opossum kidney cell-conditioned culture media with kaempferol at 70 nmol/L （70kaθOKM） and without kaempferol （oOKM） were used to stimulate MC3T3 E1 and HF proliferation. The bone morphological protein receptors I and Ⅱ （BMPR I and Ⅱ） in OK cells were identified by immune-fluorescence staining and Western blot analysis. Results： Kaempferol was found to increase OK cell growth （P〈0.05）, but alone did not promote MC3T3 E1 or HF cell proliferation. However, although OKM by itself increased MC3T3 E1 growth by 198% （P〈0.01）, the 70kaθOKM further increased the growth of these cells by an additional 127% （P〈0.01）. It indicates that the kidney cell generates a previously unknown osteoblast growth factor （OGF） and kaempferol increases kidney cell secretion of OGF. Neither of these media had any significant effect on HF growth. Kaempferol also was found to increase the level of the BMPR Ⅱ in OK cells. Conclusions： This lends strong support to the original idea that the Kidney has a significant influence over bone-formation, as suggested by some long-standing Chinese medical beliefs, kaempferol may also serve to stimulate kidney repair and indirectly stimulate bone formation.

Rheumatoid arthritis （RA） and osteoarthritis （OA） are the two most common joint diseases, and they have characterization of synovial inflammation and cartilage destruction, associated with the accumulation of numerous catabolic mediators and inflammatory cells in the synovial space and surrounding soft tissues. How these factors are cleared and if the ＂clearance＂ process contributes to pathogenesis of arthritis are not known. Recently, we found the existence of the peri-articular lymphatic system in mouse joints. The blockade of lymphangiogenesis and lymphatic draining function accelerates while stimulation of lymphatic function attenuates the severity of joint tissue lesions in mouse models of RA and OA. More importantly, we noticed the similarity between the dysfunction of lymphatic drainage in arthritic joints and ＂Bi＂ theory of Chinese medicine （CM）, and demonstrated that several Bi disease-treated herbal drugs directly affect the function of lymphatic endothelial cells. Here we review the advances about the interactions between joint inflammation and changes in the peri-articular lymphatic system and discuss our view of linking ＂Bi＂ theory of CM to lymphatic dysfunction in arthritis.

From ancient wisdom to modern science:Gut microbiota sheds light on property theory of traditional Chinese medicine

The property theory of traditional Chinese medicine(TCM) has been practiced for thousands of years,playing a pivotal role in the clinical application of TCM. While advancements in energy metabolism,chemical composition analysis, machine learning, ion current modeling, and supercritical fluid technology have provided valuable insight into how aspects of TCM property theory may be measured, these studies only capture specific aspects of TCM property theory in isolation, overlooking the holistic perspective inherent in TCM. To systematically investigate the modern interpretation of the TCM property theory from multidimensional perspectives, we consulted the Chinese Pharmacopoeia(2020 edition) to compile a list of Chinese materia medica(CMM). Then, using the Latin names of each CMM and gut microbiota as keywords, we searched the PubMed database for relevant research on gut microbiota and CMM. The regulatory patterns of different herbs on gut microbiota were then summarized from the perspectives of the four natures, the five flavors and the meridian tropism. In terms of the four natures, we found that warm-natured medicines promoted the colonization of specific beneficial bacteria,while cold-natured medicines boosted populations of some beneficial bacteria while suppressing pathogenic bacteria. Analysis of the five flavors revealed that sweet-flavored and bitter-flavored CMMs positively influenced beneficial bacteria while inhibiting harmful bacteria. CMMs with different meridian tropism exhibited complex modulative patterns on gut microbiota, with Jueyin(Liver) and Taiyin(Lung) meridian CMMs generally exerting a stronger effect. The gut microbiota may be a biological indicator for characterizing the TCM property theory, which not only enhances our understanding of classic TCM theory but also contributes to its scientific advancement and application in healthcare.

The concept of Arga and Bilig serves as a foundational principle in both ancient Mongolian philosophy and traditional Mongolian medicine (TMM). Arga, symbolized by brightness and associated with qualities of fire and activity, complements Bilig, symbolized by darkness and representing attributes of water and stillness. Together, these opposing forces permeate all aspects of existence, from the genesis of parenthood to the interplay of day and night. Understanding Arga-Bilig is crucial for diagnosing and treating diseases, as it illuminates the source of imbalance within the body. This review provides an overview of the significance of Arga-Bilig in Mongolian philosophy and its application in TMM, emphasizing the dynamic interplay of these opposing forces and their role in maintaining balance and harmony within the body.

Mechanisms of ＂kidney governing bones＂ theory in traditional Chinese medicine

Studies conducted by our group on the mechanism of ＂kidney governing bones＂ theory in traditional Chinese medicine （TCM） are reviewed in this paper. Conclusions can be summarized as follows. （1） Neuroendocrine-immune network （NIN）-osteoclast regulatory pathway OPG-RANKL-RANK is one of the mechanisms of ＂kidney governing bones.＂ Although kidney-reinforcing therapy is regarded as one of the holistic regulatory mechanisms of the body, characteristic holistic regulation in TCM can be reflected through nonselective regulation of the NIN during kidney reinforcement therapy, which can be used to treat osteoporosis through microadjustments in the microenvironment of the bone marrow. （2） Marrow exhaustion in TCM, which is the state wherein lipocytes in the bone marrow increase whereas other cells decrease, serves as the pathogenesis of osteoporosis brought about by failure of the ＂kidney governing bones.＂（3） The kidney in TCM can be regarded as a complex system comprising multiple functional units in the body, including the unit ＂governing bones.＂ Kidney deficiency refers to a deficiency in only one or more units of the kidney system and not the whole system itself, which explains the kidney-reinforcing effect of many herbs; some herbs can treat osteoporosis, but some cannot. Although both classified as kidney-reinforcing agents, the former can resolve failure of the ＂kidney governing bones＂ unit while the latter regulates the failure of other units in the kidney system. Despite the current understanding on ＂kidney governing bones＂ theory, the mechanism of ＂kidney governing bones＂ remains complicated and unresolved. Thus, further studies in this area are warranted.
