Objective To evaluate the efficacy of preventive treatment (PT) on alveolar pulmonary edema (APE) of cardiogenic origin using a monitor based on principles of internal thoracic impedance (ITI) *** We conducted blinded clinical trials on patients with ST-elevation myocardial infarction (STEMI) and monitored whether the condition would progress to APE. ITI was measured noninvasively by the Edema Guard Monitor (EGM, model RS-207) every 30 min. The measurement threshold for the diagnosis of APE was fixed at > 12% decrease in ITI from baseline as described in our methodology. The patients were divided into one group that received standard treatment after the appearance of clinical signs of APE without considering the prediction of APE by EGM devise (Group 1), and another group of asymptomatic patients in whom development of APE was predicted by using only EGM measurements (Group 2). The latter participants’PT consisted of furosemide, intravenous nitroglycerine and supplemental oxygen. Results One-hundred and fifty patients with acute STEMI were enrolled into this study. Group 1 included 100 patients (53% males, age 64.1 ±12.6 years). Treatment was started after the clinical appearance of overt signs of APE. Group 2 included 50 patients (54% males, age 65.2 ±11.9 years) who received PT based on EGMmeasurements. Group 2 had significantly fewer cases of APE (n = 4, 8%) than Group 1 (n = 100, 100%) (P > 0.001). While APE was lethal in six (6%) Group 1 patients, PT resulted in prompt resolution of APE in all four (8%) Group 2 patients. Conclusion ITI is a usefulmodality for early diagnosis and PT of pulmonary edema of cardiogenic origin.

Although compensated hypothyroidism (CH) is the most common thyroid impairment in Down syndrome (DS), its pathogenesis remains elusive. Because primary gonada l failure is another DS-associated endocrinopathy,we hypothesized that an impai red signal-transduction pathway shared by several organs may provide a unifying explanation for both endocrinopathies. We assessed two possible transduction-p athway components associated with CH in DS: the G-protein adenylate-cyclase (A C) system and β-adrenergic responsiveness, previously reported to be enhanced in DS fibroblasts. Twenty-one DS patients and 14 control subjects were studied. Peripheralmononuclear cells (PMCs)were incubated with G-proteinmodulators [pro stag-landin E1 (PGE1) and cholera toxin (CTx)], an AC stimulator (forskolin), and a β-adrenergic agonist (isoproterenol), and cAMP levels were determined. All pa rticipants had normal plasma thyroid hormone levels, but 11 of the DS patients h ad elevated TSH levels (hTSH), whereas in the 10 others, they were normal (nTSH) . cAMP levels in response to forskolin, PGE1, and CTx were similar in all groups , whereas isoprotereriol-stimulated cAMP levels were significantly higher in th e hTSH group than in the nTSH group and control subjects (45 ±30 versus 22 ±9 and 21 ±9 pmol ·106 cells-1 ·10 min-1, respectively; p = 0.02). Four patien ts in the DS hTSH subgroup had impaired sexual development. We found hyperrespon siveness of PMCs to a β-adrenergic agonist in a subgroup of DS patients with C H. If this observation is applicable to the thyroid gland, then it may reflect a mechanismin which negative effects on cell growth or responsiveness to TSH lead to CH.

Objective: To compare maternal and neonatal morbidity associated with two methods to extract the impacted fetal head during Cesarean delivery. Study design: We retrospectively analyzed cases with difficult extraction of the impacted fetal head during Cesarean section. We compared maternal and neonatal outcomes between cases that were delivered by head extraction following pushing through the vagina (“ push” method) and those that were delivered by the reverse breech technique (“ pull” method). Results: We reviewed 3105 Cesarean section reports. Difficult extraction necessitating the “ push” or “ pull” methods was noted in 48 (1.5% ) instances. Women that were delivered by the “ pull” method had significantly lower rate of postpartum fever (5% versus 46% ; odds ratios, 0.06; 95% confidence intervals, 0.007- 0.51) and extensions of the uterine incision (15% versus 50% ; odds ratio, 0.17; 95% CI, 0.04- 0.74) compared to those that were delivered by the “ push” method. Neonatal outcomes were good in all cases. Conclusions: In cases with difficult extraction of the impacted fetal head during Cesarean section, “ pull” method may result in lower maternal morbidity compared to the traditional “ push” method.

为了确定羚羊类固醇激素分泌模式与行为和社群等级的关系,我们检测了成年雄性弯角长角羚(n=15)和普通大羚羊(n=11)的粪样皮质酮和睾酮浓度。采用随意取样和目标取样法记录了共650 h的行为。结果发现,两种羚羊的睾酮水平在雌性的发情时段里都是最高的。两种羚羊皮质激素水平有季节性变化,表现为雨季皮质酮水平高于干旱季节。优势胁迫作用明显存在于普通大羚羊,而在弯角长角羚不明显。没有证据表明从属个体压力的存在,一只弯角长角羚曾经是优势个体,但是后来有两年不是优势个体,与其他雄羚相比,这只雄羚在雌性发情期的睾酮和皮质酮水平有3个交迭出现的峰值。

AIM To evaluate and describe the efficacy of fecal microbiota transplantation(FMT) for Clostridium difficile infection(CDI) in a national Israeli *** All patients who received FMT for recurrent(recurrence within 8 wk of the previous treatment) or refractory CDI from 2013 through 2017 in all the five medical centers in Israel currently performing FMT were included. Stool donors were screened according to the Israeli Ministry of Health guidelines. Clinical and laboratory data of patients were collected from patients' medical files, and they included indications for FMT, risk factors for CDI and disease severity. Primary outcome was FMT success(at least 2 mo free of CDI-related diarrhea post-FMT). Secondary outcomes included initial response to FMT(cessation of diarrhea within 7 d) and recurrence at 6 *** There were 111 FMTs for CDI, with a median age of 70 years [interquartile range(IQR): 53-82], and 42%(47) males. Fifty patients(45%) were treated via the lower gastrointestinal(LGI, represented only by colonoscopy) route, 37(33%) via capsules, and 24(22%) via the upper gastrointestinal(UGI) route. The overall success rate was 87.4%(97 patients), with no significant difference between routes of administration(P = 0.338). In the univariant analysis, FMT success correlated with milder disease(P = 0.01), ambulatory setting(P < 0.05) and lower Charlson comorbidity score(P < 0.05). In the multivariant analysis, only severe CDI [odd ratio(OR) = 0.14, P < 0.05] and inpatient FMT(OR = 0.19, P < 0.05) were each independently inversely related to FMT success. There were 35(32%) patients younger than 60 years of age, and 14(40%) of them had a background of inflammatory bowel *** FMT is a safe and effective treatment for CDI, with capsules emerging as a successful and well-tolerated route. Severe CDI is less likely to respond to FMT.

This study examines the stagnation point flow over a stretching/shrinking sheet in a hybrid nanofluid with homogeneous-heterogeneous *** hybrid nanofluid consists of copper(Cu)and alumina(Al2O3)nanoparticles which are added into water to form Cu-Al2O3/water hybrid *** similarity equations are obtained using a similarity ***,the function bvp4c in MATLAB is utilised to obtain the numerical *** dual solutions are found for limited values of the stretching/shrinking ***,the turning point arises in the shrinking region(λ<0).Besides,the presence of hybrid nanoparticles enhances the heat transfer rate,skin friction coefficient,and the concentration *** addition,the concentration gradient is intensified with the heterogeneous reaction but the effect is opposite for the homogeneous ***,the velocity and the concentration increase,whereas the temperature decreases for higher compositions of hybrid ***,the concentration decreases for larger values of homogeneous and heterogeneous *** is consistent with the fact that higher reaction rate cause a reduction in the rate of ***,the velocity and the temperature are not affected by these *** these observations,it can be concluded that the effect of the homogeneous and heterogeneous reactions is dominant on the concentration *** solutions are obtained for a single value of *** temporal stability analysis shows that only one of these solutions is stable and thus physically reliable over time.

1 Introduction The fact that the spectrum resource is underutilised in certain bands has motivated the dynamic spectrum access(DSA)approach,which enables unlicensed secondary users(SUs)equipped with cognitive radio(CR)devices to access the spectrum without causing significant interference to primary users(PUs).Nowadays,the increasing bandwidth for wireless communication in millimetre-wave and Terahertz frequency bands puts higher requirements on the performance of spectrum sensing technique,the primary enabler of DSA.
