Clinical Supervision of Midwifery Students at the University Teaching Hospital school of nursing and Midwifery in Lusaka, Zambia

Introduction: Clinical supervision plays a significant role in the acquisition of psychomotor skills by student midwives during training leading to the provision of high quality midwifery care. However, the acquisition of the psychomotor skills required for successful practice and learning can only be acquired if student midwives are supported and guided by clinical supervisors in their learning environments. Main Objective: The main objective of the study was to determine factors influencing clinical supervision of Student Midwives in Lusaka Urban District. Methodology: A cross-sectional study design that employed both quantitative and qualitative methods was used. The study population comprised 124 Nurses and Midwives working in Lusaka urban District. Self-administered questionnaires were used to collect data from the Nurses and midwives and a focus group discussion guide was used to guide discussions with the student midwives at Lusaka College of nursing and Midwifery and a total of three focus group discussions were conducted. Quantitative data were entered into by Statistical Package Social Sciences (SPSS) version 22 for windows. Chi-square test was used to test associations among variables. The confidence interval was set at 95% with a significance level of 0.05. Qualitative data were analysed using content analysis to classify words into themes and categories by looking for repeated ideas or patterns of thoughts. Results: The findings showed that a high percentage of the clinical supervisors (89.5%) had not been trained in clinical supervision and most respondents had moderate knowledge on clinical supervision of students. Majority of the respondents (63%) exhibited inadequate supervision skills. The attitudes of all the supervisors towards supervision were positive. A large percentage of respondents (73%) were competent in clinical supervision. However the learning environment considered to be poor by most respondents (61%). The study revealed a significant

Attitudes towards pain management in hospitalized cancer patients and their influencing factors

Objective： To investigate patients＇ attitudes towards cancer pain management and analyze the factors influencing these ***： The self-developed Demographic and Disease-Related Information Questionnaires, Pain Management Barriers Questionnaire-Taiwan form（BQT）, and Pain Knowledge Questionnaire were administered to 363 pairs of hospitalized cancer patients and their caregivers from the oncology departments of 7 hospitals in Beijing, ***： The average patient score for attitudes towards pain management was 2.96±0.49. The dimension scores indicated good attitudes in three areas（scores 〈2.5）, ＂Desire to be good＂（2.22±1.04）, ＂Fatalism＂（2.08±0.81） and＂Religious fatalism＂（1.86±1.00）, and poor attitudes in six areas（scores ≥2.5）, ＂Tolerance＂（3.83±0.96）, ＂Use of analgesics as needed（p.r.n.）＂（3.73±1.01）, ＂Addiction＂（3.44±1.05）, ＂Disease progression＂（3.28±1.26）, ＂Distraction of physicians＂（3.16±1.07） and ＂Side effects＂（2.99±0.68）. Two factors were entered into the regression equation：the caregivers＇ attitudes towards cancer pain management and the patients＇ pain knowledge. These two factors explained 23.2% of the total variance in the patients＇ average scores for their attitudes towards cancer pain ***： The patients＇ attitudes towards cancer pain management were poor and could be influenced by the caregivers＇ attitudes and the patients＇ pain knowledge, and thus need to be improved.

Respiratory training interventions improve health status of heart failure patients: A systematic review and network meta-analysis of randomized controlled trials

BACKGROUND Prior studies indicate that doing breathing exercises improves physical performance and quality of life (QoL) in heart failure patients. However, these effects remain unclear and contradictory. AIM To determine the effects of machine-assisted and non-machine-assisted respiratory training on physical performance and QoL in heart failure patients. METHODS This was a systematic review and network meta-analysis study. A literature search of electronic databases was conducted for randomized controlled trials (RCTs) on heart failure. Respiratory training interventions were grouped as seven categories: IMT_Pn (inspiratory muscle training without pressure or < 10% maximal inspiratory pressure, MIP), IMT_Pl (inspiratory muscle training with low pressure, 10%-15% MIP), IMT_Pm (inspiratory muscle training with medium pressure, 30%-40% MIP), IMT_Ph (inspiratory muscle training with high pressure, 60% MIP or MIP plus aerobics), Aerobics (aerobic exercise or weight training), Qi_Ex (tai chi, yoga, and breathing exercise), and none. The four outcomes were heart rate, peak oxygen uptake (VO2 peak), 6-min walking distance test (6MWT), and Minnesota Living with Heart Failure QoL. The random-effects model, side-splitting model, and the surface under the cumulative ranking curve (SUCRA) were used to test and analyze the data. RESULTS A total of 1499 subjects from 31 RCT studies were included. IMT_Ph had the highest effect sizes for VO2 peak and 6MWT, IMT_Pm highest for QoL, and Qi_Ex highest for heart rate. Aerobics had the second highest for VO2 peak, Qi_Ex second highest for 6MWT, and IMT_Ph second highest for heart rate and *** This study supports that high- and medium-intensity machine-assisted training improves exercise capacity and QoL in hospital-based heart failure patients. After hospital discharge, non-machine-assisted training continuously improves cardiac function.

Cross-cultural psychometric assessment of the parent-teen sexual risk communication (PTSRC-III) scale in Jamaica

Background: HIV/AIDS represents a serious health threat in Jamaica. Adolescent and young adult females are at particularly high risk. US studies have shown that parent-teen sexual communication may significantly influence the sexual risk-related beliefs and behaviors. However, no studies have examined parent-teen sexual communication in Jamaica or the wider Caribbean. Objective: The study aims to describe patterns of parent-teen sexual communication between Jamaican mothers and adolescent daughters and assess the psychometric properties of the Parent-Teen Sexual Risk Communication (PTSRC-III) scale when used with this population. Method: Data were collected as part of a larger randomized controlled trial study. A total of 330 urban Jamaican adolescent girls, ages 13 - 17, and their mothers or female guardians completed the 8-item PTSRC-III scale at baseline and 3-month follow-up. Results: Mothers’ and daughters’ scores covered the full range of possible scores. Internal reliability of the PTSRC-III scale was excellent (α = 0.92 and 0.93 for mothers and daughters, respectively). There was evidence of consistency over time (r = 0.57, p r = 0.27, p r = 0.427, p r = 0.146, p = 0.008). A two-factor structure was identified, although the second factor showed a lower eigen-value than was found in previous US studies. The loading pattern and explanation of variance were very similar to the factor 2 described in the initial psychometric testing of the PTSRC-III instrument. Discussion: Patterns of sexual communication and the psychometrics of the scale when used in Jamaica were consistent with US results, and provided evidence of the cross-cultural reliability and validity of the PTSRC-III scale.

Non-exhaustive double effort test is reliable and estimates the firs ventilatory threshold intensity in running exercise

Purpose: The present study aimed to investigate the reliability of the non-exhaustive double effort(NEDE) test in running exercise and its associations with the ventilatory thresholds(VT_1 and VT_2) and the maximal lactate steady state(MLSS).Methods: Ten healthy male adults(age: 23 ± 4 years, height: 176.6 ± 6.4 cm, body mass: 76.6 ± 10.7 kg) performed 4 procedures:(1) a ramp test for VT_1 and VT_2 determinations measured by ratio of expired ventilation to O_2 uptake(VE/VO_2) and expired ventilation to CO_2 output(VE/VCO_2) equivalents, respectively;(2) the NEDE test measured by blood lactate concentration(NEDE_(LAC)) and heart rate responses(NEDE_(HR));(3) a retest of NEDE for reliability analysis; and(4) continuous efforts to determine the MLSS intensity. The NEDE test consisted of4 sessions at different running intensities. Each session was characterized by double efforts at the same running velocity(E1 and E2, 180 s), separated by a passive recovery period(90 s rest). LAC and HR values after E1 and E2(in 4 sessions) were used to estimate the intensity equivalent to"null delta" by linear fit. This parameter represents, theoretically, the intensity equivalent to maximal aerobic ***: The intraclass correlation coefficient indicated significant reliability for NEDE_(LAC)(0.93) and NEDE_(HR)(0.79)(both p < 0.05). There were significant correlations, no differences, and strong agreement with the intensities predicted by NEDE_(LAC)(10.1 ± 1.9 km/h) and NEDE_(HR)(9.8 ± 2.0 km/h) to VT_1(10.2 ± 1.1 km/h). In addition, despite significantly lower MLSS intensity(12.2 ± 1.2 km/h), NEDE_(LAC) and NEDE_(HR) intensities were highly correlated with this parameter(0.90 and 0.88, respectively).Conclusion: The NEDE test applied to running exercise is reliable and estimates the VT_1 intensity. Additionally, NEDE intensities were lower but still correlated with VT_2 and MLSS.

Drug Seeking Behavior of Amphetamine Addicted Sprague-Dawley Rats Is Eliminated after Nutritional Supplementation

Aim: The effectiveness of nutritional supplementation on drug-seeking behavior of amphetamine-addicted rats during withdrawal was investigated using a biased conditioned place preference (CPP) paradigm. Method: Twenty-four male Sprague-Dawley rats exhibiting baseline preference for the black chamber during a 20-minute pre-conditioning exploration of the CPP box completed the study. On alternate days of an 8-day schedule, twelve rats (Group AMP) were randomly selected, given either amphetamine sulfate (5 mg/ml, i.p.) and confined to the white chamber;or vehicle (1 ml saline, i.p.) and confined to the black chamber. A significant increased percentage time spent and number of entries made by Group AMP to the drug-paired, white chamber on the test day confirmed amphetamine addiction. Group AMP subsequently received increasing doses of amphetamine over 6 days. Following acute drug withdrawal, their CPP performance was compared with that of vehicle treated rats (Group SAL). Groups AMP and SAL were equally divided and randomly assigned to animals fed chow reconstituted with the nutritional supplement (AMP-S and SAL-S) over 8 weeks or standard rat chow (AMP-N and SAL-N). CPP performances for all rats were determined blindly from video recordings following this period. Results: Nutritionally supplemented, amphetamine withdrawn rats (AMP-S) exhibited significantly decreased percentage entries and time spent in the white chamber (p ?ve, control animals. Conclusion: Drug-seeking behavior by amphetamine-addicted animals was eliminated after treatment with a nutritionally supplemented diet.

Effectiveness of Auricular Acupressure for Acute Postoperative Pain after Surgery: A Systematic Review and Meta-Analysis

Objective: To identify the effectiveness of auricular acupressure(AA) in patients with acute postoperative pain after surgery by systematic review. Methods: A search of randomized controlled trials was conducted in 5 English medical electronic databases and 4 Chinese databases. Two reviewers independently retrieved related studies, assessed the methodological quality, and extracted data with a standardized data form. Meta-analyses were performed using all time-points meta-analysis. Results: A total of 26 studies with 1,682 participants were included. Results showed that compared with conventional therapy, AA significantly improved the total effective rate [risk ratio=1.25, 95% confidence interval(CI), 1.13 to 1.37, P<0.0001; heterogeneity: P<0.0001, I^2=85%]. In the subgroup analysis, the results changed in different follow-up time and surgery categories. The pain relief in the AA group might be the most significant at 72 h after surgery(mean difference=–0.85, 95% CI, –1.20 to –0.50, P<0.0001) and in abdominal surgery(mean difference=–1.15, 95% CI, –1.41 to –0.90, P<0.0001). Sensitivity analysis demonstrated that the results of this meta-analysis were stable. No serious adverse effects were recorded. Conclusions: It was recommended to provide AA to patients with acute postoperative pain. However, a more accurate estimate of the effect requires further rigorously designed large-scale and high-quality RCTs for improving acute postoperative pain after surgery.
