One of th. most common cancers worldwide, colorectal cancer (CRC) h.s been associated with.significant morbidity and mortality and th.refore represents an enormous burden to th. h.alth.care system. Recent advances in CRC treatments h.ve provided patients with.primary and metastatic CRC a better long-term prognosis. Th. presence of synch.onous or metach.onous metastasis h.s been associated, h.wever, with.worse survival. Th. most common site of metastatic disease is th. liver. A variety of treatment modalities aimed at targeting colorectal liver metastases (CRLM) h.s been demonstrated to improve th. prognosis of th.se patients. Loco-regional approach.s such.as surgical resection and tumor ablation (operative and percutaneous) can provide patients with.a ch.nce at long-term disease control and even cure in select populations. Patient selection is important in defining th. most suitable treatment option for CRLM in order to provide th. best possible survival benefit wh.le avoiding unnecessary interventions and adverse events. Medical imaging plays a crucial role in evaluating th. ch.racteristics of CRLMs and disease resectability. Size of tumors, proximity to adjacent anatomical structures, and volume of th. unaffected liver are among th. most important imaging parameters to determine th. suitability of patients for surgical management or oth.r appropriate treatment approach.s. We h.rein provide a compreh.nsive overview of current-state-of-th.-art imaging in th. management of CRLM, including staging, treatment planning, response and survival assessment, and post-treatment surveillance. Computed tomograph. (CT) scan and magnetic resonance imaging (MRI) are two most commonly used tech.iques, wh.ch.can be used solely or in combination with.functional imaging modalities such.as positron emission tomograph. Role of transcath.ter arterial embolization for massive bleeding from gastroduodenal ulcers

Intractable bleeding from gastric and duodenal ulcers is associated with.significant morbidity and mortality. Aggressive treatment with.early endoscopic h.mostasis is essential for a favourable outcome. In as many as 12%-17% of patients,endoscopy is eith.r not available or unsuccessful. Endovascular th.rapy with.selective cath.terization of th. culprit vessel and injection of embolic material h.s emerged as an alternative to emergent operative intervention in h.gh.risk patients. Th.re h.s not been a systematic literature review to assess th. role for emboloth.rapy in th. treatment of acute upper gastrointestinal bleeding from gastroduo-denal ulcers after failed endoscopic h.mostasis. h.re,we present an overview of indications,tech.iques,and clinical outcomes after endovascular embolization of acute peptic-ulcer bleeding. Topics of particular relevance to tech.ical and clinical success are also discussed. Our review sh.ws th.t transcath.ter arterial embolization is a safe alternative to surgery for massive gastroduodenal bleeding th.t is refractory to endoscopic treatment,can be performed with.h.gh.tech.ical and clinical success rates,and sh.uld be considered th. salvage treatment of ch.ice in patients at h.gh.surgical risk.

Neonatal Intestinal Obstruction in Acute Renal Failure in Premature Infant: A Case Report

Introduction: Intestinal obstruction is a common cause of abdominal surgery in th. neonate. Diagnosis is straigh.forward using standard radiology, and surgical tech.ique depends on th. underlying anatomical lesion. Peritoneal dialysis (PD) is an effective, albeit invasive, th.rapy for neonatal renal failure. We report a case of neonatal obstruction with.severe renal failure treated by PD to h.gh.igh. our h.spital practice and possible remedies in a context of limited resources. Case Presentation: Th.s was a female neonate of moderate prematurity admitted on day 4 of life for management of a flat neonatal obstruction. radiological diagnosis suggested small bowel atresia. Biological tests revealed severe renal failure with.creatinine levels of 416 micromoles per liter and blood urea of 27.1 micromoles per liter. Management consisted of preoperative peritoneal dialysis for 48 h.urs followed by laparotomy. Th. intraoperative diagnosis was GROSFELD type IIIa digestive atresia. Th. postoperative course was favourable, transit was resumed on day 5 and th. patient returned h.me on day 12. Progress at 3 month. was satisfactory. Conclusion: Neonatal intestinal obstruction with.renal failure in premature infants is associated with.a poor prognosis, even more so if th.re is a delay in treatment. Peritoneal dialysis seems to be a suitable alternative for th.s management in our working conditions with.limited resources.

AIM: To define th. magnetic resonance imaging(MRI) parameters differentiating ureth.al h.permobility(Uh. and intrinsic sph.ncter deficiency(ISD) in women with.stress urinary incontinence(SUI).METh.DS: Th. static and dynamic MR images of 21 patients with.SUI were correlated to urodynamic(UD) findings and compared to th.se of 10 continent controls. For th. assessment of th. ureth.a and integrity of th. ureth.al support structures, we applied th. h.gh.esolution endocavitary MRI, such.as intraureth.al MRI, endovaginal or endorectal MRI. For th. functional imaging of th. ureth.al support, we performed dynamic MRI with.th. pelvic ph.sed array coil. We assessed th. following MRI parameters in both.th. patient and th.volunteer groups:(1) ureth.al angle;(2) bladder neck descent;(3) status of th. periureth.al ligaments,(4) vaginal sh.pe;(5) ureth.al sph.ncter integrity, length.and muscle th.ckness at mid ureth.a;(6) bladder neck funneling;(7) status of th. puborectalis muscle;(8) pubo-vaginal distance. UDs parameters were assessed in th. patient study group as follows:(1) ureth.al mobility angle on Q-tip test;(2) Valsalva leak point pressure(VLPP) measured at 250 cc bladder volume; and(3) maximum ureth.al closure pressure(MUCP). Th. Uh.type of SUI was defined with.th. Q-tip test angle over 30 degrees, and VLPP pressure over 60 cm h. O. Th. ISD incontinence was defined with.MUCP pressure below 20 cm h. O, and VLPP pressure less or equal to 60 cm h. O. We considered th. associations between th. MRI and clinical data and UDs using a variety of statistical tools to include linear regression, multivariate logistic regression and receiver operating ch.racteristic(ROC) analysis. All statistical analyses were performed using STATA version 9.0(Stata Corp LP, College Station, TX).RESULTS: In th. incontinent group, 52% h.ve h.story of vaginal delivery trauma as compared to none in control group(P h.re was no difference between th. continent volunteers and incontinent patients
