Effect of nutritional counselling on hepatic,muscle and adipose tissue fat content and distribution in non-alcoholic fatty liver disease

AIM： To assess the effectiveness of the current UK clinical practice in reducing hepatic fat （IHCL）.METHODS： Whole body mrI and IH mrS were obtained, before and after 6 mo nutritional counselling, from liver, soleus and tibialis muscles in 10 subjects with non-alcoholic fatty liver disease （NAFLD）.RESULTS： A 500 Kcal-restricted diet resulted in an average weight loss of 4% （-3.4 kg,） accompanied by significant reductions in most adipose tissue （AT） depots, including subcutaneous （-9.9%）, abdominal subcutaneous （-10.2%） and intra-abdominal- AT （-11.4%）. Intramyocellular lipids （IMCL） were significantly reduced in the tibialis muscle （-28.2%）. Decreases in both IHCL （-39.9%） and soleus IMCL （-12.2%） content were also observed, although these were not significant. Several individuals showed dramatic decreases in IHCL, while others paradoxically showed increases in IHCL content. Changes in body composition were accompanied by improvements in certain liver function tests： serum aspartate aminotransferase （AST） and alanine aminotransferase （ALT）. Significant correlations were found between decreases in IHCL and reductions in both intra-abdominal and abdominal subcutaneous AT. Improvements in liver function tests were associated with reductions in intra-abdominal AT, but not with changes in IHCL. CONCLUSION： This study shows that even a very modest reduction in body weight achieved through lifestyle modification can result in changes in body fat depots and improvements in LETs.

Hidden Secrets of the New England IPA

New England India Pale Ale (NEIPA), also known as hazy IPA or juicy IPA, is a relatively new beer style being brewed by craft breweries across the Un让ed States. These beers typically have massive hop aroma and flavor yet are not perceived as being very bitter. They can be incredibly hazy, and many impart fruity and juicy flavors. Twelve NEIPAs were analyzed by high-performance liquid chromatography for various hop compounds, and several tests were conducted on the haze itself to determine what it is and is not. Experiments conducted on these beers exposed the hidden secrets of NEIPA. The results indicated that the vast majority of the haze is polyphenol—protein haze and that the haze can act as a carrier to more of the nonpolar hop compounds than are typically found in West Coast style IPA.

A Validation Study of the Deep-Learning-Based Prostate Imaging Reporting and Data System Scoring Algorithm

Purpose: The Prostate Imaging Reporting and Data System (PI-RADS) was introduced to standardize prostate cancer diagnosis by mrI. However, the inter-reader agreement by PI-RADS scoring is not always high. The purpose of this study was to validate a deep-learning-based diagnostic algorithm of PI-RADS. Methods: We applied a Siemens Healthineers Prostate Artificial Intelligence (AI) prototype (work in progress) for fully automated prostate lesion detection, classification and reporting. More than 2000 bi-parametric mrI studies along with the PI-RADS reports were included as training, validation, and test data. This prospective validation study includes 101 consecutive patients suspected of prostate cancer, and 100 patients were included in the analysis. All subjects underwent a noncontrast-enhanced bi-parametric mrI including T2-weighted and diffusion-weighted imaging. Two board-certified radiologists independently scored the PI-RADS, and if there were disagreements;another radiologist confirmed the diagnosis. We compared the AI results with the interpretation results by the radiologists. Results: The sensitivity of our AI model for PI-RADS ≥ 4 was 0.76, and the specificity was 0.76. For the cases with PI-RADS ≥ 3, the sensitivity was 0.69, and the specificity was 0.76. In the lesion-based analysis, AI detection rates of PI-RADS 3, 4, 5 lesions in the peripheral zone were 43%, 63%, and 100%, respectively. In the transition zone, AI detection rates of PI-RADS 3, 4, 5 were 30%, 54%, and 100%, respectively. Conclusion: Our deep-learning-based algorithm has been validated and shown to help score PI-RADS.

Background: Pelizaeus-Merzbacher disease (PMD) is a rare X-linked recessive neurologic disorder caused by a mutation in the proteolipid protein (PLP) gene on chromosome Xq22. The associated depletion of PLP and severe reduction of other major myelin proteins results in dysmyelination. mrI reveals loss of T1 contrast between gray and affected white matter and T2 hyperintensities of white matter due to elevated water content. Methods: In vivo proton magnetic resonance spectroscopy (mrS) was used to determine cerebral metabolite patterns in five patients with genetically proven PMD. Absolute metabolite concentrations were obtained in cortical gray matter, affected white matter, and basal ganglia and compared to age-matched control values. Results: In comparison to age-matched controls, mrS of affected white matter resembled the metabolite pattern of cortical gray matter, as indicated by increased concentrations of N-acetylaspartate and N-acetylaspartylglutamate (tNAA), glutamine (Gln), myo-inositol (Ins), and creatine and phosphocreatine. Most remarkably, the concentration of choline-containing compounds was reduced. Parietal gray matter and basal ganglia appeared normal but showed a tendency for elevated tNAA, Gln, and Ins. Conclusions: Magnetic resonance spectroscopy (mrS)detected alterations are consistent with enhanced neuroaxonal density, astrogliosis, and reduction of oligodendroglia. These disturbances in cellular composition are in close agreement with the histopathologic features characteristic of dys-and hypomyelination. The proton mrS profile of Pelizaeus-Merzbacher disease (PMD) differs from the pattern commonly observed in demyelinating disorders and allows PMD to be distinguished from other leukodystrophies.
