Management of an Unanticipated Difficult Airway during Emergency “C” Section—A Novel Approach

Introduction: Knowledge, skill and training in addition to quick thinking, come to the rescue of Anesthesiologists when encountering an unanticipated difficult airway during emergency Caesarean section. Ability to react with time to spare will ensure maternal and fetal well being while handling this life threatening emergency. Case History: While anesthetizing a 22-year parturient for emergency Caesarean section, the endotracheal tube was inadvertently placed in the esophagus. As the “call for help” was activated, the esophageal tube was delivered thru the endoscopic port of a Patil-Syracuse face mask. After confirming our ability to ventilate the patient without distending the stomach while maintaining the oxygen saturation and end tidal carbon dioxide levels within normal limits, surgery was allowed to proceed under mask anesthesia employing oxygen, nitrous oxide and sevoflurane with rocuronium for muscle relaxation. After a healthy infant was delivered, definitive airway access was obtained with Glidescope? assisted fiberoptic intubation. The esophageal tube was then removed. Further surgery proceeded uneventfully. Discussion: By choosing to deliver the proximal end of the inadvertently placed esophageal tube thru the endoscopic port of a Patil-Syracuse mask and mask ventilating the patient, we have been able to provide that few precious minutes of oxygenation to the distressed fetus before delivery. By isolating and venting the stomach thru the esophageal tube we provided maternal air way protection during the initial phase of the delivery. Definitive airway access was obtained as soon as additional help and equipment were available. Conclusion: Difficult airway algorithm while comprehensive, does not address the question of time management. While dealing with a difficult airway in obstetric anesthesia, time is the single most important factor, which will determine the maternal and fetal well being. We in our case report have attempted to answer that question of

Evaluating the Potential Effectiveness of Moringa oleifera Seeds Biomass as an Adsorbent in the Removal of Copper (Cu) in Water

This study was based on the evaluation of the potential effectiveness of Moringa oleifera seeds biomass as a biosorbent in the removal of copper (Cu) in water which was justified by the level of toxicity, environmental unfriendliness and costly nature of chemical coagulants presently used. Fourier transform infrared (FTIR) analysis was used to identify the Moringa oleifera seeds biomass functional groups present in the adsorption of metal ions and found to be the carboxylic acid and amine functional groups (-COOH and -NH). The effects of contact time, adsorbent dosage, metal ion concentration and pH were studied. The maximum adsorption capacity at pH 5, room temperature and 0.8 g dosage was 70% for Cu(II). The adsorption data fitted better to the Langmuir than the Freundlich models as the sorption capacity (qm) of Moringa oleifera seeds biomass for Cu(II) was 3.64 mg/g. The separation factor (RL) was within the range of 0 and 1 which showed that the Cu(II) biosorption processes were favourable for Moringa oleifera biosorbent. The results showed that Moringa oleifera seed biomass is an effective adsorbent in the removal of the studied heavy metals in water. The effective pH for the Cu(II) removal was 5.0 as equilibrium was achieved practically in 35 min. The quantitative analysis of defatted Moringa oleifera should be studied in order to have a fair mixing ratio between Moringa oleifera seeds biomass and the adsorbate. There is also the ardent need to work on environmentally friendly disposal of adsorbent after saturation of adsorbent by analyte to avoid secondary pollution.

It has recently been demonstrated experimentally that a turbulent plasma created by the collision of two inhomogeneous,asymmetric,weakly magnetized,laser-produced plasma jets can generate strong stochastic magnetic fields via the small-scale turbulent dynamo mechanism,provided the magnetic Reynolds number of the plasma is sufficiently *** this paper,we compare such a plasma with one arising from two pre-magnetized plasma jets whose creation is identical save for the addition of a strong external magnetic field imposed by a pulsed magnetic field *** investigate the differences between the two turbulent systems using a Thomson-scattering diagnostic,x-ray selfemission imaging,and proton *** Thomson-scattering spectra and x-ray images suggest that the external magnetic field has a limited effect on the plasma dynamics in the *** the external magnetic field induces collimation of the flows in the colliding plasma jets and although the initial strengths of the magnetic fields arising from the interaction between the colliding jets are significantly larger as a result of the external field,the energies and morphologies of the stochastic magnetic fields post-amplification are *** conclude that,for turbulent laser-plasmas with supercritical magnetic Reynolds numbers,the dynamo-amplified magnetic fields are determined by the turbulent dynamics rather than the seed fields or modest changes in the initial flow dynamics of the plasma,a finding consistent with theoretical expectations and simulations of turbulent dynamos.

Dear Editor,The activating receptor DNAX accessory molecule-1(DNAM-1)plays an important role in T and natural killer(NK)cell-mediated cytotoxicity via the interaction with its ligands poliovirus receptor(PVR,CD155)and Nectin-2(CD112).Compared with peripheral blood NK cells,tumor-infiltrating NK cells show reduced expression of DNAM-1 across several solid tumors,including ovarian and breast cancer resulting in impaired NK cell *** studies reported an inverse correlation between DNAM-1 expression and its ligands in leukemic blasts and ovarian cancer[1].

Objective: The objective of this study is to assess the outcome of port-a-cath (PAC) maintenance every three months in patients with gynecologic malignancies with the goal of standardizing a safe and appropriate interval that would maintain patency and minimize side effects. Methods: We performed a retrospective medical record review and evaluation of all patients with a Bard? PAC who were noted to have no evidence of disease (NED) during the years 2003 to 2010. The interval between accessions and any complications related to the presence of the PAC were recorded. Relevant complications included skin infections, bacteremia, thrombosis, and occlusions. Statistical analysis was done using the Fisher’s exact test. Results: A total of 201 patients had PAC placed and 43 patients underwent PAC accessions to maintain patency. The total number of accessions was 150 with a median number per patient of 2.0 (range 1 - 10). The mean time between flushes was 112 days (SD = 57). When comparing women in maintenance who had flushes within 90 days versus those who had flushes over 90 days apart, there was no difference in infection or occlusion rates between these groups (p = 0.515). In the Conclusion: Infections and occlusions are rare in women with gynecologic malignancies undergoing maintenance of their PAC. Longer intervals between PAC flushes do not appear to affect the outcome in our patients. Our ongoing data and follow-up confirm that extending the interval of PAC accession to every 3 months, rather than monthly, is safe, effective and convenient in the patient population with gynecologic malignancies.

中国牡蛎的种类繁多,多样性极为丰富,由于其形态变异大,已报道的中国牡蛎常见种类组成及分布争议甚大。鉴于上述牡蛎分类混乱的状况,近年来作者对中国近海近50个典型海区的牡蛎进行采样,对上千个牡蛎样品进行分析,在形态学分析的基础上,对具有典型形态特征的样品进行16S,COI和28S部分序列的测定,澄清了部分常见牡蛎的分类地位。
