男性包皮环切能显著降低男性阴茎-阴道性交获得性HIV感染风险大约60%,被WHO和联合国艾滋病规划署(UNAIDS)推荐作为HIV预防策略中的一个重要干预措施。寻求一种更加安全、有效和可接受的男性包皮环切器械和手术方法,以便能够满足加快执行扩大包皮环切预防HIV感染项目的需求,已经成为相关国际组织,特别是非洲国家政府公共卫生事业的当务之急。2008年中国商环(Shang Ring)包皮环切标准化手术方案的建立,以及应用这个标准化手术方案和手术培训在中国以及2009年和2010年在肯尼亚实施中国商环成人包皮环切手术获得有用的和有意义的临床数据,证明了中国商环包皮环切术的诸多优势。手术培训手册和教学视频的多次修订为培训医护人员提供了更加准确的教学指南。经过多家相关国际机构专家的考察和评估,中国商环包皮环切技术已经成为支持在非洲HIV高发地区扩大包皮环切服务预防HIV感染项目最具潜力的候选包皮环切器械之一。可以预计,中国商环包皮环切技术的成功应用将会在改变数百万非洲人的生活方式的同时,也为中国男科学与泌尿外科学医生在包皮环切与HIV预防和生殖健康相关的临床研究领域提供了丰富的机会。本文报告了2008年2月至2010年底期间中国商环包皮环切技术临床应用的国际和国内研究进展。

Objective:To analyze the age and gender distribution characteristics of 7,632 HIV/AIDS patients at the onset of HIV infection-related high-risk intravenous drug abuse and sexual contact in Yunnan ***:Data were collected from the database of Chinese Medicine Treatment of AIDS pilot Project in Yunnan ***,age and demographics of HIV/AIDS patients were ***:The patients were almost in relatively high educational *** number of male intravenous drug users(12.90%)was more than female,and the earliest average age was 10-14 *** percentages of men in 10-19 years and 35-59 years were more than that of *** obvious difference was found in heterosexual sexual contact in both men(48.11%)and women(51.89%),and the earliest ages was 15-19 years in males and 10-14 years in *** percentage of males at 10-34 years old was less than that of females,just opposite to the age of 35-85 *** contact was more in males(92.73%)than that in females(7.27%).The earliest homosexual sexual contact associated with HIV infection was 15-19 years in males and 25-29 years in *** 128 AIDS patients with pulmonary tuberculosis infection,intravenous drug abuse accounted for the highest proportion(76.56%)of the three high-risk behaviors related to HIV ***:Reducing risk behaviors and preventing intravenous drug abuse could be effective in preventing *** with other high-risk behaviors,patients with intravenous drug use and AIDS are at greater risk of contracting tuberculosis.

Background:The Ebola virus disease spread rapidly in West Africa in 2014,leading to the loss of thousands of *** engagement was one of the key strategies to interrupt Ebola transmission,and practical community level measures needed to be explored in the field and tailored to the specific context of ***:First,community-level education on Ebola virus disease(EVD)prevention was launched for the community’s social mobilizers in six districts in Sierra Leone beginning in November ***,from January to May of 2015,in three pilot communities,local trained community members were organized to engage in implementation of EVD prevention and transmission interruption measures,by involving them in alert case report,contact tracing,and social *** epidemiological indicators of transmission interruption in three study communities were ***:A total of 6016 community social mobilizers from 185 wards were trained by holding 279 workshops in the six districts,and EVD message reached an estimated 631680 *** three pilot communities,72 EVD alert cases were reported,with 70.8%of them detected by trained local community members,and 14 EVD cases were finally *** tracing detected 64.3%of EVD *** median duration of community infectivity for the cases was 1 *** secondary attack rate was 4.2%,and no third generation of infection was *** health worker was infected,and no unsafe burial and noncompliance to EVD control measures were *** community-based measures were modeled to reduce 77 EVD cases,and the EVD-free goal was achieved four months earlier in study communities than whole country of Sierra ***: The community-based strategy of social mobilization and community engagement was effective in casedetection and reducing the extent of Ebola transmission in a country with weak health system.
