Low back pain (LBP) is one of the most prevalent, disabling and costly medical conditions affecting modern society. LBP presents a significant challenge to effective treatment due to often multifactorial or unknown etiology. Since the 1980s, the sacroiliac (SI) joint has become increasingly recognized as a common source of LBP. In contrast to other sources of LBP such as internal disc disruption and even psychosocial factors, SI joint pain and degeneration are reliably identified with provocative manual tests and diagnostic injections. Fusion of the SI joint has been shown to provide enduring symptom relief, and minimally invasive techniques developed over the past decade have further reduced the operative risks associated with open fusion surgery. Minimally invasive SI joint fusion surgeries are typically performed by placing rigid implant components across the joint space. The implants provide mechanical fixation while bony fusion develops. Decortication of the SI joint space during the procedure produces a bleeding bone surface that allows for increased availability of autologous mesenchymal stem cells and growth factors at the fusion site. Coupled with the mechanical stability provided by the implant and autologous bone graft, decortication provides an optimal environment for bone growth and subsequent fusion of the joint. This report describes the background of SI joint disease, treatment, and the minimally invasive SImmetry? Sacroiliac Joint Fusion System (Zyga Technology, Inc., Minnetonka, MN, USA), with emphasis on the decortication instrumentation and procedure.

Geostatistics-block-based characterization of the relationship between rock mass quality and powder factor and its application on open-pit limit optimization

Accurately predicting the powder factor during blasting is essential for sustainable production planning in low-grade *** research presents a method for predicting powder factor based on the heterogeneity of rock mass rating(RMR).Considering a low-grade metal mine as an example,this study exploited geostatistical methods to obtain independent RMR for each block unit.A three-dimensional spatial distribution model for the powder factor was developed on the basis of the relationships between the RMR and the powder ***,models for blasting cost and mining value were built and employed to optimize the open-pit *** multi-variable model based on the RMR performed well in predicting the powder factor,achieving a correlation coefficient of 0.88(root mean square error of 4.3)and considerably outperforming the uniaxial compressive strength *** model optimization,the mean size and standard deviation of the fragments in the blast pile decreased by 8.5%and 35.1%,respectively,whereas the boulder yield and its standard deviation decreased by 33.3%and 58.8%,***,optimizing the open-pit limit using this method reduced the amount of rock,increased the amount of ore,and lowered blasting costs,thereby enhancing the economic efficiency of the *** study provides valuable insights for blasting design and mining decisions,demonstrating the advantages and potential applications of powder factor prediction based on the heterogeneity of rock mass quality.

Modulatory effects of Azadirachta indica on benzo(a)pyrene-induced forestomach tumorigenesis in mice

AIM： To evaluate the chemopreventive effects of aqueous Azadimchta indica （A indica） leaf extract （AAILE） against benzo（a）pyrene [B（a）P]-induced forestomach tumorigenesis in Balb/c mice. METHODS： Female Balb/c mice were divided into four groups of 10-12 animals each. For induction of forestomach tumors, starting from d 14 of the experiment, mice of B（a）P and B（a）P＋A indica groups were given intra-gastric instillations of B（a）P （40 mg/kg）, twice a week for four weeks. Mice ofA indica and B（a）P＋A indica groups were orally administered with AAILE （100 mg/kg）, two weeks prior to B（a）P instillations till the end of the experiment. After 22 wk of the first B（a）P instillation, mice were sacrificed and the forestomachs were analyzed for development of tumors, scanning electron microscopy （SEM） and histopathology. RESULTS： Tumor incidence was observed to be 100% in mice that received only B（a）P. However, treatment with AAILE reduced the tumor incidence by 58.4% as observed in mice of B（a）P＋A indica group when compared to that of B（a）P group. Similarly, the tumor burden and multiplicity were seen to decrease by 87.3% and 69.6% respectively in mice of B（a）P＋A indica group when compared to those of B（a）P group. Scanning electron microscopy analysis showed that AAILE treatment itself did not cause any abnormalities on the surface architecture of forestomach epithelium. In tumorous forestomach, surface disruption was observed. Over the forestomach tumors of B（a）P group of mice certain rounded structures were seen in addition to closely placed tongue-shaped squamous cells. Interestingly, these rounded structures were not observed in B（a）P ＋ A indica group of mice. Histopathalogically, the tumors were identical and diagnosed to be papillomas. Mice from control and A indica groups of mice did not develop any forestomach tumors and showed normal histo-architecture. Lycopene modulates cellular proliferation, glycolysis and hepatic ultrastructure during hepatocellular carcinoma

AIMTo investigate the effect of lycopene extracted from tomatoes (LycT) on ultrastructure, glycolytic enzymes, cell proliferation markers and hypoxia during N-Nitrosodiethylamine (NDEA)-induced hepatocarcinogenesis. METHODSFemale BALB/c mice were randomly divided into four groups: The Control, NDEA (200 mg NDEA/kg b.w. given i.p.), LycT (5 mg/kg b.w. given orally on alternate days) and LycT + NDEA group. The mRNA and protein expression of various cell proliferation markers (PCNA, Cyclin D1, and p21) were assessed by reverse transcription-polymerase chain reaction and enzyme linked immunosorbent assay, respectively. The ultrastructure of hepatic tissue was analyzed using scanning and transmission electron microscopy. The enzymatic activity of glycolytic enzymes was estimated using standardized protocols, while glucose-6-phosphate dehydrogenase activity level was estimated using a kit obtained from Reckon Diagnostic P. Ltd. (India). RESULTSUncontrolled proliferation in the liver of NDEA (P ≤ 0.001) mice was evident from the high expression of cell-proliferation associated genes (PCNA, Cyclin D1, and p21) when compared to control and LycT mice. In addition, enhanced activities of hexokinase, phosphoglucoisomerase, aldolase, glucose-6-phosphate dehydrogenase and hypoxia-inducible factor-1α were observed in NDEA mice as compared to control (P ≤ 0.001) and LycT (P ≤ 0.001) mice. The alterations in hepatic ultrastructure observed in the NDEA group correlated with the changes in the above parameters. LycT pre-treatment in NDEA-challenged mice ameliorated the investigated pathways disrupted by NDEA treatment. Moreover, hepatic electron micrographs from the LycT + NDEA group showed increased macrophages, apoptotic bodies and well-differentiated hepatocellular carcinoma (HCC) in comparison to undifferentiated HCC as observed in the NDEA treated group. Is bactibilia a predictor of poor outcome of pancreaticoduodenectomy?

BACKGROUND:Although bile infection has been proposed to increase infective complications following pancreaticoduodenectomy,its association with infective complications and non-infective complications like pancreatic fistula is still ***:Seventy-six patients who had undergone pancreaticoduodenectomy between July 2007 and December 2008 were included in a prospective database and their data *** all patients intraoperative bile from the bile duct was ***,intra-operative,and post-operative variables were recorded and ***:Bile culture showed positive growth in 35 patients and negative growth in *** patients in the positive group underwent ERCP and *** patients with a positive bile culture had a higher incidence of infective complications including intra-abdominal abscess (n=8),wound infection (n=27),bacteremia (n=10),and renal insufficiency (n=9).There was no increase in the rate of non-infective complications of pancreaticoduodenectomy including pancreatic fistula (n=7),delayed gastric emptying (n=9),and post-operative hemorrhage (n=3).The hospital stay was significantly prolonged in the patients with a positive bile culture (P=0.0002).CONCLUSIONS:Pre-operative biliary drainage is significantly associated with bile infection,and bile infection increases the overall rates of infective complications and renal *** of the high incidence of complications is associated with infected bile,routine intra-operative bile culture is recommended in patients undergoing ***-operative prophylaxis is dependent on sensitivity of cases to perioperative antibiotics and intraoperative bile culture *** of its significant association with infected bile,biliary stenting should be used in strictly selected cases.

Ab externo canaloplasty results and efficacy: a retrospective cohort study with a 12-month follow-up

Analysis of sudden variations in photospheric magnetic fields during a large flare and their influences in the solar atmosphere

The solar active region NOAA 11719 produced a large two-ribbon flare on 2013 April 11. We have investigated sudden variations in the photospheric magnetic fields in this active region during the flare by employing magnetograms obtained in the spectral line Fe I 6173 A acquired by the Helioseismic and Magnetic Imager （HMI） onboard the Solar Dynamics Observatory （SDO） spacecraft. The analysis of the line-of-sight magnetograms from HMI show sudden and persistent magnetic field changes at different locations of the active region before the onset of the flare and during the flare. The vector magnetic field observations available from HMI also show coincident variations in the total magnetic field strength and its inclination angle at these locations. Using the simultaneous Dopplergrams obtained from HMI, we observe perturbations in the photospheric Doppler signals following the sudden changes in the magnetic fields in the aforementioned locations. The power spectrum analysis of these velocity signals shows enhanced acoustic power in these affected locations during the flare as compared to the pre-flare condition. Accompanying these observations, we have also used nearly simultaneous chromospheric observations obtained in the spectral line Ha 6562.8 A by the Global Oscillation Network group （GONG） to study the evolution of flare- ribbons and intensity oscillations in this active region. The Ha intensity oscillations also show enhanced oscillatory power during the flare in the aforementioned locations. These results indicate that the transient Lorentz force associated with sudden changes in the magnetic fields could drive localized photospheric and chromospheric oscillations, like the flare-induced oscillations in the solar atmosphere.

Selenium as a chemopreventive agent in experimentally induced colon carcinogenesis

AIM:To elucidate the chemopreventive efficacy of selenium during experimentally induced colon ***:Thirty-two male wistar rats were divided into four groups:group □(normal control);group □[1,2-dimethylhydrazine(DMH) treated];group □ (selenium treated);and group □ (DMH + selenium treated).groups □ and □ were given subcutaneous injections of DMH(30 mg/kg body weight) every week for 20 ***,in the form of sodium selenite,was given to groups □ and at 1 ppm in drinking water ad libitum for 20 *** the end of the study,rats were sacrificed and their colons were analyzed for the development of tumors,antioxidant enzyme levels and histological ***:100 of the DMH treated rats developed tumors,which was reduced to 60 upon simultaneous selenium ***,tumor multiplicity decreased to 1.1 following selenium supplementation to DMH treated *** of lipid peroxidation,glutathione-S-transferase,superoxide dismutase(SOD),catalase,and glutathione peroxidase(GPx) decreased following DMH treatment,whereas levels of glutathione(GSH) and glutathione reductase(GR) significantly increased in DMH treated *** administration to DMH treated rats led to an increase in the levels of lipid peroxidation,SOD,catalase,glutathione-S-transferase and GPx,but decreased the levels of GSH and *** studies on DMH treated rats revealed dysplasia of the colonic histoarchitecture,which showed signs of improvement following selenium ***:The study suggests the antioxidative potential of selenium is a major factor in providing protection from development of experimentally induced colon carcinogenesis.
