Surveillance recommendations for gastric cancer（GC） in current guidelines focused on advanced precancerous lesions and were based on precise diagnosis of severity/extent of baseline lesions. We aimed to develop a less endoscopy-related equipment-dependent risk-stratification tool, and assessed whether mild-precursor-lesion patients can be safely exempt from surveillance. In the multicenter communitybased cohort, 75,051 participants receiving baseline endoscopy were enrolled during 2015–2017 and followed-up until 2021. Cumulative incidence rates（CIRs） of GC for precancerous-conditions were calculated by Kaplan-Meier method and compared by Log-rank tests. Mixedeffects Cox regression models were used to detect potential factors for progression towards GC. A risk score was calculated as counts of selected factors. An independent cohort, including 26,586 participants was used for external validation. During a median follow-up of 6.25 years, CIRs of GC were 0.302%, 0.436%, and 4.756% for normal group, non-neoplastic（atrophic gastritis/intestinal metaplasia） and neoplastic lesions（low-grade/high-grade dysplasia）, respectively(Ptrend<0.001). Four predictors, including male, ≥60 years, smoking, and limited vegetable consumption, were selected for risk-stratification. High-risk patients（≥3 risk factors） with non-neoplastic lesions showed higher GC risks（adjusted HR=7.73, 95%CI: 4.29–13.92）, and their four-year CIR reached the one-year CIR of neoplastic lesions. Further categorizing non-neoplastic lesions by histological grade, both patients with moderate-to-severe lesions（aHR=3.07, 95%CI: 1.67–5.64） and high-risk patients with mild lesions（aHR=7.29, 95%CI: 3.58–14.86） showed higher risks. Consistent trends were observed in validation cohort. High-risk mild-precursor-lesion patients should receive surveillance within 3–5 years after baseline screening. Our study provides evidence on supplementing current guideline recommendations.

Cohort study on the treatment of BRAF V600E mutant metastatic colorectal cancer with integrated chinese and western medicine

BACKGROUND Patients with BRAF V600E mutant metastatic colorectal cancer(mCRC)have a low incidence rate,poor biological activity,suboptimal response to conventional treatments,and a poor *** the previous cohort study on mCRC conducted by our team,it was observed that integrated chinese and Western medicine treatment could significantly prolong the overall survival(OS)of patients with colorectal ***,we further explored the survival benefits in the population with BRAF V600E mutant *** To evaluate the efficacy of integrated chinese and Western medicine in the treatment of BRAF V600E mutant metastatic colorectal *** A cohort study was conducted on patients with BRAF V600E mutant metastatic colorectal cancer admitted to Xiyuan hospital of China Academy of chinese Medical Sciences and traditional chinese medicine hospital of Xinjiang Uygur Autonomous Region from January 2016 to December *** patients were divided into two *** A total of 34 cases were included,with 23 in chinese-Western medicine cohort(cohort A)and 11 in Western medicine cohort(cohort B).The median overall survival was 19.9 months in cohort A and 14.2 months in cohort B,with a statistically significant difference(P=0.038,hazard ratio=0.46).The 1-3-year survival rates were 95.65%(22/23),39.13%(9/23),and 26.09%(6/23)in cohort A,and 63.64%(7/11),18.18%(2/11),and 9.09%(1/11)in cohort B,*** analysis showed statistically significant differences in median OS between the two cohorts in the right colon,liver metastasis,chemotherapy,and first-line treatment subgroups(Pchinese and Western medicine can prolong the survival and reduce the risk of death in patients with BRAF V600E mutant metastatic colorectal cancer,with more pronounced benefits observed in patients with right colon involvement,liver metastasis,combined chemotherapy,and first-line treatment.

Background:Insomnia is a prevalent clinical condition and Shangxia Liangji formula(SXLJF)is a well-established method of ***,the specific mechanism of action of SXLJF remains ***:The mouse model of insomnia was established by intraperitoneal injection of ***-two mice were randomly divided into a negative control group,model group,SXLJF group(18.72 g/kg/day),and positive control group(diazepam,2 mg/kg)and treated with the corresponding drugs for 7 consecutive *** open field test and pentobarbital-induced sleeping test were ***-MS-based untargeted metabolomics and network pharmacology were applied to explore the potential targets of SXLJF for treating ***,key targets were validated using ***:Behavioral tests demonstrated that SXLJF reduced the total distance,average velocity,central distance,and sleep latency,and prolonged sleep *** and network pharmacology revealed potential targets,signaling pathways,metabolic pathways,and metabolites associated with the anti-insomnia effects of ***,tyrosine hydroxylase(TH)and tyrosine metabolism emerged as crucial metabolic pathways and targets,***-qPCR results supported the role of TH in the mechanism of SXLJF in treating ***:In conclusion,TH and tyrosine metabolism may represent significant targets and pathways for SXLJF in treating insomnia.

Effects of Acupuncture on chinese medicine Syndromes of Vascular Dementia

Objective： To study the effects of acupuncture on chinese medicine syndromes of vascular dementia （VaD）. Methods： Sixty-three VaD patients were divided into three groups. Those willing to be randomized were randomly assigned to receive either acupuncture （random acupuncture group, 24 cases） or rehabilitation training （guided rehabilitation group, 24 cases） for 6 weeks. Those unwilling to be randomized also received acupuncture for 6 weeks （non-random acupuncture group, 19 cases）. Patient syndromes and their severity were evaluated before treatment （baseline）, at the end of treatment, and at 4-week follow-up after the completion of treatment using a CM scoring system （scale of differentiation of syndromes of vascular dementia, SDSVD）. The SDSVD scores of the random and non-random acupuncture groups, and of all patients who received acupuncture （combined acupuncture group, 43 cases）, were compared with those in the guided rehabilitation group. Results： In the random, non-random, and combined acupuncture groups, SDSVD scores were significantly reduced at the end of treatment and at follow- up than at baseline. In the guided rehabilitation group, SDSVD scores were similar to baseline scores at the end of treatment and at follow-up. However, there were no significant differences in SDSVD scores among the three groups or between the combined acupuncture group and the guided rehabilitation group at any time points. In the non-random and combined acupuncture groups, SDSVD scores were significantly reduced at the end of treatment than at baseline in patients with hyperactivity of Liver （Gan）-yang or phlegm obstruction of the orifices. Conclusions： Acupuncture reduced the severity of VaD. The improvement was the greatest in patients undergoing their treatment of choice. Treatments in this study were more effective for excess syndromes, such as Liver-yang hyperactivity or phlegm obstruction of the orifices than deficiency syndromes, such as Kidney （Shen）-essence deficiency.

A Randomized Clinical Study on Optimum Proposal of Integration of Disease and Syndrome to Treat Viral Myocarditis

Objective：To determine the optimum treatment for viral myocarditis（VMC）.Methods：A total of 126 VMC patients were randomly divided into the control group（42 cases） that was treated with conventional Westem medicine,and the intervention group（84 cases） that was treated with a combination of chinese medicine（CM）and Western medicine intervention termed optimum proposal of integration of disease and syndrome（OPIDS）.Before and after 4 weeks of treatment,the integral of CM syndrome,self-rating depression and anxiety scales（SDS and SAS,respectively）,echocardiograms（ECGs）,heart rate variability and left ventricular systolic function were ***：Compared with the control group,the intervention group showed significant reductions on the SDS and SAS（P〈0.05）;improvement of premature ventricular beats,atrioventricular blocks,ST-segment abnormalities,and significant T wave changes（P〈0.05）;greater reductions in standard deviation of NN intervals（SDNN）,standard deviation for per 5 min averages NN intervals（SDANN）,and root-mean-square of successive difference of NN intervals（rMSSD）（P〈0.05）;and increases in cardiac output,stroke volume,and ejection fraction,the last of which was statistically significant（P〈0.05）.Overall,the treatment efficacy rate was significantly better（P〈0.05） in the intervention group（75.61%） compared with the control group（69.70%）.Conclusion：OPIDS is quite effective in treating VMC and improves symptoms such as anxiety and depression,left ventricular systolic dysfunction,premature ventricular contraction,and cardiac autonomic nervous system dysfunction.[Registration：chinese clinical trial center（***-TRC-00000298）]

The Qi-regulating Massotherapy for Treatment of Tonic Headache in 150 Cases

Objective: To observe the therapeutic effects of the qi-regulating massotherapy (通脉调气推拿法) for treatment of tonic headache. Methods: According to different clinical types, 150 cases of tonic headache were treated by the qi-regulating massotherapy. Evaluations were done according to the scores for physiology, behavior, intensity of pain, and the therapeutic effects. Results: After treatment, the total score in the 150 cases significantly decreased as compared with that before treatment (P<0.01). The total effective rate was 93.3%, and the effective rates for all the types were over 90%. Conclusion: The qi-regulating massotherapy is indicated for all types of tonic headache with obvious therapeutic effects.

Acupuncture Principle of Tonifying Qi and Regulating Blood,Supporting the Root and Fostering the Source on Aging and Senile Diseases

Along with the increasing life span, aging and related diseases have become a serious medical and social problem that has roused global attention. In this paper, under the guidance of traditional chinese medicine （TCM）, the author raises a theory of ＂dysfunction of Sanjiao qi activity＂ based on the studies and discussions of classical literatures on Sanjiao theory by combining knowledge in modern integrative traditional chinese and Western medicine for aging from his more than 30 years of experiences of clinical and experimental practices. The author also tries to explain the mechanisms for aging from the whole aspect of Sanjiao qi activity.
