Effects of Combined Therapy of Xuezhikang Capsule(血脂康胶囊)and Valsartan on Hypertensive Left Ventricular Hypertrophy and Heart Rate Turbulence

Objective：To observe the effect of combined therapy with Xuezhikang Capsule（血脂康胶囊,XZK） and Valsartan on left ventricular hypertrophy（LVH） and heart rate turbulence（HRT） in hypertensive patients. Methods：Ninety primary hypertensive patients with LVH were randomly assigned to three *** treatment,including aspirin,β-blockers,calcium antagonists,*** administered to all ***, Valsartan（VS,80 mg once a day） was given to the 30 patients in the VS ***（in the same dosage） and XZK（600 mg,twice a day） were given to the 32 patients in the Chinese medicine（CM） group,while none was given to the 28 patients in the control *** therapeutic course lasted for 24 *** in left ventricular mass index（LVMI） measured by cardiac ultrasonic indices,HRT parameters,including the original heart rate（TO） and slope coefficient（TS）,systolic and diastolic blood pressures（SBP and DBP）,as well as blood cholesterol level（TC） were measured before and after ***：After treatment,TO and LVMI were lowered,while TS increased in both the VS group and the CM group（P〈0.01）,but changed insignificantly in the control *** differences between the CM group and the control group were shown in terms of TO,LVMI,SBP,DBP and TS（P〈0.01）;and between the CM group and the VS group in terms of TO,LVMI and TS（P〈0.01）.Moreover,HRT parameters showed an evident correlation with LVMI（r=0.519-0.635,P〈0.01）. Conclusion：Combined therapy with XZK and Valsartan can improve hypertensive LVH and HRT parameters, and lessen the damage on the autonomous nervous system.

Epigenetic reprogramming of embryos derived from sperm frozen at-20℃

Cryopreservation of spermatozoa is a strategy that has been used to conserve the sperm of animal species and animal strains that are valuable for biomedical research. A simple method for preserving spermatozoa after application of intracytoplasmic sperm injection （ICSI） is much needed. It has been shown previously that spermatozoa frozen at -20~C can activate oocytes and support full-term embryo development. However, epigenetic reprogramming could be affected by the environment and by the in vitro manipulation of gametes. Here, we investigated embryo epigenetic reprogramming including DNA methylation and histone modification, in embryos derived from sperm preserved at -20~C without cryoprotectants. The results showed that alt- hough both fertilization and embryo developmental competence were decreased, the dynamic epigenetic reprogramming of embryos derived from frozen sperm was similar to the reprogramrning of embryos derived from fresh sperm. The results re- ported in this study indicate that sperm frozen without cryoprotectant is epigenetically safe for ICSI.

Effect of burden and origin sites of premature ventricular contractions on left ventricular function by 7-day Holter monitor

Recent studies have shown that premature ventricular contractions （PVCs） could enlarge the heart, but its risk factors are incompletely understood as a single 24-hour recording cannot reflect the true PVC burden due to day-to-day variability. Our purpose was to investigate the effect of burden and origin sites on left ventricular （LV） function in patients with PVCs by 7-day Holter electrocardiography （ecg）. From May 2012 to August 2013, 112 consecutive patients with PVCs were recruited from the authors＇ affiliated hospital. All patients received 2-dimensional transthoracic echocardiography, 12-lead routing ecg and 7-days Holter ecg. Serum N-terminal pro- brain natriuretic peptide （NT-proBNP） levels were measured. A total of 102 participants with PVCs were included in the final analysis. Origin of PVCs from the tricuspid annulus had the highest burden and NT-proBNP level. LV papillary muscle had a higher LV ejection fraction （EF） level and a lower LV end-systolic dimension （ESD） than other PVC foci （P〈0.05）. The high burden group had a higher LV end-diastolic dimension （EDD） and LVESD but lower LVEF than the other two groups （P〈0.05）. Female, older age, physical work, and history of PVCs had a significantly positive correlation with symptoms. Male, older age, physical work, and high burden were positive predictors of enlarged LVEDD, LVESD and higher serum NT-proBNP level, but lower LVEF. Seven-day dynamic ecg Holter monitor showed the true PVC burden on patients with PVCs. PVCs with a lower burden or origin from the LV papillary muscle and the fascicle were relatively benign, while PVCs with a higher burden or origin from the tricuspid annulus may lead to cardiac dysfunction.

Clinical and muscle magnetic resonance image findings in patients with late-onset multiple acyl-CoA dehydrogenase deficiency

Background:Late-onset multiple acyl-coA dehydrogenase deficiency (MADD) is an autosomal recessive inherited metabolic disorder. It is still unclear about the muscle magnetic resonance image (MRI) pattern of the distal lower limb pre- and post-treatment in patients with late-onset MADD. This study described the clinical and genetic findings in a cohort of patients with late-onset MADD, and aimed to characterize the MRI pattern of the lower ***:Clinical data were retrospectively collected from clinic centers of Peking university People's hospital between February 2014 and February 2018. Muscle biopsy, blood acylcarnitines, and urine organic acids profiles, and genetic analysis were conducted to establish the diagnosis of MADD in 25 patients. Muscle MRI of the thigh and leg were performed in all patients before treatment. Eight patients received MRI re-examinations after ***:All patients presented with muscle weakness or exercise intolerance associated with variants in the electron transfer flavoprotein dehydrogenase gene. Muscle MRI showed a sign of both edema-like change and fat infiltration selectively involving in the soleus (SO) but sparing of the gastrocnemius (GA) in the leg. Similar sign of selective involvement of the biceps femoris longus (BFL) but sparing of the semitendinosus (ST) was observed in the thigh. The sensitivity and specificity of the combination of either "SO+/GA-" sign or "BFL+/ST-" sign for the diagnosis of late-onset MADD were 80.0% and 83.5%, respectively. Logistic regression model supported the findings. Development and validation of a deep learning model to screen hypokalemia from electrocardiogram in emergency patients
