Differentiated service delivery(DsD)models for HIV often exclude children and *** that children and adolescents have lower rates of HIV diagnosis,treatment and viral load suppression,there is a need to use DsD to meet the needs of children and adolescents living with *** commentary reviews the concept of DsD,examines the application of DsD to the care of children and adolescents living with HIV,and describes national guidance on use of DsD for children and adolescents and implementation of DsD for HIV care and treatment in children and adolescents in Elizabeth Glaser Pediatric AIDs Foundation(EGPAF)-supported programmes in seven sub-saharan countries between 2017 and *** descriptions include eligibility criteria,location and frequency of care delivery,healthcare cadre delivering the care,as well as the number of EGPAF-supported facilities supporting each type of DsD model.A range of DsD models were *** facility-based models predominate,several countries support community-based *** significant uptake of various DsD models for children and adolescents,there was variable coverage within countries and variability in age criteria for each *** the recent uptake of DsD models for children and adolescents suggests feasibility,more can be done to optimise and extend the use of DsD models for children and adolescents living with *** to further DsD uptake are described and solutions *** models for children and adolescents are a critical tool that can be optimised to improve the quality of HIV care and outcomes for children and adolescents.

Histone H3.3 glycine 34 to arginine/valine(G34R/V)mutations drive deadly gliomas and show exquisite regional and temporal specificity,suggesting a developmental context permissive to their *** we show that 50%of G34R/V tumors(n=95)bear activating PDGFRA mutations that display strong selection pressure at *** considered gliomas,G34R/V tumors actually arise in GsX2/DLX-expressing interneuron progenitors,where G34R/V mutations impair neuronal *** lineage of origin may facilitate PDGFRA co-option through a chromatin loop connecting PDGFRA to GsX2 regulatory elements,promoting PDGFRA overexpression and *** the single-cell level,G34R/V tumors harbor dual neuronal/astroglial identity and lack oligodendroglial programs,actively repressed by GsX2/DLX-mediated cell fate specification.G34R/V may become dispensable for tumor maintenance,whereas mutant-PDGFRA is potently ***,our results open novel research avenues in deadly tumors.G34R/V gliomas are neuronal malignancies where interneuron progenitors are stalled in differentiation by G34R/V mutations and malignant gliogenesis is promoted by co-option of a potentially targetable pathway,PDGFRA signaling.

Context: Cardiac arrests in adults are often due to ventricular fibrillation(VF) or pulseless ventricular tachycardia(VT), which are associated with better outcomes than asystole or pulseless electrical activity(PEA). Cardiac arrests in children are typically asystole or PEA. Objective: To test the hypothesis that children have relatively fewer in-hospital cardiac arrests associated with VF or pulseless VT compared with adults and, therefore, worse survival outcomes. Design, setting, and Patients: A prospective observational study from a multicenter registry(National Registry of Cardiopulmonary Resuscitation) of cardiac arrests in 253 Us and Canadian hospitals between January 1, 2000, and March 30, 2004. A total of 36 902 adults(≥ 18 years) and 880 children(s) with pulseless cardiac arrests requiring chest compressions, defibrillation, or both were assessed. Cardiac arrests occurring in the delivery department, neonatal intensive care unit, and in the out-of-hospital setting were excluded. Main Outcome Measure: survival to hospital discharge. Results: The rate of survival to hospital discharge following pulseless cardiac arrest was higher in children than adults(27% [236/880] vs 18% [6485/36 902]; adjusted odds ratio[OR], 2.29; 95% confidence interval[CI], 1.95- 2.68). Of these survivors, 65% (154/236) of children and 73% (4737/6485) of adults had good neurological outcome. The prevalence of VF or pulseless VT as the first documented pulseless rhythm was 14% (120/880) in children and 23% (8361/36 902) in adults(OR, 0.54; 95% CI, 0.44- 0.65; Psystole was 40% (350) in children and 35% (13 024) in adults(OR, 1.20; 95% CI, 1.10- 1.40; P=.006), whereas the prevalence of PEA was 24% (213) in children and 32% (11 963) in adults(OR, 0.67; 95% CI, 0.57- 0.78; Pstment for differences in preexisting conditions, interventions in place at time of arrest, witnessed and/or monitored status, time to defibrillation of VF or pulse

Background: Adolescent girls in the United states and around the world are at a heightened risk for sexually transmitted diseases (sTDs), including human immu nodeficiency virus (HIV). Objective: To determine the efficacy of a skill-based HIV/sTD risk-reduction intervention in reducing self-reported unprotected sex ual intercourse among African American and Latino adolescent girls. Design: Rand omized controlled trial with 3-, 6-, and 12-month follow-ups. setting and Pa rticipants: sexually experienced African American and Latino adolescent girls re cruited from the adolescent medicine clinic of a children’s hospital serving a low-income inner-city community (N = 682, mean age, 15.5 years); 88.6%were re tained at the 12-month follow-up. Interventions: Three 250-minute interventio ns based on cognitive-behavioral theories and elicitation research: an informat ion-based HIV/sTD intervention provided information necessary to practice safer sex; a skillbased HIV/sTD intervention provided information and taught skills n ecessary to practice safer sex; or a health-promotion control intervention conc erned with health issues unrelated to sexual behavior. Main Outcome Measures: Pr imary outcome measure was self-reported frequency of unprotected sexual interco urse; secondary outcomes included the frequency of sexual intercoursewhile intox icated, the number of sexual partners, biologically confirmed sTDs, and theoreti cal mediator variables, including the intention to use condoms, beliefs about us ing condoms, and condom-use knowledge.
