Impact of cirrhosis-related complications on posttransplant survival in patients with acute-on-chronic liver failure

Background:Acute-on-chronic liver failure(ACLF)is a life-threatening syndrome defined as acute decompensation in patients with chronic liver *** transplantation(LT)is the most effective *** aimed to assess the impact of cirrhosis-related complications pre-LT on the posttransplant prognosis of patients with ***:This was an observational cohort study conducted between January 2018 and December *** characteristics,cirrhosis-related complications at LT and patient survival post-LT were *** liver recipients with ACLF were followed for 1 year ***:A total of 212 LT recipients with ACLF were enrolled,including 75(35.4%)patients with ACLF-1,64(30.2%)with ACLF-2,and 73(34.4%)with *** median waiting time for LT was 11(4-24)*** most prevalent cirrhosis-related complication was ascites(78.8%),followed by hepatic encephalopathy(57.1%),bacterial infections(48.1%),hepatorenal syndrome(22.2%)and gastrointestinal bleeding(11.3%).Survival analyses showed that patients with complications at LT had a significantly lower survival probability at both 3 months and 1 year after LT than those without complications(all P<0.05).A simplified model was developed by assigning one point to each complication:transplantation for ACLF with cirrhosis-related complication(TACC)*** stratification of TACC model identified 3 strata(≥4,=3,and≤2)with high,median and low risk of death after LT(P<0.001).Moreover,the TACC model showed a comparable ability for predicting the outcome post-LT to the other four prognostic models(chronic liver failure-consortium ACLF score,Chinese Group on the Study of Severe Hepatitis B-ACLF score,model for end-stage liver disease score and Child-Turcotte-Pugh score).Conclusions:The presence of cirrhosis-related complications pre-LT increases the risk of death post-LT in patients with *** TACC model based on the number of cirrhosis-related complications pre-LT could stratify posttransplant survival

Novel Coronaviruses and Astroviruses in Bats

Zoonotic transmissions of emerging pathogens from wildlife to human have shaped the history of mankind. These events have also highlighted our poor understanding of microorganisms circulated in wild animals. Coronaviruses and astroviruses,which can be found from a wide range of mammals,were recently detected in bats. Strikingly,these bat viruses are genetically highly diverse and these interesting findings might help to better understand the evolution and ecology of these viruses. The discoveries of these novel bats viruses not only suggested that bats are important hosts for these virus families,but also reiterated the role of bats as a reservoir of viruses that might pose a zoonotic threat to human health.

Identification of Immunogenic Determinants of the Spike Protein of SARS-like Coronavirus

Bat SARS-Iike coronavirus （SL-CoV） has a genome organization almost identical to that of SARS-CoV, but the N-terminus of the Spike （S） proteins, which interacts with host receptor and is a major target of neutralizing antibodies against CoVs, of the two viruses has only 63-64% sequence identity. Although there have been reports studying the overall immunogenicity of SsL, knowledge on the precise location of immunodominant determinants for SSL is still lacking. In this study, using a series of truncated expressed SsL fragments and SsL specific mouse sera, we identified two immunogenic determinants for SSL. Importantly, one of the two regions seems to be located in a region not shared by known immunogenic determinants of the SSARS. This finding will be of potential use in future monitoring of SL-CoV infection in bats and spillover animals and in development of more effective vaccine to cover broad protection against this new group of coronaviruses.

Nucleos(t)ide analogues causes HBV S gene mutations and carcinogenesis

BACKGROUND： The long-term use of nudeos（t）ide analogues causes drug resistance and mutations in the HBV reverse tran- scriptase （RT） region of the polymerase gene. The RT region overlaps the HBV surface gene （S gene） and therefore, the mutations in the RT region simultaneously modify S gene sequence. Certain mutations in the RT region bring about truncated S proteins because the corresponding changed S gene encodes a stop codon which results in the loss of a large portion of the C-terminal hydrophobic region of HBV surface protein. The rtA181T/sW172＊, rtM204I/sW196＊ and rtV191I/sW182＊ are the most frequently reported drug-resistant mutations with C-terminal truncation, these mutations have oncogenic potential. DATA SOURCES： PubMed and Web of Science were searched using terms：＂hepatitis B virus＂, ＂HBV drug resistance mutation＂＂HBV surface protein＂＂HBV truncation＂, ＂hepatocellular carcinoma＂, ＂rtA181T/sW172＊＂, ＂rtM204I/sW196＊＂, ＂rtV191I/sW182＊＂, and relevant articles published in English in the past decades were reviewed. RESULTS： The rtA181T/sW172＊ and rtV191I/sW182＊ mutants occurred more frequently than the rtM204I/sW196＊ mutant both in chronic hepatitis B patients and the HBV-related hepatocellular carcinoma tissues. Although these mutations occur naturally, nudeos（t）ide analogues therapy is the main driving force. These mutations may exist alone or coexist with other HBV mutations. All these three mutants impair the virion secretion and result in HBV surface protein retention and serum HBV DNA level reduction. These mutations possess potential carcinogenic properties. The three mutations are resistant to more than one nucleos（t）ide analogue and therefore, it is difficult to treat the patients with the truncated ***： Nucleos（t）ide analogues induce drug resistance and HBV S gene truncated mutations. These mutations have potential carcinogenesis.

Micro/nanoplastics(MNPs)are detected in human liver,and pose significant risks to human *** exposure to MNPs derived from non-biodegradable plastics can induce toxicity in mouse ***,nasal exposure to non-biodegradable plastics can cause airway dysbiosis in ***,the hepatotoxicity induced by foodborne and airborne biodegradable MNPs remains poorly *** we show the hepatotoxic effects of biodegradable polylactic acid(PLA)MNPs through multi-omics analysis of various biological samples from mice,including gut,fecal,nasal,lung,liver,and blood *** results show that both foodborne and airborne PLA MNPs compromise liver function,disrupt serum antioxidant activity,and cause liver ***,foodborne MNPs lead to gut microbial dysbiosis,metabolic alterations in the gut and serum,and liver transcriptomic *** MNPs affect nasal and lung microbiota,alter lung and serum metabolites,and disrupt liver *** gut Lachnospiraceae_NK4A136_group is a potential biomarker for foodborne PLA MNP exposure,while nasal unclassified_Muribaculaceae and lung Klebsiella are potential biomarkers for airborne PLA MNP *** relevant results suggest that foodborne PLA MNPs could affect the“gut microbiota-gutliver”axis and induce hepatoxicity,while airborne PLA MNPs could disrupt the“airway microbiota-lungliver”axis and cause *** findings have implications for diagnosing PLA MNPs-induced hepatotoxicity and managing biodegradable materials in the *** current study could be a starting point for biodegradable MNPs-induced *** research is needed to verify and inhibit the pathways that are crucial to MNPs-induced hepatotoxicity.

BACKGROUND Kaposi’s sarcoma(KS)is one of the most common cancers in human immunodeficiency virus(HIV)-positive patients and leads to a high prevalence of morbidity and *** usually appears as cutaneous or mucous *** with visceral KS are asymptomatic and clinically *** the disease advances,patients may progress from a normal condition to exhibiting severe *** SUMMARY A 27-year-old man presented with a 2-mo history of fever,bearing-down pain,and rectal *** hepatitis B virus DNA level was 2.7×107 IU/*** computed tomography(CT)indicated liver *** he was admitted to our hospital,he was diagnosed with HIV *** CD4 count was 24 cells/μ*** cavity CT suggested a thickened rectum wall accompanied by multiple enlarged lymph *** patient was initially treated as having haemorrhoidal varices with bleeding,telbivudine for anti-hepatitis B virus treatment,and antibiotics for *** half a month of treatment,the patient felt that his lower lumbus ache and bearing-down pain had not improved,and a colonoscopy was *** result revealed a rectal mass that was histologically confirmed as KS with rectal spindle cells that were positive for cluster of differentiation 117(CD117),CD34,human herpes virus 8,and *** was administered systemic chemotherapy with 36 mg/d liposomal doxorubicin six *** patient experienced no sign of lower gastrointestinal bleeding *** This case highlights the diagnosis of primary KS with lower gastrointestinal bleeding in HIV-positive patients,which means visceral KS could not be *** gold standard relies on colonoscopy and biopsy findings.

Astragaloside IV Alleviates Acute Liver Failure Induced by D-GalN/LPS by Upregulating Autophagy and Reducing Inflammation

Acute liver failure(ALF)is a life-threatening condition that manifests in an extremely serious manner and progresses *** following study investigated the protective effect of astragaloside IV(AS-IV),a traditional Chinese drug,on ALF,and its underlying mechanisms,focusing on autophagy and inflammation *** were randomly divided into a saline group,a D-galactosamine and lipopolysac-charide(D-GalN/LPS)group and an AS-IV *** analysis,immunohistochemistry,cytometric bead array,high-throughput quantitative PCR,flow cytometry and Western analysis were used to assess inflammation and liver damage 5 hours after D-GalN/LPS *** IV treatment reduced mortality by alleviating D-GalN/LPS–induced hepatic damage and decreasing inflammation(decreasing Ly6c+monocyte levels,reducing inflammatory cytokines and increasing anti-inflammatory factors)as well as upregulating ***,PCR array was used to detect expression of autophagy-related genes,which demonstrated a Log2 fold change in gene expression between the AS-IV and D-GalN/LPS groups ranging from 1.19 to-3.53,with Tnfsf10 showing the largest alteration be-tween the two *** data suggest that AS-IV may alleviate ALF by upregulating autophagy and reducing inflammation,and it may therefore be an interesting drug for alleviating ALF.

The insect cellular immune response

The innate immune system of insects is divided into humoral defenses that include the production of soluble effector molecules and cellular defenses like phagocytosis and encapsulation that are mediated by hemocytes. This review summarizes current understand- ing of the cellular immune response. Insects produce several terminally differentiated types of hemocytes that are distinguished by morphology, molecular and antigenic markers, and function. The differentiated hemocytes that circulate in larval or nymphal stage insects arise from two sources： progenitor cells produced during embryogenesis and mesodermally derived hematopoietic organs. Regulation of hematopoiesis and hemocyte differentiation also involves several different signaling pathways. Phagocytosis and encapsulation require that hemocytes first recognize a given target as foreign followed by activation of downstream signaling and effector responses. A number of humoral and cellular receptors have been identified that recognize different microbes and multicellular parasites. In turn, activation of these receptors stimulates a number of signaling pathways that regulate different hemocyte functions. Recent studies also identify hemocytes as important sources Of a number of humoral effector molecules required for killing different foreign invaders.
