BACKGROUND The self-expandable metal stent is used as a bridge to surgery in the treatment of acute malignant colorectal obstruction(AMCO). However, recent studies have shown inferior long-term outcomes and increased risk of tumor dissemination after stent placement. In addition, the optimal interval between stent placement and surgery is not clear. The aim of the current study was to present a new strategy for AMCO: stent placement followed by preoperative chemotherapy and elective *** SUMMARY Six patients were diagnosed as acute obstruction. There was one patient with descending cancer, four with sigmoid cancers and one with rectal cancer. The obstructive symptoms of these six patients were relieved within 3 d after stent placement. After receiving two cycles of preoperative chemotherapy, consisting of modified infusional fluorouracil, leucovorin and oxaliplatin [modified FOLFOX6(mFOLFOX6)], they underwent elective surgery of primary tumor resection. None of the 6 patients received colostomy or colonic lavage during surgery. There was no complication of anastomotic leak, ileus or surgical site infection after surgery. In addition, the patients had low operation time and blood loss, adequate lymph nodes harvest and fast postoperative *** The two-cycle mFOLFOX6 preoperative chemotherapy and elective surgery after stent placement is a safe and feasible strategy in the management of AMCO.

Aloin is a small-molecule drug well known for its protective actions in various models of *** brain injury(TBI)-induced cerebral edema from secondary damage caused by disruption of the blood-brain barrier(BBB) often leads to an adverse *** the role of aloin in maintaining the integrity of the BBB after TBI remains unclear,we explored the protective effects of aloin on the BBB using in vivo and in vitro TBI *** male C57 BL/6 mice underwent controlled cortical impact injury,and mouse brain capillary endothelial bEnd.3 cells underwent biaxial stretch injury,then both received aloin *** the animal experiments,we found 20 mg/kg aloin to be the optimum concentration to decrease cerebral edema,decrease disruption of the BBB,and improve neurobehavioral performance after cortical impact *** the cellular studies,the optimum concentration of 40 μg/mL aloin reduced apoptosis and reversed the loss of tight junctions by reducing the reactive oxygen species levels and changes in mitochondrial membrane potential after stretch *** mechanisms may be that aloin downregulates the phosphorylation of p38 mitogen-activated protein kinase,the activation of p65 nuclear factor-kappa B,and the ratios of B cell lymphoma(Bcl)-2-associated X protein/Bcl-2 and cleaved caspase-3/*** conclude that aloin exhibits these protective effects on the BBB after TBI through its anti-oxidative stress and anti-apoptotic properties in mouse brain capillary endothelial *** may thus be a promising therapeutic drug for TBI.

The purpose of the study was to test the biomechanical differences between a lateral wedge and a valgus knee bracing on the knee joint moment during walking in patients with early stage of medial compartment knee osteoarthritis(OA).We conducted a crossover randomized design to compare gait parameters of 32 patients(with early stage medial compartment knee OA) in three different conditions during walking:with a custom-made lateral wedge of 5(in standard shoes),with a valgus knee bracing(in standard shoes),and with control condition(in standard shoes).Both two-orthose conditions showed decreased loading patterns(knee adduction moment and knee adduction angular impulse) on the knee joint in dynamic condition(statistic probability P < 0.05).The decreased loading on OA knee in wedge condition was associated with a laterally shifted location of centre of pressure and increased ankle valgus degree and moment at the same foot(P < 0.05).It was not found significant differences in loading of the knee between these two-orthose *** results indicated that,under dynamic condition,patients wearing lateral wedge and valgus knee bracing showed changes of moments on knee *** wedge was as effective as valgus knee bracing in the treatment of early stage of knee OA.

OBJECTIVE:To investigate the efficacy of tilianin extracted from Xiangqinglan(Herba Dracocephali Moldovicae)on apoptosis of H9c2 cell after oxygenglucose deprivation/reoxygenation(OGD/R)and the ***:Tilianin was obtained from Beijing Inluck Science and Technology Development ***.,with purity≥98%.The OGD/R model was established in H9c2 *** cytometry detected the mitochondrial membrane potential,apoptosis rates,mitochondrial reactive oxygen species(ROS)and calcium ion *** dehydrogenase(SDH)activity,succinate content and levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and interleukin-1β(IL-1β)were detected with enzyme-linked immunosorbent *** blot measured protein ***:Tilianin significantly reduced the apoptotic rates,ROS levels,calcium ion concentration,succinate content,and,levels of TNF-α,IL-6 and IL-1βof OGD/R cells,while significantly increased the membrane potential and SDH activity in *** blot analysis showed that tilianin significantly up-regulated pCalmodulin-dependent protein kinaseⅡand voltagedependent anion selective channel levels in OGD/R cells,while significantly down-regulated p-protein kinase B,Bcl-2-associated X,and dynamin-related protein 1 levels related to apoptosis in the mitochondrial ***,tilianin significantly up-regulated B-cell lymphoma-2 and mitochondrial protein 2 related to the inhibition of ***,tilianin downregulated phosphorylated-apoptosis signal-regulated kinase 1,phosphorylated-p38 and C/EBP homologous protein related to endoplasmic reticulum ***:Tilianin may inhibit OGD/R-induced H9c2 cell apoptosis mediated by mitochondrial pathway and endoplasmic reticulum stress,thus protecting cardiomyocytes.

Two instances of supraventricular parasystole were presented. In both, the dominant pacemaker was located in a supraventricular focus. Such a combination has not hitherto been reported in the literature. The first case was one of chronic cor pulmonale and possible coronary atherosclerosis. The second case had rheumatic carditis with mitral stenosis and insufficiency.

AIM: To assess the efficacy and safety of a modified topical formalin irrigation method in refractory hemorrhagic chronic radiation proctitis(CRP). METHODS: Patients with CRP who did not respond to previous medical treatments and presented with grade II-III rectal bleeding according to the Common Terminology Criteria for Adverse Events were enrolled. Patients with anorectal strictures, deep ulcerations, and fistulas were excluded. All patients underwent flexible endoscopic evaluation before treatment. Patient demographics and clinical data, including primary tumor, radiotherapy and previous treatment options, were collected. Patients received topical 4% formalin irrigation in a clasp-knife position under spinal epidural anesthesia in the operating room. Remission of rectal bleeding and related complications were recorded. Defecation, remission of bleeding, and other symptoms were investigated at follow-up. Endoscopic findings in patients with rectovaginal fistulas were ***: Twenty-four patients(19 female, 5 male) with a mean age of 61.5 ± 9.5 years were enrolled. The mean time from the end of radiotherapy to the onset of bleeding was 11.1 ± 9.0 mo(range: 2-24 mo). Six patients(25.0%) were blood transfusion dependent. The median preoperative Vienna Rectoscopy Score(VRS) was 3 points. Nineteen patients(79.2%) received only one course of topical formalin irrigation, and five(20.8%) required a second course. No side effects were observed. One month after treatment, bleeding cessation was complete in five patients and obvious in14; the effectiveness rate was 79.1%(19/24). For longterm efficacy, 5/16, 1/9 and 0/6 patients complained of persistent bleeding at 1, 2 and 5 years after treatment, respectively. Three rectovaginal fistulas were found at 1 mo, 3 mo and 2 years after treatment. Univariate analysis showed associations of higher endoscopic VRS and ulceration score with risk of developing rectovaginal fistula. CONCLUSION: Modified formalin irrigation is
