AIM:To analyse the changes in magnitude and orientation of astigmatism after suture removal(SR)in keratoplasty eyes as measured by refraction,tomography,and ***:Twenty-six eyes of 25 patients after optical keratoplasty requiring SR to reduce the astigmatism during the follow-up period were prospectively *** with poor quality topography scans or if there were no sutures in the steepest semi meridian were *** distance visual acuity(CDVA),manifest refraction,corneal tomography and aberrometr y were per formed on all patients before and after ***:The mean age of the patients was 40.8±14.4 *** keratoplasty was performed in 23 eyes(89%)and deep anterior lamellar keratoplasty was done in 3 eyes(11%).There was a statistically significant reduction in the magnitude of refractive,tomographic and aberrometry astigmatism after SR(P<0.001)at 2 h after suture *** mean net reduction of the astigmatism was greater as measured by corneal tomography compared to refractive astigmatism(P<0.05).There was no statistically significant change in refractive astigmatism between 2 h and 2 mo after SR(P=0.55).Vector calculations demonstrated a greater amount of undercorrection in the tomography group and the rotational error was more towards counterclockwise *** monocular log MAR CDVA improved from 0.57 D to 0.49 D after SR(P=0.002).CONCLUSION:The net reduction in the magnitude of astigmatism after SR is greater in the tomography and aberrometry *** one episode of SR,there is no difference in the aberration profile.

Purpose: To measure the achievable perioperative aqueous concentration of the commercially available topically administered fourth generation fluoroquinolones, moxifloxacin 0.5%ophthalmic solution, and gatifloxacin 0.3%ophthalmic soluti on, and to correlate this concentration with the agents’biological efficacy in the aqueous humor of patients undergoing routine cataract surgery. Design: Prospective, randomized, parallel, double-masked, clinical trial. Participants: Fifty patients undergoing cataract extraction. Methods: Patients (n=25) were given perioperative topical moxifloxacin 0.5%or topical gati-floxacin 0.3%(n=25). One drop of antibiotic was administered every 10 minutes for 4 doses beginning 1 hour prior to surgery. Aqueous humor was sampled via paracentesis and antibiotic concentrations were determined using validated high performance liquid chromatography (HPLC) procedures. Dilution analyses were performed to determine the biological efficacy of the agents in the aqueous against Staphylococcus epidermidis, the most common cause of postcataract endophthalmitis. Main Outcome Measures: Aqueous humor antibiotic concentrations were measured using HPLC and microdilution bioassay techniques. Biological activity was measured as minimal inhibitory dil ution and minimal bactericidal dilution. Results: Aqueous humor concentrations for moxifloxacin via HPLC analysis were 1.80 (±1.21) μg/ml, whereas those for gatifloxacin were 0.48 (±0.34) μg/ml. This 3.8-fold difference in aqueous humo r antibiotic concentrations was statistically significant (P =0.00003). Similarly, the biological dilution analysis of the aqueous humor samples showed that mox ifloxacin attained an estimated activity of 2.1 μg/ml, whereas the gatifloxacin activity was approximately 0.4 μg/ml, which represented a 4.9-fold difference. Background ＂Vision 2020＂ was initiated in 1999 and many relevant activities are ongoing, but little information is available on what it has achieved and what constraints it faces. It is not yet clear if funding is the most important constraint. Nepal is one of the underdeveloped countries, but an advanced country in preventing avoidable blindness. The aim of this study was to compare the situation in China and in Nepal in prevention of avoidable blindness. Methods Data from literature review and a field visit were combined. All statistical analyses were carried out with SPSS Version 11.5. The chi-square test was used for comparisons involving categorical variables. Results The blindness rate is higher in Nepal than that in China （P〈0.05）. The average number of cataract operations performed by one ophthalmologist is much lower in China than in Nepal. The cataract surgical rate （CSR） is much lower in China than in Nepal （P〈0.001）. High cost of the surgery and inadequate qualifications of human resources were found in China. cataract surgical outcome in China compared well with international standards. Conclusions Progress towards the ＂Vision 2020＂ target in China is much slower than that in Nepal. Further attention to address this issue is urgently needed.

Risk Factors for Conjunctival Microorganism Colonization in Adults Undergoing Intraocular Surgery

Purpose: To assess prevalence of and risk factors for conjunctival colonization and types of organisms among adults undergoing elective intraocular surgery. Setting: Ha’Emek Medical Center, Afula, Israel. Methods: A prospective study conducted in the Ophthalmology Department at Ha’Emek Medical Center, Afula, Israel between May 1, 2006 and August 31, 2007. Included were adults undergoing elective intraocular surgeries. Conjunctival cultures were obtained from the lower fornix, prior to application of prophylactic decolonization treatment and were processed using routine microbiological techniques. Demographic, socioeconomic and medical data of our patient cohort were obtained from all participants. Results: Cultures were obtained from 501 patients. (Mean age 69.7 ± 12.0 years) of whom 52.1% were females. In 208 patients (40.5%) bacteria grew in conjunctival cultures, one type in 175 (34.9%) one, and two types in 28 (5.6%). In none fungi were isolated. Coagulase negative Staphylococcus was the most frequent bacteria isolated. By multivariate analysis, significant risk factors for conjunctival bacterial colonization were spring/summer seasons (OR 1.64, CI 1.15 - 2.36, P < 0.007), and showering on the day of the operation (OR 1.73, CI 1.11 - 2.69, P < 0.01). Conclusions: In addition to previously known risk factors for conjunctival microorganism colonization, the present study found showering on the morning of the operation, possibly related to bacteria on towels or in the eyelids and lashes, and time of year (spring/summer) perhaps resulting from higher temperature and humidity related to the presence of conjunctival bacteria to be significant in adults undergoing intraocular surgery.

Purpose: Non-alcoholic fatty liver (NAFL) is comorbid to obesity, metabolic syndrome and type 2 diabetes mellitus (T2DM). It is unclear whether NAFLD constitutes a direct risk factor for macrovascular disease in T2DM. This study aimed at revisiting the cardiometabolic traits linked to NADL and micro-/ macrovascular complications in a biethnic Caucasian and African cohort. Methods: Cross-sectional analysis of 568 of T2DM patients (515 Caucasians;53 Africans) in whom the presence of NAFL was identified by ultrasonography and the cardiometabolic phenotype was exhaustively characterized, including carbohydrate homeostasis, comprehensive lipidogram including Lp(a), cumulative exposure to hyperglycemia, and prevalent micro/macrovascular complications. Results: FL was present in 73% of Caucasians and 36% of Africans (p − in both groups. A marked reduction in diabetic retinopathy (DR), ocular hypertonia and cataracts were found in FL+ of the two ethnicities. In FL+ Caucasians, relative risk of DR was −38%, cataracts −35%, and ocular hypertonia −42%. In FL+ Africans, risk of overall microangiopathy was −66% and that of DR −86%. Conclusions: An inverse association was observed between NAFL and the presence of diabetic retinopathy, cataract and ocular hypertonia. Purpose To evaluate anterior and posterior segment complications and their man agement after iodine 125 plaque brachytherapy (IBT) for large uveal melanoma. De sign Retrospective nonrandomized interventional study. Participants Ninety six patients with a large uveal melanoma according to the Collaborative Ocular Melan oma Study criteria. Methods The patients underwent primary IBT (median dose to t umor apex, 87 Gy). The median tumor height and diameter were 10.7 mm (range, 4.5 -16.8) and 16.5 mm (range, 7.3-25.0) , respectively, and the median follow up time was 3.5 years (range, 0.3-10.4). Cumulative incidence analysis and compet ing risks regression were used to analyze the time to individual complications a nd to identify risk factors. Death and secondary enucleation were analyzed as co mpeting risks. Main outcome measures cataract, iris neovascularization, glaucoma , maculopathy, optic neuropathy, vitreous hemorrhage, and persistent exudative r etinal detachment (RD). Results The 5-year cumulative incidences of cataract, i ris neovascularization, and glaucoma were 69%(95%confidence interval [CI], 5 7 %-78%), 62%(95%CI, 50%-71%), and 60%(95%CI, 48%-70%), respectively. Posterior segment complications were less common. The 5-year incidences of mac ulopathy and optic neuropathy were 52%(95%CI, 35%-65%) and 46%(95%CI, 30 %-61%), and those of vitreous hemorrhage and persistent RD were 36%(95%CI, 23%-48%) and 25%(95%CI, 15%-36%), respectively. More than 80%of complic ations were diagnosed within 3 years. cataract was the earliest complication to appear. Except for cataract, the cumulative incidence of dying without developin g a particular complication was 0.24 to 0.62 times that of first developing the complication. Increasing tumor height, which correlates to increasing doses to a djacent tissues, was associated with time to cataract (P=0.017), iris neovascula rization (P=0.087), and RD (P=0.046).
