The psychiatric profile of the U.S. patient population across age groups

Introduction: As the U.S. population undergoes continuous shifts the population’s health profile changes dynamically resulting in more or less expression of certain psychiatric disorders and utilization of health-care resources. In this paper, we analyze national data on the psychiatric morbidity of American patients and their summated cost in different age groups. Methods: The latest data (2009) on the number of hospital discharges and national bill (hospital charges) linked with psychiatric disorders were extracted from the Nationwide Inpatient Sample (NIS). Results: National data shows that mood disorders are the largest diagnostic category in terms of percentage of psychiatri-crelated discharges in the 1 - 17 years age group. The proportion decreases gradually as age progresses while delirium, dementia, amnestic and other cognitive disorders increase exponentially after 65 years of age. Schizophrenia and other psychotic disorders as well as alcohol and substance-related disorders peak in the working age groups (18 - 64 years). From an economic point of view, mood disorders in the 18 - 44 age group has the highest national bill ($5.477 billion) followed by schizophrenic and other psychotic disorders in the same age group ($4.337 billion) and mood disorders in the 45 - 64 age group ($4.310 billion). On the third place come schizophrenic and other psychotic disorders in the 45 - 64 age group ($3.931 billion). Conclusion: This paper illustrates the high cost of psychiatric care in the U.S., especially the large fraction of healthcare money spent on working-age patients suffering from mood disorders. This underlines psychiatric cost-efficiency as a vital topic in the current healthcare debate.

The use of coercion to alter behavior in persons with intellectual and other developmental disabilities has a long history. Despite the work of leaders in the development of Positive Behaviour Support (PBS), the use of chemical, mechanical, physical or personal, and environmental restraints continues, resulting in traumatization, physical and emotional harm, and in some instances, death. When coercion is used, the trust between people breaks down, and the lack of trust is reinforced by the power differentials present when coercion occurs. Social justice provides an avenue for rebalancing power and regaining trust, but people affected by disabilities have historically been excluded from theories of social justice and, in some cases, do not fall within the definition of what it means to be a person in the paradigms put forth by a number of different authors. The term non-coercive has been used in an attempt to reframe behavior change theories and practices, but this term describes the absence of negative circumstances and interventions, rather than the presence of positive and affirming interactions which improve the quality of life of all stakeholders.

With the growing intelligence and popularity of mobile phones, and the trend of cellular network＇s convergence to IP based network, more and more mobile applications emerge on the market. For mission critical applications, like the electronic payment which will be discussed in this paper, the lack of trust in the underlying mobile infrastructure and secure interface to legacy systems （for this case, the banking systems） poses obstacles to their widespread presence in mobile services. Recently, the exposure of hacking of iPhone and other smart phones further emphasizes the criticality of establishing a trust platform for mobile applications. This paper analyzes the building blocks of the trusted smart phone, and proposes a framework to provide a trusted platform for mobile electronic payment. Such a proposed system may allow direct interface to the banking systems due to the banking industry recognized strong security, and hence, may enable its widespread use.

Allografts have been used in a variety of applications to augment as well as replace tissues throughout the body. A number of steps are involved in selection, harvesting, processing and testing of dermal allografts. Grafts can be obtained that are: free of antibodies to viruses and low in viral titers. Cellular material can be eliminated from the tissue and the product becomes almost exclusively a collagen fiber network. The purpose of this paper is to examine the changes in collagen structure and properties that occur during processing of decellularized dermis. The results indicate that collagen fiber swelling occurs during processing although the product preserves the native collagen banding pattern at the fibrillar structural level. Fiber swelling and decreased collagen deformability of processed dermis, may lead to stress concentration at the implant-tissue interface and up-regulation of mechanotransduction. This may lead to premature mechanical failure due to creation of a chronic inflammatory condition at the implant-tissue interface. It is suggested that all dermal allografts be oriented such that Langer’s lines of the implant match those of the host tissue, and that wound closure by suturing be done under conditions that preserve the normal tension in skin in order to minimize implant-interfacial failure.

This paper is a review of the impact on transport in the Asian region where enforced lockdowns and transport reductions have come with real human costs in the areas of economic development and access to jobs, society and services from various perspectives including urban planning and transport, economy, GHG and air pollutant emissions and gender as some of the areas of focus in the Sustainable Development Goals. Transportation systems with a high level of planning, flexible modes of transport and good communication with the public have proved more resilient than others. Furthermore, digital infrastructure has proven to be a key transport solution through this crisis. Data collection and real-time analysis have proven that their time has come to be effective in urban planning and transport management. Digital infrastructure is also driving online shopping and urban delivery services that allow people seeking to be socially distant to access daily necessities in a fast and relatively inexpensive manner. Making use of new global data sets and computing techniques can help cities and countries to plan better. Developing countries will need to improve their ability to gather relevant data from public and private sources and furthermore use the data to achieve more resilient, sustainable transportation. Achieving Sustainable Development Goals will require a reprioritization of funding and efforts to ensure that countries are building optimized infrastructure that provide transport and mobility services with minimal financial burdens including new leapfrog transit systems.

The Southern Irumide Belt(SIB)is an orogenic belt consisting of a number of lithologically varied Mesoproterozoic and Neoproterozoic terranes that were thrust upon each *** belt lies along the southwest margin of the Archaean to Proterozoic Congo Craton,and bears a Neoproterozoic tectonothermal overprint relating to the Neoproterozoic-Cambrian collision between the Congo and Kalahari *** preserves a record of about 500 million years of plate interaction along this part of the Congo *** zircon samples from the SIB were analysed for U-Pb and Lu-Hf isotopes,as well as trace element *** data are used to constrain sediment-source relationships between SIB terranes and other Gondwanan terranes such as the local Congo Craton and Irumide belt and wider afield to Madagascar(Azania)and *** correlations are then used to interpret the Mesoproterozoic to Neoproterozoic affinity of the rocks and evolution of the *** zircon samples from the Chewore-Rufunsa and Kacholola(previously referred to as Luangwa-Nyimba)terranes of the SIB yield zircon U-Pb age populations and evolvedε(Hf)(t)values that are similar to the Muva Supergroup found throughout eastern Zambia,primarily correlating with Ubendian-Usagaran(ca.2.05-1.80 Ga)phase magmatism and a cryptic basement terrane that has been suggested to underlie the Bangweulu Block and Irumide *** data suggest that the SIB was depositionally connected to the Congo Craton throughout the *** more eastern Nyimba-Sinda terrane of the SIB(previously referred to as Petauke-Sinda terrane)records detrital zircon ages andε(Hf)(t)values that correlate with ca.1.1-1.0 Ga magmatism exposed elsewhere in the SIB and Irumide *** ascribe this difference in age populations to the polyphase development of the province,where the sedimentary and volcanic rocks of the Nyimba-Sinda terrane accumulated in extensional basins that developed in the *** deposition wou

The lattice Boltzmann method(LBM)is implemented in the Particle Flow Code(PFC)as a pore-scale CFD module and coupled with the particulate discrete element assemblage in PFC using an immersed boundary *** implementation of LBM and LBM-PFC coupling is validated with the analytical solutions in a couple of hydrodynamics and fluid-particle interaction problems,i.e.,the accuracy of LBM as a CFD solver is verified by solving channel flow driven by a pressure gradient for which the closed-form solution is also derived;the accuracy of LBM-PFC coupling is validated by solving flow across a cylinder,Taylor-Couette flow,Karman vortex street,and fluid flow through a cylinder *** demonstrate potential applications of this coupling code,a perforation cavity subjected to axial fluid flush is then tested,showing that the collapse and reconstruction of sand arch in the perforation cavity can be reproduced in this coupling *** developed system is ready for exploring more complicated physical issues involved in sand production.
