Analysis of Changes of Extreme Temperature during June to August Season over tanzania

Natural and human systems are exposed and vulnerable to climate extremes, which contributes to the repercussions of climate variability and the probability of disasters. The impacts of both natural and human-caused climate variability are reflected in the reported changes in climate extremes. Particularly at the local community levels in the majority of the regions, there is currently a dearth of information regarding the distribution, dynamics, and trends of excessive temperatures among the majority of tanzanians. Over the years 1982-2022, this study examined trends in tanzania’s extreme temperature over the June to August season. Based on the distinction between absolute and percentile extreme temperatures, a total of eight ETCCDI climate indices were chosen. Mann-Kendall test was used to assess the presence of trends in extreme climatic indices and the Sen’s Slope was applied to compute the extent of the trends in temperature extremes. The study showed that in most regions, there is significant increase of warm days and nights while the significant decrease of cold days and nights was evident to most areas. Moreover, nighttime warming surpasses daytime warming in the study area. The study suggests that anthropogenic influences may contribute to the warming trend observed in extreme daily minimum and maximum temperatures globally, with tanzania potentially affected, as indicated in the current research. The overall results of this study reflect patterns observed in various regions worldwide, where warm days and nights are on the rise while cold days and nights are diminishing.

Assessment of the Spatio-Temporal Trends of Annual Extreme Temperature Indices over tanzania during the Period of 1982-2022

Extreme weather and climatic phenomena, such as heatwaves, cold waves, floods and droughts, are expected to become more common and have a significant impact on ecosystems, biodiversity, and society. Devastating disasters are mostly caused by record-breaking extreme events, which are becoming more frequent throughout the world, including tanzania. A clear global signal of an increase in warm days and nights and a decrease in cold days and nights has been observed. The present study assessed the trends of annual extreme temperature indices during the period of 1982 to 2022 from 29 meteorological stations in which the daily minimum and maximum data were obtained from NASA/POWER. The Mann-Kendall and Sen slope estimator were employed for trend analysis calculation over the study area. The analyzed data have indicated for the most parts, the country has an increase in warm days and nights, extreme warm days and nights and a decrease in cold days and nights, extreme cold days and nights. It has been disclosed that the number of warm nights and days is on the rise, with the number of warm nights trending significantly faster than the number of warm days. The percentile-based extreme temperature indices exhibited more noticeable changes than the absolute extreme temperature indices. Specifically, 66% and 97% of stations demonstrated positive increasing trends in warm days (TX90p) and nights (TN90p), respectively. Conversely, the cold indices demonstrated 41% and 97% negative decreasing trends in TX10p and TN10p, respectively. The results are seemingly consistent with the observed temperature extreme trends in various parts of the world as indicated in IPCC reports.

Assessing the Relationship between October-November-December Rainfall and Indian Ocean Dipole in Recent Decades over tanzania Following the 2011 Abrupt Change

The present study explored how the Indian Ocean Dipole (IOD) influences October-November-December (OND) rainfall over tanzania in recent decade following the 2011 abrupt change. The study spans 50 years, from 1973 to 2022. Notable abrupt changes were observed in 1976 and 2011, leading us to divide our study into two periods: 1976-2010 and 2011-2022, allowing for a close investigation into the existing relationship between OND IOD and OND rainfall and their associated large-scale atmospheric circulations. It was found that the relationship between OND IOD and OND rainfall strengthened, with the correlation changed from +0.73 during 1976-2010 to +0.81 during 2011-2022. Further investigation revealed that, during 1976-2010, areas that received above- normal rainfall during positive IOD experienced below-normal during 2011- 2022 and vice versa. The same pattern relationship was observed for negative IOD. Spatial analysis demonstrates that the percentage departure of rainfall across the region mirrors the standardized rainfall anomalies. The study highlights that the changing relationship between OND IOD and OND rainfall corresponds to the east-west shift of Walker circulation, as well as the north-south shift of Hadley circulation. Analysis of sea surface temperature (SST) indicates that both positive and negative IOD events strengthened during 2011-2022 compared to 1976-2010. Close monitoring of this relationship across different timescales could be useful for updating OND rainfall seasonal forecasts in tanzania, serving as a tool for reducing socio-economic impacts.

Accuracy and precision of dried urine spot method for the detection of Schistosoma mansoni circulating cathodic antigens in resource-limited settings

Background The World Health Organization recommends the use of Schisto point-of-care circulating cathodic anti-gens(Schisto POC-CCA)for screening of Schistosoma mansoni as it offers better sensitivity than microscopy.However,there are limitation facing the use of this method including timely availability of the test cassettes.The aim of this study was to determine the reliability of dried urine spot(DUS)method for collection of urine and detection of S.mansoni using Schisto POC-CCA cassettes in a resource-limited settings.Methods A cross-sectional study was conducted between October and November 2022 among 250 primary school children in Sengerema District,northwestern tanzania.S.mansoni CCA was detected in filter paper-based DUSs,liquid urine using DUS Schisto POC-CCA(index),and direct urine Schisto POC-CCA(comparator)methods respectively.S.mansoni eggs in stool were detected using duplicate Kato-Katz(KK)method.The measures of accuracy were com-puted and compared between the index and comparator methods.The strength of agreement between inter-raters precisions was tested using Cohen's kappa(k).Results This study revealed S.mansoni prevalence rates of 28.8%,54.0%and 50.8%by duplicate KK,direct urine Schisto POC-CCA and DUS Schisto POC-CCA methods respectively.The mean intensity of infection among infected participants Was 86.3 eggs per gram of stool(EPG)ranging from 12.0 EPG to 824.0 EPG.The sensitivity of DUS Schisto POC-CCA and direct urine Schisto POC-CCA Was 94.44%(95%CI:89.15-99.74%)and 97.22%(95%CI:93.43-100.00%)respectively.The DUS Schisto POC-CCA method had slightly higher specificity(66.85%)than direct urine Schisto POC-CCA method(63.48%).The accuracy of the DUS Schisto POC-CCA Was found to be slightly high(74.80%,95%CI:68.94-79.06%)compared to that of direct urine Schisto POC-CCA(73.20%,95%CI:67.25-78.59%).There was good agreement between two laboratory technologists who performed the DUS Schisto POC-CCA method on similar samples(k=0.80,95%CI:0.59-0.95).Conclusions The DUS Schisto POC-CCA method had comp

Recent advances in deep neural networks have shed new light on physics,engineering,and scientific computing.Reconciling the data-centered viewpoint with physical simulation is one of the research hotspots.The physicsinformedneural network(PINN)is currently the most general framework,which is more popular due to theconvenience of constructing NNs and excellent generalization ability.The automatic differentiation(AD)-basedPINN model is suitable for the homogeneous scientific problem;however,it is unclear how AD can enforce fluxcontinuity across boundaries between cells of different properties where spatial heterogeneity is represented bygrid cells with different physical properties.In this work,we propose a criss-cross physics-informed convolutionalneural network(CC-PINN)learning architecture,aiming to learn the solution of parametric PDEs with spatialheterogeneity of physical properties.To achieve the seamless enforcement of flux continuity and integration ofphysicalmeaning into CNN,a predefined 2D convolutional layer is proposed to accurately express transmissibilitybetween adjacent cells.The efficacy of the proposedmethodwas evaluated through predictions of several petroleumreservoir problems with spatial heterogeneity and compared against state-of-the-art(PINN)through numericalanalysis as a benchmark,which demonstrated the superiority of the proposed method over the PINN.

Clinical Presentation and Treatment Outcomes of Pregnancy-Related Acute Kidney Injury among Pregnant Women Admitted at the Benjamin Mkapa Hospital in tanzania

Background: Globally, PRAKI is among the leading causes of death in pregnant women. The prevalence, causes and outcome of this condition vary among countries due to differences in environmental, socioeconomic, and health delivery systems. The common causes that have been reported in several studies are PIH, Haemorrhages and Sepsis while the outcomes may be either complete renal recovery, progression to CKD and hence dialysis dependency or death. This study aimed at determining clinical presentation and treatment outcomes of Pregnancy-Related Acute Kidney Injury in Pregnant women admitted at the Benjamin Mkapa Hospital, Dodoma, tanzania. Results: Out of 4007 pregnant women who were admitted to the maternity ward 51 pregnant women were found to have PRAKI. Of those with PRAKI, 74.5% were between 21 to 25 years. The leading causes of PRAKI were PPH 12 (23.53%), Eclampsia 12 (23.53%), and pre-eclampsia 12 (23.5%). Hemodialysis therapy was provided to 22 (43.1%) patients, 15 (29.4%) individuals recovered spontaneously with medical management and 14 (27.5%) missed haemodialysis therapy due to various reasons. The mortality due to PRAKI was 17 (33.3%). Conclusion and Recommendation: Pre-eclampsia/eclampsia and post-partum haemorrhage were found to be the main causes of PRAKI. The mortality related to PRAKI is high and Hemodialysis therapy is vital help to prevent deaths for pregnant women with PRAKI. Pregnant women who develop acute kidney injury should be followed closely and a nephrologist should be consulted early. Early referral should be done by the lower level facilities for all at-risk pregnant women to a specialized multidisciplinary health facility.

Advanced Insights into Goliathus orientalis (Coleoptera: Scarabaeidae) of tanzania

Members of the Goliathus group in the order Coleoptera, family Scarabaeidae and the sub-family Cetoniidae is one among essential ecological insects worldwide with scanty documentation in tanzania. The country was among the first point of collection of the specimen by Moser (1909) but subsequent attempts to locate and collect the specimen were unsuccessful possibly due to changed habitat and behaviour of Goliathus. Much of published work about the insect are based on the morphological characteristics of collected specimen which led to designation of six major species namely;Goliathus goliatus, G. cacicus, G. regius, G. albosignatus, G. orientalis and G. usambarensis, the last two being considered to exist in tanzania. The current study reports on successful identification of habitats using biological cues and subsequently collected several Goliathus specimen that had been difficult to trace for more than a century. New insights into correct identity of the species by using a combination of morphological and molecular techniques have been set. The study has resolved that only one species, Goliathus orientalis with five sub-species exist in tanzania.

Water, Sanitation and Hygiene in Lower-Level Health Care Facilities of Dar es Salaam Region in tanzania: Status towards Achieving the Sustainable Development Goals and Way Forward

Background: Improving Water, Sanitation and Hygiene in health care settings is a critical prerequisite for achieving national health goals and Sustainable Development Goals (SDGs). The World Health Organization (WHO) has set a target for each United Nations member state to reach by 2030. Each member state is required to reach by 2022, 2025 and 2030 at least 60%, 80% and 100%, respectively of basic level of service of the five elements which are water, sanitation, hygiene, waste management and environmental cleaning. Methods: This study aimed to evaluate and document the current state of basic water, sanitation, and hygiene services in all lower-level health care facilities in the Dar es Salaam region of tanzania as of July 2022. A cross-sectional study was conducted in 99 public dispensaries in the Dar es Salaam region’s five councils: Ubungo, Kigamboni, Kinondoni and Temeke Municipalities, and Ilala City. The interviewee form and observational checklists were both digitalized using the Kobo tool software. The respondents were health care facility in-charges or nurse in-charges. Data were downloaded, validated, and imported to Stata version 15 for analysis. Results: The basic WASH level per JMP is far below the target in 2022. Each member state by 2022 is required to reach at least 60% of the basic level of service of each element. We found a low coverage of basic WASH in the 99 dispensaries included in this study. The basic WASH coverage was met in only 10 (10.1%) of the dispensaries, while the remaining 89 (89.9%) dispensaries fall on limited WASH services. Conclusion: This study revealed lower coverage of basic WASH services in dispensaries. An urgent need is required to improve the status of WASH in all the dispensaries and facilitate the provision of quality health care services, patient safety and reduce health care associated infections.

Histopathological Patterns and Early Postoperative Complications among Patients with Surgically Treated Thyroid Diseases at Bugando Medical Centre, Mwanza, tanzania

Background: Thyroid diseases are among the leading endocrine disorders affecting a large proportion of people worldwide and show geographical variation in incidence and histopathological pattern related to age, sex, dietary and environmental factors. Histopathological patterns of surgically treated thyroid diseases play an important role in early diagnosis and management of these diseases. There is, however, limited published data regarding histopathological reports on thyroid disease in our local setting. This study aimed to determine the histopathological patterns and highlight early postoperative complications among patients with surgically treated thyroid diseases at Bugando Medical Centre (BMC). Methods: This was a longitudinal study involving all patients with surgically treated thyroid diseases seen at BMC over a period of 6 months from October 2019 to March 2020. Results: A total of 84 patients were studied. Females outnumbered males by a female to male ratio of 11:1. The median age of patients was 44 [IQR, 35 - 54] years old, the youngest was 14 years old and the oldest was 76 years old. Colloid goiter was the most common non-neoplastic lesion accounting for 34 (44.7%) patients. Among the neoplastic lesions, follicular adenoma was the most commonly encountered benign pathologies (n = 16;21.1%), while papillary carcinoma was the most commonly encountered malignancy (n = 4;50%). Following thyroidectomy, 12 (14.3%) patients developed early complications, of which hemorrhage sometimes requiring blood transfusion was the leading intra/postoperative complications accounting for 4 (33.3%) patients. Other complications include temporary recurrent laryngeal nerve palsy 2 (16.7%), surgical site infection 2 (16.7%) and tracheomalacia, bronchospasm, thyroid abscess and respiratory obstruction in 1 (8.3%) patient each, respectively. Factors Affecting Cotrimoxazole Prophylactic Therapy Compliance in HIV Patients Attending a Care and Treatment Clinic at Bugando Medical Centre in Mwanza, tanzania

Introduction: Cotrimoxazole Prophylactic Therapy (CPT) compliance lowers the risk of opportunistic infections and other Acquired Immune Deficiency Syndrome (AIDS)-related diseases. The aim of this study was to examine factors that influence compliance with CPT among HIV patients in the Care and Treatment Clinic (CTC) at Bugando Medical Centre (BMC) in Mwanza, tanzania. Methods: A descriptive cross-sectional study was conducted at the BMC between April 1, 2021, and June 30, 2021. Data were collected using face-to-face interviews and a semi-structured questionnaire. Data are presented in frequency, percentages, and cross-tabulation tables. A P-value of less than 0.05 was considered statistically significant. Results: The prevalence of compliance with CPT by self-reported measurement was 158 (63.7%). Most CPT-compliant participants were more likely to have a spouse who is familiar with CPT, have a family member who is aware of their HIV status, and be aware of the benefits of CPT. The majority of participants who complied with CPT were more likely to have experienced counseling during refill, felt that the length of time spent seeing doctors for treatment was reasonable, and received accurate information from them. Conclusion: Most adult HIV patients attending CTC at BMC were reported to be in compliance with CPT. These findings suggest that improving social support and patient-provider communication may be effective strategies for improving compliance with CPT among HIV patients.
