Role of ultrasound in the diagnosis of cervical tuberculous lymphadenitis in children

Background To describe sonographic characteristics of cervical tuberculous lymphadenitis(CTBL)in children,clinical infor-mation,and sonograms of 348 lymph nodes(LNs)from 57 children with CTBL were retrospectively analyzed in this *** We retrospectively reviewed the clinical data and sonograms of 348 LNs from 31 boys and 26 girls with CTBL,who were confirmed by pathology or laboratory examination,at the Hangzhou Red Cross hospital between June 2014 and December *** age of the children ranged from 1 to 14 years(average 7.1±2.9 years).Results Night sweats,fatigue and loss of appetite were the most common clinical symptoms observed in children with *** LN involvements were *** was found in healthy children with no *** sonography,the hilus was absent or unclear in all *** short-to-long axis(S/L)ratio was 20.5,and the edges were unclear in most *** accompanying findings included necrosis(47.4%),an echogenic thin layer(36.8%),surround-ing soft-tissue edema(38.5%)。multiple intra-nodal strong echo(28.2%),sinus(22.7%)and abscess formation(6.9%).The Doppler ultrasound showed that the majority of vascularity patterns of CTBL were capsular or peripheral(3.3%).Conclusions Ultrasound is a recommended examination method for children from different age groups with cervical *** ultrasonic signs of hilus absence,S/L ratio≥0.5,unclear edge,necrosis,echogenic thin layer,strong echoes and capsular or peripheral vascularity may aid in the diagnosis of cervical tuberculous lymphadenitis.

Lactobacillus levels and prognosis of patients with acute myocardial infarction

Gadolinium Enhances Dual-energy Computed Tomography Scan of Pulmonary Artery

Objective To evaluate the feasibility of using gadopentetate dimeglumine(Gd-DTPA)for dual-energy computed tomography pulmonary angiography(CTPA).Methods Sixty-six patients were randomly divided into three groups and underwent *** A had a turbo flash scan using an iohexol injection,Group B had a turbo flash scan using Gd-DTPA,and Group C had a dual-energy scan using *** original images of Group C were linearly blended with a blending factor of 0.5 or reconstructed at 40,50,60,70,80,90,100,and 110 keV,*** groups were compared in terms of pulmonary artery CT value,image quality,and radiation *** The pulmonary artery CT values were significantly higher in Group C40keV than in Groups B and C,but lower than in Group *** was no significant difference in the image noise of Groups C40keV,B,and ***,Group A had the largest beam hardening artifacts of the superior vena cava(SVC),followed by Groups B and *** C40keV showed better vascular branching than the other three groups,among which Group B was superior to Group *** subjective score of the image quality of Groups A,B,and C showed no significant difference,but the score was significantly higher in Group C40keV than in Groups A and *** radiation dose was significantly lower in Group B than in Groups A and *** Gd-CTPA is recommended to patients who are unsuitable for receiving an iodine-based ***,a turbo flash scan could surpass a dual-energy scan without consideration for virtual monoenergetic imaging.

Propofol can Protect Against the Impairment of Learning-memory Induced by Electroconvulsive Shock via Tau Protein Hyperphosphorylation in Depressed Rats

Objective To explore the possible neurophysiologic mechanisms of propofol and N-methyl-Daspartate(NMDA) receptor antagonist against learning-memory impairment of depressed rats without olfactory bulbs. Methods Models of depressed rats without olfactory bulbs were established. For the factorial design in analysis of variance, two intervention factors were included: electroconvulsive shock groups(with and without a course of electroconvulsive shock) and drug intervention groups [intraperotoneal(ip) injection of saline, NMDA receptor antagonist MK-801 and propofol. A total of 60 adult depressed rats without olfactory bulbs were randomly divided into 6 experimental groups(n=10 per group): ip injection of 5 ml saline; ip injection of 5 ml of 10 mg/kg MK-801; ip injection of 5 ml of 10 mg/kg MK-801 and a course of electroconvulsive shock; ip injection of 5 ml of 200 mg/kg propofol; ip injection of 5 ml of 200 mg/kg propofol and a course of electroconvulsive shock; and ip injection of 5 ml saline and a course of electroconvulsive shock. The learning-memory abilities of the rats was evaluated by the Morris water maze test. The content of glutamic acid in the hippocampus was detected by high-performance liquid chromatography. The expressions of p-AT8Ser202 in the hippocampus were determined by Western blot analysis. Results Propofol, MK-801 or electroconvulsive shock alone induced learning-memory impairment in depressed rats, as proven by extended evasive latency time and shortened space probe time. Glutamic acid content in the hippocampus of depressed rats was significantly up-regulated by electroconvulsive shock and down-regulated by propofol, but MK-801 had no significant effect on glutamic acid content. Levels ofphosphorylated Tau protein p-AT8Ser202 in the hippocampus was up-regulated by electroconvulsive shock but was reduced by propofol and MK-801 alone. BACKGROUND Bedaquiline is among the prioritized drugs recommended by the World Health Organization for the treatment of extensively drug-resistant tuberculosis(XDRTB).Many patients have not achieved better clinical improvement after bedaquiline is stopped at 24 ***,there is no recommendation or guideline on bedaquiline administration beyond 24 wk,which is an important consideration when balancing the benefit of prognosis for XDR-TB against the uncertain safety concerning the newer *** SUMMARY This paper reported 2 patients with XDR-TB(a female of 58 years of age and a female of 18 years of age)who received bedaquiline for 36 wk,as local experience to be *** 2 cases had negative cultures after 24 wk of treatment,but lung imaging was still *** discussion among experts,the consensus was made to bedaquiline prolongation by another 12 *** 36-wk prolonged use of bedaquiline in both cases achieved a favorable response without increasing the risk of cardiac events or new safety *** Longer regimen,including 36-wk bedaquiline treatment,might be an option for patients with *** studies are needed to explore the effectiveness and safety of prolonged use of bedaquiline for 36 wk vs standard 24 wk in the treatment of multidrug-resistant/XDR-TB or to investigate further the biomarkers and criteria indicative for extension of bedaquline to facilitate clinical use of thisnovel drug.
