Adult split liver transplantation to treat liver cancer: a single-center retrospective study

BACKGROUND: The increasing morbidity of liver cancer has led to a growing demand for transplantation. Split liver transplantation(SLT) is a promising way to ameliorate organ shortages. However, the safety and efficacy of SLT are still controversial. The aim of this study was to assess the clinical outcome of SLT in liver cancer patients at our center. METHODS: A total of 74 patients who received liver transplantation at a tertiary hospital from March 2019 to July 2023 were retrospectively studied, of whom 37 recipients underwent SLT and 37 recipients underwent whole-graft liver transplantation(WGLT). Clinical data were analyzed and compared between patients who underwent SLT and ***: SLT and WGLT were successfully performed, with no intraoperative transplantrelated mortality. Postoperatively, no significant differences in total bilirubin(tb, P=0.266), alanine transaminase(ALT, P=0.403) and aspartate transaminase(AST, P=0.160) levels within 30 d were detected between the two groups. The transplant-related mortality rates were 8.1% in the SLT group and 5.4% in the WGLT group within 30 d of surgery(P=1.000), and 10.8% and 8.1%, respectively, at 90 d after surgery(P=1.000). There were no significant differences in overall survival(OS) and progress-free survival(PFS) between the SLT and WGLT groups(P=0.910, P=0.190). CONCLUSION: Our results show that SLT does not imply additional risks in treating liver cancer compared with WGLT.

Characterization of tb/HIV Co-Infected Patients Receiving tb Treatment at a DOTS Clinic, in a Tertiary hospital in South-Eastern Nigeria

Background: Tuberculosis (tb) is a specific infectious disease caused by mycobacterium tuberculosis while acquired immune deficiency syndrome (AIDS) is a fatal illness caused by human immunodeficiency virus (HIV). Both of them constitute the main burden of infectious public health disease in many parts of the world, particularly in resource limited countries like Nigeria. This study sets out to describe tb/HIV co-infected patients accessing care at the DOTS clinic in a tertiary hospital in South-Eastern Nigeria. Methods: This study was conducted retrospectively at the DOTS clinic of NAUTH Nnewi. A structured proforma was used to extract specific characteristics of tb/HIV co-infected patients who received tb treatment for the period of 1st January 2013 to 31st December 2013. The collected data were analyzed with SPSS version 20. Results: Ninety eight patients (40.6%) were tb/HIV co-infected, out of the two hundred and forty one patients treated for tuberculosis in the DOTS clinic during the period under review. These were the findings among the tb/HIV co-infected patients: there were more females (51%) than males (49%);the commonest age group affected was the group 30 - 39 years (34.7%);majority of the patients (91.8%) had pulmonary tb as against extrapulmonary tb (8.2%) and most of the patients had negative sputum AFB result (43.9%) as against those with positive result (36.7%). Conclusion: This study demonstrated some important characteristics of tb/HIV co-infected patients. Such knowledge if taken into consideration in both the tuberculosis control and HIV control programs will improve the outcomes of the programs.

An HIV 1/2 point of care test on sputum for screening tb/HIV coinfection in central India-Will it work?

Objective:To determine whether the OraQuick(?).HIV-1/2 Assay(OraSure Technologies, Inc.,Bethlehem,PA,USA) in sputum is a valid tool for HIV surveillance among tb patients. Methods:A cross sectional study was carried out on sputa of patients diagnosed with *** were tested for antibodies to HIV using OraQuick. HIV-1/2 Assay(OraSure Technologies,Inc.,Bethlehem,PA,USA).The results were compared with results of serum ***:Compared to serum ELISA,the OraQuick(?).HIV-1/2 Assay in sputum specimens reported 90%sensitivity(9/10)and 100%specificity(307/307),with a positive predictive value of 100%(95%CI:66.37%-100.00%)and a negative predictive value of 99.68%(95%CI:98.20%-99.99%). Conclusions:This testing method may provide a useful strategy for conducting HIV surveillance in possible co-infected tb patients at peripheral *** there is no investment on infrastructure,it may be possible for paramedical health professionals to carry out the test. particularly in areas with low HIV endemicity.

BACKGROUND Bedaquiline is among the prioritized drugs recommended by the World Health Organization for the treatment of extensively drug-resistant tuberculosis(XDRtb).Many patients have not achieved better clinical improvement after bedaquiline is stopped at 24 ***,there is no recommendation or guideline on bedaquiline administration beyond 24 wk,which is an important consideration when balancing the benefit of prognosis for XDR-tb against the uncertain safety concerning the newer *** SUMMARY This paper reported 2 patients with XDR-tb(a female of 58 years of age and a female of 18 years of age)who received bedaquiline for 36 wk,as local experience to be *** 2 cases had negative cultures after 24 wk of treatment,but lung imaging was still *** discussion among experts,the consensus was made to bedaquiline prolongation by another 12 *** 36-wk prolonged use of bedaquiline in both cases achieved a favorable response without increasing the risk of cardiac events or new safety *** Longer regimen,including 36-wk bedaquiline treatment,might be an option for patients with *** studies are needed to explore the effectiveness and safety of prolonged use of bedaquiline for 36 wk vs standard 24 wk in the treatment of multidrug-resistant/XDR-tb or to investigate further the biomarkers and criteria indicative for extension of bedaquline to facilitate clinical use of thisnovel drug.

Fully Intermittent, Thrice-Weekly Dose of Anti-Tuberculosis Drugs; Their Efficacy under Routine Program Conditions of India

When discovered, anti-tuberculosis drugs were always administered in Daily-doses; it worked wonders! Later, studies claimed these drugs could cure even in thrice-weekly doses （intermittently）. Not everyone believed that. Three Reviews （Cochrane, Thorax, Azhar） appear unconvinced! Also, most nations （including 20 High tb Burden Countries） continue to practice time-tested Daily dosing! INDIA adopted a unique low-cost, thrice-weekly regimen around 2000. During 15 years, under national program, Thrice-weekly doses have been administered to 15 million tb patients, self-claiming 85% success. To revisit long-term outcome of all registered patients in a district; identify the one, who didn＇t achieve long-lasting recovery with a single course, was later found re-registered for a second innings; his name figures twice in national registry （a Repeater）. Author heads District Tuberculosis Center, Faridabad, India; monitors thousands of current/ex-patients on Thrice-weekly regimen. By providence, he enjoys unique access to registered patients, data, district tb-workers. Two methods of data collection： 1. Author motivated district work-force; jointly conducted systematic Retrospective Record Review of hand-written registers of past 15 years, one at a time--thus identified over 1,575 Repeaters. 2. Author has been conducting ongoing Clinical Audit in his busy tb OPD for 6 years. He is able to cherry-pick （average） one Repeater per day; thus tracked 3,100 Repeaters-about half come in from Delhi＇s premier tb institutes, duly diagnosed, referred in for local registration for Re-treatment. Long-term fate of 36,785 registered tb patients could be analyzed. 4,675 patients observed to have returned sick, meriting Re-registration, Re-treatment; thus over 12.7 % registered twice/thrice （names, unique govt. ID＇s, irrefutable evidence enclosed）-an adverse outcome, unreported thus far-a bubble. Besides, study recorded 1,590 （5%） Deaths; 2,590 （8%） Defaulters. Under India＇s routine program cond
