Background: The role of Helicobacter pylori in patients with cirrhosis and increased prevalence of peptic ulcers is still poorly defined. The objective is to evaluate the effect of H pylori eradication on ulcer recurrence in patients with cirrhosis. Methods: The study was conducted at a single, tertiary, referral hospital with 1200 beds. Patients with cirrhosis and duodenal ulcers were tested for H pylori and were enrolled in the study. Patients with positive H pylori received eradication therapy. Patients with duodenal ulcers received antisecretory therapy and regular endoscopic examinations. Main outcome measurements were the recurrence of duodenal ulcers within 1 year. Results: A total of 104 patients with cirrhosis and duodenal ulcers were enrolled. Fifty- four patients (52% ) were H pylori positive, and 50 patients (48% ) were H pylori negative. Forty- four patients received antimicrobial treatment and 36 patients achieved eradication of H pylori. Recurrent duodenal ulcers within 1 year were noted in 21 of 36 patients (58% ) who achieved H pylori eradication. Recurrent duodenal ulcers also were noted in 8 of the 18 patients (44% ) who remained H pylori positive and in 24 of the patients (48% ) who were H pylori negative since their enrollment in the study (p = 0.53). The limitation was a relatively small sample size. Conclusions: The results of our study showed that the prevalence of H pylori in patients with cirrhosis and duodenal ulcers was only 52% . Eradication of H pylori in patients with cirrhosis and duodenal ulcers did not effectively reduce the recurrence of ulcers.

寻找量子色动力学(Quantum Chromodynamics,QCD)相变信号是当前重离子碰撞实验的一个基础科学目标,对理解强相互作用物质的性质、大爆炸初期的宇宙演化、超新星的爆发机制、致密星的内部结构以及双中子星并合产生的引力波等重要前沿问题都具有重要意义。当重离子碰撞中发生非连续QCD相变时,引发的强相互作用物质的密度涨落与关联可以通过轻原子核的产额观察量来探测。核子间的多体关联决定了重离子碰撞中轻核的产额。特别地,基于核子并合模型对于轻核产额的计算表明质子(p)、氘核(d)和氚核(t)的产额比值N_(t)N_(p)/Nd_(2)敏感于核子密度涨落与关联,是一个较好的寻找QCD相变信号的可观测量。进一步,基于输运模型对于重离子碰撞中的手征相变的模拟发现,当系统的相轨迹经过一阶相变区域时,轻核产额比值N_(t)N_(p)/Nd_(2)有明显增加。这些结果为利用重离子碰撞中的轻核产生寻找QCD相变提供了理论依据。

Rhegmatogenous retinal detachment followed peribulbar injection in a patient w ith preexisting sympathetic ophthalmia. Inadvertent globe perforation resulted i n subsequent rhegmatogenous retinal detachment. Scleral buckling and pars plana vitrectomy reattached the retina. The histopathology of the subretinal fibrotic tissue confirmed the diagnosis.

患SARS的医护人员中的腹泻状况调查

Background and goal: Several known coronavirus species cause a variety of dise ases, including respiratory or enteric diseases. The purpose of this study was to investigate the interesting enteric symptoms of the medical care workers who were evidently infected with SARS by means of respiratory transmission. Study: Between May 1 and June 16, 2003, we enrolled 16 medical care workers who fulfilled the definition of probable SARS. Samples used for the detection of coronavirus RNA by RT-PCR were collected from throat and rectal swabs during acute phase. S erum anti SARS IgG was checked by enzyme linked immunosorbent assays at the co nvalescent phase. Results: The incidence of watery diarrhea was 18.8%(3 of 16). The RT PCR of coronavirus was positive in three (18.8%) of 16 throat swabs an d in none (0%) of seven rectal swabs. Serum anti coronavirus IgG was positive in 13 of the 15 patients (86.7%). The mortality rate was 6.25%(1 of 16). The d iarrhea rate in our hospital was significantly lower in comparison with the 73% (55 of 75) of the Amoy Gardens outbreak in Hong Kong (P = 0.000073), and similar to the 19.6%(27 of 138) of the hospital acquired outbreak in the Prince of Wa les Hospital in Hong Kong (P= 0.798). In contrast to the high positive rate of f eces RT-PCR (97%)in Amoy Gardens, our positive rate in rectal swab RT PCT (0 %) was significantly lower (P = 0.00000002). Conclusions: Hospital acquired SA RS cases infected mainly by respiratory route less commonly presented with diarr hea. Lower intestinal viral load, when the virus spread by respiratory route, may be contributive to lower diarrhea rate and lower positive rate in rectal swab RT-PCR.

Statins are widely used in the treatment of hyperlipidemia disease,which are refractory in the municipal sewage treatment *** photocatalytic degradation of statins by the Z-scheme heterostructured photocatalyst is confirmed,but the degradation mechanism of statins needs to be further *** this study,the effects of photocatalyst dosage,solution pH and humic acid(HA)on the photocatalysis of fluvastatin by ZnIn_(2)S_(4)/Bi_(2)WO_(6) Z-scheme heterostructured photocatalyst(ZIS/BWO photocatalyst)were investigated and the degradation mechanism was *** showed that adsorption of fluvastatin was improved with the increase of photocatalyst dosage,but photoinduced desorption and light scattering resulted in the decrease of the removal of fluvastatin with high dosage.0.2 g/L of the ZIS/BWO photocatalyst was optimal dosage.65.21%of fluvastatin was removed at pH=9,because high concentration of OH^(-)was conducive to produce·*** change of pH,competition of photons and active sites,and trapping of reactive oxygen species(ROS)by carboxyl group of HA combined to inhibit the photocatalysis of fluvastatin in the presence of *** C–C,C=C and C–N bonds of fluvastatin were attacked by a variety of ROS to generate degradable intermediates that were easily mineralized to H_(2)O and CO_(2).

The ACE （angiotensin converting enzyme） inhibitors are not only drugs widely prescribed drugs in cardiovascular diseases, but also potentially therapeutic agents in dementia. Based on the findings that the ACE inhibitors could activate the c-Jun N-terminal kinase signal to increase the ACE gene expression and that the Alu element of the human ACE gene involved in regulating ACE promoter activity, we aimed to investigate whether there are different pharmacogenetic responses of ACE I/D polymorphism to the ACE inhibitors in neurons. The three reporter vectors, pACEpro（0-SEAP, p-I-ACEpro-SEAP, and p-D-ACEpro-SEAP were used to examine the transcriptional activity of the vectors responding to the lisinopril treatment using a transient-transfection method in SH-SY5Y cells. Our results showed that lisinopril increased the promoter activity of an ACE gene by 16.7%. Additionally, we found the lisinopril enhanced the ACE promoter activity of the I-form vector by 17.2%, but adversely reduced that of the D-form vector by 16.8%, as compared with the respective control without the lisinopril treatment. Firstly, our findings had proved that the UD polymorphism of ACE gene contrarily responds to the ACE inhibitors in regulating the ACE expression in neurons, which provide a novel insight suggesting genetic testing to tailor the treatment regimens in AD （Alzheimer＇s disease） patients.
