Clinical efficacy of laparoscopic cholecystectomy combined with endoscopic papillary balloon dilation in treatment of gallbladder stones with common bile duct stones: A retrospective study

BACKGROUND The incidence of cholelithiasis has been on the rise in recent years,but the choice of procedure is *** To investigate the efficacy of laparoscopic cholecystectomy(LC)combined with endoscopic papillary balloon dilation(EPBD)in patients with gallbladder stones(GS)with common bile duct stones(CBDS).METHODS The clinical data of 102 patients with GS combined with CBDS were selected for retrospective analysis and divided into either an LC+EPBD group(n=50)or an LC+endoscopic sphincterotomy(EST)group(n=52)according to surgical ***-related indexes,postoperative recovery,postoperative complications,and expression levels of inflammatory response indexes were compared between the two *** Total surgical time,stone free rate,rate of conversion to laparotomy,and successful stone extraction rate did not differ significantly between the LC+EPBD group and LC+EST *** hemorrhage,time to ambulation,and length of hospitalization in the LC+EPBD group were lower than those of the LC+EST group(P<0.05).The rate of total complications of the two groups was 9.80%and 17.65%,respectively,and the difference was not statistically *** serious complications occurred in either *** 48 h postoperatively,the expression levels of interleukin-6,tumor necrosis factor-α,high-sensitivity Creactive protein,and procalcitonin were lower in the LC+EPBD group than in the LC+EST group(P<0.05).At 3 d postoperatively,the expression levels of aspartate transaminase,alanine transaminase,and total bilirubin were lower in the LC+EPBD group than in the LC+EST group(P<0.05).CONCLUSION LC combined with EPBD and LC combined with EST are both effective procedures for the treatment of GS with CBDS,in which LC combined with EPBD is beneficial to shorten the patient’s hospitalization time,reduce the magnitude of elevated inflammatory response indexes,and promote postoperative recovery.

Effect of the extrahepatic bile duct anatomy on choledocholithiasis and its clinical significance

BACKGROUND A comprehensive understanding of the extrahepatic bile duct anatomy is vital to guide surgical procedures and perform endoscopic retrograde *** irregularities within the extrahepatic bile duct may increase susceptibility to bile duct *** To investigate the anatomical risk factors associated with extrahepatic bile ducts in patients diagnosed with choledocholithiasis,with a specific focus on preventing stone recurrence after surgical intervention and endoscopic *** We retrospectively analyzed the medical records of 124 patients without choledocholithiasis and 108 with confirmed choledocholithiasis who underwent magnetic resonance cholangiopancreatography examinations at our center between January 2022 and October *** regression analyses were conducted to identify the anatomical risk factors influencing the incidence of common bile duct *** Multivariate logistic regression analysis revealed that several factors independently contributed to choledocholithiasis *** independent risk factors for choledocholithiasis were diameter of the common hepatic[adjusted odds ratio(aOR)=1.43,95%confidence interval(CI):1.07-1.92,adjusted P value=0.016]and common bile(aOR=1.68,95%CI:1.27-2.23,adjusted P value<0.001)ducts,length of the common hepatic duct(aOR=0.92,95%CI:0.84-0.99,adjusted P value=0.034),and angle of the common bile duct(aOR=0.92,95%CI:0.89–0.95,adjusted P value<0.001).The anatomical features of the extrahepatic bile duct were directly associated with choledocholithiasis *** risk factors include an enlarged diameter of the common hepatic and bile ducts,a shorter length of the common hepatic duct,and a reduced angle of the common bile duct.

Problems with repairing gut sphincters malfunctions

Correcting a gut sphincter malfunction is a difficult *** each sphincter has two opposite functions,that of closure and opening,repairing one there is a risk of damaging the ***,widening a narrow sphincter,such as lower esophageal sphincter(LES)and anal sphincter,may cause gastroeso-phageal reflux and fecal incontinence,respectively,whereas narrowing a wide sphincter,may cause a difficult *** the corrective treatments for difficult or retrograde transit concerning LES and anal sphincter with their unwanted consequences have been analyzed and *** overcome the drawbacks of sphincter surgical repairs,researchers have devised devices capable of closing and opening the gut lumen,named artificial sphincters(ASs).Their function is based on various mechanisms,e.g.,hydraulic,magnetic,mechanical etc,operating through many complicated components,such as plastic cuffs,balloons,micro-pumps,micromotors,connecting tubes and wires,electromechanical clamps,rechargeable batteries,magnetic devices,elastic bands,***,these structures may facilitate the onset of infections and induce a local fibrotic reaction,which may cause device malfunctioning,whereas the compression of the gut wall to occlude the lumen may give rise to ischemia with erosions and other *** ASs are already being used in clinical practice,despite their considerable limits,while others are still at the research *** view of the adverse events of the ASs mentioned above,we considered applying bioengineering methods to analyze and resolve biomechanical and biological interaction problems with the aim to conceive and build efficient and safe biomimetic ASs.

There is shortage of organs,including kidneys,*** with deceased kidney transplantation,there is a significant rise in live kidney *** prevalence of prediabetes(PD),including impaired fasting glucose and impaired glucose tolerance,is on the rise across the *** teams frequently come across prediabetic kidney donors for *** are at risk of diabetes,chronic kidney disease,cardiovascular events,stroke,neuropathy,retinopathy,dementia,depression and nonalcoholic liver disease along with increased risk of all-cause ***,most of the studies done in prediabetic kidney donors are retrospective in nature and have a short follow up *** is lack of prospective long-term studies to know about the real risk of complications after ***,there are variations in recommendations from various guidelines across the globe for donations in prediabetics,leading to more confusion among *** increases the responsibility of transplant teams to take appropriate decisions in the best interest of both donors and *** review focuses on pathophysiological changes of PD in kidneys,potential complications of PD,other risk factors for development of type 2 diabetes,a review of guidelines for kidney donation,the potential role of diabetes risk score and calculator in kidney donors and the way forward for the evaluation and selection of prediabetic kidney donors.

Inhaled nitric oxide as a salvage therapy for refractory hypoxemia in the post-transplantation period of hepatopulmonary syndrome:An explorative report of three cases

Liver transplantation(LT)is the only effective treatment for hepatopulmonary syndrome(HPS).Moreover,perioperative refractory hypoxemia(pRH)is a prevalent life-threatening condition and has extremely limited treatment ***,we report three patients with HPS who experienced pRH after LT and were consecutively treated with different salvage therapies,ephedrine inhalation,intravenous use of methylene blue with nitric oxide(NO)inhalation,and NO inhalation *** results showed that unresolved severe hypoxia may induce fatal morbidity such as early biliary leakage and acute kidney *** initiation of NO inhalation,rather than ephedrine,can significantly improve oxygenation in patients with pRH and may help prevent hypoxia-related ***,based on the response to these exploratory salvage treatments,we further demonstrate the unique ventilation-perfusion mismatch pathophysiology in specific lung regions during pRH in *** propose that early inhalation of NO is an important treatment option to rescue severe hypoxia in patients with HPS during the perioperative period of LT.

BACKGROUND Extragastric lesions are typically not misdiagnosed as gastric submucosal tumor(SMT).However,we encountered two rare cases where extrinsic lesions were misdiagnosed as gastric *** SUMMARY We describe two cases of gastric SMT-like protrusions initially misdiagnosed as gastric SMTs by the abdominal contrast-enhanced computed tomography(CT)and endoscopic ultrasound(EUS).Based on the CT and EUS findings,the patients underwent gastroscopy;however,no tumor was identified after incising the gastric *** surgical exploration revealed no gastric lesions in both patients,but a mass was found in the left triangular ligament of the *** patients underwent laparoscopic tumor resection,and the postoperative diagnosis was hepatic *** During EUS procedures,scanning across different layers and at varying degrees of gastric cavity distension,coupled with meticulous image analysis,has the potential to mitigate the likelihood of such misdiagnoses.
