Laparoscopic approach to retrorectal cyst

Retrorectal cysts are rare benign lesions in the presacral space which are frequently diagnosed in middle-aged females. We report here our experience with two symptomatic female patients who were diagnosed as having a retrorectal cyst and managed using a laparoscopic approach. The two patients were misdiagnosed as having an ovarian cystic lesion after abdominal ultrasonography. Computer tomograghy (CT) scan was mandatory to establish the diagnosis. The trocar port site was the same in both patients. An additional left oophorectomy was done for a coexisting ovarian cystic lesion in one patient in the same setting. There was no postoperative morbidity or mortality and the two patients were discharged on the 5th and 6th post operative days, respectively. Our cases show that laparoscopic management of retrorectal cysts is a safe approach. It reduces surgical trauma and offers an excellent tool for perfect visualization of the deep structures in the presacral space.

Introduction: Urethral prolapse, a rare benign lesion of the female urethra, is considered unusual in children, but it is far from exceptional in girls aged 18 to 24. African origin. The diagnosis is clinical but can be overlooked or confused with other pathologies. Treatment remains controversial. The aim of this work is to study the epidemiological, clinical and therapeutic data of urethral mucosal prolapse in young girls in our department. Materials and methods: Retrospective and descriptive study on urethral mucosal prolapse in little girls identified from the hospitalization records of the Sino-Guinean Friendship hospital in Conakry. The study period was 5 years. Patients were selected randomly. Outcome assessment focused on the occurrence of recurrences and urinary incontinence. The mean follow-up duration was 24 months. The parameters studied were: Age, clinical and histological findings, therapeutic data and patient outcomes. Results: The incidence of urethral mucosal prolapse in girls in the urology department was 2.2 cases/year. The mean age of the patients was 6.7 years (with extremes of 5 months and 10 years). The age group of 7 to 10 years was the most represented, observed in 63.63% of patients. The most frequent reason for consultation was vulvar bleeding in 7 patients, or 63.63%. Malpighian hyperplasia with a congested chorion was the most observed histological type, in 4 patients. surgical treatment consisting of complete excision of the prolapsed part had given excellent results. Conclusion: Urethral prolapse is a rare pathology, observed mainly in young black girls. Its diagnosis is based mainly on clinical examination. surgical treatment gives satisfactory results, both clinically and aesthetically.

Treatment strategies for colorectal carcinoma with synchronous liver metastases: Which way to go?

AIM: To offer an up-to-date review of all availabletreatment strategies for patients with synchronous colorectal liver metastases(CLM).METHODS: A comprehensive literature search was performed to identify articles related to the management of patients with synchronous CLM. A search of the electronic databases PubMed, MEDLINE, and Google Scholar was conducted in September *** following search terms were used: synchronous colorectal liver metastases, surgery, stage Ⅳ colorectal cancer, liver-first approach, and up-front *** terms were employed in various combinations to maximize the search. Only articles written in English were included. Particular attention was devoted to studies and review articles that were published within the last six years(2009-2014). Additional searches of the cited references from primary articles were performed to further improve the review. The full texts of all relevant articles were accessed by two independent ***: Poor long-term outcomes of patients with synchronous CLM managed by a traditional treatment strategy have led to questions about the timing and sequence of possible therapeutic interventions. Thus,alternative paradigms called reverse strategies have been proposed. Presently, there are four treatment strategies available:(1) primary first approach(or traditional approach) comprises resection of the primary colorectal tumor followed by chemotherapy;subsequent liver resection is performed 3-6 mo after colorectal resection(provided that CLM are still resectable);(2) simultaneous resection of the primary colorectal tumor and CLM during a single operation presents intriguing options for a highly select group of patients, which can be associated with significant postoperative morbidity;(3) liver-first(or chemotherapy-first) approach comprises preoperative chemotherapy(3-6 cycles) followed by liver resection,adjuvant chemotherapy, and resection of the primary colorectal tumor(it is best suited for patient

Introduction: Valvular heart diseases, often linked to rheumatic heart disease, represent a major cause of cardiovascular morbidity in sub-Saharan Africa. This study reports the preliminary outcomes of surgical treatment for valvular heart diseases initiated at the Yaoundé General hospital (YGH) by an entirely local team. Materials and Methods: Between September 2022 and November 2024, 37 patients underwent surgery for valvular heart diseases at YGH. A retrospective analysis of clinical, operative, and postoperative data was conducted using R software (version 4.4.0). Results: Male patients predominated, with a sex ratio of 1.17. The median age at the time of surgery was 40 years (interquartile range [IQR]: 25.0 - 51.0). Dyspnea was the main symptom at admission, reported in 36 patients (97.3%). Rheumatic etiologies were the most frequent (25 cases, 67.57%), followed by atherosclerotic lesions (6 cases, 16.2%) and degenerative lesions (4 cases, 10.83%). Among the 37 patients, 19 (51.4%) underwent mitral valve replacement, 11 (29.7%) aortic valve replacement, 6 (16.2%) double mitral-aortic valve replacement, and 1 (2.7%) tricuspid valve replacement. Mechanical prostheses were implanted in 27 patients (73%), while 10 patients (27%) received biological prostheses. The median duration of cardiopulmonary bypass and aortic clamping was 101 minutes (IQR: 84.75 - 146.25) and 73 minutes (IQR: 55.75 - 116.25), respectively. The median duration of mechanical ventilation and ICU stay were 2 hours (IQR: 2 - 3) and 3 days (IQR: 2 - 4), respectively. The most frequent complication was postoperative anemia requiring blood transfusion in 16 patients (43.2%). Three patients (8.1%) required reoperation for bleeding. There was no in-hospital mortality. Conclusion: Valvular heart diseases treated at YGH are predominantly of rheumatic origin. Valve replacement was by far the most commonly used technique. Early outcomes are satisfactory and encouraging.
