g>Introduction:g> The volvulus of the sigmoid colon is the twist of the sigmoid handle on its mesocolic axis, achieving a low occlusion by strangulation. g>Methodology:g> The study was conducted in the surgery “A” department of the teaching hospital of point g in Bamako. The study is retrospective and descriptive, over 5 years, ranging from January 2014 to December 2018. We conducted a comprehensive recruitment of all patients operated on for sigmoid volvulus during the study period. The only criterion for inclusion was patients operated on for volvulus of the sigmoid colon in the surgery “A” department of the point g hospital and the non-inclusion criteria were all patients operated on for other sigmoid pathologies without volvulation and patients operated on for other types of occlusions. g>Result:g> We conducted an exhaustive recruitment of 55 patients operated on for sigmoid volvulus during the study period. Sigmoid volvulus accounted for 13.75% of intestinal obstructions. The average age of patients was 48.013 ± 18.042 years with extremes of 24 years and 82 years. The age group 40 - 49 was the most represented at 21.8%. The sex ratio (M/F) was 8 in favour of male sex. The duration of the disease was less than 1-day in 50.94% of patients. There were two cases of ileo-sigmoid nodes. Immediate anastomosis resection was performed in 27 patients or 49.2% of cases. The time to restore continuity when specified was between 60 - 90 days and the median incision was the most common route of recovery at 80.8% of cases. The average length of hospitalization was 9 days with extremes of 2 days and 42 days. The morbidity rate was 7.3%. Objective: We aim to analyze the surgical nursery of abdominal wall hernias in adults between two poorly fitted medical environments, the Regional hospital Center of Tsévié (RHC-T) and the Prefectural hospital Center of Kpalimé (PHC-K) in Togo. Methodology: It was a retrospective, descriptive and comparative study carried out over five years (2018-2022) focusing on abdominal wall hernias in adults. Information was gathered from the patients’clinical notes and registers on the frequency of hernias, the anatomo-clinical forms of hernias, the type of anesthesia, the hernia repair technique and the postoperative evolution. Results: Of the 1022 and 1026 operations performed, 312 and 412 were hernial repairs, representing 30.5% and 41.2% of operations at RHC-T and PHC-K respectively (p = 0.001). The inguinal hernia was the most often found in 83.7% (n = 261) at RHC-T versus 76.6% (n = 324) at PHC-K. Herniorrhaphy was the most frequently used repair method, in 93.6% (n = 292) of patients at RHC-T and in 91.3% (n = 376) at PHC-K (p = 0.11). Postoperative complications were noted in 5.1% of cases (n = 16) at RHC-T versus 3.5% of cases (n = 15) at PHC-K (p = 0.307). These complications included scrotal hematomas, surgical areas infections, and orchitis. We recorded 1.3% (n = 4) and 0.8% (n = 3) deaths at RHC-T and PHC-K respectively (p = 0.496). Conclusion: There is homogeneity in the treatment of hernia in these two hospitals in Togo.

Introduction: Stroke is a worldwide leading cause of mortality and disability, and there are substantial economic costs for post-stroke. A large hemispheric ischemic stroke is a severe form of stroke that may occur in up to 10% of stroke cases. In these cases, decompressive craniectomy (DC) is an effective strategy to reduce early case fatality. In this study, the authors present their experience in the management of large hemispheric stroke in patients who underwent decompressive craniectomy. Patients and Methods: This study is a retrospective analysis of 9 patients who underwent decompressive craniotomy for large hemispheric stroke from February 2021 to January 2024. Demographic, clinical, and radiologic variables were collected from the hospital archives. All patients underwent head CT scan or MRI showing the involvement of the majority or complete middle cerebral artery distribution area. Hemicraniectomy with duraplasty was the main surgical procedure. The mortality and the outcome using Modified Rankin Scale (mRS) were reported. Results: A total of 9 patients with large hemispheric stroke who underwent DC were reviewed retrospectively. The mean age was 53 years with extremes of 39- and 67-year-old. There were 6 males and 3 females. The main risk factors of stroke were hypertension and diabetes in 5 and 3 cases, respectively. The large craniectomy was made and the dura was opened in the stellate fashion. Duraplasty was performed using epicrane in 7 patients and fascia lata aponeurosis in 2 patients. In the postoperative course, 4 (44%) patients died in 1 month and the mortality rate was 66.6% in 6 months. Conclusion: Decompressive craniectomy is a life-saving procedure used to manage cerebral edema after stroke onset. In our study, the high mortality rate could be explained by many factors, such as patient selection criteria and timing of surgery. Background: Left ventricular size is a factor independently associated with increased mortality in mitral surgery, particularly, in the setting of ischemic mitral regurgitation (IMR). In some patients with dilated ventricles, reverse remodelling does not occur, with a high risk of residual or recurrent mitral regurgitation and therefore associated with worse outcome. We conducted multi-centers retrospective observational study on a cohort of patients who had combined CABg and mitral surgery for moderately severe IMR to correlate a relationship between perioperative left ventricular dimensions to postoperative mortality rates in both genders. Results: 287 patients had combined CABg and mitral valve surgery. The exclusion criteria were patients with very low function Conclusion: Failure of early ventricular remodeling with persistently elevated end systolic dimensions is a risk factor for postoperative death in both genders. Background: Diarrheal diseases have globally decreased over the past few decades, yet they remain one of the top three causes of mortality in children under five years, especially in sub-Saharan Africa and Nigeria. Seasonal peaks of diarrheal episodes continue to contribute significantly to childhood mortality in these regions. One of the notable causes of diarrhea in children is parasitic infections, particularly Cryptosporidium, which poses a serious health risk. In Nigeria, the burden of Cryptosporidium diarrhea is under-researched, making it imperative to investigate its prevalence and associated risk factors. Study Objectives: The study aims to determine the prevalence and risk factors associated with Cryptosporidium diarrhea among children aged five years and below in selected health institutions in Abakaliki, South-East Nigeria. Methodology: This cross-sectional study was conducted from January to May 2017, recruiting 200 children under five years with diarrhea from health institutions in Abakaliki. Fecal specimens were analyzed for Cryptosporidium oocysts using light microscopy with modified Ziehl-Neelsen staining and immunofluorescent antibody test (IFAT). Deoxyribonucleic acid (DNA) was extracted from positive samples using QIAmp® DNA stool kit, followed by Polymerase Chain Reaction (PCR) and molecular genotyping. Results: Cryptosporidium was detected in 0.5% (1/200) of children via light microscopy and 6.5% (13/200) via IFAT. All positive samples were confirmed as Cryptosporidium hominis by PCR. Objectives: Coronary artery bypass grafting (CABg) is the preferred revascularisation option for diabetics with multivessel coronary artery disease, in current guideline recommendations. Not infrequently, coronary artery disease causes minimal symptoms in diabetic patients, so they present late for surgery and this could have implications for clinical outcomes. We sought to examine differences in symptom severity between diabetics and non-diabetics at presentation for CABg, and the impact on in-hospital outcomes. Methods: We retrieved prospectively collected data for all patients who had CABg between January 2000 and December 2022. Perioperative variables were compared between diabetic and non-diabetics. The association between in-hospital major adverse cardiac and cerebrovascular events (MACCE) and perioperative variables was determined using multivariate analysis. Results: Of 10,834 patients, diabetics constituted 24.8% (n = 2687) with mean age 66.4 ± 8.7 years compared to non-diabetics 65.9 ± 9.4, p = 0.02. More diabetics (p Conclusions: Diabetics more often presented for non-elective CABg with advanced symptoms, impaired left ventricular function, prior myocardial infarction and coronary stenting.
