Multimodal abnormalities of brain structures in adolescents and young adults with major depressive disorder:An activation likelihood estimation meta-analysis

BACKGROUND Major depressive disorder(MDD)in adolescents and young adults contributes significantly to global morbidity,with inconsistent findings on brain structural changes from structural magnetic resonance imaging *** likeli-hood estimation(ALE)offers a method to synthesize these diverse findings and identify consistent brain *** We performed a comprehensive literature search in PubMed,Web of science,Embase,and Chinese National Knowledge Infrastructure databases for neuroi-maging studies on MDD among adolescents and young adults published up to November 19,*** independent researchers performed the study selection,quality assessment,and data *** ALE technique was employed to synthesize findings on localized brain function anomalies in MDD patients,which was supplemented by sensitivity *** Twenty-two studies comprising fourteen diffusion tensor imaging(DTI)studies and eight voxel-based morphome-try(VBM)studies,and involving 451 MDD patients and 465 healthy controls(HCs)for DTI and 664 MDD patients and 946 HCs for VBM,were ***-based ALE demonstrated significant reductions in fractional anisotropy(FA)values in the right caudate head,right insula,and right lentiform nucleus putamen in adolescents and young adults with MDD compared to HCs,with no regions exhibiting increased FA ***-based ALE did not demonstrate significant alterations in gray matter *** analyses highlighted consistent findings in the right caudate head(11 of 14 analyses),right insula(10 of 14 analyses),and right lentiform nucleus putamen(11 of 14 analyses).CONCLUsION structural alterations in the right caudate head,right insula,and right lentiform nucleus putamen in young MDD patients may contribute to its recurrent nature,offering insights for targeted therapies.

BACKGROUND sleep deprivation is a prevalent issue that impacts cognitive *** numerous neuroimaging studies have explored the neural correlates of sleep loss,inconsistencies persist in the reported results,necessitating an investigation into the consistent brain functional changes resulting from sleep *** To establish the consistency of brain functional alterations associated with sleep deprivation through systematic searches of neuroimaging *** metaanalytic methods,signed differential mapping(sDM)and activation likelihood estimation(ALE),were employed to analyze functional magnetic resonance imaging(fMRI)*** A systematic search performed according to PRIsMA guidelines was conducted across multiple databases through July 29,*** that met specific inclusion criteria,focused on healthy subjects with acute sleep deprivation and reported whole-brain functional data in English were considered.A total of 21 studies were selected for sDM and ALE *** Twenty-one studies,including 23 experiments and 498 subjects,were *** to pre-sleep deprivation,post-sleep deprivation brain function was associated with increased gray matter in the right corpus callosum and decreased activity in the left medial frontal gyrus and left inferior parietal *** revealed increased brain functional activity in the left striatum and right central posterior gyrus and decreased activity in the right cerebellar gyrus,left middle frontal gyrus,corpus callosum,and right *** This meta-analysis consistently identified brain regions affected by sleep deprivation,notably the left medial frontal gyrus and corpus callosum,shedding light on the neuropathology of sleep deprivation and offering insights into its neurological impact.

Thrombotic Thrombocytopenic Purpura in Pregnancy Presented with stroke at 29 Weeks: A Case Report

Thrombotic thrombocytopenic purpura (TTP) is a rare but acute, life-threatening condition which may be precipitated by pregnancy. This disorder that presents with thrombocytopenia, haemolytic anemia, and clinical consequences of microvascular thrombosis such as stroke. The exact cause is not known but it is associated with a deficiency of ADAMTs13 enzymes. Immune mediated TTP is more common and can present in pregnancy. The aim of this case is to bring awareness as many clinicians are unaware of this condition in pregnancy, its diagnosis may be missed or delayed, leading to fetal loss or serious maternal implications. In this case the patient presented at 29 weeks with stroke in Emergency department, referred to delivery suit for Obstetric review, with suspicion of Pre-eclampsia/HELLP. The diagnosis of TTP was achieved by a multidisciplinary team who worked tirelessly together. The patient was transferred to a specialist Tertiary Care Centre for further management. The pregnancy continued until 33 weeks and 5 days. she underwent an emergency caesarean section for fetal distress. steroids and Rituximab were continued postnatally. The outcome was favourable due to fast and efficient multidisciplinary care. Awareness of this rare but important condition can lead to recognition of clinical presentation, prompt diagnosis and appropriate management.

A Potential Role for GLUT4 in Predicting sepsis in Critically Ill Children

Background: This study investigated serum Glucose transporter (GLUT) 4 levels and examined the relationship between serum GLUT4 levels and sepsis in critically ill children. Methods: This was a retrospective study of 77 critically ill children and 33 non-diabetic healthy children (controls) who were admitted between 07/2015 and 05/2016. Patient data, clinical information, and blood samples were collected on admission, alongside a large number of laboratory parameters that were routinely assessed. Critically ill patients were divided into sepsis and non-sepsis/systemic inflammatory response syndrome (sIRs). serum GLUT4 was measured using western blotting and enzyme-linked immunosorbent assays. Insulin resistance indexes, clinical data, laboratory parameters, and inflammatory cytokines were assessed. Results: GLUT4 serum levels were higher in critically ill children than in healthy children (90.5 vs. 30.3 μg/L, P 0.05). Compared to healthy children, hyperglycemic patients (n = 48) had elevated GLUT4 serum levels (30.3 vs. 103.7 g/L, P Conclusions: GLUT4 serum levels might be significantly increased in critically ill children compared with healthy children, particularly those in septic shock. serum GLUT4 could predict disease severity.

Emergency Care for Children and Adolescents after sexual Violence, in Brazil’s Capital, Brasilia

Objective: sexual violence affects women of all ages but, more commonly, children and adolescents. This study aimed to identify the extent to which services are being provided at the hospitals responsible for the emergency care of children and adolescents who have suffered sexual violence and to determine whether there is any difference in the provision of such services as a function of the victim’s age. Method: This cross-sectional study investigated differences in the care received by female children and adolescents who are victims of sexual violence in a capital city, comparing the treatment given to girls under 15 with that given to adolescents of 15 to 19 years of age. The data were extracted from the state Department of Health database between 01/01/2012 and 31/12/2016. Results: Children under 15 were much less likely to receive prophylaxis against HIV, sexually transmitted infections and hepatitis B, with the number of adolescent girls treated adequately corresponding to between twice and almost three times the number of children treated prophylactically. Only 10% of girls under 15 received emergency contraception. Conclusion: The lack of standard care established by the Ministry of Health highlights the need for urgent debates both locally and probably throughout the country, as the situation may be similar in other regions of the country.

BACKGROUND Adolescent major depressive disorder(MDD)is a significant mental health concern that often leads to recurrent depression in ***-state functional magnetic resonance imaging(rs-fMRI)offers unique insights into the neural mechanisms underlying this ***,despite previous research,the specific vulnerable brain regions affected in adolescent MDD patients have not been fully *** To identify consistent vulnerable brain regions in adolescent MDD patients using rs-fMRI and activation likelihood estimation(ALE)*** We performed a comprehensive literature search through July 12,2023,for studies investigating brain functional changes in adolescent MDD *** utilized regional homogeneity(ReHo),amplitude of low-frequency fluctuations(ALFF)and fractional ALFF(fALFF)*** compared the regions of aberrant spontaneous neural activity in adolescents with MDD vs healthy controls(HCs)using *** Ten studies(369 adolescent MDD patients and 313 HCs)were *** the ReHo and ALFF/fALFF data,the results revealed that the activity in the right cuneus and left precuneus was lower in the adolescent MDD patients than in the HCs(voxel size:648 mm3,Psed *** on the ALFF data,we found decreased activity in the right cuneus and left precuneus in adolescent MDD patients(voxel size:736 mm3,Ps exhibiting increased *** Through ALE meta-analysis,we consistently identified the right cuneus and left precuneus as vulnerable brain regions in adolescent MDD patients,increasing our understanding of the neuropathology of affected adolescents.

The effect of porosity on surface wave scattering by a vertical porous barrier over a rectangular trench is studied here under the assumption of linearized theory of water *** fluid region is divided into four subregions depending on the position of the barrier and the *** the Havelock’s expansion of water wave potential in different regions along with suitable matching conditions at the interface of different regions,the problem is formulated in terms of three integral *** the edge conditions at the submerged end of the barrier and at the edges of the trench,these integral equations are solved using multi-term Galerkin approximation technique taking orthogonal Chebyshev’s polynomials and ultra-spherical Gegenbauer polynomial as its basis function and also simple polynomial as basis *** the solutions of the integral equations,the reflection coefficient,transmission coefficient,energy dissipation coefficient and horizontal wave force are determined and depicted *** was observed that the rate of convergence of the Galerkin method in computing the reflection coefficient,considering special functions as basis function is more than the simple polynomial as basis *** change of porous parameter of the barrier and variation of trench width and height significantly contribute to the change in the scattering coefficients and the hydrodynamic *** present results are likely to play a crucial role in the analysis of surface wave propagation in oceans involving porous barrier over submarine trench.

Objective:To analyze the effect of perioperative comfort care for patients with hysteromyomectomy(HYs).Methods:seventy cases of HYs patients admitted to our hospital from October 2022 to October 2023 were randomly selected and divided into Group A(control group,35 cases,conventional care)and Group B(observation group 35 cases,comfort care).The effects of the two groups before and after nursing care were ***:The self-rating anxiety(sAs)scores,exercise of self-care agency(EsCA)scores,and nursing satisfaction scores at the time of discharge of Group B were better than in Group A after nursing care(Psion:Perioperative comfort care for HYs patients improved the patient’s adverse emotions,self-care ability,and nursing satisfaction.
