Resectable left lower lobe non-small cell lung cancer with lymph node metastasis is related to unfavorable outcomes

Background:Despite numerous previous studies,the consideration of tumor location as a prognostic factor in resectable non-small cell lung cancer(NSCLC)remains *** present study analyzed the association between tumor location and clinical outcome in patients with resectable NSCLC who had undergone lobectomy with systematic lymphadenectomy and who had presented with varying nodal ***:The data from a cohort of 627 eligible patients treated in sun yat-sen university Cancer Center between January 2000 and December 2008 were retrospectively collected,and the nodal statuses of patients with different tumor locations were *** proportional hazards regression model was used to determine the independent factors related to cancer-specific survival(CSS).Results:Multivariate analysis demonstrated that left lower lobe(LLL)tumors[hazard ratio(HR):1.465,95%confidence interval(CI)1.090-1.969,P=0.011],lymph node metastasis(HR:2.742,95%CI 2.145-3.507,P4 cm(HR:1.474,95%CI 1.151-1.888,P=0.002)were three independent prognosticators in patients with resectable ***,LLL tumors were associated only with CSS in node-positive patients(HR:1.528,95%CI 1.015-2.301,P=0.042),and a tumor size of>4 cm was the only independent risk predictor in the node-negative subgroup(HR:1.889,95%CI 1.324-2.696,P<0.001).Conclusions:Tumor location is related to the long-term CSS of NSCLC patients with lymph node *** tumors may be upstaged in node-positive patients to facilitate an optimal treatment strategy.

Inversion of the paleo-geothermal gradient using low-temperature thermochronology: A case study of the Haiyuan -Liupanshan region

The paleo-geothermal gradient is a crucial parameter for converting the thermal history to the exhumation history. However， the precise estimation of this parameter has been a challenge. This paper presents a simple two-step method to model the paleo-geothermal gradient using low-temperature thermochronology. (1) It uses the Monte Carlo approach to generate thermal histories in a vertical section randomly and calculates the entire thermal history within the goodness-of-fit thresholds based on different paleo-geothermal gradients. (2) It selects the optimum paleo-geothermal gradient by comparing the entire thermal history within different goodness-of-fit thresholds. We validated the method with apatite (U-Th)/He and fission track data collected from two drill cores in the Haiyuan-Liupanshan region. The result revealed that the best-fit paleo-geothermal gradient was ～42 ℃/km during the Early Cretaceous–Miocene and has decreased rapidly to 20 ℃/km since ～10 Ma. The crust thickening in the study area may explain the rapid reduction in the paleo-geothermal gradient since ～10 Ma. Our results are consistent with earlier studies in the region， suggesting that our simple and more intuitive approach provides an alternative method for paleo-geothermal gradient modeling.

The FUT2 loss-of-function mutations are highly prevalent and are associated with inflammatory bowel disease(IBD).To investigate the impact of FUT2 loss-of-function mutation on the gut microbiota in patients with IBD,81 endoscopically confirmed IBD patients were genotyped and divided into 3 groups:homozygous for functional FUT2 genes(SeSe),with one copy of non-functional FUT2 gene(Sese),or homozygous for non-functional FUT2 genes(sese).Escherichia,which attaches to fucosylated glycoconjugates,was the only abundant genus exhibiting decreased abundance in sese *** with SeSe or Sese patients,sese patients exhibited higher abundance in CD8+inducing Alistipe and Phascolarctobacterium and Th17 inducing Erysipelotrichaceae ***-intuitively,butyrate-producing bacteria were more abundant in sese ***,metabolomics analysis found higher levels of butyrate in sese *** data support the hypothesis that FUT2 loss-of-function mutation participates in the IBD pathogenesis by decreasing binding sites for adherent bacteria and thus altering the gut *** abundances of adherent bacteria may allow the overgrowth of bacteria that induce inflammatory T cells,leading to intestinal *** FUT2 loss-of-function mutations are highly prevalent,the identification of T cell inducing bacteria in sese patients could be valuable for the development of personalized microbial intervention for IBD.

Comparison of breast-conserving surgery and mastectomy in early breast cancer using observational data revisited:a propensity score-matched analysis

Recent observational studies showed that breast-conserving surgery(BCS) resulted in superior survival compared to mastectomy in breast cancer patients. This study compared the clinical outcomes of BCS and mastectomy using propensity score(PS)matching analysis, which had advantages over conventional methods in reducing bias. Nonmetastatic breast cancer patients who underwent BCS and mastectomy were matched 1:1 based on their PS. We used the Kaplan-Meier method and Cox-regression model to estimate the treatment effects. A total of 2,866 patients with a median follow-up time of 67 months were included in the original study population. Although the mastectomy cohort(N=1,219) had more advanced disease compared to the BCS cohort(N=1,647), LRFS was similar between the two groups(93.8% vs. 92.4%, P>0.05). BCS(vs. mastectomy) was associated with improved DFS(73.8% vs. 58.7%, P0.05) or CSS(87.5% vs. 84.9%, P>0.05). We found that PS methods reduce bias when estimating treatment effects using observational data. BCS and mastectomy show equivalent outcomes in nonmetastatic breast cancer patients.

Discrimination of Pb-Zn deposit types using sphalerite geochemistry: New insights from machine learning algorithm

Due to the combined influences such as ore-forming temperature,fluid and metal sources,sphalerite tends to incorporate diverse contents of trace elements during the formation of different types of Lead-zinc(Pb-Zn)***,trace elements in sphalerite have long been utilized to distinguish Pb-Zn deposit ***,previous discriminant diagrams usually contain two or three dimensions,which are limited to revealing the complicated interrelations between trace elements of sphalerite and the types of Pb-Zn *** this study,we aim to prove that the sphalerite trace elements can be used to classify the Pb-Zn deposit types and extract key factors from sphalerite trace elements that can dis-criminate Pb-Zn deposit types using machine learning algorithms.A dataset of nearly 3600 sphalerite spot analyses from 95 Pb-Zn deposits worldwide determined by LA-ICP-MS was compiled from peer-reviewed publications,containing 12 elements(Mn,Fe,Co,Cu,Ga,Ge,Ag,Cd,In,Sn,Sb,and Pb)from 5 types,including Sedimentary Exhalative(SEDEX),Mississippi Valley Type(MVT),Volcanic Massive Sulfide(VMS),skarn,and epithermal *** Forests(RF)is applied to the data processing and the results show that trace elements of sphalerite can successfully discriminate different types of Pb-Zn deposits except for VMS deposits,most of which are falsely distinguished as skarn and epithermal *** further discriminate VMS deposits,future studies could focus on enlarging the capacity of VMS deposits in datasets and applying other geological factors along with sphalerite trace elements when con-structing the classification ***’s feature importance and permutation feature importance were adopted to evaluate the element significance for ***,a visualized tool,t-distributed stochastic neighbor embedding(t-SNE),was used to verify the results of both classification and *** results presented here show that Mn,Co,and Ge display significant impacts on classifica

A nomogram to predict prognosis for gastric cancer with peritoneal dissemination

Objective： To identify independent prognostic factors to be included in a nomogram to predict the prognosis ofgastric cancer patients with peritoneal ***： This is a retrospective study on 684 patients with a histological diagnosis of gastric cancer withperitoneal dissemination from the sun yat-sen university Cancer Center as the development set, and 62 gastriccancer patients from the Sixth Affiliated Hospital of sun yat-sen university as the validation group. Chi-square testand Cox regression analysis were used to compare the clinicopathological variables and the prognosis of gastriccancer patients with peritoneal dissemination. The Harrell＇s concordance index （C-index） and calibration curvewere determined for comparisons of predictive ability of the ***： Univariate and multivariate analyses showed that serum carcinoembryonic antigen （CEA） level（P=0.032）, ascites grading （P=0.008）, presence of extraperitoneal metastasis （P〈0.001）, seeding status （P=0.016） andperformance status （P=0.009） were independent prognostic factors for gastric cancer patients with peritonealdissemination in the development set. The nomogram model was constructed using these five factors. Internalvalidation showed that the C-index of the model was 0.641. For the external validation, the C-index of this modelwas ***： We developed and validated a nomogram to predict the prognosis for gastric cancer patients withperitoneal dissemination. This nomogram may play an important clinical role in guiding palliative therapy for thesetypes of patients, although it may need more data for optimization.

Serosa-penetration in human T4aN0M0 gastric carcinoma correlates with worse prognosis after D2 gastrectomy

Background In both the seventh edition of the International Union Against Cancer （UICC） staging system for gastric cancer and the 14th edition of the Japanese Gastric Cancer Association （JGCA） system, T4a is defined as ＂Tumor perforates serosa （visceral peritoneum） without invasion of adjacent structures.＂ The aim of this study was to investigate the differences in prognosis between patients with serosa-penetrating and serosa-invading T4aN0M0 gastric carcinomas. Methods data were collected from 221 patients with T4aNoMo gastric carcinoma who underwent D2 resection at our cancer center between January 1990 and December 2008. The cohort included 42 patients with serosa-penetrating tumors and 179 patients with serosa-invading tumors. The average follow-up time was 85.5 months. Spearman＇s rank correlation, Kaplan-Meier plots, and Cox proportional hazards regression models were used to analyze the data. Results The 5-year survival rate of patients with serosa-penetrating and serosa-invading gastric cancers were 31% and 62% respectively （P 〈0.05）. The relapse rates after D2 radical surgery were 16.2% in patients with serosa-invading gastric cancer versus 59.5% in those with serosa-penetrating tumors （P 〈0.05）. Peritoneal dissemination and distant organ/lymph node metastatic rates were 76.0% and 24.0% respectively in patients with penetrating tumors, versus 44.8% and 55.2% respectively in patients with invading tumors （P 〈0.05）. Multivariate analysis showed that penetration of the serosa was an independent prognostic indicator of overall survival. Conclusions Our findings provide a basis for the concept that serosa-penetrating TaaNoMo gastric carcinoma represents a more aggressive cancer than serosa-invading T4aN0M0 gastric carcinoma. Serosa penetration is an independent factor for poor prognosis of patients with gastric carcinoma.

Tropical storms(TSs)can induce sea surface cooling,freshening,and phytoplankton *** dissolved oxygen(DO)concentration response to TSs within an anticyclone is still unclear due to the rarity of in situ *** this study,we investi-gate the variations in DO concentration attributed to TS‘Haitang’within an anticyclonic eddy in the northern South China Sea based on Chinese underwater glider *** concentrations have a higher value at the edge of eddy than at the *** by TS,DO concentrations decrease remarkably in the subsurface layer in all three regions(inside,edge,and outside of the anticyclonic eddy).The mean DO concentrations decrease more at the edge of the anticyclone than those inside the *** recovery time of DO concentration after TS is around one week at the edge of the anticyclone and is>10 days within the *** observations show that the DO concentrations decrease above the subsurface chlorophyll a maxima *** analysis shows that variations in DO concentration are dominated by horizontal advection and vertical advection terms in the subsurface layer.
