Defects in phospholipase C zeta cause polyspermy and low fertilization after conventional IVF:not just ICSI failure

Phospholipase C zeta(PLC)is a key sperm-borne oocyte-activating factor that triggers Ca^(2+)oscillations and the subsequent block to polyspermy following gamete *** in PLCZ1,the gene encoding PLCζ,cause male infertility and intracytoplasmic sperm injection(ICSI)fertilization failure;and PLCζ expression and localization patterns are significantly correlated with ICSI fertilization rate(FR).However,in conventional in vitro fertilization(cIVF),whether and how sperm PLCζ affects fertilization remain ***,we identified one previously reported and two novel PLCZ1 mutations associated with polyspermy in vitro that are characterized by excessive sperm-zona binding and a delay in pronuclei(PN)*** staining and oocyte activation testing revealed that virtually all spermatozoa from patients lacked functional PLCζ and were thus unable to evoke Ca^(2+) *** with an artificial oocyte activation treatment successfully rescued the polyspermic phenotype and resulted in a live ***,we analyzed PLCζ in an additional 58 males after cIVF treatment in the Reproductive and Genetic Hospital of CiTiC-Xiangya(Changsha,china)between February 2019 and January *** found that the proportion of spermatozoa that expressed PLCζ was positively correlated with both 2PN rate and total *** optimal cutoff value below which males were likely to experience low FR(total FR≤30%)after clVF was 56.7%for the proportion of spermatozoa expressing *** study expands the mutation and the phenotypic spectrum of PLCZ1 and further suggests that PLCζ constitutes a promising biomarker for identifying low FRs cases in cIVF due to sperm-related oocyte activation deficiency and that sperm PLCζ analysis may benefit the widermale population and not onlymen with IcsI failure.

The high mortality rate in hepatocellular carcinoma(HCC)is partially due to the fact that a significant number of patients are diagnosed at an intermediate or advanced stage,with surgical treatment options *** therapy,which involves both locoregional and systemic treatments,has the potential to downstage tumors in selected patients with initially unresectable HCC,thereby making surgical treatment a possibility and potentially increasing long-term *** optimize the conversion rate,it is necessary to maximize successful conversions and clearly define the target population for conversion treatment through a collaborative *** this review article,we summarize the clinical experience and evidence for conversion therapy in patients with‘potentially resectable’HCC from four perspectives:1)defining the target population for conversion therapy,2)selecting the appropriate conversion strategy,placing emphasis on the utilization of combination therapy that exhibits a significant objective response rate,3)determining the timing and urgency of surgical resection,4)promoting the adoption of a multidisciplinary team *** authors are optimistic that with the continuous progress in treatment and a deeper understanding of HCC,the success rate of HCC conversion therapy will increase,and the overall survival of HCC patients will be prolonged.

Increased retinal venule diameter as a prognostic indicator for recurrent cerebrovascular events:a prospective observational study

Serum amylase as a novel prognostic marker of organophosphorus poisoning:a retrospective study

Background:This study aims to explore the significance of serum amylase levels in patients with organophosphorus pesticide(OP)poisoning and to provide a new perspective for evaluating the severity and prognosis of OP ***:The clinical data of 67 patients with acute OP poisoning,who had been treated in the emergency department of the General Hospital of Ningxia Medical University between September 2020 and August 2022,were retrospectively *** Spearman rank correlation,serum amylase levels during admission and severity of poisoning correlated with discharge *** on serum amylase levels,receiver operating characteristic curves were plotted to predict the severity and mortality of patients who experience organophosphate *** optimal threshold serum amylase level at admission was determined based on the maximum Youden ***:Using Spearman rank correlation analysis,serum amylase levels at admission positively correlated with the severity of poisoning and discharge outcomes,but the degree of correlation was weak(r_(s):0.344;0.264;P194.1 U/L at admission was 15.944(95%CI:1.825-139.274).Conclusion:Serum amylase levels in patients with organophosphate poisoning correlate with the degree of poisoning and discharge *** serum amylase level was a risk factor for organophosphorus poisoning death.

Intussusception is relatively rare in adults but common in children.[1,2]An untreated intussusception often causes intestinal obstruction,intestinal perforation,peritonitis,and shock.[3]Diabetic ketoacidosis(DKA)is the most common acute hyperglycemic emergency in uncontrolled type 1 diabetes(T1DM).[4]More than 60%of DKA patients experience gastrointestinal symptoms such as nausea,vomiting,and abdominal pain,[5,6]which sometimes lead to the neglect of other serious underlying *** delayed gastric emptying,intestinal obstruction,electrolyte imbalance,and metabolic acidosis are associated with DKA,the underlying mechanism has not been fully elucidated.[7]This study aimed to report a DKA patient complicated with intussusception.

Mindfulness Facets and Psychological Well-Being among Meditators:Serenity as a Mediating Process

Guided by the theoretical processes of mindfulness and psychological well-being,this study examined serenity as a mediator between mindfulness facets and psychological well-being,as indexed by depressive symptoms and life *** were 133 mindfulness practitioners who took part in a 3-day transnational meditation event in Hong *** informed consent,participants completed a self-report *** findings from structural equation modeling showed that serenity mediated the relation between two facets of mindfulness,including describing and nonreacting to inner experience,and life *** also mediated the relation between the mindfulness facet of describing and depressive *** associations were indicated between two mindfulness facets,including observing and nonjudging of inner experience,and depressive *** together,the findings revealed mindfulness facets as major correlates of serenity and psychological outcomes among Chinese meditation *** foster psychological well-being,researchers,educators,and practitioners should pay attention to the role of serenity,describing,and nonreacting to inner experience in mental health.

AIM:To explore the factors influencing individuals’willingness to participate in ophthalmic clinical ***:A questionnaire survey was conducted from January to April 2021 among patients and their family members at Zhongshan Ophthalmic Center,Sun Yat-sen University,in Guangzhou,*** survey gathered data on respondents’willingness,demographic and socioeconomic profiles,as well as their reasons and concerns regarding engagement in clinical ***:Of the 1078 residents surveyed(mean age 31.2±13.1y;65.8%females)in Guangzhou,749(69.5%)expressed a willingness to participate in future ophthalmic clinical *** characteristics associated with greater willingness included a younger age,lower annual income,higher education,prior participation experience,previous ophthalmic treatment,and a better understanding of clinical *** the exception of age,these characteristics were significantly linked to a higher *** primary barrier to participation,expressed by 64.8%of those willing and 54.4%of those unwilling,was“Uncertain efficacy”.In terms of motivations,the willing group ranked“Better therapeutic benefits”(35.0%),“Professional monitoring”(34.3%),and“Trust in healthcare professionals”(33.1%)as their top three reasons,whereas the unwilling participants indicated“Full comprehension of the protocol”(46.2%)as the key ***:This study reveals a substantial willingness to participate in ophthalmic clinical trials and demonstrates the predictive role of demographic and socioeconomic *** in motivators and concerns between willing and unwilling participants highlight the significance of tailored recruitment ***,the need for and trust in healthcare professionals stand out as powerful motivations,underscoring the importance of enhancing physician-patient relationships,adopting patient-centered communication approaches,and addressing individualized needs to improve accrual rates.
