Resistance of Helicobacter pylori to furazolidone and levofloxacin: A viewpoint

In their review, Arslan et al[1] did not describe the status of Helicobacter pylori(H. pylori) treatment with furazolidone and the resistance to this antibiotic. We have presented different surveys showing the resistance of H. pylori to furazolidone from Asia and South America. The resistance rates varied but were mostly low(< 5%). There are not enough data on its efficacy and resistance in the United States and Europe. H. pylori mutations occurring in the oor D gene, including A041 G, A122 G, C349A(G), A78 G, A112 G, A335 G, C156 T and C165 T, and in the por D gene, including G353 A, A356 G, C357 T, C347 T, C347 G and C346 A, have been indicated to be possibly related to the observed resistance. Additionally, to complete Arslan et al 's statement regarding levofloxacin resistance, it should be noted that compound mutations of N87 A, A88 N and V65 I at codon Asn-87 were recently observed in the gyr A gene for the first time. However, the results on these topics are not sufficient, and more worldwide studies are suggested.

Introduction:Rare cases of autoimmune hemolytic anemia(AHA)associated with using ciprofloxacin or hydrochlorothiazide(HCTZ)alone have been reported before.However,simultaneous use of both drugs could lead to a severe clinical condition.This study presents the combination of acute kidney injury(AKI)and AHA following the simultaneous use of ciprofloxacin and HCTZ for three days.Case Description:A 42-year-old Iranian woman presented to the emergency department with symptoms of fatigue,lethargy,nausea and vomiting,ataxia,oliguria,dark urine,and jaundice.The patient reported using HCTZ due to high blood pressure and ciprofloxacin for a urinary tract infection three days before presentation.Early laboratory findings revealed hemolytic anemia with a hemoglobin of 7 g/dl,the strongly positive direct and indirect Coombs test,high level of lactate dehydrogenase(820 IU/L),and hyperbilirubinemia(total:3 mg/dL and direct:1.2 mg/dL).Furthermore,hyperkalemia(5.2 mEq/L),hyperphosphatemia(6.2 mg/dL),high levels of BUN(100 mg/dL),and creatinine rise(6.8 mg/dL)were found.Urine analysis showed 2+blood,4-6 red blood cells,and cola-colored urine.Based on the findings,druginduced AHA,followed by AKI,was diagnosed.Following,the drugs were stopped and steroid therapy was initiated.The patient underwent four sessions of hemodialysis to improve the AKI.Conclusion:Healthcare providers should be aware of the life-threatening adverse effects of commonly used drugs such as ciprofloxacin or HCTZ.The timely diagnosis of the offending drugs leads to avoiding the persistence of the risk factor and the deterioration of the patient's clinical condition.

Rationale:Anthrax is a zoonotic disease caused by spores of Gram-positive Bacillus anthracis,commonly affects mammals and in rare cases birds.Human infection occurs accidentally through direct or indirect exposure to animal or their products.Patient concerns:A 63-year-old man was referred to our hospital with flu-like symptoms and severe swelling and redness on the face,the roof of the mouth,and nostrils.He had a history of direct contact with a slaughtered mutton two days ago.He declared controlled diabetes,hypertension,hypertriglyceridemia,and heart failure.Lungs were normal in lung high resolution CT,but multiple lymphadenopathies were seen in the mediastinum.Bilateral axillary lymphadenopathy with a maximum sad of 23 mm and pleural effusion on the right side was observed.CT scan of the nose and sinuses showed an increased density of polyps in the left maxillary sinus.Slides were prepared from the patient's lesions and examined under a light microscope.Bacillus shape with Streptococcus bacteria was seen.Diagnosis:Anthrax co-infection with herpes systemic virus and Streptococcus pyogenes.Interventions:Multidrug therapy started with appropriate antibiotics.Outcomes:The symptoms of the patient gradually disappeared.The patient was discharged without any complications.Lessons:Cutaneous anthrax in endemic areas in patients with skin presentations and a history of contact with infected animals or products should be considered a differential diagnosis.This is more important in mixed infections where the main cause of the problem may be hidden.

Zataria multiflora Boiss is a perennial plant with a wide spectrum of biological and pharmacological activities including antidiabetic,antinociceptive,anti-asthmatic,anti-fever,anti-spastic,anti-oxidative,anti-inflammatory,and antimicrobial properties.This paper reviews the therapeutic effects of Zataria multiflora based on recent reports.The relevant reports were extracted by checking the electronic databases including PubMed,Scopus,Web of Science,and Google Scholar from the beginning of 2010 until the end of May 2023.The neuroprotective effects of Zataria multiflora can be attributed to inhibition of acetylcholinesterase,enhancement of brain-derived neurotrophic factor,and alleviation of brain oxidative damage.Zataria multiflora also exerts its protective effects on the respiratory system,liver,and kidney by reducing the level of inflammatory cytokines,scavenging the free radicals,and augmenting the antioxidant enzymes.Additionally,Zataria multiflora accelerates wound healing via upregulating transforming growth factor-β,insulin-like growth factor 1,fibroblast growth factor 2,and vascular endothelial growth factor,and inducing angiogenesis and collagen biosynthesis.Overall,the protective impacts of Zataria multiflora on different organs are mainly attributed to its antioxidant and antiinflammatory properties.

Effect of quercetin on secretion and gene expression of leptin in breast cancer

OBJECTIVE: To investigate the possible inhibitory action of pure quercetin on secretion and gene expression of leptin in the T47 D breast cancer cell line.METHODS: In this experimental study, T47 D cells were cultured in monolayers in RPMI 1640. IC50 was determined by MTT(3-[4,5-dimethylthiazol-2-yl]-2,5-diphenyltetrazolium bromide) assay after 24 h treatment at different concentrations of quercetin.The levels of leptin gene expression were measured by reverse-transcription real-time polymerase chain reaction(PCR). Secreted leptin was measured in the supernatant of cells by an enzyme-linked immuno sorbent assay.RESULTS: Analysis of MTT assay data showed that quercetin has a cytotoxic effect on T47 D breast cancer cells with 40 μM IC50 after 24 h exposure. Dataanalysis of real-time PCR showed that with increases in quercetin concentration, a decreasing trend was seen in m RNA levels of leptin of treated cells compared with the control cells(P < 0.05). Also,measurement of secreted leptin in the culture media showed a similar decreasing trend in the amount of leptin protein in the treated cells compared with the control cells(P < 0.05).CONCLUSION: Quercetin significantly inhibits the growth of T47 D cells through inhibition of leptin secretion and gene expression in T47 D breast cancer cells. Therefore, it might be an alternative approach to breast cancer therapy through leptin targeting.

BACKGROUND It has been said that the number of orthopaedic claims has increased in the last few years. Investigation through the most prevalent cause would help to prevent further cases.AIM To review the cases of medical complaints in orthopedic patients who had been involved in a traumatic accident.METHODS A retrospective multi-center review of trauma orthopaedic-related malpractice lawsuits from 2010 to 2021 was conducted utilizing the regional medicolegal database. Defendant and plaintiff characteristics along with fracture location, allegations, and litigation outcomes were investigated.RESULTS A total of 228 claims referred to trauma-related conditions with a mean age of 31.29 ± 12.56 were enrolled. The most common injuries were at hand, thigh, elbow, and forearm, respectively.Likewise, the most common alleged complication was related to malunion or nonunion. In 47% of the cases, the main problem that led to the complaint was the inappropriate or insufficient explanation to the patient, and in 53%, there was a problem in the surgery. Eventually, 76% of the complaints resulted in a defense verdict, and 24% resulted in a plaintiff verdict.CONCLUSION Surgical treatment of hand injuries and surgery in non-educational hospitals received the most complaints. The majority of litigation outcomes were caused by a physician’s failure to fully explain and educate the traumatic orthopedic patients and technological errors.

Tissue engineering has yet to reach its ideal goal,i.***.creating profitable off-theshelf tissues and organs,designing scaffolds and three-dimensional tissue architectures that can maintain the blood supply,proper biomaterial selection,and identifying the most efficient cell source for use in cell therapy and tissue engineering.These are still the major challenges in this field.Regarding the identification of the most appropriate cell source,aging as a factor that affects both somatic and stem cells and limits their function and applications is a preventable and,at least to some extents,a reversible phenomenon.Here,we reviewed different stem cell types,namely embryonic stem cells,adult stem cells,induced pluripotent stem cells,and genetically modified stem cells,as well as their sources,i.***.autologous,allogeneic,and xenogeneic sources.Afterward,we approached aging by discussing the functional decline of aged stem cells and different intrinsic and extrinsic factors that are involved in stem cell aging including replicative senescence and Hayflick limit,autophagy,epigenetic changes,miRNAs,mTOR and AMPK pathways,and the role of mitochondria in stem cell senescence.Finally,various interventions for rejuvenation and geroprotection of stem cells are discussed.These interventions can be applied in cell therapy and tissue engineering methods to conquer aging as a limiting factor,both in original cell source and in the in vitro proliferated cells.

COVID-19 vaccination,although is a promising tool to overcome the pandemic,has side effects.There are in-creasing reports of oral lesions after COVID-19 vaccination.The aim of this review is to identify the occurrence of some oral lesions after COVID-19 vaccination,and highlight the underlying immune mechanisms involved.A narrative literature review was performed by searching electronic databases including PubMed,Scopus and Web of Science to investigate the oral lesions after COVID-19 vaccination.The inclusion criteria were original stud-ies,including the case reports,case series,letter to the editor,and cross-sectional studies.The exclusion criteria included the studies which examined the oral lesions caused by COVID-19 infection.The information,including the number of participant(s)receiving vaccine,type of vaccine,dose number,side effect(s),time of onset fol-lowing vaccination,healing time,treatment strategies for the existing lesions,and related mechanisms were then summarized in a data extraction sheet.The results of this review showed that some vaccines had side effects with oral involvement such as pemphigus vulgaris,bullous pemphigoid,herpes zoster,lichen planus,Stevens-Johnson syndrome and Behçet’s disease.Future research needs to elucidate the physiopathology of oral manifestations after the COVID-19 vaccination,and better understand the risk factors associated with such responses.Sometimes vaccine’s side effects may be due to the nocebo effect,which means that the person expects some adverse events to occur following the vaccine administration.
