A literature review of cost-effectiveness of intravenous recombinant tissue plasminogen activator for treating acute ischaemic stroke

Background Intravenous recombinant tissue plasminogen activator(IV rtPA)is recommended treatment for patients with acute ischaemic stroke,but the cost-effectiveness of IV rtPA within different time windows after the onset of acute ischaemic stroke is not well *** To conduct a literature review of the cost-effectiveness studies about IV rtPA by treatment *** of review A literature search was conducted using MEDLINE,EMBASE,CINAHL and Cochrane Library,with the keywords acute ischemic stroke,tissue plasminogen activator,cost,economic benefit,saving and incremental cost-effectiveness *** review is limited to original research articles published during 1995-2016 in English-language peer-reviewed *** found 16 studies meeting our criteria for this *** of them were cost-effectiveness studies of IV rtPA treatment within 0-3 hours after stroke onset,2 studies within 3-4.5 hours,3 studies within 0-4.5 hours and 2 studies within 0-6 *** rtPA is a cost-saving or a cost-effectiveness strategy from most of the study *** one study showed incremental cost-effectiveness ratio of IV rtPA within 1 year was marginally above US$50000 per quality-adjusted life year *** rtPA within 0-3 hours after stroke led to cost savings for lifetime or 30 years and IV rtPA within 3-4.5 hours after stroke increased costs but still was *** The literature generally showed that IV rtPA was a dominant or a cost-effective strategy compared with traditional treatment for patients with acute ischaemic stroke without IV *** findings from the literature lacked generalisability because of limited data and various assumptions.

Understanding cost data collection tools to improve economic evaluations of health interventions

Background:Immune checkpoint inhibitors(ICIs)are effective in a subset of patients with metastatic solid ***,the patients who would benefit most from ICIs in biliary tract cancer(BTC)are still *** and methods:We molecularly characterized tissues and blood from 32 patients with metastatic BTC treated with the ICI pembrolizumab as second-line ***:All patients had microsatellite stable(MSS)type *** of the 32 patients achieved partial response(PR),with an objective response rate(ORR)of 9.4%(95%confidence interval[CI],2.0–25.2)and nine showed stable disease(SD),exhibiting a disease control rate(DCR)of 37.5%(95%CI,21.1–56.3).For the 31 patients who had access to PD-1 ligand 1(PD-L1)combined positive score(CPS)testing(cut-off value≥1%),the ORR was not different between those who had PD-L1-positive(PD-L1+;1/11,9.1%)and PDL1-(2/20,10.0%)tumors(p=1.000).The tumor mutational burden(TMB)of PD-L1+BTC was comparable to that of PD-L1-BTC(p=0.630).TMB and any exonic somatic mutations were also not predictive of pembrolizumab *** analysis of blood and tumor samples demonstrated a relatively high natural killer(NK)cell proportion in the peripheral blood before pembrolizumab treatment in patients who achieved tumor ***,the tumors of these patients presented high enrichment scores for NK cells,antitumor cytokines,and Th1 signatures,and a low enrichment score for cancer-associated ***:This study shows the molecular characteristics associated with the efficacy of pembrolizumab in BTC of the MSS type.

Life-Cycle Bearing Capacity for Pre-Stressed T-beams Based on Full-Scale Destructive Test

To investigate the evolution of load-bearing characteristics of pre-stressed beams throughout their service life and to provide a basis for accurately assessing the actual working state of damaged pre-stressed concrete T-beams,destructive tests were conducted on full-scale pre-stressed concrete *** on the measurement and ana-lysis of beam deﬂection,strain,and crack development under various loading levels during the research tests,combined with the veriﬁcation coefﬁcient indicators speciﬁed in the codes,the veriﬁcation coefﬁcients of bridges at different stages of damage can be *** results indicate that the T-beams experience complete,incom-plete linear,and non-linear stages during the destructive test *** the complete linear elastic stage,both the deﬂection and bottom strain veriﬁcation coefﬁcients comply with the speciﬁcations,indicating a good structural load-bearing capacity no longer adheres to the code’s *** the non-linear stage,both coefﬁcients exhi-bit a sharp increase,resulting in a further decrease in the structure’s load-bearing *** to the pro-visions of the current code,the beam can be in the incomplete linear stage when both values fall within the code’s speciﬁed *** strain veriﬁcation coefﬁcient sourced from the compression zone at the bottom of theﬂange is not recommended for assessing the bridge’s load-bearing capacity.
