Flashlight contamination and effectiveness of two disinfectants in a stomatology hospital

Aim: This study aims to assess the infection risks of flashlight contamination in a stomatology hospital and compare the disinfection effectiveness of alcohol (75%) and disinfecting ***: The flashlight is a basic non-critical medical device in oral and maxillofacial surgery wards. Wounds are mostly found in oral cavities;therefore, reusable flashlights may be a potential source of nosocomial infections (NIs). However, the microbial flora present in flashlights used in hospitals has not yet been explored. Methods: This study investigated the microbial contamination of 41 flashlights used in a stomatology hospital in Guangzhou in March 2016. Results: Results indicated that 75.6%(31/41) of the flashlights had microbial contamination. Gram-positive bacteria accounted for 72.7%(24/33)of the microbial groups contaminating the flashlights, and Gram-negative bacteria (21.2%, 7/33), and fungi (6.1%, 2/33) constituted the remaining contaminants. The predominantly isolated species was Staphyloccus (66.7%, 22/33), especially Staphylococcus aureus (24.2%, 8/33). Approximately 77.3%(17/22) of the types of bacteria detected in the hands were same as those in the corresponding flashlights. Both the bacterial overstandard and S. aureus detection rates of doctors' flashlights were higher than those of nurses' flashlights (16/17 vs. 14/23, 7/17 vs. 1/23, respectively) (P 0.05). Conclusion: Flashlights are potential causes of NIs. Disinfecting flashlights could be an effective and practical infection control method.

Analysis of Screening Results of Four Common Infectious Diseases in One stomatology hospital

Objective: To understand the status and characteristics of hepatitis B, hepatitis C, syphilis and HIV antibody screening in one dental hospital, and to provide scientific basis for the prevention and treatment of common infectious diseases in dental medical institutions. Methods: Collect all samples of patients screened for hepatitis B, hepatitis C, syphilis and HIV from one stomatological hospital in 2018-2019, and make statistics on the distribution of common infectious diseases among patients in stomatology, such as age, gender, type of treatment, department of treatment, etc. Results: The positive rate of hepatitis B was 2.27%, 39.42% in males and 60.58% in females. It was mainly detected by outpatient department, accounting for 87.5%. The age was concentrated in 30 - 60 years old, accounting for 77.88%. It was mainly distributed in oral and maxillofacial surgery, orthodontics and implant, accounting for 82.69%. The positive rate of hepatitis C was 0.1%, 33.33% in men and 66.67% in women. It was mainly detected by outpatient department, accounting for 83.33%. The age is concentrated in 20 - 50 years old, accounting for 100%. It is mainly distributed in oral and maxillofacial surgery, orthodontics and implant, accounting for 100%. The positive rate of syphilis was 0.18%, 33.33% for men and 66.67% for women. It was mainly detected by outpatient department, accounting for 83.33%. The age is concentrated in 30 - 50 years old, accounting for 100%. It was mainly distributed in oral and maxillofacial surgery, orthodontics and implant, accounting for 83.33%. HIV was not detected. Conclusion: The positive detection rate of common infectious diseases in the stomatological hospitals is generally lower than the average level. It is mainly distributed in the department of implant, oral and maxillofacial surgery and orthodontics. The highest positive detection rate of common infectious diseases is hepatitis B which is 2.27%, and the lowest is HIV which is 0.

Assessment of Two Years of Free Surgical Treatment of Cleft Lip, Palate and Alveolar (CLPA) in the Maxillofacial Surgery and stomatology Department of the hospital University of Treichville-Abidjan

Introduction: Cleft lip, palate and alveolar (CLPA) are congenital malformations of the face due to a defect in the fusion of embryonic buds during the first weeks of embryogenesis. These malformations affect the upper lip, the alveolar bone and the palate. The incidence in Africa ranges from 1/2000 to 1/500 births. Their multidisciplinary management is long and costly. Thus, the help provided by humanitarian organisations during free care campaigns is welcome. Materials and Methods: This is a retrospective descriptive study conducted from August 2014 to July 2016 in the Maxillofacial Surgery and stomatology Department of the Treichville University hospital in Abidjan, Côte d’Ivoire. The objective was to describe the epidemiological, clinical and therapeutic aspects of CLPA during a humanitarian campaign for free care. Results: 51 cases of CLPA were operated on. Males were involved in 54.9% of the cases, i.e. a sex ratio of 1.2. The average age of the patients at the time of the operation was 3.44 years with extremes of 3 months and 52 years. Patients with low socioeconomic status represented 84.3% of the cases. Cleft lips (31.4%) and cleft palates (33.33%) predominated. For cleft lips, unilateral forms were the most frequent (73.5%) and the left side was most often affected (59.2%). The most common surgical techniques used were MILLARD cheiloplasty for cleft lips (79.36%) and Dorrance pushback for cleft palates (78.05%). The postoperative course was simple in the majority of cases (80.47%). Patients and/or parents were satisfied with the postoperative results in over 90% of cases. Discussion: Cleft lip and palate are common. Their management by humanitarian missions through mass campaigns allows us to receive a large number of patients affected by this pathology who are treated with a high satisfaction rate. Treatment and Sequelae of Panfacial Fractures in the Maxillofacial Surgery and stomatology Department of the hospital University of Treichville-Abidjan (Cote d’Ivoire)

Introduction: The treatment of panfacial fractures is complex and constitutes a challenge for the maxillofacial surgeon, who, despite therapeutic progress, frequently suffers functional and aesthetic sequelae. This study aimed to describe the treatment and evaluate the functional and aesthetic sequelae of panfacial fractures in the stomatology and maxillofacial surgery department of the Treichville University hospital. Materials and Methods: We conducted a retrospective study over a 6 years in the stomatology and maxillofacial surgery department of Treichville University hospital. Forty-two patients with panfacial fractures were included in the study. Results: Forty-two patients were registered. The average time to osteosynthesis was 12.4 days. Nasotracheal intubation was used most often (88%), and in the majority of cases, mixed osteosynthesis combining a screwed plate and steel wire was performed (64.29%). The “Bottom-up and Outside-in” surgical sequence was the most commonly used (64.29%). All patients had at least one functional and/or cosmetic sequela after treatment. Functional sequelae were dominated by occlusal problems and aesthetic sequelae by nasal deformities. Discussion: Panfacial fractures are characterised by their complexity, presenting maxillo-facial surgeons with a therapeutic and evolutionary challenge. The quality of the initial, often multidisciplinary, management of panfacial fractures is an essential factor in both functional and aesthetic prognosis. Conclusion: The treatment of panfacial fractures, even if well managed, is sometimes a source of sequelae, requiring often complex secondary management.

Tumours of salivary glands in the stomatology and maxillo-facial surgery unit at the university hospital centre —Yalgado Ouédraogo

The aim of this study was to contribute to strengthening the efficiency in the treatment of the salivary gland tumours in the stomatology and Maxillo-facial surgery service at the University hospital Centre/Yalgado OUEDRAOGO of Ouagadougou. It has been a retrospective and descriptive study covering seven (7) years [January 2006-December 2012] on epidemiology and the treatment of 54 cases of the salivary glands tumours histologically confirmed. Benign tumours (non-malignant tumours were prevalent (61.11% of cases). The annual incidences were 4.71 cases for non malignant tumours and 3 cases for malignant tumours. The average age was 34-45 years for non malignant tumours and 44-33 years for malignant ones. In both groups, the tumour was discovered lately with an average 32-48 months before consulting in case of non malignant tumours and 18-29 months for malignant tumours. The location of non malignant tumours was very often the parotid (42.42%), under mandible gland (27.28%). Malignant tumours very often concerned the parotid (76.18%) and the under mandible gland (14.29%). The histology has revealed that pleomorphic adenoma was the leading type of non-malignant tumours (93.94% of cases) whereas carcinoma was predominant in malignant tumours (80.95% of cases). Long term track keeping was difficult and it has been complicated by the loss of contact with some patients. The low living standard, the time wasted before consulting, limited human resources and therapeutic means constitute handicaps to the treatment in our African context.

Introduction: Epulis is a pseudotumor very common in women. If neglected, it can reach a considerable volume. The marginal gingiva is very involved in the tumor either in isolation or with periodontal and alveolar bone. The epulis sits classically on the vestibular gum, rarely on the palatal side. We report a case of palatal giant epulis which posed a diagnostic problem. Case report: This was a 50-year-old patient with a large tumor occupying the entire oral cavity and pushing all nearby organs. This lesion had been evolving for ten years, with a tendency to gradually increase of its volume. The orthopantomogram revealed the destruction of the underlying alveolar bone without signs of tumor invasion. Surgical excision was performed, removing all the tumor in the vicinity of the nasopalatine vascular pedicle. The pathological examination concludes with an inflammatory epulis. Conclusion: The epulis is a benign tumor of the gum, circumscribed, localized preferentially at the neck of one or two contiguous teeth. It occurs more in women, with a predilection for the vestibular side, rarely the palatal side. In the absence of appropriate treatment it can lead to a large tumor, posing a diagnostic problem and causing functional or aesthetic discomfort. Complete surgical excision with histological examination remains the solution. Healing without sequelae is the rule.

Carboxymethyl chitosan as a polyampholyte mediating intrafibrillar mineralization of collagen via collagen/ACP self-assembly

The significant role of the polyelectrolytic nature of non-collagenous proteins (NCPs) in regulating the in vivo mineralization of collagen provides important insights for scientists searching for analogues of NCPs to achieve in vitro collagen mineralization. Polyampholyte carboxymethyl chitosan (CMC) has both carboxyl and amino groups, which allows it to act as a cationic or anionic polyelectrolyte below or above its isoelectric point (IP), respectively. In this study, CMC was employed as the analogue of NCPs to stabilize amorphous calcium phosphate (ACP) under acidic conditions (pH < 3.5) via the formation of CMC/ACP nanocomplexes. In the presence of both ACP nanoparticles and acid collagen molecules, ACP nanoparticles could be integrated into collagen fibrils during the process of collagen self-assembly and achieve intrafibrillar mineralization of collagen in vitro (i.e., synchronous self-assembly/mineralization (SSM) of collagen). This mode of mineralization is different from established mechanisms in which mineralization follows the self-assembly (MFS) of collagen. Thus, SSM provides a new strategy for developing materials from mineralized collagen scaffolds.

Hydrogel scaffolds have numerous potential applications in the tissue engineering ***,tough hydrogel scaffolds implanted in vivo are seldom reported because it is difficult to balance biocompatibility and high mechanical *** by Chinese ramen,we propose a universal fabricating method(printing-P,training-T,cross-linking-C,PTC&PCT)for tough hydrogel scaffolds to fill this ***,3D printing fabricates a hydrogel scaffold with desired structures(P).Then,the scaffold could have extraordinarily high mechanical properties and functional surface structure by cycle mechanical training with salting-out assistance(T).Finally,the training results are fixed by photo-cross-linking processing(C).The tough gelatin hydrogel scaffolds exhibit excellent tensile strength of 6.66 MPa(622-fold untreated)and have excellent ***,this scaffold possesses functional surface structures from nanometer to micron to millimeter,which can efficiently induce directional cell ***,this strategy can produce bionic human tissue with mechanical properties of 10 kPa-10 MPa by changing the type of salt,and many hydrogels,such as gelatin and silk,could be improved with PTC or PCT *** experiments show that this scaffold can effectively promote the new generation of muscle fibers,blood vessels,and nerves within 4 weeks,prompting the rapid regeneration of large-volume muscle loss injuries.

Fabrication and characterization of selective laser melting printed Ti–6Al–4V alloys subjected to heat treatment for customized implants design

Selective laser melting（SLM） is a promising technique capable of rapidly fabricating customized implants having desired macro- and micro-structures by using computer-aided design models. However, the SLM-based products often have non-equilibrium microstructures and partial surface defects because of the steep thermal gradients and high solidification rates that occur during the laser melting. To meet clinical requirements, a heat treatment was used to tailor the physiochemical properties, homogenize the metallic microstructures, and eliminate surface defects, expecting to improve the cytocompatibility in vitro. Compared with the as-printed Ti–6Al–4V substrate, the heat-treated substrate had a more hydrophilic, rougher and more homogeneous surface,which should promote the early cell attachment, proliferation and osseointegration. More importantly, a crystalline rutile TiO;layer formed during the heat treatment, which should greatly promote the biocompatibility and corrosion resistance of the implant. Compared to the untreated surfaces, the adhesion and proliferation of human bone mesenchymal stem cells（hBMSCs） on heat-treated substrates were significantly enhanced, implying an excellent cytocompatibility after annealing. Therefore, these findings provide an alternative to biofunctionalized SLM-based Ti–6Al–4V implants with optimized physiochemical properties and biocompatibility for orthopedic and dental applications.
