Background:The establishment of preoperative chemotherapy(PCT)with FOLFIRINOX and gemcitabine/nab-paclitaxel in recent years has enabled resectability in many patients with initially locally advanced pancreatic cancer(LAPC).Nevertheless,information about the impact of PCT on surgical results is ***:All patients with initial LAPC who received surgery after chemotherapy at the high-volume centre for pancreatic surgery of ***-hospital Bochum between 2015 and 2022 were included in this retrospective cohort ***:A total of 139 patients underwent surgery after pre-treatment with FOLFIRINOX(76.3%),gemcitabine/nab-paclitaxel(11.5%),both(5.8%)and other regimens(6.5%).Eighty-five tumors(61.2%)were resectable after PCT.R0 resection was achieved in 92.9%,R1 in 7.1%and R2 in 0%of ***-four tumors were still not resectable at the time of *** results of the patients did not show increased postoperative mortality and morbidity compared to the literature *** 30-day mortality was 1.4%.Rates for pancreas-specific complications[postoperative pancreatic fistula(POPF),delayed gastric emptying(DGE),postpancreatectomy hemorrhage(PPH),and others]were not *** occurred in 10.5%and DGE in 26.3%after *** distal pancreatectomy,POPF was detected in 37.5%and DGE in 12.5%.Median postoperative survival(31 vs.13 months)and overall survival after initial diagnosis(40 vs.20 months)were significantly longer in resected patients(Pstoperative recurrence-free survival in resected patients amounted to 12 ***:This study underlines that PCT allows resectability of primarily unresectable patients with LAPC without increasing perioperative mortality and *** may lead to a significant prolongation of recurrence-free and overall survival in resected patients after PCT.

One of the major challenges in emergency medicine is out-of-hospital cardiac arrest(OHCA).Every year,about 53–62/100000 people worldwide suffer an out-of-hospital cardiac arrest with serious consequences,whereas persistent brain injury is a major cause of morbidity and mortality of those surviving a cardiac ***,only few and insufficient strategies are known to limit neurological damage of ischemia and reperfusion *** aim of the present study was to investigate whether teriflunomide,an approved drug for treatment of relapsing-remitting-multiple-sclerosis,exerts a protective effect on brain cells in an in vitro model of ***,organotypic slice cultures from rat hippocampus and cerebellum were exposed to oxygen-glucose-deprivation and subsequently treated with *** administration of teriflunomide in the reperfusion time on both hippocampal and cerebellar slice cultures significantly decreased the amount of detectable propidium iodide signal compared with an untreated culture,indicating that more cells survive after ***,hippocampal slice cultures showed a higher vulnerability to ischemic conditions and a more sensitive response to teriflunomide compared with cerebellar slice *** study suggests that teriflunomide,applied as a post-treatment after an oxygenglucose-deprivation,has a protective effect on hippocampal and cerebellar cells in organotypic slice cultures of *** procedures were conducted under established standards of the German federal state of North Rhine Westphalia,in accordance with the European Communities Council Directive 2010/63/EU on the protection of animals used for scientific purposes.

AIM: Evaluation of the wide range of normal findings in asymptomatic women undergoing dynamic magnetic resonance (MR) defecography. METHODS: MR defecography of 10 healthy female volunteers (median age: 31 years) without previous pregnancies or history of surgery were evaluated. The rectum was filled with 180 mL gadolinium ultrasound gel mixture. MR defecography was performed in the supine position. The pelvic floor was visualized with a dynamic T2-weighted sagittal plane where all relevant pelvic floor organs were acquired during defecation. The volunteers were instructed to relax and then to perform straining maneuvers to empty the rectum. The pubococcygeal line (PCGL) was used as the line of reference. The movement of pelvic floor organs was measured as the vertical distance to this reference line. Data were recorded in the resting position as well as during the defecation process with maximal straining. Examinations were performed and evaluated by two experienced abdominal radiologists without knowledge of patient history. RESULTS: Average position of the anorectal junction was located at -5.3 mm at rest and -29.9 mm during straining. The anorectal angle widened significantly from 93° at rest to 109° during defecation. A rectocele was diagnosed in eight out of 10 volunteers showing an average diameter of 25.9 mm. The bladder base was located at a position of +23 mm at rest and descended to -8.1 mm during defecation in relation to the PCGL. The bladder base moved below the PCGL in six out of 10 volunteers, which was formally defined as a cystocele. The uterocervical junction was located at an average level of +43.1 mm at rest and at +7.9 mm during straining. The uterocervical junction of three volunteers fell below the PCGL; described formally as uterocervical prolapse. CONCLUSION: Based on the range of standard values in asymptomatic volunteers, MR defecography values for pathological changes have to be re-evaluated.

Unicentric Castleman＇s disease of the pancreas is extremely rare, with only six cases described in the worldwide literature. An asymptomatic case of unicentric, hyaline, vascular-type Castleman＇s disease （UCD） localized to the tail of bhe pancreas with central calcification imitating a primary neoplasm of the pancreas is presented. This is the first description of endosonographic and endoscopic retrograde pancreatographic findings of pancreatic UCD. Additionally, computed tomography, histological and serologic findings are reported.
