Predictors of heart rhythm dist.rbances in hypertensive obese patients with obst.uctive sleep apnea

Objective To assess the incidence and predictors of heart rhythm and conduction dist.rbances in hypertensive obese patients with andwithout obst.uctive sleep apnea （OSA）. Methods This is an open, cohort, prospective st.dy. Out of 493 screened patients, we selected 279hypertensive, obese individuals without severe concomitant diseases： 75 patients without sleep-disordered breathing （non-SDB group）, and204 patients with OSA （OSA group）. At baseline, all patients underwent examination, including ECG, Holter ECG monitoring, and sleepst.dy. During follow-up （on 3, 5, 7 and 10th years; phone calls once per 6 months）, information about new events, changes in therapy and lifest.le was collected, diagnost.c procedures were performed. As the endpoints, we regist.red significant heart rhythm and conduction disordersas following： atrial fibrillation （AF）, ventricular tachycardia, atrioventricular block （AV） 2-3 degree, sinoatrial block, significant sinus pauses（〉 2000 ms）, and the required pacemaker implantation. Results The median follow-up was 108 （67.5-120） months. The frequency of heartrhythm disorders was higher in OSA patients （29 cases, X^2= 5.5; P = 0.019） compared to the non-SDB patients （three cases; OR： 3.92, 95%CI： 1.16-13.29）. AF was regist.red in 15 patients （n = 12 in OSA group; P = 0.77）. Heart conduction dist.rbance developed in 16 patients,without an association with the rate of coronary artery disease onset. Regression analysis showed that only hypertension duration was anindependent predictor olAF （OR： 1.10, 95% CI： 1.04-1.16; P = 0.001）. In case of heart conduction dist.rbances, apnea duration was thest.ongest.predictor （P = 0.002）. Conclusions Hypertensive obese patients with OSA demonst.ate 4-fold higher incidence of heart rhythmand conduction dist.rbances than subjects without SDB. Hypertension duration is an independent predictor for AF development, while sleepapneaJhypopnea duration is the main factor for heart conduction disorders onset in hypertensiv

Evolutionarily conserved features of the retained intron in alternative transcripts of the <i>nxf1</i>(nuclear export factor) genes in different organisms

One of the features of intron-containing genes of the nxf (nuclear export factor) family in different organisms is the presence of an evolutionarily conserved exon-intron block: exon 110nt-intron-exon 37nt. The intron in this evolutionarily conserved block, which we call a “cassette” intron, can be excised or retained in alternative transcripts of nxf1. It corresponds to intron 10 -11 inthe genes that are orthologous to nxf1 in vertebrates, and intron 5 -6 inthe genes that are orthologous to nxf1 in Drosophilidae. The alignment of sequences of cassette introns in nxf1 genes in vertebrates has revealed four evolutionarily conserved sequences: 1)5’flanking sequence, 2) a region containing СТЕ (const.tutive transport element), 3) third conserved sequence, and 4)3’flanking sequence. Introns 5-6 of nxf1 in Drosophilidae have no similar conserved sequences. The results of sequence alignment demonst.ate a similarity between cassette introns of nxf1 in Drosophilidae in two poly(A) sequences. The prevalence of Dm nxf1 transcripts containing cassette intron 5-6 under completely spliced transcripts in the heads of adult Drosophila melanogast.r suggest. a functional importance of transcripts that contain a retained intron. Evolutionary conservation, which in Drosophilidae is evident in the presence of poly(A) sequences in cassette introns of the nxf1 genes, is an adaptive feature: the poly(A) sequences are capable of mimicking the 3’-end of transcripts, promote transport from the nucleus to the cytoplasm, or are involved in NMD control. The ability to form characterist.c secondary st.uctures is a common feature of nxf1 cassette introns.

On the Gamma-Ray Burst. Origin

Gamma-ray (GRBs) and X-ray Burst. are millisecond-10 and 1000 seconds-long events of unknown origin. Recent simulations of the merger of binary neutron st.r syst.ms do not generate a magnetically dominated called funnel nor a relativist.c outflow. New models for the detection the afterglow of GRB 121024A, measured 0.15 days after the burst. invoke anisotropy as required to produce the complex microphysics of realist.c shocks in relativist.c jets. On the other hand the non-thermal gamma-rays are supposed to be produced by a fireball of relativist.c e?e+ pairs that are created by annihilation of neutrino-antineutrino pairs in the vicinity of the hot, merged object. It is also known that in a syst.m of a large number of fermions with pairs, gravitational interaction occurs a spontaneous breaking of the vacuum spatial symmetry, accompanied by gravitational mass defect. If spherical symmetry is broken, as in the known case of the merger scenario where a rapidly rotating disk can be formed and material is pulled away from rotation axis by centrifugal forces, then a baryon-free funnel along the rotation axes may allow relativist.c beam of γ’s and e?e+ to escape. It might lead to matter ejection with Lorentz factors of ~102 - 103 which are in the right range to enable copious gamma production during shock interaction with ambient interst.llar gas. Here we show that the space rays generation mechanism on a method of direct transformation of intergalactic gamma-rays to the proton current on spin shock-waves ensure precise agreement between generated proton currents (spin shock waves theory) with the angular dist.ibution data of Galactic gamma-rays as well as for the individual pulses of gamma-/X-ray burst.. Etiology of invasive candidosis agents in Russia： a multicenter epidemiological survey

A multicenter prospective epidemiological survey on the etiologic agents of invasive candidosis was conducted in Russia in the period of 2012-2014. Samples were collected from 284 patients with invasive candidosis and Candida species isolated by culture. The species were identified by DNA sequencing and MALDI-TOF mass- spectrometry. A total of 322 isolates were recovered, in which 96% of Candida species belonged to six major species, namely, C. albicans （43.2%）, C. parapsilosis （20.2%）, C. glabrata （11.5%）, C. tropicalis （9.6%）, C. krusei （6.2%）, and C. guilliermondii （5.3%）. Most.Candida species were isolated from blood samples （83.23%）. Notably, the prevalence rate of C. albicans reduced from 52.38% to 32.79% （2012 vs. 2014）（P = 0.01） whereas that of non-C. albicans increased from 47.62% （2012） to 67.21% （2014）（P〈 0.01）. Species dist.ibution differed among geographical regions; specifically, the prevalence rate of C. albicans as an etiologic agent of invasive candidosis in Siberian Federal region was significantly higher than that in other Federal regions. Results indicated a shift from C. albicans to non-C, albicans. Therefore, a detailed invest.gation on the contributing factors and appropriate treatment of invasive candidosis is needed.

Different discharge morphologies in atmospheric Ar and He plasmas are excited by using a pulsed microwave hairpin *** plasmas form an arched plasma plume at the opened end of the hairpin,whereas He plumes generate only a contracted plasmas in between both tips of metal *** this different point,their discharge processes have three similar characterist.cs:(i)the ionization occurs at the main electrode first.y and then develops to the slave electrode,(ii)during the shrinking st.ge the middle domain of the discharge channels disappears at last.and(iii)even at zero power input(in between pulses)a weak light region always exist. in the discharge *** experimental results and electromagnetic simulations suggest.that the discharge is resonantly excited by the local enhanced electric *** addition,Ar ionization and excitation energies are lower than those of He,the effect of Ar gas flow is far greater than that of He gas,and the contribution of accelerated electrons only locates at the domain with the st.ongest.electric *** reasons could be used to interpret the different characterist.c plume morphologies of the proposed atmospheric Ar and He plasmas.

Influence of dust.particles on spatial dist.ibutions of particles and fluxes in positive column of glow discharge

The research herein examined the results of numerical simulations of the positive column of a glow discharge in argon dust. plasma using COMSOL Multiphysics software under conditions similar to the project known as *** scenarios dealing with formations of spatial dist.ibutions of densities and fluxes for charged particles were st.died,and evaluations of the influence of dust.particles on the discharge were obtained in a wide range of dust.*** extreme cases were dist.nguished:weak dust.influence when the densities,fluxes and electric field profiles are not perturbed,and st.ong dust.influence when all three density profiles(electrons,ions and charged dust.in the dust.cloud are similar(parallel)to each other,resulting in all created charges in the dust.cloud being lost.inside the *** such a case,the ambipolar field and the transport of charged particles are decreased in the dust.cloud,and any ambipolar flux is almost.absent within the cloud.
