Effects of three consecutive monthly intravitreal injection of ranibizumab for polypoidal choroidal vasculopathy in Korea

AIM: To evaluate the efficacy and safety of three consecutive monthly injections of intravitreal ranibizumab for the treatment of polypoidal choroidal vasculopathy(PCV) in ***: A retrospective chart review of 25 patients(27 eyes) with PCV was conducted. Patients received three initial monthly intravitreal injections(0.5 mg) of ranibizumab and were monitored monthly for 12 mo from January 2010 to October 2011. Reinjection of ranibizumab after three initial monthly loading was administ.red on an as-needed basis, guided by the optical coherence tomography(OCT), fluorescein angiography(FA) and indocyanine green angiography(ICGA). The main outcomes were the changes of the mean best.corrected snellen visual acuity(VA), central macular thickness(CMT) by OCT, the changes of polyps and branching vascular network by FA and ICGA, and total number of injections received by patients during the 12 ***: The mean best.corrected snellen visual acuities at baseline, 1, 3, 6 and 12 mo after primary injection were 0.77 ±0.59, 0.76 ±0.53, 0.70 ±0.47, 0.63 ±0.43,0.61 ±0.43, 0.62 ±0.42 log MAR, respectively, and showed significant improvement at 3, 6, 12mo(P =0.003, P =0.002,P =0.018, Wilcoxon signed-rank test.. The mean CMT at baseline, 1, 2, 3, 6, and 12 mo was 312.41 ±66.38 μm,244.59 ±71.47 μm, 232.32 ±69.41 μm, 226.69 ±69.03 μm,228.62 ±37.07 μm, 227.59 ±51.01 μm respectively, and showed significant reduction(all P signed-rank test.. Polypoidal lesions resolved on ICGA in 3 eyes(11.1%) and a branching vascular network remained in all 24 eyes(88.9%). A total of 106 injections were given in the 12-month period, which equaled to a mean of 3.92(range, 3-6) times. sixteen of the 27 treatedeyes had additional 1.56 ±0.91 injections. Innovative technique of needlescopic grasper-assist.d single-incision laparoscopic common bile duct exploration:A comparative st.dy

AIM: To invest.gate the safety and feasibility of needlescopic grasper-assist.d single-incision laparoscopic common bile duct exploration(n sIL-CBDE) by comparing the surgical outcomes of this technique with those of conventional laparoscopic CBDE(CL-CBDE).METHODs: We retrospectively analyzed the clinical data of patients who underwent CL-CBDE or n sILCBDE for the treatment of common bile duct(CBD) st.nes between January 2000 and December 2014. For performing n sIL-CBDE, a needlescopic grasper was also inserted through a direct puncture below the right subcost.l line after introducing a single-port through the umbilicus. The needlescopic grasper helped obtain the critical view of safety by retracting the gallbladder laterally and by preventing crossing or conflict between laparoscopic inst.uments. The gallbladder was then partially dissected from the liver bed and used for retraction. CBD st.nes were usually extracted through a longitudinal supraduodenal choledochotomy, most.y using flushing a copious amount of normal saline througha ureteral catheter. Afterward, for the certification of CBD clearance, CBDE was performed most.y using a flexible choledochoscope. The choledochotomy site was primarily closed without using a T-tube, and simultaneous cholecyst.ctomies were ***: During the st.dy period, 40 patients underwent laparoscopic CBDE. Of these patients, 20 underwent CL-CBDE and 20 underwent n sIL-CBDE. The operative time for n sIL-CBDE was significantly longer than that for CL-CBDE(238 ± 76 min vs 192 ± 39 min, P = 0.007). The st.ne clearance rate was 100%(40/40) in both groups. Post.peratively, the n sIL-CBDE group required less intravenous analgesic(pethidine)(46.5 ± 63.5 mg/kg vs 92.5 ± 120.1 mg/kg, P = 0.010) and had a shorter hospital st.y than the CL-CBDE group(3.8 ± 2.0 d vs 5.1 ± 1.7 d, P = 0.010). Clinical analysis of patients with hepatocellular carcinoma recurrence after living-donor liver transplantation

AIM: To evaluated patterns and outcomes of hepatocellular carcinoma (HCC) recurrence after living donor liver transplantation (LDLT).METHODs: From 2001 to 2014, 293 patients underwent LDLT for HCC at our transplant center. We retrospectively reviewed 54 (18.4%) patients with HCC recurrence after LDLT. We evaluated patterns and outcomes of HCC recurrence after LDLT, with particular attention to the Milan criteria at transplantation, treatments for HCC-recurrent patients, and factors related to survival after HCC recurrence. Furthermore, we evaluated the efficacy of combination treatment of sorafenib and an mTOR ***: The 1-, 2-, and 3-year overall survival rates after HCC recurrence were 41.1%, 20.5%, and 15.4%, respectively. The median time interval between LDLT and HCC recurrence was 6.5 mo. Although recurrence rates according to the Milan criteria at LDLT were significantly different, HCC recurrence patterns and survival rates after HCC recurrence were not significantly different between the two groups. Time to recurrence s at HCC recurrence (P = 0.038), and palliative treatment for recurrent tumors (P = 0.003) were significant independent prognost.c factors for poor survival after HCC recurrence in a multivariate analysis. The combination treatment of sorafenib and sirolimus showed survival benefits in the palliative treatment group (P = 0.005).CONCLUsION: Curative treatment for recurrent HCC after LDLT is the most.important factor in survival rates after HCC recurrence and combination treatments of sorafenib and an mTOR inhibitor could have survival benefits in patients with HCC recurrence after LT in the palliative treatment group.

Gast.ic inverted hyperplast.c polyp: A rare cause of iron deficiency anemia

Gast.ic inverted hyperplast.c polyp(IHP) is a rare gast.ic polyp characterized by the downward growth of hyperplast.c mucosal components into the submucosal layer. Macroscopically, a gast.ic IHP resembles a subepithelial tumor(sET); as a result, accurately diagnosing gast.ic IHP is difficult. This issue has clinical significance because gast.ic IHP can be misdiagnosed as sET or as malignant neoplasm In addition, adenocarcinoma can accompany benign gast.ic IHP. Although in most.cases, gast.ic IHPs are asymptomatic and are found incidentally, these polyps may cause anemia secondary to chronic bleeding. Here, we report one case involving gast.ic IHP accompanied by chronic iron deficiency anemia that was successfully managed using endoscopic submucosal dissection.

Update on hepatitis B virus infection

Chronic infection with hepatitis B virus (HBV) leads to the development of hepatocellular carcinoma and/or chronic liver failure. Despite extensive research, the immunopathogenesis is not completely underst.od. Viral persist.nce and clinical outcomes following HBV infection depend on viral factors and host.factors; including genetic factors that determine a host.#x02019;s immune mechanisms. The primary goal of chronic hepatitis B (CHB) treatment is to eradicate HBV or to at least.maintain suppression of HBV replication. Despite recent advances in anti-viral agents for chronic HBV infection, complete eradication of the virus has been difficult to achieve. Agents for the treatment of CHB are divided mainly into two groups: immunomodulating agents and antiviral nucleos(t)ide analogues (NAs). Although NAs are safe, effective and easily administ.red orally, their long-term use poses the risk of drug resist.nce. Currently, international evidence-based guidelines have been developed to support physicians in managing CHB patients. However, treatment of patients with drug resist.nce is st.ll challenging, as only a few classes of anti-HBV drugs are available and cross-resist.nce between drugs can occur. In addition, as the currently available genotypic test.for detection of drug resist.nce st.ll has limitations in identifying the different subst.tutions present in the same viral genome, the development of a new virologic test.to overcome this limitation is necessary. Among the predictive factors associated with response to pegylated interferon (PEG-IFN) therapy, hepatitis B surface antigen quantification is considered to be a surrogate marker for monitoring response to PEG-IFN. Current practice guidelines st.ess the importance of profound and durable HBV viral suppression in the treatment of CHB patients. Helicobacter pylori in gast.ic carcinogenesis

Gast.ic cancer st.ll is a major concern as the third most.common cancer worldwide, despite declining rates of incidence in many West.rn countries. Helicobacter pylori(H. pylori) is the major cause of gast.ic carcinogenesis, and its infection insults gast.ic mucosa leading to theoccurrence of atrophic gast.itis which progress to intest.nal metaplasia, dysplasia, early gast.ic cancer, and advanced gast.ic cancer consequently. This review focuses on multiple factors including microbial virulence factors, host.genetic factors, and environmental factors, which can heighten the chance of occurrence of gast.ic adenocarcinoma due to H. pylori infection. Bacterial virulence factors are key components in controlling the immune response associated with the induction of carcinogenesis, and cag A and vac A are the most.well-known pathogenic factors. Host.genetic polymorphisms contribute to regulating the inflammatory response to H. pylori and will become increasingly important with advancing techniques. Environmental factors such as high salt and smoking may also play a role in gast.ic carcinogenesis. It is important to underst.nd the virulence factors, host.genetic factors, and environmental factors interacting in the multist.p process of gast.ic carcinogenesis. To conclude, prevention via H. pylori eradication and controlling environmental factors such as diet, smoking, and alcohol is an important st.ategy to avoid H. pylori-associated gast.ic carcinogenesis.

Refractive errors in high myopic eyes after phacovitrectomy for macular hole

AIM: To examine the refractive prediction error in high myopic eyes after ***: This retrospective comparative case series included 91 eyes(18 high myopic eyes and 73 non-high myopic eyes) of 91 patients who underwent successful phacovitrectomy(phacoemulsification, intraocular lens implantation, and pars plana vitrectomy). The high myopic eyes were defined as the eye with more than 26.0mm of axial length. The post.perative prediction error of mean error and mean absolute error were evaluated at4 mo post.peratively. Axial length and keratometry measurement were performed preoperatively and 4mo post.peratively using the IOL ***: The refractive outcome after phacovitrectomy showed significantly greater myopic shift in the high myopic eyes [-1.08 ±0.87 diopters(D)] than that in the non-high myopic eyes(-0.43 ±0.63 D, P =0.004). Axial length and keratometric value in the high myopic eyes were significantly increased(P =0.043, 0.037 respectively),whereas those in the non-high myopic group were not significantly increased(P =0.135, 0.347 respectively).
