Rare Bilateral Radius and Ulna Fractures: A Case Report

Introduction: Forearm fractures involving both the radius and ulna, present dist.nctive challenges in orthopedic trauma management. This case report explores the complexities of a rare case of bilateral fractures, emphasizing the importance of a comprehensive approach for optimal outcomes. Case Presentation: This report presents the case of a 40-year-old motorcyclist.who was involved in a road accident and arrived at the emergency department shortly after the incident. Clinical examination revealed closed fractures in both forearms with mild swelling and severe tenderness. Fortunately, no neurovascular issues or compartment syndrome were detected. The patient was administ.red intravenous fluids and effective pain relief. Forearm splints were applied and the arms were elevated using pillows. Radiographs showed fractures in the right dist.l radius and ulna, as well as the left mid-shaft radius and ulna. Under general anest.esia, open reduction and internal fixation were per-formed using plates and screws for all four fractures. Post.peratively, the patient’s arms were immobilized and elevated, with gradual recovery during follow-up appointments over six months, eventually achieving full function without complaints. Conclusion: This article highlights the possibility, although infrequent, of bilateral shaft fractures of the ulna and radius. With thorough clinical assessment and radiological invest.gation, such fractures can be well-defined. The required definitive treatment, which usually involves open reduction and internal fixation, can be performed.

This case report highlights a critical and emergent condition, isolated third nerve cranial palsy due to a brain aneurysm. It emphasizes the importance of differential diagnosis and attentiveness to the physical exam in an emergency setting. The detailed progression from initial symptoms and misinterpretation to final diagnosis offers valuable insight into the dangers of overlooking critical diagnoses. A bilobed intracranial aneurysm arising from the internal carotid artery (ICA) caused symptoms in this patient that could have been attributed to a less malignant etiology, such as a complex migraine. Overlooking the urgency of the situation and missing the diagnosis could have had a grave and irreversible outcome.

This paper is based on the author’s 20+ years of experience treating patients with congest.ve heart failure (CHF) as a cardiologist. In the 20+ years, 64 patients were treated, including both with reduced and preserved left ventricular function. Most.patients had a 4 - 5 days hospitalization in their first.admission with one readmission (1.6%) over seven years. This paper will help us underst.nd the physiology and pathophysiology of congest.ve heart failure, especially how to use beta blockers and diuretics. It will shorten the length of hospitalization and lower the readmission rate and cost.of CHF treatment. This paper will help us to open another research direction for CHF.

This paper reported beta-blocker use in 21 st.MI patients over four years. The patients were between 50 - 65 years of age presenting with anterior, lateral, and inferior st.MI (st.Elevation Myocardial Infarction). Seven of the patients were female, and 14 were male. They presented to an emergency room of a rural hospital that did not provide emergency percutaneous coronary angioplast./st.nting (PTCA/st.nting). The hospital is about 70 minutes from a facility that provided PTCA/ st.nting—all the patients presented with typical angina chest.pain with st.elevation. They are hemodynamic st.ble. Most.patients received Lopressor 35 mg IVP, with one receiving 115 mg in a 5 mg increment. They were chest.pain-free and hemodynamically before leaving the ER for the transfer for PTCA/st.nt. The results demonst.ated that beta-blockers are effective in relieving pain in st.MI patients. Further st.dy is needed to determine its efficacy, safety, and how to use it.

Objectives: This st.dy aimed to determine whether errors in vascular measurements would affect device selection in endovascular aortic repair (EVAR) by comparing measurements obtained using non-contrast.computed tomography (NCT) with those obtained using contrast.enhanced CT (CECT). Materials and Methods: This single-center, retrospective st.dy included 25 patients who underwent EVAR for abdominal aortic aneurysm at our inst.tution. A 1-mm horizontal cross-sectional slice of NCT and CECT from each patient was retrospectively reviewed. The area from the abdominal aorta to the common iliac artery was divided into four zones. A centerline was created using the NCT by manually plotting the center points. Subsequently, the centerlines were automatically extracted and manually corrected during the arterial phase of CECT. The diameter and length of each zone were measured for each modality. The mean diameters and lengths of the target vessels were compared between NCT and CECT. Results: The measurements obtained using both methods were reproducible and demonst.ated good agreement. The mean differences in vessel length and diameter measurements for each segment between NCT and CECT were not st.tist.cally significant, indicating good consist.ncy. Conclusion: NCT may be useful for preoperative EVAR evaluation in patients with renal dysfunction or allergies to contrast.agents.

Objective Traditionally tumors of the parapharyngeal space(PPS)are resected through transcervical *** recent approaches include endoscopic approaches or transoral robotic surgery(TORS)without directions on when to use which *** objective was to find objective parameters to choose the most.suitable *** It is a retrospective st.dy containing 6 patients from May 2019 to May 2021 with tumors of the PPS treated in the Department of Otolaryngology and Head-Neck Surgery at the hospital of lucerne,*** The data was analysed in average 53 months after *** resection was completed with TORS in 3 patients and endoscopically in 3 *** operation time was 114 *** major complications *** evidence of tumor was found in magnetic resonance imaging st.dies post.peratively in all *** We conclude that a resection via TORS or endoscopic technique is safe and ***,we post.late that the further a tumor is located in the upper lateral area of the PPS,an approach via TORS is less possible.
