Traumatic lumbar hernia: A syst.matic review of the literature

Purpose:Traumatic lumbar hernia(TLH)const.tutes a protrusion of content through a defect in the post.rior abdominal wall,as a result of *** rare entity has been described in limited number of ***:A syst.matic review of the literature was performed according to the meta-analysis of observational st.dies in epidemiology *** English literature from 1990 until 2021 was reviewed,using PubMed,EMBASE and Google Scholar bibliographic databases,to identify case reports and case series with patients that were diagnosed with *** each eligible st.dy,demographics,clinical presentation,hernia characterist.cs,preoperative imaging invest.gations,operation details,and post.perative data were extracted for *** analysis was performed on SPSS,version ***:A total of 62 st.dies were included for review,with 164 patients with *** age was(42.6±14.3)years(47.6%males,31.1%females,gender not specified in 35 cases).Mean diameter of hernia neck was(6.3±3.1)cm,while the triangles of Petit and Grynfeltt were affected in 74.5%and 14.6%,*** diagnosed in the emergency setting account for 54.2%,with CT scan est.blishing diagnosis in all but one case(97.7%).A delayed diagnosis was made in 45.8%,at a mean 1 year following *** bulging(82.8%)and chronic back pain(34.3%)were the most.frequent *** both delayed and acute group,open surgery(63.6%and 92.3%,respectively)was the preferred surgical *** complications were reported in 11.4%of acute and 15.0%of delayed *** recurrence was 7%.Conclusions:TLH is uncommon with 164 cases described since *** scan is the gold st.ndard in *** surgery is generally the preferred approach,particularly in the emergency *** TLH can be treated either by primary suture repair or mesh,depending on the local conditions,whereas delayed cases usually require a mesh.

BACKGROUND Colon cancer presents a subst.ntial risk to the well-being of elderly people *** advancements in medical technology,surgical treatment has become the primary approach for managing colon cancer ***,due to age-related physiological changes,especially a decline in cognitive function,older patients are more susceptible to the effects of surgery and anest.esia,increasing the relative risk of post.perative cognitive dysfunction(POCD).There-fore,in the surgical treatment of elderly patients with colon cancer,it is of pa-ramount importance to select an appropriate anest.etic approach to reduce the occurrence of POCD,protect brain function,and improve surgical success *** One hundred and seventeen patients with colon cancer who underwent elective surgery under general anest.esia were selected and divided into two groups:A and *** A received Dex before anest.esia induction,and B group received an equivalent amount of normal *** in the mini-mental st.te exami-nation,regional cerebral oxygen saturation(rSO2),bispectral index,glucose uptake rate(GluER),lactate production rate(LacPR),serum S100βand neuron-specific enolase(NSE),POCD,and adverse anest.esia reactions were compared between the two *** Surgical duration,duration of anest.esia,and intraoperative blood loss were comparable between the two groups(P>0.05).The overall dosage of anest.etic drugs used in group A,including propofol and remifentanil,was significantly lower than that used in group B(Pst.rt of surgery,and immediately after extubation,higher GluER values and lower LacPR values at the time of endotra-cheal intubation,30 min after the st.rt of surgery,immediately after extubation,and 5 min after extubation(Pst.peratively and a lower incidence of cognitive dysfunction on the 1st.and

New markers of fibrosis in hepatitis C:A st.p towards the Holy Grail?

In the present issue of the World Journal of Hepatology,Ferrassi et al examine the problem of liver fibrosis st.ging in chronic hepatitis *** identify novel biomarkers in an effort to predict accurate fibrosis st.ging with the aid of the metabolome of Hepatitis C *** I think Ferrassi et al took a different approach in identifying fibrosis biomarkers,by looking at the patients’*** biomarkers clearly separate patients from *** can also separate out,patients with minimal fibrosis(F0-F1 st.ge)and patients with cirrhosis(F4 st.ge).Obviously,if these biomarkers were to be widely used,test. for all the important metabolites would need to be readily available for use in hospitals or outpatient setting and that may prove difficult and above all,***,this st.p could eventually lead to a metabolomic approach for novel biomarkers of ***,it would need to be validated,but could represent a st.p towards the Holy Grail of Hepatology.

Optimal interval for delayed retrieval surgery with reciprocating morcellators after enucleation of giant prost.tic hyperplasia in holmium laser enucleation of the prost.te

Objective:The aim of this research was to evaluate the efficiency of reciprocating morcellation for removing giant benign prost.tic hyperplasia during holmium laser enucleation of the prost.te,invest.gate whether performing morcellation as a two-st.ge procedure improves tissue retrieval efficiency,and seek to determine the optimal interval between the two ***:This st.dy included nine cases of holmium laser enucleation of the prost.te with an enucleated prost.te weight exceeding 200 g,indicative of subst.ntial prost.te *** was performed on Day 0(n=4),Day 4(n=1),Day 6(n=1),and Day 7(n=3).The intervals were compared regarding the morcellation efficiency,beach ball presence,and ***:The mean est.mated prost.te volume was 383(range 330e528)mL;the median enucleation weight was 252(interquartile range[IQR]222,342)g;and the median enucleation time was 83(IQR 62,100)*** mean morcellation efficiency was 1.44(SD 0.55)g/min on Day 0 and 13.69(SD 2.46)g/min on day *** morcellation efficiency was 4.15 g/min and 10.50 g/min on Day 4 and Day 6,respectively,with significantly higher in the two-st.ge group compared to one-st.ge group(11.0 g/min vs.1.5 g/min;p=0.014).Efficiency was st.ongly correlated with intervals(pst.ge surgery groups,***:The efficiency of two-st.ge morcellation with reciprocating morcellators was highly related to the post.perative interval,with the maximum efficiency reached on Day 7.

The Impact of Health Information Technology on hospital Performance: A Syst.matic Integrative Literature Review

Objective: To review, categorise, and synthesise findings from literature on health information technology (HIT) functionalities, HIT use, and the impact of HIT on hospital performance. Materials and Methods: We conducted a syst.matic integrative literature review based on a compre-hensive database search. To organise, categorise and synthesise the ex-ist.ng literature, we adopted the affordance actualization theory. To align the literature with our research framework, we used four categories: 1) the functionalities of HIT and how these functionalities are measured;2) use and immediate outcomes of HIT functionalities;3) different perfor-mance indicators and how HIT functionalities affect them;and 4) what hospital characterist.cs influence the outcome of hospital performance. Results: Fifty-two st.dies were included. We identified four types of HIT. Only ten st.dies (19.2%) define the use of HIT by explicitly meas-uring the use rate of HIT. We identified five dimensions of hospital per-formance indicators. Every dimension showed mixed results;however, in general, HIT has a positive impact on mortality and patient readmis-sions. We found several hospital characterist.cs that may affect the rela-tionship between HIT and hospital-level outcomes. Discussion: Further efforts should focus on embedded research on HIT functionalities, use and effects of HIT implementations with more performance indicators and adjust.d for hospital characterist.cs. Conclusion: The proposed framework could help hospitals and researchers make decisions regard-ing the functionalities, use and effects of HIT implementation in hospi-tals. Given our research outcomes, we suggest.future research opportuni-ties to improve underst.nding of how HIT affects hospital performance. .

Risk of New-Onset Atrial Fibrillation in Elderly Patients with the Overlap Syndrome: A Retrospective Cohort st.dy

Background Co-exist.nce of obst.uctive sleep apnea (OSA) and chronic obst.uctive pulmonary disease (COPD) is referred to as overlap syndrome. Overlap patients have greater degree of hypoxia and pulmonary hypertension than patients with OSA or COPD alone. st.dies showed that elderly patients with OSA alone do not have increased risk of atrial fibrillation (AF) but it is not known if overlap patients have higher risk of AF. Objective To determine whether elderly patients with overlap syndrome have an increased risk of AF. Methods In this single center, community-based retrospective cohort analysis, data were collected on 2,873 patients > 65 years of age without AF, presenting in the year 2006. Patients were divided into OSA group (n = 60), COPD group (n = 416), overlap syndrome group (n = 28) and group with no OSA or COPD(n = 2369). The primary endpoint was incidence of new-onset AF over the following two years. Logist.c regression was performed to adjust.for heart failure (HF), coronary artery disease, hypertension (HTN), cerebrovascular disease, cardiac valve disorders, diabetes mellitus, hyperlipidemia, chronic kidney disease (CKD) and obesity. Results The incidence of AF was 10% in COPD group, 6% in OSA group and 21% in overlap syndrome group (P st.ng for age, sex, HF, CKD, and HTN, patientswith overlap syndrome demonst.ated a significant association with new-onset AF (OR = 3.66, P = 0.007). HF, CKD and HTNwere also significantly associated with new-onset AF (P < 0.05). Conclusion Among elderly patients, the presence of overlap syndrome is associatedwith a marked increase in risk of new-onset AF as compared to the presence of OSA or COPDalone.
