Dear Editor,The molecular pathogenesis of estrogen receptor(ER)-negative,especially triple-negative breast cancer(TNBC),is unclear,thus,the treatment of these types of cancers is still a great challenge[1].Chemotherapy remains the primary adjuvant treatment choice for TNBC patients[2],but a large number of these tumors are resistant to chemotherapy and relapse ormetastasize quickly after adjuvant therapy[3].

目的:系统评价针刺治疗原发性高血压的疗效和安全性.方法:通过计算机检索中国知网(CNKI)、重庆维普数据库(CQVIP),万方学术期刊全文数据库(Wanfang)、中国生物医学文献数据库(CBM)、PubMed、荷兰医学文摘数据库(EMBASE)和Cochrane Library数据库自建库至2021年1月30日所收录的以针刺作为主要干预手段治疗原发性高血压的临床随机对照试验.根据Cochrane手册对纳入研究进行偏倚风险评估.使用Review Manager 5.4.1软件和Stata 15.0软件进行数据分析.结果:共纳入46项随机对照试验,共包括3859例受试者.主要结局指标包括干预后舒张压变化{8项研究显示针刺联合西药治疗组优于单用降压药组[MD=1.45,95%CI(0.48,2.43),P=0.004,固定效应模型;I2=39%]}和干预后收缩压变化{11项研究表明针刺联合西药治疗组优于单用降压药组[MD=8.60,95%CI(7.12,10.07),P<0.00001,固定效应模型;I2=26%]}.次要结局指标为降压疗效,12项单独针刺治疗组的研究[RR=1.20,95%CI(1.12,1.28),P<0.00001,固定效应模型;I2=36%]与15项针刺联合降压药组的研究[RR=1.27,95%CI(1.20,1.34),P<0.00001,固定效应模型;I2=6%]皆显示其降压效果优于单用降压药.在不良反应方面,4项研究表明单独针刺组不良反应少于单用降压药组[RR=0.10,95%CI(0.04,0.25),P<0.00001,固定效应模型;I2=0%].结论:Meta分析表明,针刺联合降压药物的降压效果优于单用西药,且针刺治疗原发性高血压有效、安全、副作用少.然而,该领域尚缺乏高质量、多中心的随机双盲对照试验,仍需要严谨设计的大样本临床研究对上述结论进行验证.

Poly(ADP-ribose)polymerase(PARP)inhibitors are one of the most exciting classes of targeted therapy agents for cancers with homologous recombination(HR)***,many patients without apparent HR defects also respond well to PARP inhibitors/*** biomarker responsible for this mechanism remains ***,we identified a set of ribosomal genes that predict response to PARP inhibitors/cisplatin in HR-proficient *** inhibitor/cisplatin selectively eliminates cells with high expression of the eight genes in the identified panel via DNA damage(ATM)signaling-induced pro-apoptotic ribosomal stress,which along with ATM signaling-induced pro-survival HR repair constitutes a new model to balance the cell fate in response to DNA ***,the combined examination of the gene panel along with HR status would allow for more precise predictions of clinical response to PARP inhibitor/*** gene panel as an independent biomarker was validated by multiple published clinical datasets,as well as by an ovarian cancer organoids library we *** importantly,its predictive value was further verified in a cohort of PARP inhibitor-treated ovarian cancer patients with both RNA-seq and WGS ***,we identified several marketed drugs capable of upregulating the expression of the genes in the panel without causing HR deficiency in PARP inhibitor/cisplatin-resistant cell *** drugs enhance PARP inhibitor/cisplatin sensitivity in both intrinsically resistant organoids and cell lines with acquired ***,our study identifies a marker gene panel for HR-proficient patients and reveals a broader application of PARP inhibitor/cisplatin in cancer therapy.

AIM: To investigate the relationship between contrastenhanced ultrasound(CEUS),basic fibroblast growth factor(b FGF),endothelin-1(ET-1),and hepatocellular carcinoma(HCC) recurrence after ablation. METHODS: A total of 51 HCC patients(38 males and 13 females) who received radiofrequency ablation in our hospital from June 2012 to July 2014 were enrolled in this study. The patients were divided into two groups: recurrence group and non-recurrence group. Routine abdominal examination was first performed in the horizontal position. Then the patients underwent CEUS and immunohistochemical staining before receiving radiofrequency ablation. All patients were followed-up every three months for one *** results of CEUS and serum tumor marker levels were evaluated and combined together to estimate HCC recurrence and metastasis. Patients were divided into two groups: recurrence group and non-recurrence group. Quantitative parameters of CEUS and tumor expression levels of b FGF and ET-1 were compared between the two groups,respectively. Binary logistic regression analysis was used to analyze the relationship between CEUS quantitative parameters,expression levels of ET-1 and b FGF,and HCC recurrence after ablation. RESULTS: Based on the quantitative parameters of CEUS before patients received radiofrequency ablation,the levels of tumor rise time(t RT),tumor time to peak(t TTP),tumor peak intensity(t PI) and tumorparenchymal peak intensity(t-p PI) in the recurrence group were significantly lower than those in the nonrecurrence group(16.6 ± 6.1 vs 23.2 ± 7.0,P = 0.000; 41.2 ± 10.2 vs 59.6 ± 14.2,P = 0.000; 23.8 ± 6.7 vs 31.4 ± 6.4,P = 0.000; 7.1 ± 3.4 vs 14.6 ± 7.4,P = 0.000; respectively). The expression levels of b FGF in the recurrence group were significantly higher than those in the non-recurrence group(P < 0.05). 1 BACKGROUND With the rapid development of immune checkpoint inhibitors(ICIs)over the past decades,they have become a major area of interest in the treatment of colorectal cancer(CRC)[1,2].There are evidence pointing that programmed cell death protein-1(PD-1)blockade,alone or in combination with anti-cytotoxic T-lymphocyte-associated protein 4(anti-CTLA4)therapy,achieved durable responses in patients with mismatch repair-deficient(dMMR)or microsatellite instability-high(MSI-H)metastatic CRC(mCRC)[3–6].However,the optimal diagnostic method for detecting dMMR/MSI-H disease as well as the optimal anti-PD-1-based treatment modality still remains controversial in this patient *** addition,for the majority of mCRC cases that are mismatch repair-proficient(pMMR)or microsatellite stable(MSS),the clinical benefits from these agents are generally minimal[3,7],driving extensive research efforts to develop effective combination therapies in this disease ***,investigations of anti-PD-1-based treatments have also been initiated in the nonmetastatic settings of CRC,with some encouraging preliminary evidence[8].Medical oncologists and surgeons from the Committee of Colorectal Cancer of the Chinese Society of Clinical Oncology had a panel discussion on immunotherapy for patients with colorectal cancer during a seminar on June 16,2020,in Guangzhou,***,the expert opinions have been summarized along with relevant clinical evidence(Table 1)to guide real-world treatment decision-making regarding the use of ICIs in patients with CRC.

Atrial fibrillation(AF)is a common cardiac disease with high prevalence in the general *** a mild manifestation at the onset stage,it causes serious consequences,including sudden death,when the disease progresses to the late *** available treatments of AF focus on symptom management or alleviation,due to a lack of fundamental knowledge and the fact that considerable variations of AF *** the popularisation of the next-generation sequencing technology,several causal genetic factors,including MYL4,have been discovered to contribute to AF,giving hope to developing its gene *** this study,we attempted to treat a previously established rat AF model,which carried Myl4E11K/E11K loss of function mutation,via overexpression of exogenous wild-type Myl4 by AAV9 *** results showed that delivery of Myl4 expressing AAV9 to postnatal rat models rescued the symptoms of AF,indicating the therapeutic potential that early gene therapy intervention can achieve long-term effects in treating cardiac arrhythmias caused by gene mutations.

Intermittent hypoxia（IH） can damage endothelial cells and lead to apoptosis in obstructive sleep apnea-hypopnea syndrome（OSAHS）.Hypoxia inducible factor-1 alfa（HIF-1α） plays a key role in the hypoxic stress *** investigated an approach to diminish the negative effect of HIF-1α while maintaining its protective *** umbilical vein endothelial cells（HUVECs）were subjected to IH,and the responses of HIF-1α,C/EBP β and ET-1 were assessed by western blotting.A luciferase reporter system was employed to verify potential binding sites for C/EBP βin the ET-1 *** specificity of regulation of ET-1 by HIF-1α via C/EBP β was evaluated by a lentiviral *** effects of silencing of C/EBP β on IH-induced apoptosis,VEGF protein levels,proliferation and in vitro tube formation were *** found that IH significantly increased HIF-1α,C/EBP β and ET-1 in *** of both HIF-1α and C/EBP β inhibited the IH-induced increase in ET-1.C/EBP β was shown to regulate ET-1 through the predicted binding site,*** C/EBP β impaired IH-induced apoptosis but did not affect VEGF expression,proliferation or in vitro tube formation.C/EBP βwas shown to mediate increased ET-1 expression by HIF-1α through transcriptional regulation and silencing.C/EBP β can suppress apoptosis but does not affect the protective role of HIF-1α in the hypoxic stress response.

Evolution of placental mammals over the past 160 million years witnesses the relative sparing of muscles from cancer attacks. In 1) nude mice with human gastrointestinal or lung tumors, and 2) human subjects with liver, lung or gastrointestinal tumors, intra-tumor implantation of allogeneic human myoblasts induced cancer apoptosis, inhibiting metastasis and tumor growth. We postulate four mechanisms of cancer apoptosis: a) myoblasts releasing tumor necrosis factor-α (TNF-α);b) deprivation of nutrients and oxygen;c) local inflammatory and immunologic attacks;and d) prevention from metastasis. These basic and clinical studies demonstrated preliminary safety and efficacy of intra-tumor myoblast implantation in the development of prevention and treatment for cancer, now the number one disease killer of mankind.

In recent years,natural orifice specimen extraction surgery(NOSES)in the treatment of colorectal cancer has attracted widespread *** potential benefits of NOSES including reduction in postoperative pain and wound complications,less use of postoperative analgesic,faster recovery of bowel function,shorter length of hospital stay,better cosmetic and psychological effect have been described in colorectal *** significant decrease in surgical trauma of NOSES have been observed,the potential pitfalls of this technique have been ***,several issues including bacteriological concerns,oncological outcomes and patient selection are raised with this new ***,it is urgent and necessary to reach a consensus as an industry guideline to standardize the implementation of NOSES in colorectal *** three rounds of discussion by all members of the International Alliance of NOSES,the consensus is finally completed,which is also of great significance to the long-term progress of NOSES worldwide.
