Esophageal dysphagia and reflux symptoms before and after oral IQoro^(R) training

AIM: To examine whether muscle training with an oral IQoroR screen(IQS) improves esophageal dysphagia and reflux ***: A total of 43 adult patients(21 women a n d 2 2 m e n) w e re c o n s e c u t i ve l y re fe r re d t o a swallowing center for the treatment and investigation of long-lasting nonstenotic esophageal dysphagia. Hiatal hernia was confirmed by radiologic examination in 21 patients before enrollment in the study(group A; median age 52 years, range: 19-85 years). No hiatal hernia was detected by radiologic examination in the remaining 22 patients(group B; median age 57 years,range: 22-85 years). Before and after training with an oral IQS for 6-8 mo, the patients were evaluated using a symptom questionnaire(esophageal dysphagia and acid chest symptoms; score 0-3), visual analogue scale(ability to swallow food: score 0-100), lip force test(≥ 15 N), velopharyngeal closure test(≥ 10 s), orofacial motor tests, and an oral sensory test. Another twelve patients(median age 53 years, range: 22-68 years) with hiatal hernia were evaluated using oral IQS traction maneuvers with pressure recordings of the upper esophageal sphincter and hiatus canal as assessed by high-resolution ***: Esophageal dysphagia was present in all 43 patients at entry, and 98% of patients showed improvement after IQS training [mean score(range): 2.5(1-3) vs 0.9(0-2), P < 0.001]. Symptoms of reflux were reported before training in 86% of the patients who showed improvement at follow-up [1.7(0-3) vs 0.5(0-2), P < 0.001). The visual analogue scale scores were classified as pathologic in all 43 patients, and 100% showed improvement after IQS training [71(30-100) vs 22(0-50), P < 0.001]. No significant difference in symptom frequency was found between groups A and B before or after IQS training. Operative stabilization of the remaining mobile segment in ankylosed cervical spine in systemic onset- juvenile idiopathic arthritis:A case report

We describe a case of a 19-year-old young man with oligoarthritis type of juvenile idiopathic arthritis, who presented with several month duration of lower neck pain and progressive muscular weakness of all four limbs. X-rays of the cervical spine demonstrated spontaneous apophyseal joint fusion from the occipital condyle to C6 and from C7 to Th2 with marked instability between C6 and C7. Surgical intervention began with anterolateral approach to the cervical spine performing decompression, insertion of cage and anterior vertebral plate and screws, followed by posterior approach and fixation. Care was taken to restore sagittal balance. The condition was successfully operatively managed with multisegmental, both column fixation and fusion, resulting in pain cessation and resolution of myelopathy. Postoperatively, minor swallowing difficulties were noted, which ceased after three days. Patient was able to move around in a wheelchair on the sixth postoperative day. Stiff neck collar was advised for three months postoperatively with neck pain slowly decreasing in the course of first postoperative *** the follow-up visit six months after the surgery patient exhibited no signs of spastic tetraparesis, X-rays of the cervical spine revealed solid bony fusion at single mobile segment C6-C7. He was able to gaze horizontally while sitting in a wheelchair. Signs of myelopathy with stiff neck and single movable segment raised concerns about intubation, but were successfully managed using awake fiber-optic intubation. Avoidance of tracheostomy enabled us to perform an anterolateral approach without increasing the risk of wound infection. Regarding surgical procedure, the same principles are obeyed as in management of fracture in ankylosing spondylitis or Mb. Forestrier.

Sensori-Neural Hearing Loss Client’s Performance with Receiver-In-Canal (RIC) Hearing Aids

Background: Individuals fitted with hearing aids complain of the unnatural sound quality of their voice, other internally generated sounds such as chewing and swallowing sounds “hollow”, “muffled” sounds. Receiver-In-Canal hearing aids are favored due to small size, discrete appearance and ability to minimize occlusion. Aim: To compare the performance of Receiver-In-Canal (RIC) to traditional ear tip (ET), ear moulds (EM) fittings using Functional gain measures. Method: Ten subjects with flat moderately severe sensori neural hearing loss participated in the study. Subjective unaided and aided measures for digital BTE hearing aids with ear tip, ear mould or Receiver-In-Canal for pure tones of 250 Hz, 500 Hz, 1000 Hz, 2000 Hz, 4000 Hz were obtained. Results and Discussion: Higher scores were obtained with Receiver-In-Canal fitting on Functional gain measures. No significant difference between all the three conditions was obtained at low frequencies especially at 500 Hz, as Receiver-In-Canal hearing aids attenuate low frequency sounds automatically when the ear is left open (up to 30 dB less amplification at 500 Hz) especially for hearing in noisy situations. Conclusion: The results suggest that Receiver-In-Canal fittings are an effective means of overcoming the major barriers to the acceptance of amplification and further suggest the clinical importance of subjective measures in measuring aided benefit of open-fit devices in the rehabilitation of person’s with moderately severe to severe SN hearing loss.

Esophageal disease is a common disorder of the digestive system that can severely affect the quality of life andprognosis of patients. Esophageal stenting is an effective treatment that has been widely used in clinical ***, esophageal stents of different types and parameters have varying adaptability and effectiveness forpatients, and they need to be individually selected according to the patient’s specific situation. The purposeof this study was to provide a reference for clinical doctors to choose suitable esophageal stents. We used 3Dprinting technology to fabricate esophageal stents with different ratios of thermoplastic polyurethane (TPU)/(Poly-ε-caprolactone) PCL polymer, and established an artificial neural network model that could predict the radial forceof esophageal stents based on the content of TPU, PCL and print parameter. We selected three optimal ratios formechanical performance tests and evaluated the biomechanical effects of different ratios of stents on esophagealimplantation, swallowing, and stent migration processes through finite element numerical simulation and in vitrosimulation tests. The results showed that different ratios of polymer stents had different mechanical properties,affecting the effectiveness of stent expansion treatment and the possibility of postoperative complications of stentimplantation.

Background: Ptosis and dysphagia are important features in oculopharyngeal muscular dystrophy (OPMD). Objective: Retroflexion of the head is a well known compensatory mechanism for ptosis, but generally retroflexion has a negative effect on swallowing. We hypothesised that severity of ptosis is related to degree of retroflexion and that this compensation is responsible for deteriorating dysphagia. Methods: Nine OPMD patients were examined in the conditions “head position adapted to ptosis" and “head position slightly flexed". Ptosis was quantified by photogrammetry and retroflexion of the head by digital photographs. The severity of dysphagia was measured using visual analogue scales (VAS) and by calculating swallowing volumes and oropharyngeal swallow efficiency (OPSE) based on videofluoroscopy. Results: Statistical analyses show a significant relationship between ptosis and degree of retroflexion. The degree of retroflexion of the head correlated significantly with VAS scores and with the maximum swallowing volume. The slightly flexed head position significantly improved VAS scores as well as swallowing volumes and OPSE. Conclusion: In OPMD patients, ptosis significantly correlates with retroflexion of the head, which has a negative effect on swallowing. Subjective and objective reduction of swallowing problems was found when patients were instructed to eat and drink with a slightly flexed head position.
