Marriage-Age and Fertility Pattern among Reproductive Age Women in Abakaliki, Ebonyi State, South East Nigeria

Introduction: In Nigeria, one of the major reasons for marriage is procreation and married couples look forward to having children within a year or two of marriage. The age of marriage is on the increase in this part of the world. Many women are now increasingly getting engulfed in career and academic pursuits at the detriment of early marriage. Many men are also not interested in marrying women who have no job. There is age-related acceleration of primordial follicle depletion which is of great clinical importance because it is associated with a significant decrease in fecundity. Objective: This study assessed the ages of the marriage of women who attended Fertility and Antenatal clinics at the National Obstetric Fistula Centre and Smile specialist Hospital, Abakaliki, and their relationship with the fertility pattern of these women. Methods: This was a cross-sectional study of 352 consenting married women who sought and received fertility and Antenatal care at the National Obstetric Fistula Centre and Smile specialist Hospital Abakaliki, South-East Nigeria between February 1, 2022 and January 31, 2024. Semi-structured questionnaires were administered to the participants over a two-year period. Data was analyzed using IBM SPSS version 20. A test of statistical significance was done using Chi Square. Results: The age group with the highest education was 26 - 30. This was statistically significant (X2 = 90.893, P-value 2 = 294.555, P-value 2 = 173.007, p-value 2 = 129.494, P-value 2 = 169.300, p-value 2 = 279.230, p-value Conclusion: There is increased difficulty at conception at marriage age above 25 years. The major reasons for delayed marriage were academic pursuit and lack of gainful employment. Education of the masses and inclusion of biological dynamics of the female reproductive behaviours as part of the core school curriculum will help enlighten the female and the male folks alike to enable them to make informed decisions with regard to the timing of marriage

Efficacy of Single Dose of Mifepristone Combined with Two Doses of Misoprostol in Early Medical Abortions

Objective To study the efficacy of single dose of mifepristone and two doses of misoprostol in women undergoing early medical abortion （EMA） up to 9 weeks of gestation. Methods An audit was performed on a retrospective data analysis of 162 women who underwent an early medical abortion in Southampton U.K. Relevant data on completeness of abortion and other details were analyzed. Results Majority of the women （67%） aged 20-30 years old, 22% were between 30 and 40 years old and 1% were above 40 years old when they came for abortion. About 94.5% women had complete abortion following the use of single dose mifepristone combined with two doses of misoprostol, 4.9% had an incomplete abortion and 0.6% had failure of the procedure. These figures had significantly improved over the results of similar audit in 2005 with single dose of misoprostol to women below 7 weeks of pregnancy. The results in this audit were 82.1% complete abortion, 7.2% incomplete abortion and O. 7% failed procedure. Conclusion Mifepristone in combination with two doses of misoprostol appears to be more effective in all cases of early medical abortion when compared with single dose of misoprostol for termination below 49 d （7 weeks） and two doses of misoprostol between 49 d and 63 d （7-9 weeks） of pregnancy.

Analgesic Efficacy of Paracervical Block for Manual Vacuum Aspiration of Incomplete Abortions: A Randomized Controlled Trial

Introduction: Pain control during Manual Vacuum Aspiration is one of the most important aspects of postabortion care. This study assessed the analgesic efficacy, requirement for additional analgesia, and overall satisfaction using Paracervical blocks of 1% lignocaine compare with normal saline as placebo among women undergoing manual vacuum aspiration for incomplete abortion in OAUTHC, Ile-Ife. Methods: This was a double blind randomized controlled trial that occurred between January 2019 and February 2020. We randomized one hundred and twenty eligible women equally into 2 groups. Group A received paracervical block using 1% lignocaine while those in group B received paracervical block using normal saline as placebo. We obtained ethical clearance from the Ethics and Research Committee of the hospital. Preoperatively, we obtained relevant data and evaluated the degree of anxiety and pain on a visual analog scale (VAS). Intraoperative pain was evaluated from 2 viewpoints: that of the external observer on a 0 - 4 scale and that of the patient scale of 0 - 10 in the immediate postoperative period, followed by overall satisfaction at the point of discharge. Background: The metabolic syndrome affects more and more global people. Although it shows increasing prevalence in general population, the syndrome affects more women than men, what makes its risk of being developed during pregnancy period. Also, possible perinatal adverse effects are always lurking. Objective: the objective was demonstrated what’s new in literature on metabolic syndrome and pregnancy. Methods: A literature review was performed to extract the articles published on metabolic syndrome and pregnancy, its prevalence, obstetrical complications and its perinatal adverse effects. This review was conducted by online researching in PubMed, Lilacs, Medline, Embase, Scopus, Medscape, Libertas Academica and CINAHL database, Science database and also by researches in books. 27 selected articles on metabolic syndrome after this research were all published between 1988 and 2015. Results: Among those 27 articles and two books studied, SM rate in obstetric population ranged from 3% to 42% depending on the previously manifested components of the syndrome, age and region. Women with previously manifested components showed more adverse perinatal effects. Conclusion: Women with pregestational DM or SM and SM develop more during pregnancy, obstetric complications and adverse perinatal outcomes.

Treatment patterns of primary care physicians vs specialists prior to subspecialty urogynaecology referral for women suffering from pelvic floor disorders

BACKGROUND There are approximately 25% of women in the United States suffering from pelvic floor disorders (PFDs) and this number is predicted to rise. The potential complications and increasing healthcare costs that exist with an operation indicate the importance of conservative treatment options prior to attempting surgery. Considering the prevalence of PFDs, it is important for primary care physician and specialists (obstetricians and gynecologists) to be familiar with the initial work-up and the available conservative treatment options prior to subspecialist (urogynecologist) referral. AIM To assess the types of treatments that specialists attempted prior to subspecialty referral and determine the differences in referral patterns. METHODS This is a retrospective cohort study of 234 patients from a community teaching hospital referred to a single female pelvic medicine and reconstructive surgery (FPMRS) provider for PFD. specialist vs primary care provider (PCP) referrals were compared. Number, length and treatment types were studied using descriptive statistics. RESULTS There were 184 referrals (78.6%) by specialists and 50 (21.4%) by PCP. Treatment (with Kegel exercises, pessary placements, and anticholinergic medications) was attempted on 51%(n = 26) of the PCP compared to 48%(n = 88) of the specialist referrals prior to FPMRS referral (P = 0.6). There was no significant difference in length of treatment prior to referral for PCPs vs specialists (14 mo vs 16 mo, respectively, P = 0.88). However, there was a significant difference in the patient’s average time with the condition prior to referral (35 mo vs 58 mo for PCP compared to specialist referrals)(P = 0.02). CONCLUSION One half of the patients referred to FPMRS clinic received treatment prior to referral. Thus, specialists and generalists can benefit from education regarding therapies for PFD before subspecialty referral.

BACKGROUND Asherman’s syndrome is characterized by reduced menstrual volume and adhesions within the uterine cavity and cervix,resulting in inability to carry a pregnancy to term,placental malformation,or *** present the case of a 40-year-old woman diagnosed with Asherman’s syndrome who successfully gave birth to a live full-term neonate after hysteroscopic adhesiolysis under laparoscopic observation,intrauterine device insertion,and Kaufmann *** SUMMARY A 40-year-old woman(Gravida 3,Para 0)arrived at our hospital for specialist care to carry her pregnancy to *** had previously undergone six sessions of dilation and curettage owing to a hydatidiform mole and persistent trophoblastic disease,followed by *** subsequently became pregnant twice,but both pregnancies resulted in spontaneous miscarriages during the first *** menstrual periods were very light and of short *** adhesiolysis with concurrent laparoscopy was performed,and Asherman’s syndrome was *** uterine adhesions covered the area from the internal cervical os to the uterine *** Kaufmann therapy was administered,and endometrial regeneration was confirmed using *** became pregnant 9 mo postoperatively and delivered through elective cesarean section at 37 wk of *** postpartum course was uneventful,and she was discharged on postoperative day *** Hysteroscopic adhesiolysis with concurrent laparoscopy enables identification and resection of the affected area and safe and accurate surgery,without complications.

Objective:To determine the relationship between asymptomatic malaria parasitemia and some oxidative stress parameters in pregnant Nigerian ***:This is a cross-sectional study involving 130 normal pregnant women at various trimesters,who were attending antenatal clinic at the University of Nigeria Teaching Hospital(UNTH) and Kenechukwu specialist Hospital in Enugu.A comparable group(control),made of 30 non pregnant women was also *** a 24 hour dietary recall,serum levels of vitamin A,C and malondialdehyde(MDA) were determined by colorimetric method,while vitamin E was determined by absorptiometric ***:There were no statistically significant differences in age,parity,estimated calorie,vitamins A,C and E intake between the pregnant and non pregnant groups(P>0.05).The serum level of the vitamins(umol/L) and MDA(umol/L) in control,1st,2nd and 3rd trimesters respectively were:(l)Vitamin A:1.6±0.36 vs 0.6±0.26 vs 0.62±0.33 vs 0.46±0.21(P0.05). Conclusions:Asymptomatic malaria parasitemia does not induce additional oxidative stress on pregnant women in *** enormity of acute and complicated attack should be further investigated.

A Review of Caesarean Delivery at the Kogi State specialist Hospital, Lokoja, Nigeria

Background: Caesarean delivery rate is increasing globally including in Nigeria. Caesarean delivery is a life-saving surgery for both mother and child. Objective: The aim of the study was to assess the caesarean delivery rate at the Kogi State specialist Hospital, Lokoja, Nigeria over a one-year period. Methods: This was a prospective assessment of the demographic characteristics of patients, indications, types, rates and outcomes of Caesarean delivery in Kogi State specialist Hospital, Lokoja, Nigeria from December 15, 2020 to December 14, 2021. Data collected were analyzed using SPSS version 20.0 statistical package (IBM Corp. Released 2011. IBM SPSS Statistics for Windows, Version 20.0. Armonk, NY: IBM Corp). The categorical data were displayed using tables and charts. Results: There was a total of 491 deliveries, out of which were 113 Caesarean deliveries and 378 spontaneous vaginal deliveries making the Caesarean delivery rate in Kogi State specialist Hospital, Lokoja, Nigeria 23.0%. There were no instrumental deliveries during the study period. The commonest indication for Caesarean section was obstructed labour in 31% of cases. There was no maternal mortality. There were three early neonatal deaths due to severe birth asphyxia. Conclusion: The rate of Caesarean delivery in this study was 23.0% and obstructed labour was the leading indication in 31% of cases. The rate of Caesarean delivery in this study is high, albeit the overall outcome was good for both mother and child. Caesarean delivery, therefore, remains one of the means for reducing maternal and perinatal morbidity and mortality in Nigeria.

Psychological Issues among Women Undergoing Fertility Treatment in a specialist Fertility Hospital, South East Nigeria

Introduction: In Nigeria, a major reason for marriage is procreation and married couples look forward to having children within a year or two of marriage. The inability to achieve conception and subsequently have a child among couples may lead to psychological complications. Objective: This study assessed the psychological impact of infertility and its management on women receiving treatment at a Fertility specialist Hospital in a poor resource setting. Methods: This was a cross-sectional study of 376 consenting married women who sought and received fertility treatment at a private Fertility specialist Hospital, located at Abakaliki, South-East Nigeria between May 1, 2017 and May 31, 2022. Socio-demographic and Structured questionnaires were administered to the participants over a 5-year period. Results: The mean age of the women at presentation was 36.9 (±7.2 SD) years. The majority of the women (227) presented with secondary infertility accounting for 60.4%, while the rest had primary infertility (χ2 = 16.18, P = 0.001). Male-only factor infertility accounted for 22.9% of all the infertility cases, female-only factor 21.3% while both (male and female factors co-existing) accounted for 52.7% of all the cases. The commonest cause of infertility in the study was poor sperm parameters (176) accounting for 46.8% of cases, tubal factor 19.1% and anovulatory factor 22.3% (χ2 = 214.21, P = 0.001). Three hundred and thirty four (88.8%) felt depressed, 266 (70.7%) felt guilty about the past, and 222 (59.0%) had suicidal tendencies because of the infertility ordeal. Two hundred and fifty one (66.8%) felt inferior, 237 (63.0%) cried often and 174 (46.3%) were socially withdrawn. However, 10.4% of the women felt satisfied and well. These negative psychological feelings were statistically significant. Concerning the effect on marriage, the closeness of couples was reduced significantly in 57.7%, 62.8% had reduced coital intimacy, 79.3% had frequent quarrel
