Comparison of Air Pollution−Mortality Associations Using Observed Particulate Matter Concentrations and Reanalysis Data in 33 spanish Cities

Air pollution poses a health hazard in all ***,complete data on ambient particulate matter(PM)concentrations are not available in all world *** data is already a valuable source of exposure data in epidemiological studies examining the relationship between temperature and ***,the performance of reanalysis data in assessing the short-term health effects of particulate air pollution remains *** assessed the performance of CAMS reanalysis(EAC4)data from the European Centre for Medium-Range Weather Forecasts,compared with daily PM concentrations from field monitoring stations,to estimate short-term exposure to PM with an aerodynamic diameter less than 10μm(PM_(10))on daily mortality in 33 spanish provincial capital cities using a two-stage time series regression *** shape of the PM_(10)distribution varied substantially between PM observations and CAMS global reanalysis of atmospheric composition(EAC4)reanalysis data,with correlation ranging from 0.21 to *** pooled mortality risk for a 10μg/m^(3)increase in PM_(10)showed similar estimates using PM concentrations{relative risks(RR)=1.007,95%confidence intervals(95%CI)=[1.002,1.011]}and EAC4 reanalysis data(RR=1.011,95%CI=[1.006,1.015]).However,the city-specific PM_(10)beta coefficients estimated using PM concentrations and EAC4 reanalysis data showed a low correlation(r=0.22).The use of reanalysis data should be approached with caution when assessing the association between particulate matter air pollution and health outcomes,particularly in cities with small populations.

Impact of socioeconomic and health-related factors on consumption of homeopathic and natural remedies in Spain in 2006, 2011 and 2017

Objective: Complementary and alternative medicine use and type of use may be influenced by sociodemographic and economic determinants through which we could identify characteristics of patients with greater trend to use it. This paper aims to describe the changes in the consumption of homeopathic and natural remedies in Spain for three time points in order to discern changes in rate of consumption, associated factors and whether their use has been affected by a period of economic ***: This study utilized 2006, 2011 and 2017 cross-sectional data from the spanish National health Survey, a nationally representative survey of the population aged more than 15 years old and resident in Spain. Independent bivariate and multivariate descriptive analyses for each of the 3 years studied were ***: The rate of consumption of both homeopathic and natural remedies has decreased over the periods studied. In spite of this decrease, the consumer profile appears to remain stable over the three periods. The sociodemographic factors associated with their consumption were being female, being 30–64 years old, being separated/divorced, having higher education qualifications, being employed and belonging to a higher social class. Psychiatric morbidity, chronic health problems such as pain, mental health problems or malignant tumors, and absence of major cardiovascular events were the clinical factors ***: It can be concluded that beyond the economic situation, the use of homeopathic and natural remedies obeys to the needs of the patients related to their state of health and the response they receive from the health system. It may be that women have different needs and expectations of the healthcare system and, given this breach of expectations, seek remedy to alleviate their needs outside the system and conventional medicine.

Climate change has increased interest in the effects of the thermal environment on cardiovascular *** studies have focused on mortality ***,pre-hospital care data are better able to evaluate these effects,as they can register the full spectrum of the disease in real *** scoping review aims to synthesize the epidemiological evidence regarding the effects of the thermal environment on cardiovascular morbidity in the pre-hospital setting,evaluated through ambulance calls.A staged literature search was performed using the PubMed database for the period between 1st January 2000 and 30th March 2023,using the MeSH terms“Weather”AND“Emergency Medical Services”.A total of 987 publications were identified that examined the correlation between the thermal environment and ambulance call-outs for cardiovascular *** studies were mostly ecological time series,with significant variability in the methodological aspects *** increase in the number of ambulance call-outs has been observed in association with low temperatures,both for overall cardiovascular pathologies and for certain pathological *** high temperatures,no effect has been observed in overall call-outs,although an increase has been observed during heat *** demand for ambulances for cardiac arrests is increased by both low and high temperatures and during heat *** call-outs for cardiovascular causes increase with low temperatures and heat waves,with no significant increase in the overall demand associated with high *** call-outs for cardiac arrests are the only subtype that is increased by high temperatures.

Analyzing predictors of graft survival in patients undergoing liver transplantation with donors aged 70 years and over

AIM To increase the number of available *** This is a single-center comparative analysis performed between April 1986 and May 2016. Two hundred and twelve liver transplantation(LT) were performed with donors ≥ 70 years old(study group). Then, we selected the first cases that were performed with donors < 70 years old immediately after the ones that were performed with donors ≥ 70 years old(control group).RESULTS Graft and patient survivals were similar between both groups without increasing the risk of complications, especially primary non-function, vascular complications and biliary complications. We identified 5 risk factors as independent predictors of graft survival: recipient hepatitis C virus(HCV)-positivity [hazard ratio(HR) = 2.35; 95% confidence interval(CI): 1.55-3.56; P = 0.00]; recipient age(HR = 1.04; 95%CI: 1.02-1.06; P = 0.00); donor age X model for end-stage liver disease(D-MELD)(HR = 1.00; 95%CI: 1.00-1.00; P = 0.00); donor value of serum glutamic-pyruvic transaminase(HR = 1.00; 95%CI: 1.00-1.00; P = 0.00); and donor value of serum sodium(HR = 0.96; 95%CI: 0.94-0.99; P = 0.00). After combining D-MELD and recipient age we obtained a new scoring system that we called DR-MELD(donor age X recipient age X MELD). Graft survival significantly decreased in patients with a DR-MELD score ≥ 75000, especially in HCV patients(77% vs 63% at 5 years in HCV-negative patients, P = 0.00; and 61% vs 25% at 5 years in HCV-positive patients; P = 0.00). CONCLUSION A DR-MELD ≥ 75000 must be avoided in order to obtain the best results in LT with donors ≥ 70 years old.

Occupational determinants of continued smoking during pregnancy. INMA Valencia cohort study

To describe the smoking habits of the mothers participating in the INMA (INfancia y Medio Ambiente or Childhood and the Environment) cohort of Valencia and to determine which occupational factors are associated with smoking cessation during pregnancy. Cohort study comprising 855 pregnant women from health Service Areas 6 and 7 was set up in the Autonomous Region of Valencia, Spain. Data on smoking and smoking cessation during pregnancy, and on sociodemographic and work related variables were collected through structured questionnaires at the first and third trimester of pregnancy. Multivariate logistic regression models were constructed to assess factors related to smoking at the beginning of the pregnancy and to continued smoking. 787 women followed in the project and completed all the data. Among them 40.9% smoked at the beginning of pregnancy and 56.2% continued until gestational week 32. Time and effort spent on housework was associated with smoking at the beginning of pregnancy while stress in the workplace related variables contributed to both the prevalence of smoking at the beginning of the pregnancy and the persistence in the habit throughout pregnancy. We found a high prevalence of smoking among the pregnant mothers. Both, conditions in the workplace, as well as the burden of housework were found to be risk factors for smoking and not quitting during pregnancy. These factors should be taken into account when developing health programs designed to encourage the incorporation of healthy habits into the lives of pregnant women.

Dear Editor,In the European region,which shares 22.8%of the global cancer burden for 10%of the global population,there were around 4.4 million new cancer cases and 1.9 million deaths from cancer in 2020[1].The reasons for the high cancer incidence rates are complex;however,diet and dietary components are among the main contributors to cancer risk[2].In modern-day living,a growing proportion of people include in their diets ultra-processed *** of food processing and home-prepared foods are so-called dietary advanced glycation endproducts(AGEs),which are reactive metabolites emerging during the breakdown of reducing *** production is preponderant in dry high-heat processes(e.g.,baking,roasting);hence foods such as cakes,crisps,crackers,cereal products,meat and meat-derived products represent a major source of dietary AGEs[3].

The reduction of nitrate to nitrite by the oral microbiota has been proposed to be important for oral health and results in nitric oxide formation that can improve cardiometabolic conditions. Studies of bacterial composition in subgingival plaque suggest that nitrate-reducing bacteria are associated with periodontal health, but the impact of periodontitis on nitrate-reducing capacity(NRC)and, therefore, nitric oxide availability has not been evaluated. The current study aimed to evaluate how periodontitis affects the NRC of the oral microbiota. First, 16S rRNA sequencing data from five different countries were analyzed, revealing that nitratereducing bacteria were significantly lower in subgingival plaque of periodontitis patients compared with healthy individuals(P healthy individuals. Salivary NRC was found to be diminished in periodontal patients before treatment(P healthy levels 90 days post-treatment. Additionally, the subgingival levels of nitrate-reducing bacteria increased after treatment and correlated negatively with periodontitis-associated bacteria(P health should be explored in future studies.
