Clinical pathological characteristics of“crawling-type”gastric adenocarcinoma cancer:A case report

BACKGROUND Gastric cancer(GC)is a significant health problem worldwide,and early detection and accurate diagnosis are crucial for improving patient outcomes.Crawling-type gastric adenocarcinoma is a rare subtype of GC that has unique histopathological and clinical characteristics,and its diagnosis and management can be challenging.This pathological type of GC is also rare.CASE SUMMARY Here,we report the case of a patient who underwent ordinary endoscopy,na-rrow-band imaging,and endoscopic ultrasonography intending to determine the extent of tumor invasion and upper abdominal enhanced computed tomography and whether there was tumor metastasis.Then,endoscopic submucosal dissection was performed.After pathological and immunohistochemical examination,the pathological diagnosis was crawling-type gastric adenocarcinoma.This is a very rare and special pathological type of tumor.This case highlights the importance of using advanced endoscopic techniques and pathological examination in diagnosing and managing gastric crawling-type adenocarcinoma.Moreover,the findings underscore the need for continued research and clinical experience in this rare subtype of GC to improve patient outcomes.CONCLUSION The“crawling-type”GC is a rare and specific tumor pathology.It is difficult to identify and diagnose gliomas via endoscopy.The tumor is ill-defined,with a flat appearance and indistinct borders due to the lack of contrast against the background mucosa.Pathology revealed that the tumor cells were hand-like,so the patient has diagnosed with“crawling-type”gastric adenocarcinoma.

Autism spectrum disorder(ASD)is a highly heritable neurodevelopmental disorder characterized by deficits in social interactions and repetitive behaviors.Although hundreds of ASD risk genes,implicated in synaptic formation and transcriptional regulation,have been identified through human genetic studies,the East Asian ASD cohorts are still under-represented in genome-wide genetic studies.Here,we applied whole-exome sequencing to 369 ASD trios including probands and unaffected parents of Chinese origin.Using a joint-calling analytical pipeline based on GATK toolkits,we identified numerous de novo mutations including 55 high-impact variants and 165 moderate-impact variants,as well as de novo copy number variations containing known ASD-related genes.Importantly,combined with single-cell sequencing data from the developing human brain,we found that the expression of genes with de novo mutations was specifically enriched in the pre-,post-central gyrus(PRC,PC)and banks of the superior temporal(BST)regions in the human brain.By further analyzing the brain imaging data with ASD and healthy controls,we found that the gray volume of the right BST in ASD patients was significantly decreased compared to healthy controls,suggesting the potential structural deficits associated with ASD.Finally,we found a decrease in the seed-based functional connectivity between BST/PC/PRC and sensory areas,the insula,as well as the frontal lobes in ASD patients.This work indicated that combinatorial analysis with genome-wide screening,single-cell sequencing,and brain imaging data reveal the brain regions contributing to the etiology of ASD.

Circular RNA (circRNA) is a novel type of noncoding RNA that originates from eukaryotic precursor messenger RNA (pre-mRNA).1 These circRNAs regulate tumor processes through diverse mechanisms, including serving as a miRNA sponge to modulate expression of miRNA-target genes.2 Eukaryotic translation initiation factor 3 subunit C (eIF3c) is a core subunit of the IF3 complex, involving in the initiation of the translation process of tumor cells.3 Currently, little is known about the underlying molecular mechanisms of circEIF3C in the pathogenesis of intrahepatic cholangiocarcinoma (ICC). A better understanding of the biological functions of circEIF3C in response to tumor cellular growth and how this affects the tumor signaling pathways would provide important clinical value related to the pathogenesis of ICC.

This letter to the editor discusses the publication on gut microbiome supple-mentation as therapy for metabolic syndrome.Gut microbiome dysbiosis disrupts intestinal bacterial homeostasis and is related to chronic inflammation,insulin resistance,cardiovascular diseases,type 2 diabetes mellitus,and obesity.Previous research has found that increasing the abundance of beneficial microbiota in the gut modulates metabolic syndrome by reducing chronic inflammation and insulin resistance.Prebiotics,probiotics,synbiotics,and postbiotics are often used as supplements to increase the number of beneficial microbes and thus the produc-tion of short-chain fatty acids,which have positive effects on the gut microbiome and metabolic syndrome.In this review article,the author summarizes the available supplements to increase the abundance of beneficial gut microbiota and reduce the abundance of harmful microbiota in patients with metabolic disorders.Our group is also researching the role of the gut microbiota in chronic liver disease.This article will be of great help to our research.At the end of the letter,the mechanism of the gut microbiota in chronic liver disease is discussed.

Background:Multiparous women are at a higher risk of gestational diabetes mellitus(GDM) than primiparas.Physical activity during pregnancy has been shown to be beneficial for GDM,but there is little evidence on the association between physical activity and glycemic control among women with GDM,whether primiparas or multiparas.Thus,the objective of the present study was to examine the association between physical activity and glycemic control in women with GDM and to determine what,if any,effects result from number of parous events.Methods:A prospective cohort of 1162 women with GDM was recruited,with 604 multiparas(51.98%).The general linear model was used to calculate the risk difference and its 95% confidence interval(95%CI) to quantify the impact of parous events on glycemic control in pregnancy as well as the association between physical activity time and glycemic control.Results:Among 1162 women with GDM,the median daily activity time was 65 mm(interquartile range(IQR):45-90 min),and the abnormal plasma glucose(PG) percentage,calculated as number of abnormal PG tests divided by the total number of PG tests,was 40.00%(IQR:22.22%-66.67%).The percentage of abnormal PG was stabilized and statistically lower with daily physical activity time exceeding 60 min among primiparas(IQR:30.89%-44.43%) and exceeding 90 min among multiparas(ranged from 27.76% to 38.82%).After adjusting for potential confoumders,primiparas tended to have a lower percentage of abnormal PG than do multiparas(rate difference=-0.39,95%CI:-3.61 to2.84).The same amount of physical activity time was significantly less effective for multiparas than for primiparas(trend p-value <0.01).Conclusion:In women with GDM,being multiparous is associated with less effective glycemic control through physical activity,such that multiparas need more physical activity to achieve glycemic control at a similar level to primiparas.

目的:运用数据挖掘技术分析现代针灸治疗多囊卵巢综合征(PCOS)的经穴规律,挖掘治疗PCOS的潜力穴位,为针灸治疗PCOS提供选穴思路和参考.方法:收集截至2018年4月针灸治疗PCOS的相关文献,利用MicrosoftExcel 2007软件自编数据挖掘程序,建立针灸治疗PCOS的数据库,进行穴位的使用频次及穴位组合的关联分析,并进行聚类分析.结果:最终纳入92篇文献,使用频次最高的穴位、经络和部位分别为三阴交、任脉和胸腹部,关联规则结果显示:置信度最高的穴位组合为三阴交和足三里,支持度最高的穴位组合是三阴交和关元.聚类分析结果显示:存在5个有效聚类群,即关元、三阴交;中脘、天枢;中极、足三里、气海;血海、肾俞、太冲和归来;以及肝俞、太溪、阴陵泉和脾俞.结论:根据置信度和支持度结果,推荐针灸治疗PCOS时选用三阴交加足三里,或三阴交加关元,并依据不同证型加用配穴,如加用中脘、天枢以健脾除湿;加用中极、气海以益气养血;加用血海、肾俞、太冲和归来以补肾化瘀;加用肝俞、太溪、阴陵泉、脾俞以益肾补脾调肝.

Mild traumatic brain injury(mTBI)-induced post-traumatic headache(PTH)is a pressing public health concern and leading cause of disability worldwide.Although PTH is often accompanied by neurological disorders,the exact underlying mechanism remains largely unknown.Identifying potential biomarkers may prompt the diagnosis and development of effective treatments for mTBI-induced PTH.In this study,a mouse model of mTBI-induced PTH was established to investigate its effects on cerebral structure and function during short-term recovery.Results indicated that mice with mTBI-induced PTH exhibited balance deficits during the early post-injury stage.Metabolic kinetics revealed that variations in neurotransmitters were most prominent in the cerebellum,temporal lobe/cortex,and hippocampal regions during the early stages of PTH.Additionally,variations in brain functional activities and connectivity were further detected in the early stage of PTH,particularly in the cerebellum and temporal cortex,suggesting that these regions play central roles in the mechanism underlying PTH.Moreover,our results suggested that GABA and glutamate may serve as potential diagnostic or prognostic biomarkers for PTH.Future studies should explore the specific neural circuits involved in the regulation of PTH by the cerebellum and temporal cortex,with these two regions potentially utilized as targets for non-invasive stimulation in future clinical treatment.

On April 27,2022,an international flight KL857 from Amsterdam,the Netherlands arrived at Pudong International Airport,Shanghai Municipality.Passengers were transferred to the quarantine hotel for a routine 14-day medical observation in songjiang District and regularly tested for severe acute respiratory syndrome coronavirus 2(SARS-CoV-2).One of the passengers(a 37-year-old Chinese male)was reported positive and diagnosed as a mild case on April 29.The case set out from Uganda(flight KL535)on April 25 and transferred at Amsterdam,the Netherlands on April 26 and Seoul,the Republic of Korea(KL857)on April 27.The patient has been vaccinated in four doses against coronavirus disease 2019(COVID-19)(Beijing institute of Biological Products Co.,Ltd)in China and Uganda.After diagnosis,he was transferred to Shanghai Public Health Clinical Center for treatment.He recovered after treatment and was discharged on May 12.

We present here the development of cholesterol(Chol)-modified dendrimer system for targeted chemotherapy of folate(FA)receptor-expressing cancer cells. In our study, poly(amidoamine)(PAMAM) dendrimers of generation 5(G5) were functionalized stepby-step with Chol, fluorescein isothiocyanate(FI), and FA via a poly(ethylene glycol)(PEG) spacer(PEG-FA), and then acetamide to shield their remaining surface amines. The synthesized G5.NHAc-Chol-FI-PEG-FA(for short, G5-CFPF) dendrimers were utilized to encapsulate 10-hydroxycamptothecin(HCP), a hydrophobic anticancer drug. We find that each G5-CFPF dendrimer can encapsulate 13.8 HCP molecules. The complexes show a slower release profiles of HCP in a pH-dependent manner than the control complexes formed using the same dendrimers without Chol under the same conditions. Thanks to the targeting role played by FA, the complexes display a specific inhibition efficacy to FA receptor-expressing cervical cancer cells. The designed Chol-modified dendrimers may be adopted as a promising carrier for application in targeted cancer therapy.
