An Investigation and Suggestions for the Improvement of Brain Death Determination in China

Background: Brain death is the irreversible cessation of the function of the brain including the brainstem. In 2013, the Brain Injury evaluation Quality Control centre (BQCC) of the National Health and Family Planning Commission issued criteria and practical guidelines for the determination of brain death. This study aimed to evaluate whether the institutions have adopted these guidelines and to make suggestions for the improvement of the current criteria and practical guidelines for brain death determination in China. Methods: Consecutive brain death cases from 44 hospitals were evaluated for summary statistics for the following data: the performance of BQCC criteria and practical guidelines, clinical examination, apnea testing, ancillary testing, and the number of examinations as well as the waiting periods between examinations and details of who determined brain death. Data analysis was conducted from January 2013 to December 2017. Results: A total of 550 cases were obtained. All patients were determined to have deep coma and met the prerequisites for clinical testing. The performance rates of four brainstem reflex examinations (except cough reflex) ranged from 97.5% to 98.0%, and the completion rate as well as the coincidence rate were both 100.0%. The 238 cases (50.7%) completed apnea testing, and 231 cases (42.0%) had to stop apnea testing during the examination because of instability. The performance rates of the three ancillary tests, including electroencephalogram, short-latency somatosensory evoked potential, and transcranial Doppler, were 89.5%, 67.5%, and 79.5%, respectively;furthermore, the coincidence rates were 98.6%, 96.5%, and 99.5%, respectively. The combination of two ancillary tests was more accurate than one single ancillary test. A total of 401 (72.9%) cases successfully underwent two separate examinations to determine brain death with at least a 12-h waiting period. A scheme on automated test data generation and its evaluation

By analyzing some existing test data generation methods, a new automated test data generation approach was presented. The linear predicate functions on a given path was directly used to construct a linear constrain system for input variables. Only when the predicate function is nonlinear, does the linear arithmetic representation need to be computed. If the entire predicate functions on the given path are linear, either the desired test data or the guarantee that the path is infeasible can be gotten from the solution of the constrain system. Otherwise, the iterative refining for the input is required to obtain the desired test data. Theoretical analysis and test results show that the approach is simple and effective, and takes less computation. The scheme can also be used to generate path-based test data for the programs with arrays and loops.

Introduction: Oesophageal mycosis (OM) is one of the most common opportunistic infections in patients infected with HIV (Human Immunodeficiency Virus). However, this condition is increasingly observed in immunocompetent subjects. The aim of this study was to determine the endoscopic prevalence, clinical characteristics and risk factors for the occurrence of oesophageal mycosis in our department. Patients and Method: This was a prospective cross-sectional study of all patients who underwent oeso-gastroduodenal fibroscopy during the period from 1st January to 31st December 2022, i.e. one year, at the digestive endoscopy unit of the hepato-gastroenterology department of the Donka CHU national hospital in Conakry. All patients found to have oesophageal mycosis by FOGD were included. The endoscopy was performed using appropriate equipment: A Fujinon 4400 video endoscopy column;Three Fujinon EG 590 video gastroscopes;A hoover;Data were collected using a pre-established survey form and analysed using Epi info software version 6.0.4;Pearson’s Chi2 test as a test of independence and the exact 5% threshold ficher test. Results: Out of 1343 upper gastrointestinal endoscopies performed, 107 cases of oesophageal mycosis were found, representing a prevalence of 7.96%. The mean age was 40 years, with a male predominance of 55.42%. The sex ratio M/F was 1.24. The 45 and over age group was the most affected, with a prevalence of 40.43%, followed by the [35 - 45] age group, with a prevalence of 22.43%. Clinical symptoms were dominated by epigastralgia in 74.76% of cases, followed by odynophagia in 37.38% of cases, nausea and vomiting in 28.03% of cases, and pyrosis in 26.16% of cases. Oesophageal mycosis without oesophagitis was the most common endoscopic finding in 70% of cases. The main associated endoscopic lesions were erythemato-erosive and congestive gastropathy in 28.03% of cases, peptic oesophagitis (9.34%) and gastric ulcer (5.60%). The main risk factor

Effects of external temperature and dead volume on laboratory measurements of pore pressure and injected volume in a rock fracture

The accurate evaluation of pore pressure and injected volume is crucial for the laboratory characterization of hydromechanical responses of rock fractures. This study reports a series of laboratory experiments to systematically demonstrate the effects of external temperature and dead volume on laboratory measurements of pore pressure and injected volume in a rock fracture. We characterize the hydraulic aperture of the fracture as a function of effective normal stress using the exponential aperture *** model is then employed to predict the pore pressure change and injected volume in the fracture without the influences of external temperature and dead volume. The external temperature changes in the cyclic loading test due to the Joule-Thompson effect for fluids. The effect of external temperature on pore pressure change in the fracture can be well explained by thermal pressurization of fluids. Our results also show that the external dead volume can significantly lower the pore pressure change in the fracture during the cyclic loading test under undrained conditions. The injected volume can also be substantially enlarged due to the external dead volume in a typical pore pressure system. Internal measurement of the pore pressure in the fracture using a fiber optic sensor cannot exclude the influences of external temperature and dead volume, primarily because of the good hydraulic communication between the fracture and pore pressure system. This study suggests that the effects of external temperature and dead volume on pore pressure response and injected volume should be evaluated for accurate laboratory characterization and inter-laboratory comparison.

Optimization of Three-Dimensional Culture Conditions of HepG2 Cells with Response Surface Methodology Based on the VitroGel System

Minimizing FWM Impact in DWDM ROF DP-DQPSK System for Optical

The demonstration of a higher data rate transmission system was amajor aspect to be considered by researchers in recent years. The most relevantaspect to be studied and analyzed is the need for a reliable system to handlenonlinear impairments and reduce them. Therefore, this paper examines theinfluence of Four-Wave Mixing (FWM) impairment on the proposed highdata rate Dual polarization–Differential Quadrature phase shift keying (DPDQPSK)system using the Optisystem software. In the beginning, the impactof varied input power on the proposed system’s performance was evaluated interms of QF and BER metrics. More power is used to improve system ***, increasing power would raise theFWMeffects. Accordingly,a−10dBminput power and the proposed system are used to reduce the impactof FWM. Additionally, a hybrid amplification method is proposed to enhancesystem performance by utilizing the major amplification methods of erbiumdopedfiber amplifier (EDFA): semiconductor optical amplifier (SOA) andRadio optical amplifier (ROA). The evaluation demonstrates that the OAEDFAoutperformed the other two key amplification techniques of (EDFASOA)and (EDFA-ROA) in improving Quality factor (QF) and Bit error rate(BER) system results for all distances up to 720 km. Consequently, the methodcontributes to minimizing the impact of FWM. In the future, other forms ofnonlinearity will be investigated and studied to quantify their impact on theproposed system.
