Sandfly fever is a febrile emerging arboviral illness which is a huge threat posed to public *** fever viruses(Sicilian,Naples,and Toscana)are endemic to the Mediterranean *** suggests an increase in the activity of sandfly-borne phleboviruses and their extension to other *** infection is relatively benign but may cause severe clinical manifestations and *** fever viruses belongs to the genus Phlebovirus(family Bunyaviridae),and the four main serotypes include sandfly fever Sicilian virus,sandfly fever Naples virus,sandfly fever Cyprus virus,and Toscana *** fever has flu-like symptoms such as fever,headache,photophobia,malaise,myalgia,and retro-orbital *** incubation period of the disease is 4-8 days,and the patient usually recovers within a *** first infection provides the host with *** fever viruses are transmitted through the bites of infected female sandflies,thus,the epidemiology of sandfly fever is closely related to the presence and activity of sandfly species that act as the vectors for *** and outbreaks of the disease have been reported worldwide for more than 100 *** in all,this disease is important for non-endemic countries because it is a travel-related and/or imported illness.

We have presented an integrated approach based on supervised and unsupervised learning tech- nique to improve the accuracy of six predictive models. They are developed to predict outcome of tuberculosis treatment course and their accuracy needs to be improved as they are not precise as much as necessary. The integrated supervised and unsupervised learning method (ISULM) has been proposed as a new way to improve model accuracy. The dataset of 6450 Iranian TB patients under DOTS therapy was applied to initially select the significant predictors and then develop six predictive models using decision tree, Bayesian network, logistic regression, multilayer perceptron, radial basis function, and support vector machine algorithms. Developed models have integrated with k-mean clustering analysis to calculate more accurate predicted outcome of tuberculosis treatment course. Obtained results, then, have been evaluated to compare prediction accuracy before and after ISULM application. Recall, Precision, F-measure, and ROC area are other criteria used to assess the models validity as well as change percentage to show how different are models before and after ISULM. ISULM led to improve the prediction accuracy for all applied classifiers ranging between 4% and 10%. The most and least improvement for prediction accuracy were shown by logistic regression and support vector machine respectively. Pre-learning by k- mean clustering to relocate the objects and put similar cases in the same group can improve the classification accuracy in the process of integrating supervised and unsupervised learning.

Aim: The first purpose is to survey the cumulative trauma disorder (CTD) prevalence in 4 zones of neck, shoulder, hand/wrist and back, and the second purpose is to survey the risk factors producing upper limb musculoskeletal disorders by using OCRA index method. In this analytic-descriptive and cross-sectional study, incorporation of four data collection methods was contained;observing and interviewing methods, using questionnaire and check list, were utilized. Randomly, 384 samples for CTD prevalence and 423 samples for OCRA index exposure survey were collected by cluster ratio sampling. In this study, findings indicated that CTD prevalence was high in studied bakery workers and most disorder and pain were reported in back, knees and hand/wrist. Through different tasks, most percentage of neck CTD (7.1%) and shoulder CTD (27.8%) was related to selling and baker tasks respectively, and most percentage of hand/wrist CTD (33/3%) and back CTD (38.1%) was related to bread baker task. There was a significant correlation between shoulder, hand/wrist and back CTD with bakery and task type, and effect of job background on CTD preva-lence was improved in the above four zones. The rather high prevalence of CTD in the young pop-ulation with average age of 30 years which have a few work experiences and the results of per-formed risk assessment can be taken account of serious warning, if it is not considered.

After controls,including engineering and management,theﬁnal way to control noise is to use hearing protection *** ofastandardized questionnaireregarding investigatingworkers’use of hearing protection devices on the basis of the BASNEF behavioral model,the present study was conducted to investigate the effect of health education based on the BASNEF model on the use of hearing protection devices in workers of an auto-mobile manufacturing plant in *** quasi-experimental and prospective intervention study was performed on 80 workers at an automobile manufacturing plant who are exposed to noise levels above 85 decibels and donot use hearing protection *** this study,40 people working in a cast iron foundry were selected as the inter-vention group,and 40 working in aluminum casting were chosen as the control *** were ana-lyzed at the beginning of the intervention and three months after the intervention in the intervention and control *** was no signiﬁcant difference between the intervention and control groups before the *** was a signiﬁcant difference between the intervention and control groups after the intervention in the area of knowledge.A signiﬁcant correlation was observed between the intervention and control groups after the inter-vention program in all areas of the BASNEF educational model except behavioral *** this study,the effect of educational intervention on the use of hearing protection devices was investigated,and with the educa-tional intervention,it was tried to get help from inﬂuential people and enabling factors in education based on the BASNEF *** results showed that the educational intervention based on the BASNEF model can improve the knowledge of individuals in both the intervention and control groups.

health-related quality of life (HRQOL) has not been investigated among Iranian industrial workers. The present paper aimed to study the influence of the socio-demographic, health and work-related factors on HRQOL among Iranian industrial workers. In this cross-sectional study, participants were 280 workers of two factories. The Persian version of World health Organization Quality of Life-Brief (WHOQOL-BREF) was used to assess the HRQOL. A questionnaire was developed to assess the socio-demographic, health and work-related factors. Results showed that the means (SD) of physical health, psychological health, social relationships, and environment domains of HRQOL were 13.2 (2.7), 13.3 (2.6), 14.2 (3.5) and 12.6 (2.5), respectively. A multiple linear regression showed that types of job, exercise activity, working schedule, sleep quality, smoking, and conflict between work and social life were significantly associated with physical health domain;whereas, working schedule, marital status, working demand, sleep quality, BMI, and conflict between work and individual life were significantly associated with psychological health domain. Working schedule, working demand, sleep quality, conflict between work and individual life, and having children over two years were significantly associated with social relationship domain;however, working demand, working schedule, smoking, sleep quality, working hour, job satisfaction, marital status and exercise activity were significantly associated with environment domain. Collectively, work-related factors including unhealthy working conditions, unsafe working environments, long working hours, irregular working schedules, and the lack of occupational training may negatively influence the HRQOL of workers. To improve workers’ HRQOL, intervention programs should focus on improving work environment, working schedule, occupational training and restricting working hours.

Background: Kind of delivery affect many aspects of individuals, families and health systems situations. The aim of this study was to identify the Role of non-medical factors on choice of delivery (CS/NVD) in hospitals affiliated to Tehran University of Medical Sciences. Materials and Method: This cross-sectional study was performed to identify the factors involved in the choice of delivery (CS/NVD) among women attending hospitals affiliated to Tehran University of Medical Sciences. Data was derived from the medical records of women who gave birth in the hospitals. Results: A significant statistical relationship was found between factors such as age of mother, level of education, occupational status, type of previous delivery, person supervising the pregnancy and dissatisfaction about delivery were more frequent in women who underwent CS than those who gave birth by NVD. Conclusion: The highly increasing rise in the rate of unnecessary CS during throughout the world, and its adverse effect on maternal and child health, the financial burden imposed on families and health systems, has highlighted the importance of studies to identify the non-medical factors that affect decision-making concerning type of delivery as well as to determine the appropriate medical indications of CS.

Background:Avian influenza(AI)is a global public health *** the knowledge that butchers have about it and the precautionary practices they take against it is crucial for designing future preparedness *** study aimed to identify the social determinants of knowledge and precautionary measures of AI among butchers in the Kathmandu district in ***:The study was based on a cross-sectional study design using structured interview questionnaires and checklists to observe social determinants and the precautionary measures of 120 butchers aged 15 years and above from the Kathmandu ***:The majority of the respondents were male(69.2%)and more than half(53.3%)were from the age group of 25-39 years(mean:31.08,SD:±9.82).Nearly two-thirds(61.3%)of the respondents had a‘poor knowledge’,and the remaining had‘some knowledge’,about *** than half(55.4%)of the respondents were in the category of displaying‘poor practice’towards AI and the remaining half were in the‘satisfactory practice’*** of the respondents had‘adequate knowledge’or displayed‘good practice’.The respondents in the>25 years of age group were less likely[OR 0.169;95%CI(0.056-0.512)]compared to those in the<25 years age group to have a poor knowledge about AI;and the respondents with‘primary education’were more likely[OR 3.265;95%CI(1.326-8.189)]to have a poor knowledge about AI as compared to those who had a secondary or above level of *** who did not know the correct definition of AI were more likely to follow poor practices[OR 4.265;95%CI(1.193-15.242)];and the respondents who did not know the risk groups associated with AI were also more likely to follow poor practices[OR 3.103;95%CI(1.191-8.083)].Conclusion:This study points out the need to address butchers to improve their knowledge of,and more importantly their compliance with,the precautionary measures to prevent avian influenza.

Comparison between Azithromycin and Cephalexin for Preventing Infection after Cesarean Section in Obese Patient

Introduction: Postcesarean infection can cause maternal mortality and morbidity. Use of prophylactic antibiotics could decrease surgical site infection. Despite using prophylactic antibiotics in obese women, compared to normal weighted women the rate of wound infection is higher in this group. The aim of this study is to compare prophylactic effect of azithromycin and cephalexin on febrile morbidity and postcesarean infection in women with BMI > 30. Methods and Materials: This randomized controlled double blind clinical trial was done on 231 women with BMI > 30 who underwent elective cesarean section. Patients were randomly divided to intervention and control groups. Keflin 2 gr IV was administered 0.5 hr before surgery for both intervention and control groups. Cefalexin (placebo) every 6 hr and azythromycin (placebo) every 12 hr for control group and cephalexin 500 mg every 6 hr and azithromycin 250 mg every 12 hr for intervention group were administered for 48 hrs. Patients were under observation till 1 month after surgery. Fever, wound infection, endometritis and hospitalization were compared between two groups with SPSS v. 18. Results: A total of 231 patients 113 (48.9%) were enrolled in intervention group and 118 (51.1%) in control group with mean age of (28.53 ± 5.51) recruited. BMI distribution did not differ in the two groups. Hospitalization in control group was significantly higher than that in intervention group (2.58 ± 0.99) vs (2.11 ± 0.45) (P value < 0.001). BMI in intervention group with and without fever was (34.62 ± 2.64) and (30.89 ± 2.80), respectively (P value < 0.001). In control group, BMI in patients with and without fever was (38.60 ± 2.80) and (31.29 ± 1.28), respectively (P value = 0.001). Fever and endometritis simultaneously was seen in 3 (2.7%) of intervention group and 8 (6.8%) of control group.
