A new perspective on intervertebral disc calcification—from bench to bedside

Disc degeneration primarily contributes to chronic low back and neck ***,there is an urgent need to understand the spectrum of disc degeneration phenotypes such as fibrosis,ectopic calcification,herniation,or mixed *** these phenotypes,disc calcification is the least *** calcification,by definition,is the pathological mineralization of soft tissues,widely studied in the context of conditions that afflict vasculature,skin,and ***,disc calcification is associated with poor surgical outcomes and back pain refractory to conservative *** is frequently seen as a consequence of disc aging and progressive degeneration but exhibits unique molecular and morphological characteristics:hypertrophic chondrocyte-like cell differentiation;TNAP,ENPP1,and ANK upregulation;cell death;altered Pi and PPi homeostasis;and local *** studies in mouse models have provided a better understanding of the mechanisms underlying this *** is essential to recognize that the presentation and nature of mineralization differ between AF,NP,and EP ***,the combination of anatomic location,genetics,and environmental stressors,such as aging or trauma,govern the predisposition to ***,the systemic regulation of calcium and Pi metabolism is less important than the local activity of PPi modulated by the ANK-ENPP1 axis,along with disc cell death and differentiation *** there is limited understanding of this phenotype,understanding the molecular pathways governing local intervertebral disc calcification may lead to developing disease-modifying drugs and better clinical management of degeneration-related pathologies.

MENSTRUAL HEALTH Menstrual health is a general biological marker for many cisgender women,transgender men and non-binary *** more than half of the population being people who menstruate,stigma,lack of conversation and pressing social needs around menstrual health persists throughout medicine.1 Discussions around menstruation and menstrual management can be difficult for individuals,whether it is with friends or family,or in the healthcare setting.1 Patients who have never discussed menstruation with a clinician may not know what is healthy,assume that an abnormal experience is normal and may endure periods that negatively affect their life,career or well-being.2–5 Menstruation plays a vital role in overall well-being and contributes significantly to an individual’s quality of *** their scope of care,family medicine clinicians are poised to identify red-flag menstrual symptoms in their routine visits with patients,reducing time to diagnosis of menstrual *** urge family medicine clinicians to have renewed conversations surrounding menstrual health with their *** purpose of this report is to supply a brief overview of the importance of menstrual communication in primary care and serve as a resource to enhance menstrual communication between patient and clinician,with the ultimate goal of decreasing menstrual stigma and promoting improved menstrual health and experiences for patients.

This work provides a theoretical investigation into the strong coupling between a single quantum emitter(QE)and the surface plasmons of sodium metals in two representative plasmonic systems,i.e.,the semi-infinite metal-dielectric interface and the metal nanoparticles(NPs)of monomer/dimer *** both configurations,sodium metals exhibit distinctly stronger coupling strength and lower optical loss in the optical region than their noble metal counterparts,demonstrating the ideal candidate characteristics for single-molecule-level strong couplings with distinctly facile operation *** results provide new insights into extreme light-matter interactions with potential applications in quantum information,optical sensors,quantum chemistry,etc.

Mounting evidence suggests that insect hormones associated with growth and development also participate in pathogen *** have discovered a previously undescribed midgut transcriptional control pathway that modulates the availability of 20-hydroxyecdysone(20E)in a worldwide insect pest(Plutella xylostella),allowing it to defeat the major virulence factor of an insect pathogen Bacillus thuringiensis(Bt).A reduction of the transcriptional inhibitor(PxDfd)increases the expression of a midgut microRNA(miR-8545),which in turn represses the expression of a newly identified ecdysteroid-degrading glucose dehydrogenase(PxGLD).Downregulation of PxGLD reduces 20E degradation to increase 20E titer and concurrently triggers a transcriptional negative feedback loop to mitigate 20E *** moderately elevated 20E titer in the midgut activates a MAPK signaling pathway to increase Bt tolerance/*** findings deepen our understanding of the functions attributed to these classical insect hormones and help inform potential future strategies that can be employed to control insect pests.

No-reference image quality assessment based on AdaBoost_BP neural network in wavelet domain

Considering the relatively poor robustness of quality scores for different types of distortion and the lack of mechanism for determining distortion types, a no-reference image quality assessment(NR-IQA) method based on the Ada Boost BP neural network in the wavelet domain(WABNN) is proposed. A 36-dimensional image feature vector is constructed by extracting natural scene statistics(NSS) features and local information entropy features of the distorted image wavelet sub-band coefficients in three scales. The ABNN classifier is obtained by learning the relationship between image features and distortion types. The ABNN scorer is obtained by learning the relationship between image features and image quality scores. A series of contrast experiments are carried out in the laboratory of image and video engineering(LIVE) database and TID2013 database. Experimental results show the high accuracy of the distinguishing distortion type, the high consistency with subjective scores and the high robustness of the method for distorted images. Experiment results also show the independence of the database and the relatively high operation efficiency of this method.

In order to achieve the model-based fault monitoring and diagnosis,an accurate model for the rotor system is necessary to locate and quantify *** the dynamic characteristics of a blade-rotor system is influenced by foundation flexibility,the modeling and dynamic analyses on the foundation were sequentially ***,the effect of element size on the model convergence was investigated using the forward difference quotient as the slope of the frequency difference,which found that the model converged when the element size refined to ***,a modal analysis and a harmonic response analysis were performed to obtain the dynamic characteristics of the foundation ***,an optimization to the foundation utilizing an additional stiffener was conducted to reduce the foundation response and make the critical speed far away from the working frequency band of 20—50Hz.

Optimal timing and route of nutritional support after esophagectomy: A review of the literature

Some controversy surrounds the postoperative feeding regimen utilized in patients who undergo *** in practices during the perioperative period exists including the type of nutrition started,the delivery route,and its *** nutrition is essential for this patient population as these patients often present with weight loss and have altered eating patterns after surgery,which can affect their ability to regain or maintain *** of feeding after an esophagectomy include total parenteral nutrition,nasoduodenal/nasojejunal tube feeding,jejunostomy tube feeding,and oral *** evidence suggests that early oral feeding is associated with shorter LOS,faster return of bowel function,and improved quality of *** recovery pathways after surgery pathways after esophagectomy with a component of early oral feeding also seem to be safe,feasible,and cost-effective,albeit with limited ***,data on anastomotic leaks is mixed,and some studies suggest that the incidence of leaks may be higher with early oral *** risk of anastomotic leak with early feeding may be heavily modulated by surgical *** definitive data is currently available to definitively answer this question,and further studies should look at how these early feeding regimens vary by surgical *** review aims to discuss the existing literature on the optimal route and timing of feeding after esophagectomy.
