A multimodal integration pipeline for accurate diagnosis, pathogen identification, and prognosis prediction of pulmonary infections

Pulmonary infections pose formidable challenges in clinical settings with high mortality rates across all age groups *** diagnosis and early intervention are crucial to improve patient *** intelligence(AI)has the capability to mine imaging features specific to different pathogens and fuse multimodal features to reach a synergistic diagnosis,enabling more precise investigation and individualized clinical *** this study,we successfully developed a multimodal integration(MMI)pipeline to differentiate among bacterial,fungal,and viral pneumonia and pulmonary tuberculosis based on a real-world dataset of 24,107 *** area under the curve(AUC)of the MMI system comprising clinical text and computed tomography(CT)image scans yielded 0.910(95%confidence interval[CI]:0.904–0.916)and 0.887(95%CI:0.867–0.909)in the internal and external testing datasets respectively,which were comparable to those of experienced ***,the MMI system was utilized to rapidly differentiate between viral subtypes with a mean AUC of 0.822(95%CI:0.805–0.837)and bacterial subtypes with a mean AUC of 0.803(95%CI:0.775–0.830).Here,the MMI system harbors the potential to guide tailored medication recommendations,thus mitigating the risk of antibiotic ***,the integration of multimodal factors in the AI-driven system also provided an evident advantage in predicting risks of developing critical illness,contributing to more informed clinical *** revolutionize medical care,embracing multimodal AI tools in pulmonary infections will pave the way to further facilitate early intervention and precise management in the foreseeable future.

Vortex genesis over the Bay of Bengal in spring and its role in the onset of the Asian Summer Monsoon

Physical processes associated with onset of the 1998 Asian summer monsoon were examined in detail using multi-source datasets. We demonstrated that strong ocean-atmosphere-land interaction in the northern Indian Ocean and tropical Asian area during spring is a fundamental factor that induces the genesis and development of a monsoon onset vortex over the Bay of Bengal (BOB), with the vortex in turn triggering onset of the Asian summer monsoon. In spring, strong surface sensible heating over India and the Indochina Peninsula is transferred to the atmosphere, forming prominent in situ cyclonic circulation, with anticyclonic circulations over the Arabian Sea and northern BOB where the ocean receives abundant solar radiation. The corresponding surface winds along the North Indian Ocean coastal areas cause the ocean to produce the in situ offshore currents and upwelling, resulting in sea surface temperature (SST) cooling. With precipitation on the Indochina Peninsula increasing from late April to early May, the offshore current disappears in the eastern BOB or develops into an onshore current, leading to SST increasing. A southwest-northeast oriented spring BOB warm pool with SST >31°C forms in a band from the southeastern Arabian Sea to the eastern BOB. In early May, the Somali cross-equatorial flow forms due to the meridional SST gradient between the two hemispheres, and surface sensible heat over the African land surface. The Somali flow overlaps in phase with the anticyclone over the northern Arabian Sea in the course of its inertial fluctuation along the equator. The convergent cold northerlies on the eastern side of the anticyclone cause the westerly in the inertial trough to increase rapidly, so that enhanced sensible heat is released from the sea surface into the atmosphere. A hallmark of adaptive evolution is innovation in gene function,which is associated with the development of distinct roles for genes during plant evolution;however,assessing functional innovation over long periods of time is not *** buckwheat(Fagopyrum tataricum)originated in the Himalayan region and has been exposed to intense UV-B radiation for a long time,making it an ideal species for studying novel UV-B response mechanisms in ***,we developed a workflow to obtain a co-functional network of UV-B responses using data from more than 10,000 samples in more than 80 projects with multi-species and multiomics *** the entire network revealed that flavonoid biosynthesis was most significantly related to the UV-B ***,we found that the regulatory factor MYB4R1,which resides at the core of the network,has undergone *** vitro and in vivo experiments demonstrated that MYB4R1 regulates flavonoid and anthocyanin accumulation in response to UV-B in buckwheat by binding to L-box motifs in the FtCHS,FtFLS,and FtUFGT *** used deep learning to develop a visual discrimination model of buckwheat flavonoid content based on natural populations exposed to global UV-B *** study highlights the critical role of gene neofunctionalization in UV-B adaptation.

Fe(Ⅱ)is an essential trace element for anaerobic ammonium oxidation bacteria(AAOB)metabolism,and can improve the nitrogen removal efficiency of anaerobic ammonia oxidation(Anammox).Here we oper-ated two identical expanded granular sludge bed(EGSB)reactors at low temperature(15±3℃)for 154 *** 1(R_(1))received additional Fe(Ⅱ)(0.12 mmol/L)during the late startup phase,while reactor 0(R_(0))served as the control and did not receive extra Fe(Ⅱ).Nitrogen removal in R_(1)became stable at 55 d of operation,ten days earlier than R_(0).The nitrogen removal rate(NRR)of R_(1) was 1.64 kg N m^(−3)d^(−1)and its TN removal rate was as high as 89%,while R_(0)only reached 75%.The addition of Fe(Ⅱ)was fur-ther beneficial to aggregation and stability of the granular sludge,and the used sludge of both reactors showed enrichment for AAOB populations compared to the inoculum,for instance,increased abundance of Candidatus-Kuenenia and in particular of Candidatus-Brocadia(from 0.17%to 10.10%in R_(0)and 7.79%in R_(1)).Diverse microbial species and complex microbial network structure in R_(1)compared to R_(0)promoted the coupled denitrogenation by Anammox,dissimilatory nitrate reduction to ammonium(DNRA),nitrate-dependent Fe oxidation(NDFO),and ferric ammonium oxidation(Feammox).In addition,the microbial community in R_(1)was more resistant to short-term low temperature(2-7℃)starvation,illustrating a further positive effect of adding Fe(Ⅱ)during the startup phase of an Anammox reactor.

Analysis of typhoon wind hazard in Shenzhen City by Monte-Carlo Simulation

As one of the most serious natural disasters,many typhoons affect southeastern China every *** Shenzhen,a coastal city in southeast China as an example,we employed a Monte-Carlo simulation to generate a large number of virtual typhoons for wind hazard *** analyzing 67-year historical typhoons data from 1949 to 2015 using the Best Track Dataset for Tropical Cyclones over the Western North Pacific recorded by the Shanghai Typhoon Institute,China Meteorological Administration(CMASTI),typhoon characteristic parameters were extracted and optimal statistical distributions established for the parameters in relation to *** employed the Monte-Carlo method to sample each distribution to generate the characteristic parameters of virtual *** addition,the Yah Meng(YM)wind field model was introduced,and the sensitivity of the YM model to several parameters *** the YM wind field model,extreme wind speeds were extracted from the virtual *** extreme wind speeds for different return periods were predicted and compared with the current structural code to provide improved wind load information for wind-resistant structural design.

A suite of semi-idealized numerical experiments are conducted to investigate the sensitivity of tropical cyclone(TC) intensity to changes of sea surface temperature(SST) over different radial extents. It is found that the increase of inner SST within the range 1.5–2.0 times the radius of maximum wind(RMW), defined as the effective radius(ER), contributes greatly to the increase of TC intensity and the reduction of TC inner-core size, whereas the increase of outer SST(defined as SST outside the ER)reduces TC intensity and increases TC inner-core size. Further analysis suggests that,the effects of SST inside and outside the ER on TC intensity rely on the factors that influence the TC *** the SST increases inside the ER, more surface enthalpy flux enters TC eyewall and less enters the outer spiral rainbands. This will decrease the RMW, leading to a smaller eyewall radius where strong latent heating is released. As a result, the central pressure of the TC deepens with stronger radial pressure gradient. Meanwhile,difference between SST and upper-tropospheric temperature increases. All above factors contribute to TC intensification as the inner SST increases. The opposite happen as the SST increases outside the ER. How TC intensity responds to the change of entire SST depends on the competitive and opposite effects of inner and outer ***, understanding the mechanisms is vital to the forecast of variations in TC intensity and inner-core size when a TC comes across an ocean cold or warm pool.

Discussion on Health Management Model of Patients with Chronic Diseases

The new medical reform program puts forward new requirements for the prevention and control of chronic diseases and the construction of community health service system. Through the health management of patients with chronic diseases, the health management experience of chronic disease patients is summarized, including collecting data, establishing health records, assessing health risk factors, adopting health interventions, dietary interventions, exercise interventions, medication interventions, psychological interventions, and health education. It is believed that strengthening the health management of patients with chronic diseases can alleviate the suffering of patients, improve the quality of life of patients, and save medical resources.

climate CHANGE:LONG-TERM TRENDS AND SHORT-TERM OSCILLATIONS

Identifying the Northern Hemisphere （NH） temperature reconstruction and instrumental data for the past 1000 years shows that climate change in the last millennium includes long-term trends and varioas oscillations. Two long-term trends and the quasi-70-year oscillation were detected in the global temperature series tbr the last 140 years and the NH millennium series. One important feature was emphasized that temperature decreases slowly but it increases rapidly based on the analysis of different series. Benefits can be obtained of climate change from understanding various long-term trends and oscillations. Millennial temperature proxies from the natural climate system and time series of nonlinear model system are used in understanding the natural climate change and recognizing potential benefits by using the method of wavelet transform analysis. The results from numerical modeling show that major oscillations contained in numerical solutions on the interdecadal timescale are consistent with that of natural proxies. It seems that these oscillations in the climate change are not directly linked with the solar radiation as an external tbrcing. This investigation may conclude that the climate variability at the interdecadal timescale strongly depends on the internal nonlinear effects in the climate system.
