Lack of correlation between surface water area and infection with Pseudomonas aeruginosa and the non-tuberculous mycobacteria (NTMs) in patients with cystic fibrosis (CF)

Background People with cystic fibrosis(CF)may develop clinically significant chronic respiratory infections with Pseudomonas aeruginosa(PA)and non-tuberculous mycobacteria(NTM).Open water has been suggested to be an important source for continuous or intermittent exposure to these *** date,there has been a paucity of studies examining the relationship between chronic PA and NTM infection in CF patients and surfaces waters,including blue *** aim of this study was therefore to examine the relationship between chronic pulmonary infection with PA and NTMs in children and adults with CF in European countries and area of surface waters,including blue *** European CF registry data detailing incidence of chronic PA and NTM infection in adults and children with CF in Europe(n=41,486 in 24 European countries)was correlated with surface water area data from the same countries(approx.678,278 km2)employing Spearman *** Correlation of chronic PA infection in children and adults and surface water area were not significant(p=0.0680 and p=0.8448,respectively),as was NTM infection(p=0.7371 and p=0.0712,respectively).Conclusions Acquistion of PA and its avoidance in people with CF is a complicated dynamic,not solely driven by close association with surface water,but through the integration of several other factors,including mitigations by people with CF to avoid high risk scenarios with surface *** study was unable to demonstrate a correlation between PA and NTM infection in people with cystic fibrosis and surface water area at a national *** patients should continue to be vigilant about potential infection risks posed by water and take evidence-based decisions regarding their behaviour around water to protect them for acquiring these organisms from these sources.

A Scan of Obesogenic Environments and a Spatial Inference of Obesity Prevalence in Chinese children and Adolescents: Based on the Chinese Health and Nutrition Survey 2011 Data

Objective To identify the characteristics of Chinese obesogenic environments at a provincial level, infer a spatial distribution map of obesity prevalence in 31 provinces, and provide a foundation for development of policy to reduce obesity in children and adolescents. Methods After scanning obesity data on subjects aged 7-17 years from 12 provinces in the China Health and Nutrition Survey 2011 and environmental data on 31 provinces from the China Statistical Yearbook 2011 and other sources, we selected 12 predictors. We used the 12 surveyed provinces as a training sample to fit an analytical model with partial least squares regression and prioritized the 12 predictors using variable importance in projection. We also fitted a predictive model with Bayesian analysis. Results We identified characteristics of obesogenic environments. We fitted the predictive model with a deviance information criterion of 61.96 and with statistically significant（P 〈 0.05） parameter estimates of intercept [95% confidence interval（CI）： 329.10, 963.11], log（oil）（CI： 13.11, 20.30）, log（GDP）（CI： 3.05, 6.93）, log（media）（CI：-234.95,-89.61）, and log（washing-machine）（CI： 0.92, 5.07）. The total inferred average obesity prevalence among those aged 7-17 was 9.69% in 31 Chinese provinces in 2011. We also found obvious clustering in occurrences of obesity in northern and eastern provinces in the predicted map. Conclusion Given complexity of obesity in children and adolescents, concerted efforts are needed to reduce consumption of edible oils, increase consumption of vegetables, and strengthen nutrition, health, and physical activity education in Chinese schools. The northern and eastern regions are the key areas requiring intervention.

A high maintenance dose of clopidogrel improves short-term clinical outcomes in patients with acute coronary syndrome undergoing drug-eluting stent implantation

Background Recurrent ischemic events occurred even during routine use of 75 mg clopidogrel in addition to aspirin, that indicated a potentially insufficient maintenance dosage of clopidogrel. The aim of the present study was to evaluate the short-term efficacy and safety of a 150 mg maintenance dose of clopidogrel following a 600 mg loading dose in patients with an acute coronary syndrome （ACS） undergoing drug eluting stent （DES） implantation. Methods Between November 2005 and November 2006, a total of 813 consecutive ACS patients undergoing DES implantation were enrolled. A 600 mg loading dose was administered before percutaneous coronary intervention （PCI） and patients were randomized to receive clopidogrel 75 mg or 150 mg for 30 days in addition to 300 mg aspirin daily. Primary end points were the composite of cardiac death, non-fatal myocardial infarction （MI） and urgent target vessel revascularization （UTVR）. Secondary end points included stent thrombosis （ST）, major and minor bleeding events at 30 days. Results At a follow-up period of 30 days, 4 （1.0%） patients in the 150 mg group and 9 （2.2%） patients in the 75 mg group （P 〉0.05） reached the primary end points. There was no significant difference in the incidences of MI （0.5% vs 1.2%, P〉0.05）, UTVR （0.7% vs 2.0%, P 〉0.05）, and cardiac death （0.2% vs 0.2%, P 〉0.05） between the two groups. The incidence of ST （0 vs 1.5%, P 〈0.05） was significantly lower in the 150 mg group than that in the 75 mg group. There were no significant differences between both groups regarding the risk of major （0.2% vs 0, P 〉0.05） or minor （0.5% vs 0.2%, P 〉0.05） bleedings. Conclusion A high clopidogrel maintenance dose of 150 mg daily following a 600 mg loading dose for the first month after PCI procedure reduces the risk of ST and appears to be safe in patients with ACS undergoing DES implantation.

Endovascular stent-grafts for acute and chronic type B aortic dissection： comparison of clinical outcomes

Background Endovascular stent-graff treatment has emerged as an alternative for patients with type B aortic dissection （AD）, either at acute or chronic phase, in selected patients. This study aimed to investigate the results of endovascular stent-graft repair for acute and chronic type BAD. Methods From May 2002 to July 2007, 67 patients with type BAD were treated by endovascular stent-graft placement. There were 32 patients in the acute phase （AAD group） and 35 patients in the chronic phase （CAD group）. The patients were followed up from 1 to 65 months （average, 17_＋16 months）. The immediate and follow-up clinical outcomes were documented and compared between the 2 groups. Results Placement of endovascular stent-grafts across the primary entry tears was technically successful in all 67 patients. Compared with patients in the CAD group, those in the AAD group had higher percentages of pleural effusion （15.6% vs 0, P=-0.02） and visceral/leg ischemia （21.9% vs 2.9%, P=0.02）. Procedure related complications, including endoleak and post-implantation syndrome occurred more frequently in AAD group than in CAD group （21.9% vs 2.9% and 31.3% vs 8.6%, respectively; P=0.02 and P=0.02）. Kaplan-Meier analysis showed no significant difference in survival rate at 4 years between the 2 groups （86.4% vs 92.3%, P=0.42 by Log-rank test）. But the 4-year event-free survival rate was higher in patients with chronic dissection than in patients with acute dissection （96.2% vs 73.9%; P=0.02 by Log-rank test）. Conclusions Endovascular repair with stent-graff was safe and effective for the treatment of both acute and chronic type BAD. However, both immediate and long term major complications occurred more frequently in patients with acute dissection than in those with chronic dissection.
