Epidemiology and clinical characteristics of burns in China's Mainland from 2009 to 2018

Background:burn injuries place a heavy burden on the global healthcare ***,there is still a lack of nationwide studies on the epidemiological characteristics of burn patients in China's *** present study aims to accurately analyze the clinical characteristics of burn patients by collecting data in China's Mainland from 2009 to 2018,which will provide effective strategies for healthcare systems and the government in China's ***:Patients admitted for burn injuries to 196 hospitals in 31 provinces,autonomous regions and municipalities in China's Mainland from 2009 to 2018 were *** data collected included sex,age,month distribution,etiology,region,clinical outcome,injury anatomical location,total burn surface area and *** 19.0 software was used to analyze the ***:From 2009 to 2018,the burn patients were 333,995(0.76%),which included 222,480(66.61%)males and 111,515(33.39%)*** 2009 to 2018,the number of individuals admit-ted to hospitals for burns showed a downward trend year by *** burn patients accounted for the highest proportion of inpatients in 0-10 years(38.10%),followed by 40-50 years(13.14%).The highest cure ratio of burn inpatients was in the 20-30 age group(31394,71.53%).Among 31 provinces,autonomous regions and municipalities,the province with the highest proportion of total inpatients caused by burns was Inner Mongolia(4.61%),followed by Zhejiang(3.17%),Hainan(2.88%)and Xinjiang(2.64%).Summer(29.16%)was the season with the highest incidence of burn patients admitted to hospitals,followed by spring(25.6%).Scalding(60.19%)was the most frequent kind of burn treated,followed by fire(20.45%).The patients had multiple burn sites(68.89%)most often,followed by burns on the lower limbs(10.91%).From 0%to 10%total body surface area(TBSA)accounted for the highest ratio(37.19%),followed by 90-100%TBSA(21.74%).Conclusions:The present study is the first to describe the associated situation and trend

The validation study on a three-dimensional burn estimation smart-phone application: accurate, free and fast?

Background: Accurate total body surface area burned (TBSAB) estimation is a crucial aspect of early burn management. It helps guide resuscitation and is essential in the calculation of fluid requirements. Conventional methods of estimation can often lead to large discrepancies in burn percentage estimation. We aim to compare a new method of TBSAB estimation using a three-dimensional smart-phone application named 3D burn Resuscitation (3D burn) against conventional methods of estimation—Rule of Palm, Rule of Nines and the Lund and Browder chart. Methods: Three volunteer subjects were moulaged with simulated burn injuries of 25%, 30% and 35% total body surface area (TBSA), respectively. Various healthcare workers were invited to use both the 3D burn application as well as the conventional methods stated above to estimate the volunteer subjects’burn percentages. Results: Collective relative estimations across the groups showed that when used, the Rule of Palm, Rule of Nines and the Lund and Browder chart all over-estimated burns area by an average of 10.6%, 19.7%, and 8.3% TBSA, respectively, while the 3D burn application under-estimated burns by an average of 1.9%. There was a statistically significant difference between the 3D burn application estimations versus all three other modalities (pburn application, although slower, allowed more accurate TBSAB measurements when compared to conventional methods. The validation study has shown that the 3D burn application is useful in improving the accuracy of TBSAB measurement. Further studies are warranted, and there are plans to repeat the above study in a different centre overseas as part of a multi-centre study, with a view of progressing to a prospective study that compares the accuracy of the 3D burn application against conventional methods on actual burn patients.

Vitamin D status and its influence on outcomes following major burn injury and critical illness

Vitamin D deficiency is common among the general population. It is also observed in up to 76% of critically ill *** the high prevalence of hypovitaminosis D in critical illness, vitamin D is often overlooked by medical staff as the clinical implications and consequences of vitamin D deficiency in acute contexts remain to be fully *** D has a broad range of pleotropic effects on various processes and systems including the immuneinflammatory response. 1α,25-dihydroxyvitamin D (1,25(OH)2D), has been shown to promote a tolerogenic immune response limiting deleterious inflammatory effects, modulation of the innate immune system, and enhancement of anti-microbial peptides. Vitamin D deficiency is frequently observed in critically ill patients and has been related to extrinsic causes (i.e., limited sunlight exposure), magnitude of injury/illness, or the treatment started by medical doctors including fluid resuscitation. Low levels of vitamin D in critically ill patients have been associated with sepsis, organ failure, and mortality. Despite this, there are subpopulations of critical illness, such as burn patients, where the literature regarding vitamin D status and its influence on outcomes remain insufficient. Thermal injury results in damage to both burned and non-burned tissues, as well as induces an exaggerated and persistent immune-inflammatory and hypermetabolic response. In this review, we propose potential mechanisms in which burn injury affects the vitamin D status and summarizes current literature investigating the influence of vitamin D status on outcomes. In addition, wereviewed the literature and trials investigating vitamin D supplementation in critically ill patients and discuss the therapeutic potential of vitamin D supplementation in burn and critically ill patients. Palpebral Reconstruction in a Secondary Ophthalmology Center in Mali: About a Case

Introduction: Significant trauma to the periocular region can seriously damage ocular structures and their adnexa. The eyelids can be damaged during these traumas. The most frequent lesions are lacerations, the surgical treatment of which is generally simple. In some cases, there are complex traumas where there is a loss of tissue, which is difficult to treat. We report the case of a 26-year-old young man, farmer with no medical and surgical history, referred by the odontostomatology and maxillofacial surgery department for burns to the left hemi face by the exhaust pipe of his motorbike which occurred following a public road accident (AVP). Observation: A 26-year-old young man with no medical-surgical history, visual acuity was 5/10 with good mobility of the globe. In collaboration with the maxillofacial surgeon, a graft of the hemi face was performed using a flap from the inner side of the thigh first. In the second time we carried out a recovery by a flap which consisted in taking a supra-superciliary flap and suturing it at the level of the palpebral edges which, in spite of a fragile vitality of the tissues. Conclusion: The reconstruction of the eyelid is a real problem in our service because of the technical platform and the availability of consumables, which limits us in the choice of the operating technique.
