Comparison of post-urethroplasty complication rates in pediatric cases with hypospadias using Vicryl or polydioxanone sutures

Objective:Hypospadias is a common congenital problem among male newborns.Both rapid absorbable sutures(polyglactin,Vicryl)and delayed absorbable sutures(polydioxanone,PDO)are used in hypospadias repair based on the surgeon's preference.This study was conducted to compare post-urethroplasty complication rates in pediatric patients with hypospadias using Vicryl or PDO sutures.Methods:This is a retrospective study which was designed and performed on 583 children aged 1-7 years old who had undergone hypospadias repair from January 2012 to December 2018.Required data were obtained from the patients'medical records.Results:Overall,post-surgical complications were observed in 60(10.3%)patients comprising urethro-cutaneous fistula(n=39,6.7%),meatal stenosis(n=10,1.7%),urethral stricture(n=7,1.2%),and glans dehiscence(n=4,0.7%).The mean age of the children with complications was 3.0±1.3 years.According to Kaplan-Meier estimate,the interval between surgery and development of complications was significantly shorter in the Vicryl group(p=0.037).Overall,complications were more prevalent in Vicryl suture than PDO suture(15.1%vs.5.3%,p<0.001).Regression model revealed that in comparison to the distal type,proximal hypospadias(odds ratio[OR]:103.9,95%confidence interval[CI]:32.2-334.9,p<0.001)and mid-shaft hypospadias(OR:82.9,95%CI:25.9-264.6,p<0.001)while using Vicryl suture instead of PDO suture(OR:62.4,95%CI:21.2-183.8,p<0.001)increased the odds of developing post-urethroplasty complications.Conclusion:We suggest PDO suture in the repair of hypospadias due to its lower complication rate,especially in cases of proximal and mid-shaft hypospadias which can get more complicated than the distal type.

Iran experienced one of the largest methanol poisoning outbreaks shortly after the coronavirus pandemic due to misinformation about the beneficial effects of alcohol in preventing and treating this viral infection.^([1])According to the Iran Legal Medicine Organization(LMO),over 3,100 methanol toxicity cases and 728 deaths were reported across the country,resulting in a syndemic of methanol poisoning.^([2])There is very little literature on the electrographic predictors of cardiovascular mortality in methanol poisoned patients.^([3])

Inter-regulatory role of microRNAs in interaction between viruses and stem cells

MicroRNAs(miRNAs)are well known for post-transcriptional regulatory ability over specific mRNA targets.miRNAs exhibit temporal or tissue-specific expression patterns and regulate the cell and tissue developmental pathways.They also have determinative roles in production and differentiation of multiple lineages of stem cells and might have therapeutic advantages.miRNAs are a part of some viruses’regulatory machinery,not a byproduct.The trace of miRNAs was detected in the genomes of viruses and regulation of cell reprograming and viral pathogenesis.Combination of inter-regulatory systems has been detected for miRNAs during viral infections in stem cells.Contraction between viruses and stem cells may be helpful in therapeutic tactics,pathogenesis,controlling viral infections and defining stem cell developmental strategies that is programmed by miRNAs as a tool.Therefore,in this review we intended to study the interregulatory role of miRNAs in the interaction between viruses and stem cells and tried to explain the advantages of miRNA regulatory potentials,which make a new landscape for future studies.

Association of risky driving behavior with psychiatric disorders among Iranian drivers:A case-control study

Purpose:This study aimed to investigate the possible association between psychological disorders and riskydriving behavior(RDB)in Iran.Methods:This case-control study conducted in shiraz,Iran in 2021.The case group included drivers with psychological disorders and the control group included those without any disorders.The inclusion criteria for selecting patients were:active driving at the time of the study,being 18-65 years old,having a driving license,having a psychological disorder including depression,bipolar disorder,anxiety spectrum disorder,or psychotic disorder spectrum confirmed by a psychiatrist,and completing an informed consent form.The exclusion criterion was the existence of conditions that interfered with answering and understanding the questions.The inclusion criteria for selecting the healthy cases were:active driving at the time of the study,being 18-65 years old,having a driving license,lack of any past or present history of psychiatric problems,and completing an informed consent form.The data were gathered using a researcher-made checklist and Manchester driving behavior questionnaire.First,partition around medoids method was used to extract clusters of RDB.Then,backward logistic regression was applied to investigate the association between the independent variables and the clusters of RDB.Results:The sample comprised of 344(153 with psychological disorder and 191 without confirmed psychological disorder)drivers.Backward elimination logistic regression on total data revealed that share of medical expenditure≤10%of total household expenditure(OR=3.27,95%Cl:1.48-7.24),psychological disorder(OR=3.08,95%Cl:1.67-5.70),and substance abuse class(OR=6.38,95%CI:3.55-11.48)wereassociatedwithhighlevelof RDB.Conclusion:Substance abuse,psychological illnesses,and share of medical costs from total household expenditure were found to be main predictors of RDB.Further investigations are necessary to explain the impact of different psychological illnesses on driving behavior.

Propose:In this study,we re-assessed the criteria defined by the radiological society of North America(RSNA)to determine novel radiological findings helping the physicians differentiating COVID-19 from pulmonary contusion.Methods:All trauma patients with blunt chest wall trauma and subsequent pulmonary contusion,COVID-19-related signs and symptoms before the trauma were enrolled in this retrospective study from February to May 2020.Included patients(Group P)were then classified into two groups based on polymerase chain reaction tests(Group Pa for positive patients and Pb for negative ones).Moreover,44 patients from the prepandemic period(Group PP)were enrolled.They were matched to Group P regarding age,sex,and trauma-related scores.Two radiologists blindly reviewed the CT images of all enrolled patients according to criteria defined by the RSNA criteria.The radiological findings were compared between Group P and Group PP;statistically significant ones were re-evaluated between Group Pa and Group Pb thereafter.Finally,the sensitivity and specificity of each significant findings were calculated.The Chi-square test was used to compare the radiologica丨findings between Group P and Group PP.Results:In the Group PP,73.7%of all ground-glass opacities(GGOs)and 80%of all multiple bilateral GGOs were detected(p<0.001 and p=0.25,respectively).Single bilateral GGOs were only seen among the Group PP.The Chi-square tests showed that the prevalence of diffused GGOs,multiple unilateral GGOs,multiple consolidations,and multiple bilateral consolidations were significantly higher in the Group P(p=0.001,0.01,0.003,and 0.003,respectively).However,GGOs with irregular borders and single consolidations were more significant among the Group PP(p=0.01 and 0.003,respectively).Of note,reticular distortions and subpleural spares were exclusively detected in the Group PP.Conclusion:We concluded that the criteria set by RSNA for the diagnosis of COVID-19 are not appropriate in trauma patients.The clinical signs and symptoms are not always usefu

Efficacy of tadalafil on improvement of men with erectile dysfunction caused by COVID-19:A randomized placebo-controlled trial

Objective: According to the high prevalence of COVID-19 and the subsequent risk of men's sexual health, we decided to investigate the efficacy of tadalafil on improvement of men with erectile dysfunction caused by COVID-19.Methods: In this study, 70 outpatients who were recovered from COVID-19 without acute respiratory distress syndrome with negative polymerase chain reaction test and a complaint of erectile dysfunction were divided into two groups: 35 patients who received tadalafil 5 mg daily and 35 who received placebo. For each patient, basic assessment of sexual function was performed using the 5-item version of the International Index of Erectile Function (IIEF-5) questionnaire. Then, treatment was started from 2 months after complete recovery of COVID-19 with negative polymerase chain reaction test for 3 months. At the end of the treatments, the patients were re-evaluated for sexual function using the complete version of IIEF questionnaire. Finally, the results before and after treatment in the intervention group were compared with those of the control group.Results: Treatment with both tadalafil and placebo improved the patients' sexual function criteria compared to the baseline. However, this improvement was significantly higher in the intervention group with tadalafil than the control group with placebo (p<0.05).Conclusion: Daily administration of tadalafil 5 mg seems to be effective and safe for improvement of erectile dysfunction caused by COVID-19.

A cross-sectional study of complementary and alternative medicine use in patients with coagulation disorders in Southern Iran

OBJECTIVE：Complementary and alternative medicine（CAM）use has an increasing trend around the world.Despite the wild application of CAM in patients with coagulation disorders,its efficacy is still questioned by many studies.The aim of this study was to determine the frequency and types of CAM usage,and the factors affecting CAM use among patients with bleeding disorders.METHODS：This cross-sectional study investigated the usage of CAM with a standard validated questionnaire in coagulopathic patients who were referred to Dastgheib Hospital,an educational therapeutic center affiliated to the shiraz University of Medical Sciences in shiraz,Southern Iran.RESULTS：Between December 2015 and May 2016,75 patients were included in this survey.Severe hemophilia A and rare bleeding disorders were the most frequent among our participants.Overall,nearly half of the studied population（49.3%）used at least one CAM product or practices.The most common products were multivitamin（29.3%）,followed by folic acid（21.3%）and chamomile（12%）.32%of the patients resorted to faith healing.The main reasons of using CAM were reported to be increased general health,correction of anemia and thrombocytopenia and control of bleeding tendency.CONCLUSION：CAM is being used frequently in patients with coagulation disorders like many other chronic illnesses all around the world.Due to emerging concern regarding the safety and possible interaction with the conventional medicine,it is necessary to develop basic data about the hematologic effects of these methods in order to use them more safely.

Effect of cOVID-19 pneumonia infection control protocols on nosocomial infection incidence in trauma patients

Purpose:Nosocomial infection is a major threat to the health care system and patient welfare.After the pandemic,new protocols were established in hospitals and communities to protect against the transmission of cOvID-19,which may have changed the incidence of nosocomial transmission.This study was conducted to compare the incidence of nosocomial infection before and after the covID-19 pandemic.Methods:This was a retrospective cohort study performed on trauma patients who were admitted,from May 22,2018 to November 22,2021,to the largest level-1 trauma center in shiraz,Iran(Shahid Rajaei Trauma Hospital).All the trauma patients over 15 years old admitted during the study time were included in this study.Individuals who were declared dead upon arrival were excluded.Patients were evaluated in 2 periods:before the pandemic(May 22,2018-February 19,2020)and after the pandemic(February 19,2020-November 22,2021).Patients were assessed based on demographic information(age,gender,length of hospital stay,and patient outcome),the occurrence of hospital infection,and the type of infection.The analysis was done using SPSS version 25.Results:Overall,60,561 patients were admitted,with a mean age of 40 years.Nosocomial infection was diagnosed in 4.00%(n=2423)of all admitted patients.The incidence rate of post-COVID-19 hospitalacquired infections decreased by 16.28%(p<0.001)when compared to before the pandemic;in contrast,surgical site infection(p<0.001)and urinary tract infection(p=0.043)were responsible for this change,while hospital-acquired pneumonia(p=0.568)and bloodstream infection(p=0.156)were not significantly different.Overall mortality was 1.79%,while 28.52%of all patients with nosocomial infections died.During the pandemic,there was a 25.78%increase(p<0.001)in the overall incidence rate of mortality,which was also observed among patients with nosocomial infections(17.84%).Conclusion:The incidence of nosocomial infection has decreased during the pandemic,possibly due to the use of more personal protective equipment and mo

Incidence and pattern of traumatic spine injury in a single level I trauma center of southern Iran

Purpose:Spine injury is one of the leading causes of death and mortality worldwide.The objective of this study was to determine the incidence,pattern and outcome of trauma patients with spine injury referred to the largest trauma center in southern Iran during the last 3 years.Methods:This is a cross-sectional study conducted between March 2018 and June 2021 in the largest trauma center in the southern Iran.The data collection form included the age,sex,injury location(cervical,thoracic,and lumbar),cause of injury(traffic accidents,falls,and assaults),length of hospital stay,injured segment of spine injury,severity of injury,and outcome.Statistical analyzes were performed using SPSS software version 24.Results:Totally 776 cases of spine injury were identified.The spine injury rate was 17.0%,and the mortality rate was 15.5%.Cervical spine injury(20.4%)more often occulted in motorcycle accident,and thoracic spine injury(20.1%)occulted in falls.The highest and lowest rates of spine injurys were related to lumbar spine injury(30.2%)and cervical spine injury(21.5%),respectively.There was a statistically significant relationship between the mechanism of injury and the location of spine injury(p<0.001).And patients with lumbar spine injury had the highest mortality rate(16.7%).Injury severity score(OR=1.041,p<0.001)and length of stay(OR=1.018,p<0.001)were strong predictors of mortality in trauma patients with spine injury.Conclusion:The results of the study showed that the incidence of traumatic spine injury rate was approximately 17.0%in southern of Iran.Road traffic injury and falls are the common mechanism of injury to spine.It is important to improve the safety of roads,and passengers,as well as work envi-ronment,and improve the quality of cars.Also,paying attention to the pattern of spine injury may assist to prevent the missing diagnosis of spine injury in multiple trauma patients.

Background： Lipid-lowering effect ofRhus coriaria L. （Rhus） has been investigated in multiple animal stud- ies with promising results. Nonetheless, its clinical efficacy has not been adequately examined. Objective： The aim of this study was to evaluate the lipid-lowering effects of Rhus among patients with hyperlipidemia. Design, setting, participants and interventions： The study was designed as a two-arm, double-blind placebo-controlled randomized clinical trial, using a parallel design. Eighty patients with primary hyper- lipidemia were randomly assigned to receive Rhus capsules or placebo for 6 weeks. Main outcome measures： The serum lipid levels, apolipoprotein-A1 （Apo-A1） and apolipoprotein-B （Apo-B） were measured. Results： Mean serum high-density lipoprotein cholesterol （HDL-C） and Apo-Al levels were significantly increased in the Rhus group, compared with the placebo group, after 6 weeks of intervention （P= 0.001）. The analysis of covariance test including age, gender, body mass index （BMI）, and smoking as co-variables revealed that the increase in HDL-C and Apo-A1 levels remained significant, and increases in HDL-C were dependent on the increase in Apo-A1 levels. No significant difference was observed between Rhus and placebo groups in terms of mean reductions in total cholesterol, low-density lipopro- tein cholesterol and triglyceride levels; however, more significant improvement was observed among obese patients （BMI≥ 30 kg/m^2）. Conclusion： The study showed significant increases in HDL-C and Apo-Al levels in response to Rhus sup- plementation in patients with hyperlipidemia.
