Sequential neoadjuvant chemotherapy using pegylated liposomal doxorubicin and cyclophosphamide followed by taxanes with complete trastuzumab and pertuzumab treatment for HER2-positive breast cancer: A phase Ⅱ single-arm study

Objective: Despite cardiotoxicity overlap, the trastuzumab/pertuzumab and anthracycline combination remains crucial due to significant benefits. Pegylated liposomal doxorubicin(PLD), a less cardiotoxic anthracycline, was evaluated for efficacy and cardiac safety when combined with cyclophosphamide and followed by taxanes with trastuzumab/pertuzumab in human epidermal growth factor receptor-2(HER2)-positive early breast cancer(BC).Methods: In this multicenter, phase II study, patients with confirmed HER2-positive early BC received four cycles of PLD(30-35 mg/m^(2)) and cyclophosphamide(600 mg/m^(2)), followed by four cycles of taxanes(docetaxel,90-100 mg/m^(2) or nab-paclitaxel, 260 mg/m^(2)), concomitant with eight cycles of trastuzumab(8 mg/kg loading dose,then 6 mg/kg) and pertuzumab(840 mg loading dose, then 420 mg) every 3 weeks. The primary endpoint was total pathological complete response(tp CR, yp T0/is yp N0). Secondary endpoints included breast p CR(bp CR),objective response rate(ORR), disease control rate, rate of breast-conserving surgery(BCS), and safety(with a focus on cardiotoxicity).Results: Between May 27, 2020 and May 11, 2022, 78 patients were treated with surgery, 42(53.8%) of whom had BCS. After neoadjuvant therapy, 47 [60.3%, 95% confidence interval(95% CI), 48.5%-71.2%] patients achieved tp CR, and 49(62.8%) achieved bp CR. ORRs were 76.9%(95% CI, 66.0%-85.7%) and 93.6%(95% CI,85.7%-97.9%) after 4-cycle and 8-cycle neoadjuvant therapy, respectively. Nine(11.5%) patients experienced asymptomatic left ventricular ejection fraction(LVEF) reductions of ≥10% from baseline, all with a minimum value of >55%. No treatment-related abnormal cardiac function changes were observed in mean N-terminal pro-BNP(NT-pro BNP), troponin I, or high-sensitivity troponin.Conclusions: This dual HER2-blockade with sequential polychemotherapy showed promising activity with rapid tumor regression in HER2-positive BC. Importantly, this regimen showed an acceptable safety profile,especially a low risk of cardiac events,

Clinical characteristics and prognostic factors of primary malignant cardiac tumors

Primary malignant cardiac tumors(PMCTs)are rare but highly lethal,and up to 75%of them are sarcomas,followed by lymphoma and mesothelioma.[1]Due to the rarity of this disease,most studies to date were small series or single case report.Although a few of them investigated the clinical characteristics and survival of PMCT patients,the factors influencing overall survival have not been well elucidated.[2,3,4]The Surveillance,Epidemiology,and End Results(SEER)program covers about 28%of the US population,collecting data on patient demographics,primary tumor site,tumor morphology,stage at diagnosis,survival,and mortality,including rich data of cardiac tumors.[5]In the present study,we described the clinical characteristics,identified the variables affecting the prognosis,and constructed a nomogram for patients with PMCTs using the data from the SEER database.

Background:The mortalities of hepatobiliary malignancies are high.With the failure of conventional chemotherapy and unsatisfactory outcome of molecular targeted drugs,immune-based therapy has become a new focus of research in hepatobiliary cancers treatment.Data sources:We performed a Pub Med search with relevant articles published up to May 2022 and the following keywords:cellular immunotherapy,hepatobiliary cancer,antigen receptor T cell therapy,and receptor-engineered T cell.Information of clinical trials was obtained from https://clinicaltrials.gov/.Results:Cell therapies for hepatobiliary malignancies are at early stage of development.The current review showed that cellular therapies are safe and feasible in patients.These findings provide an important platform for future lager scale clinical trials on immunotherapy in patients with hepatobiliary malignancies.Conclusions:With the continuous advances of cellular immunotherapy,the combination of cellular immunotherapy with surgery,chemotherapy and radiotherapy will be new therapeutic strategies for patients with hepatobiliary cancer.

The development of implantable bioelectronics is driven by emerging applications including continuous health monitoring and human-machine interfacing.The mechanical mismatch between implanted bioelectronics and tissues not only compromises device accuracy,but also causes interference with tissues undesirably.To address this issue,it is necessary to develop ultrasoft and biocompatible fiber strain sensor with proper stretchability and sensitivity.Here,we fabricate a bacterial cellulose-based sensing fiber which possesses the stretchability and elastic modulus(~102 kPa)close to those of soft tissues.In addition,such fiber has high sensitivity to tiny tensile force/strain(8.8×10^(−3)N/2.5%)and low cell cytotoxicity.These excellent properties make the bacterial cellulose(BC)-based sensing fiber an excellent candidate of implantable bioelectric devices for monitoring subtle motions of organs.We demonstrate this by applying it for continuous monitoring of human pulse and bullfrog heartbeats.The(BC/oxBC)@PANI(ox=oxidized and PANI=polyaniline)fibers can further be woven into an array sensor and serve more complex sensing functions,such as multipoint force perception and shape recognition as demonstrated.

The healing of critical-sized bone defects(CSD)remains a challenge in orthopedic medicine.In recent years,scaffolds with sophisticated microstructures fabricated by the emerging three-dimensional(3D)printing technology have lighted up the treatment of the CSD due to the elaborate microenvironments and support they may build.Here,we established a magnesium oxide-reinforced 3D-printed biocompos-ite scaffold to investigate the effect of magnesium-enriched 3D microenvironment on CSD repairing.The composite was prepared using a biodegradable polymer matrix,polycaprolactone(PCL),and the disper-sion phase,magnesium oxide(MgO).With the appropriate surface treatment by saline coupling agent,the MgO dispersed homogeneously in the polymer matrix,leading to enhanced mechanical performance and steady release of magnesium ion(Mg^(2+))for superior cytocompatibility,higher cell viability,advanced osteogenic differentiation,and cell mineralization capabilities in comparison with the pure PCL.The in-vivo femoral implantation and critical-sized cranial bone defect studies demonstrated the importance of the 3D magnesium microenvironment,as a scaffold that released appropriate Mg^(2+) exhibited remarkably increased bone volume,enhanced angiogenesis,and almost recovered CSD after 8-week implantation.Overall,this study suggests that the magnesium-enriched 3D scaffold is a potential candidate for the treatment of CSD in a cell-free therapeutic approach.

Objective:To explore experiences of Chinese nurses who study for Master of Science in Professional Practice(health care)in the United Kingdom.Methods:A qualitative approach was applied,consisting of semi-structured telephone interviews(n=8).The Colaizzi seven-step framework analysis method was used to extract themes.Results:The themes that emerged are as follows:(1)gains from studying overseas,(2)difficulties during studying,and(3)impacts of learning experience on work.The gains for nurses studying overseas include the improvement of scientific research capabilities,reconstruction of critical thinking,and development of reflection ability.Difficulties while studying mainly include language barrier,strict assessment,and boring life.Conclusions:The education of nursing postgraduate attaches impor tance to the cultivation of thinking.Nurses are very interested in nursing research including qualitative and quantitative methods.Nurses encountered language barriers in their study and life even though they knew a cer tain level of English.

Psoriasis is a systemic immune-mediated inflammatory disease that presents at any age and is characterized by excessive proliferation and abnormal differentiation of keratinocytes as well as infiltration of multiple inflammatory cells.It has been gradually attracting more attention because of not only the increased risk of cardiovascular,metabolic,and autoimmune-related diseases but also the accompanying depression,anxiety,self-affirmation impairments,dementia,sleep disorders,and other mental disorders that place a substantial psychosocial burden on society and individuals.Mental and psychological disorders are involved in the occurrence,development,outcome,recurrence,and treatment of psoriasis through the neuroendocrine-immune signaling network.This review summarizes the pathogenesis of psychological and psychological disorders in patients with psoriasis with the aim of deepening clinicians’awareness of the mental and psychiatric comorbidities in patients with psoriasis.

CRISPR screening has been broadly used to discover new therapy targets,but these finding’successful clinical transition is still limited.In a recent study published in Nature,Jin et al.1 adopted CRISPR screening to dissect the resistance of hepatocellular carcinoma to lenvatinib,discovering the synergistic effect of EGFR inhibitor(gefitinib)and lenvatinib(multi-tyrosine kinase inhibitor,muti-TKI)combination therapy with encouraging clinical benefits.

Erianthus produces substantial biomass,exhibits a good Brix value,and shows wide environmental adaptability,making it a potential biofuel plant.In contrast to closely related sorghum and sugarcane,Erianthus can grow in degraded soils,thus releasing pressure on agricultural lands used for biofuel production.However,the lack of genomic resources for Erianthus hinders its genetic improvement,thus limiting its potential for biofuel production.In the present study,we generated a chromosome-scale reference genome for Erianthus fulvus Nees.The genome size estimated by flow cytometry was 937 Mb,and the assembled genome size was 902 Mb,covering 96.26%of the estimated genome size.A total of 35065 proteincoding genes were predicted,and 67.89%of the genome was found to be repetitive.A recent wholegenome duplication occurred approximately 74.10 million years ago in the E.fulvus genome.Phylogenetic analysis showed that E.fulvus is evolutionarily closer to S.spontaneum and diverged after S.bicolor.Three of the 10 chromosomes of E.fulvus formed through rearrangements of ancestral chromosomes.Phylogenetic reconstruction of the Saccharum complex revealed a polyphyletic origin of the complex and a sister relationship of E.fulvus with Saccharum sp.,excluding S.arundinaceum.On the basis of the four amino acid residues that provide substrate specificity,the E.fulvus SWEET proteins were classified as monoand disaccharide sugar transporters.Ortho-QTL genes identified for 10 biofuel-related traits may aid in the rapid screening of E.fulvus populations to enhance breeding programs for improved biofuel production.The results of this study provide valuable insights for breeding programs aimed at improving biofuel production in E.fulvus and enhancing sugarcane introgression programs.
