The COVID-19 pandemic has posed a serious threat to global mental *** lines of evidence suggest that there is a varying yet considerable increase in mental health issues among the general population and vulnerable groups.12 The aftermath is obscure and speculative from a social,economic,individual and public mental health *** published studies support the existence of an emotional epidemic curve,describing a high probability of an increase in the burden of mental health issues in the post-pandemic era.34 Furthermore,previous major public health emergencies showed that more than half of the population devel-oped mental health problems and required mental health intervention.45 There is,therefore,an urgent need to reorganise existing mental health services to address the current unmet needs for mental health and to prepare for future challenges in the postpandemic era in terms of prevention and management.

Bacteriophages,or viruses of microbes,when used as a medical strategy,might be able to solve the current crisis mankind faces with the increasing number of pathogens being antibiotic-resistant,where chemical drugs seized to show any therapeutic *** socalled phage therapy may be one of the most promising alternatives to treat infections caused by antibiotic-resistant bacteria,which are killed after infection by a *** phages that destroy the host by lysis are chosen for therapy,many pharmacological and immunological aspects of phages as medicines have not been established so *** immune system plays an important role in a process called phage acceptance where both,innate and adaptive immune responses of the host are ***,not only medical aspects but also social ones such as lacking public awareness or acceptance,and lack of structured regulatory guidelines are challenges that have to be addressed in the near future to establish phage therapy as a reliable and safe alternative for the treatment of *** review focuses on the unique pharmacological and immunological aspects of phages used in therapy.

Background Soil-transmitted helminths (STH) affect approximately 1.5 billion people globally. The current STH control strategy is annual or twice-annual preventive chemotherapy, typically school-based deworming targeting children and women of reproductive age. Mathematical modeling suggests that it may be possible to interrupt STH transmission through high-coverage community-wide mass drug administration (cMDA). DeWorm3 is a cluster randomized trial testing cMDA for prevalence reduction and transmission interruption. The purpose of this study is to describe coverage of cMDA in study clusters over time and correlates of coverage at individual and cluster *** From 2018-2020, DeWorm3 delivered six rounds of cMDA with 400 mg albendazole at sites in Benin, India, and Malawi. We report coverage, treatment uptake, and directly observed therapy across all rounds. Factors associated with coverage at the cluster level were identified using binomial generalized estimating equations, while factors associated with non-treatment at the individual level were identified using binomial mixed-effects *** Coverage was high across all clusters and rounds, exceeding the WHO target of 75% in all sites and across all rounds (78% to 95%); cluster-level coverage tended to increase over time. Younger, unmarried, and migratory adults were more likely to be untreated at all sites; adult males were more likely to be untreated in Benin and Malawi. Among children, girls were more likely to be untreated, as were non-school-attending and migratory children. Higher adult education was associated with greater odds of non-treatment among adults, but lower odds among children in the household. Factors Affecting Maternal Coping in Korean Mothers of Children with Cancer

Background: The childhood cancer is increasing throughout the world. The incidence of children and adolescent (1 - 19 years old) mortality by cancer was 4.49 by 2006 (Korea Statistics, 2014). Mothers of children experienced a difficult situation in coping when their children were diagnosed with cancer. Based on Mishel’s Uncertainty in Illness Theory, maternal uncertainty and perceived social support are important factors that may have an impact on maternal coping in mothers of children with cancer. Therefore, it was worth to investigate this phenomenon in the context of Korea where no such study was conducted. Objective: To identify the factors affecting maternal coping in mothers of children with cancer. Methods: The study was conducted by using a descriptive design. This study was approved by the Institutional Review Board, Yonsei Health System, Severance hospital, Seoul, Korea and informed consent forms were obtained from the participants. Fifty-nine mothers completed three questionnaires: 1) Parent’s Perception of Uncertainty Scale, 2) Multidimensional Scale of Perceived Social Support, and 3) Coping Health Inventory for Parents. The internal consistency of these instruments was satisfactory, all with alpha coefficients over 0.86. Results: Hierarchical multiple regression analysis revealed that perceived social support and maternal uncertainty together explained 35% of the variance in the maternal coping in mothers of children with cancer (R2 = 0.353, F = 15.255, p = 0.000). Perceived social support and maternal uncertainty had an impact on maternal coping. Conclusions: The findings provided information that could be used in a further intervention study to increase maternal coping in mothers of children with cancer.

Human giardiasis is a common waterborne/foodborne parasitic disease worldwide,especially in developing *** and molecular identity of Giardia parasites are largely *** present study was conducted to determine the occurrence of Giardia parasites and the genetic profile of circulating assemblage (s) in patients attended the outpatient clinic at Kafrelsheikh University hospital,Kafr El sheikh Province,Egypt.A total of 318 patients,of different age and sex,referred to the clinic were subjected to fecal *** results revealed that 181/318 (56.9%) were positive for Giardia *** genotyping by PCR/sequencing of beta-giardin (bg),triose phosphate isomerase (tpi),and glutamate dehydrogenase (gdh) genes of representative number of positive samples (65) revealed that assemblages A,B and mixed infections (A+B) occurred in 26/65 (40.0%),32/65 (49.2%) and 10.8%(7/65) of the analyzed isolates,*** analysis indicated that assemblage A sequences clustered in two novel types of AⅡ*** assemblage B sequences,BⅢwas the predominant (22/23,95.7%) sub-assemblage compared to BⅣ(1/23,4.3%).Collectively,assemblage B MLGs displayed greater levels of genetic diversity compared to assemblage *** data indicate that assemblages A and B of *** circulate in humans at Kafr El sheikh Province,Egypt,and that high genetic diversity exists at the assemblage and/or sub-assemblage levels.

目的:在真实世界的前瞻性队列研究中观察心内科门诊“极高危ASCVD”患者在治疗起始阶段采用联合降脂策略的疗效与安全性。方法:连续入组2021年1月至6月在湘雅医院心内科高脂血症专病门诊就诊的“极高危ASCVD”患者,根据其实际降脂策略分为三组:阿托伐他汀单药组;阿托伐他汀联合依折麦布组;阿托伐他汀联合依洛尤单抗组。主要观察终点为治疗一个月后低密度脂蛋白胆固醇(LDL-C)、脂蛋白a[Lp(a)]、非-HDL-C变化,次要终点为总胆固醇(TC)、甘油三酯(TG)、LDL-C、超敏C反应蛋白(hs-CRP)变化及安全性指标。结果:无论哪种联合方式,在“极高危ASCVD”患者降脂起始阶段联合降脂治疗的疗效均显著优于阿托伐他汀单药组:LDL-C、Log[Lp(a)]、非-HDL-C、TC的降幅更大,差异均有统计学意义(均P0.05)。以“LDL-C<1.4 mmol/L或<1.8 mmol/L”作为降脂达标的标准时,两组联合降脂治疗的LDL-C达标率均高于阿托伐他汀单药组,差异均有统计学意义(均P<0.05);阿托伐他汀联合依洛尤单抗组的LDL-C达标率高于阿托伐他汀联合依折麦布组,差异均有统计学意义(均P<0.05)。单独或联合“LDL下降超过50%”的降幅为达标标准,三组均无人能达标。治疗后三组间的肝脏转氨酶水平变化差异均无统计学意义(均P<0.05);三组间心肌酶同工酶(CK-MB)均下降,但差异无统计学意义(P<0.05)。与阿托伐他汀单药组相比,两联合降脂组治疗组的血糖均下降,差异具有统计学意义(P<0.05);阿托伐他汀联合依折麦布组的血糖比阿托伐他汀联合依洛尤单抗组的降幅更大,差异有统计学意义(P<0.05)。结论:起始联合降脂治疗1个月对心内科门诊“极高危ASCVD”患者的降脂效果及LDL-C的达标率优于阿托伐他汀单药组。以LDL-C<1.4 mmol/L或<1.8 mmol/L为降脂达标目标时,阿托伐他汀联合依洛尤单抗组治疗1个月后LDL-C达标率高于联合依折麦布组;以“LDL下降超过50%”的降幅作为降脂达标标准时,1个月内很难达标。门诊“极高危ASCVD”患者采用起始联合的降脂治疗1个月无不良反应。起始联合降脂策略可用于心内科门诊“极高危ASCVD”患者中LDL-C数值需早期达标的人群,阿托伐他汀联合依洛尤单抗可用于需在1个月内LDL-C<1.4 mmol/L或<1.8 mmol/L的人群。
