Impact of gender on outcomes after transcatheter aortic valve implantation

Backgound The influence of gender on clinical outcomes following transcatheter aortic valve implantation （TAVI） was considerably discrepant in previous studies. We aimed to investigate the impact of gender in our registry. Methods The study is a retrospective observational analysis of a prospectively designed cohort （546 consecutive patients treated at the University hospital Zurich who were enrolled in Swiss TAVI Cohort from May 2008 to April 2014）. The Study took place in University Heart Centre at University hospital Zurich, Switzerland. Results Both genders were equally represented with 51.5 % （n = 281） of the populations being females, who were significantly older and had a more pronounced history of hypertension （P 〈 0.001 ）. Males on the other hand showed a higher incidence of diabetes （P = 0.004）, coronary artery disease （P 〈 0.001）, chronic obstructive pulmonary disease （P 〈 0.001） and renal failure requiring renal replacement therapy （P = 0.018）. Patients were followed up for a median of 391 days with a 100% complete follow-up at one year. The primary outcome （all-cause mortality） occurred in 6.8% （n = 37） and 15% （n = 82） of patients at 30 days and one year, respectively. The 30-day all-cause mortality outcome did not significantly differ between females （7.5%） and males （6.0%）（P = 0.619）, but one year all-cause mortality occurred significantly more in males than in females （18.7% vs. 11.7%, P 〈 0.037）. Conclusion After TAVI implantation for severe aortic stenosis, males have a less favorable long-term （one year） mortality outcome than females.

Background: As the half-life of intact parathyroid hormone (iPTH) is very low, it reflects parathyroid insufficiency within minutes to hours after total thyroidectomy. Therefore, iPTH level assessment in the postoperative period can be used to predict the development of hypocalcaemia. The optimal time point to measure serum iPTH is important for the accurate prediction of hypocalcaemia. Aim: This paper aims to evaluate the ability of iPTH as an early predictive marker of hypocalcaemia and determine which time iPTH is more able to predict postoperative hypocalcaemia. Method: This prospective observational study was conducted in the Department of Otolaryngology-Head & Neck Surgery, BSMMU, Dhaka, from July 2020 to December 2021, with 67 patients who underwent total thyroidectomy. iPTH levels were measured on the day before the operation and at 1 hour, 4 hours, and 24 hours after the operation. *** levels were measured on the day before the operation and 1st postoperative day. All the data were compiled and sorted properly and were analyzed statistically. Results: Postoperative hypocalcaemia developed in 18 cases, with an incidence of 26.9%. Pearson correlation showed a significant correlation between postoperative iPTH at 1 hr, 4 h, and 24 hr with 1st postoperative calcium value. The Receiver operating characteristic (ROC) curve was processed for the postoperative iPTH at 1 hr, 4 h, and 24 hr. The sensitivity, specificity, cut-off value, and mean AUC found 93.9%, 94.4%, ≤14.0, 0.988;95.9%, 94.4%, ≤09.5, 0.993 and 91.8%, 94.4%, ≤11.0, 0.993 respectively. Conclusion: iPTH can be used as an early predictor of post-thy-roidectomy hypocalcaemia. 4 hr iPTH showed more sensitivity and specificity for a cut-off value near the laboratory reference range.

Aplasia Cutis Congenita (ACC) is a rare condition characterized by absence of skin layers, usually on the scalp, but can also affect the foot. ACC can occur as an isolated condition or in the presence of the other congenital anomalies. Here we describe a case of a 1-day-old baby boy with an isolated ACC of the left foot, with no family or siblings positive disease history. The patient was managed by both conservatively and surgically until the defect has formed scar tissue 5 months later.

Risk Factors Associated with Various Subtypes of Ischemic Stroke: A Prospective Study in BSMMU, Dhaka, Bangladesh

Introduction: Stroke is the main cause of adult disability and the second most leading cause of death worldwide. The number of deaths due to stroke is 5.54 million worldwide. Globally, 70% of strokes and 87% of both stroke-related deaths and disability-adjusted life years occur in low and middle-income countries. Method: This was a prospective observational cross-sectional study conducted at Indoor, Outdoor, Stroke and Neuro-Intervention clinic of the Department of Neurology, Bangabandhu sheikh Mujib medical University hospital (BSMMU), Dhaka, Bangladesh, from June-2019 to May-2020. A total of 220 ischemic stroke patients aged above 30 years confirmed by CT scan/MRI of brain. Data were collected with a pre-structured questionnaire from the patients, investigations reports and face-to-face interviews and analyzed using IBM SPSS software version 23.0. Proper consent was taken from the participants. The collected data were ANNOVA tests and association within the risk factors and the patterns of subtypes of ischemic stroke where P Results: Among the 220 stroke patients, large-artery atherosclerosis (LAA), cardio-embolism (CE), small-vessel occlusion (SVO, stroke of other determined etiology (SODE) and stroke of undetermined etiology (SUDE) were being observed 84 (38.18%), 14 (6.36%), 63 (28.64%), 12 (5.45%) and 47 (18.18%). Eighty-Six (39.09%) prevalence of subtypes was found in rural areas whereas 134 (60.91%) were in urban areas. The significant risk factors are associated with the prevalence of various subtypes (p Conclusion: This study prevails the people of Bangladesh are vulnerable to various subtypes of ischemic stroke. The higher prevalence of large-artery atherosclerosis (LAA) in our patients suggests the presence of uncontrolled hypertension, diabetes mellitus, smoking habit and dyslipidemia.

Background:Systemic lupus erythematosus(SLE)is a systemic autoimmune disease characterized by the involvement of multiple organs and autoantibody *** study aimed to determine the association of human leukocyte antigen(HLA)-DQ and-DR alleles with SLE at a tertiary hospital in ***:This case-control study was conducted in the Department of Rheumatology,Bangabandhu sheikh Mujib medical University(BSMMU),Dhaka,*** patients who fulfilled the 1997 revised American college of Rheumatology classification criteria for SLE were considered cases(n=41).The controls(n=32)were selected from the hospital staff of BSMMU,including doctors and nurses who did not suffer from any known rheumatic diseases,and there was no such disease in their first-degree *** chain reaction using a sequence-specific primer(PCRSSP)in a low-resolution typing method was carried out on blood samples collected for HLA-DQ and-DR allele ***:The cases and controls had comparable mean ages(31 vs.32 years),and most cases were female(95.1%vs.34.4%).The most frequent clinical manifestations were mucocutaneous(85.4%),followed by musculoskeletal(70.7%)and constitutional symptoms(58.5%).The cases had a significantly higher frequency of HLA-DR2 allele compared with healthy controls(70.7%vs.43.8%;odds ratio:3.1;95%confidence interval:1.2-8.2).In addition,HLADQ6 allele was found to have a significant association with mucocutaneous manifestations of the cases(88.9%,P=0.03).Conclusions:Bangladeshi adults having HLA-DR2 may be more susceptible to ***-DQ6 allele might be associated with mucocutaneous manifestations of SLE.

Currently,there are no harmonized guidelines which govern skin banking in the Asia Pacific ***,skin banks are either unregulated or rely on their nation’s legislation or international accreditation to uphold their quality standards.A new set of skin banking guidelines was devel-oped through a comprehensive review and collation of best international practices for the Asia Pacific Burn Association(APBA)members,from donor screening and testing,to skin recovery,processing,storage and distribution,and quality *** regulatory requirements reviewed include the European directives,Australia’s Therapeutic Goods Administration and Sin-gapore’s tissue banking *** technical and quality management recommendations are referenced from the American Association of Tissue Banks(AATB),the United States Food and Drug Administration standards and guidance documents,various relevant European guides,Japanese Society of Tissue Transplantation guidelines and the Asia Pacific Association of Surgical Tissue *** mainly from the AATB standards,the new Asia Pacific Burn Association Guidelines for Skin Banking in Therapeutic Applications offer a comprehensive manual,address-ing:governance and contracts;staff responsibilities;quality management;facilities,equipment and supplies management;donor consent and testing;and recommendations of good practices pertaining to skin recovery,processing,storage and *** complementing current generic regulations,they provide technical specifications of major aspects unaddressed in most *** inaugural set of new regional skin banking guidelines would be a start for regional members of the APBA to adopt,and will hopefully culminate in a set of standards so that,in the long run,skin allografts from this region can be of similar quality,which can simplify import process and facilitate the exchange of allografts between members.

Diagnostic Accuracy of Gray Scale Sonography for the Detection of Malignant Breast Tumour

Background: Gray scale sonography is an important diagnostic tool for the detection of malignant breast tumour. Objective: The purpose of the present study was to find out the diagnostic validity gray scale sonography to detect malignant lesions of breast. Methodology: This cross-sectional study was carried out in the department of Radiology and Imaging, in collaboration with the department of Surgery and Pathology at Banghabandhu sheikh Mujib medical University (BSMMU), Dhaka, from July 2008 to June 2009 for a period of one year. Women presented with clinically suspected breast mass from outpatients’ department (OPD) or inpatient department (IPD) were purposively selected and was Ultrasonographic diagnosis. 2-D Real-Time B-Mode Ultrasonographic examinations were done using high frequency (7.5 MHz) linear transducer. Result: Test of validity was done for color Doppler Sonographic findings in evaluation of benign and malignant breast lesion. Out of 50 cases, 23 cases were true positive;4 cases were false negative;1 case was false positive;22 cases were true negative confirmed by histopathology. Here sensitivity and specificity of Gray scale ultrasonography were 85.18% and 95.0% respectively. Here accuracy of Gray scale study was 90.0%. Positive predictive value was 95.83% and negative predictive value was 84.61%. Conclusion: Gray scale ultrasonography is a useful method in the differentiation between benign and malignant breast masses.

Management of Sepsis in Intensive Care Units in Rajshahi medical college, Bangladesh

Background: Sepsis was a life-threatening situation affecting roughly 27 million individuals globally every year. Devoid of proper management, sepsis could develop to austere sepsis and septic shock that reason certain eight million expiries every year. Initial identification and analysis of contaminations and quick introductions of antimicrobial management had long been predictable as essential principles of treatment. Methods: All patients were evaluated thoroughly by history, clinical and laboratory findings. Serum lactate levels were collected at 0 hr and 24 hrs of the total admission in the ICU. Blood culture and sensitivity was performed prior to antibiotic administration in this study. Results: In this research, total sample was 1367 patients. Among these sepsis patients were 305 (nearly 22%). These 305 studied sepsis cases, majority 128 (42%) of the patients were within the age group of 41 - 65 years, followed by 18 - 30 years consisting of 116 (38%) and the least number of sepsis patients were in the age group of >65 years (20%). This study found that 129 (42.3%) out of 305 blood samples screened from assumed sepsis infection were affirmative for the presence of sepsis causing pathogens. Among the 129 culture positive samples, majority of the isolated micro-organisms were gram negative bacteria 58% (75), followed by gram positive bacteria 38% (49), and 3.8% (5) were the fungi Candida albicans. Conclusion: At present, mortality & morbidity of sepsis subject was too big due to late identification, wrong & inadequate management of sepsis in the ward and also in the intensive care unit of Bangladesh.

Detection of <i>β</i>-Hemoglobin Gene and Sickle Cell Disorder from Umbilical Cord Blood

Sickle cell disease (SCD) is one of the most common hemoglobinopathies, which is caused by the replacement of glutamic acid with valine at the sixth position of the beta-globin amino acid chain which sickling of the entire red blood cells in the homozygous (Hb S/S) condition. There are many analyses and screening procedures were developed to detect sickle cell anemia in the early age of birth, especially from heel prick blood, but in case of developing countries, it would be more acceptable to detect sickle cell disorder using umbilical cord blood just after birth rather than using heel prick blood. In this study, umbilical cord blood (UCB) was used to detect β-hemoglobin gene and sickle cell disorder. Polymerase chain reaction (PCR) based analysis was done using two primers (wild-type and mutant type) to detect this disorder. A total number of 22 samples were enrolled in this experiment for PCR amplification among which nineteen samples were identified by amplification of both 267 bp and 517 bp fragments revealing heterozygous sickle cell trait (Hb A/S), whereas three samples were found to amplify of 517 bp only revealing healthy individuals. The result from PCR analysis was then collaborated with the information of the mothers of each sample to analyze the result more conveniently and found that the mothers of all individuals except the three samples had anemia or mild form of anemia, thus it was expected that the newborn might have anemia trait (Hb A/S) the exception was found in case of sample No. 9 and sample No. 15. Both samples showed the bands on 267 bp and 517 bp thus expressed the sickle cell disease trait although the mothers of these samples were not anemic. However, no samples were recorded having sickle cell anemia (9 Hb S/S). The inherent simplicity and low cost of this PCR based analysis with umbilical cord blood will be considered as an effective tool in future newborn screening in Bangladesh.

Evaluation of the Risk Factors for Pelvic Lymph Node Metastasis in Early Stage Cervical Cancer

Objective: This study evaluated the risk factors for pelvic lymph node metastasis in patients with early stage cervical cancer. Material and Methods: A cross sectional study was carried out in the Division of Gynaecologic Oncology at Bangabandhu sheikh Mujib medical University (BSMMU) between July 2014 and July 2015 to evaluate demographic factors and histopathological findings of women with cervical cancer stage IB-IIA who underwent primary radical hysterectomy and bilateral pelvic lymph node dissection. Results: Fifty women aged 30 years or above were included in the study. Lymph node metastases were identified in 20% of patients. By univariate analysis, preoperative anemia, a history of smoking, tumor size greater than 4 cm and lack of cervical inflammatory reaction by histopathology were significant variables associated with lymph node metastasis. Multivariate analysis showed that the lack of inflammatory reaction in the uterine cervix was the most important predictor for pelvic lymph node metastasis when adjusting for other variables. Moderate to severe inflammatory reaction in the uterine cervix was 18 times less likely to have pelvic lymph node metastasis than those who had mild inflammation. Conclusion: An association was found between the presence of pelvic lymph node metastasis in cervical cancer and certain variables: the lack of contraceptive use, smoking, preoperative anemia, bulky tumors, invasion of deep two-third of the uterine cervix, mild inflammatory reaction in the cervix, and keratinizing carcinoma. After adjusting for other factors, presence or absence of inflammatory reaction in the cervix was found to be the most important predictor for pelvic lymph node metastasis.
