We describe a 62-year-old male patient. He had been diagnosed with hypertrophic obstructive cardiomypathy and were refractory to medical treatments. He was admimitted to our hospital for percutaneous transluminal septal myocardial ablation (PTsMA). Temporary cardiac pacemaker was inserted before the injection of ethanol. Completely atrial ventricular block happened soon after PTsMA. One hour after PTsMA, Ventricular arrhythmia storms occurred. They were refractory to lidocaine, amiodarone and magenesium sulfate injection,six times of electrical conversions and defibrillations were performed. The ventricular fibrillations did not stopped until the injection of esmolol hydrochloride. Completely atrial-ventricular block disappered 36 hours after PTsMA. Transthocic echocardiography (TTE) was performed before and 1-week after PTsMA. The left ventricular outflow-tract pressure gradient (LVOTG) was much lower than that of Pre-PTsMA, with 25mmHg vs 53 mmHg. TTE was peformed 3 months after PTsMA, it showed sustained improved in LVOTG (13 mmHg). Patient’s symptoms impoved, and electcardiograph showed that atrial ventricular block and ventricular arrhythmias didn’t occurred anymore. In conclution,PTsMA is effective in reducing the left ventricular outflow tract pressure gradient in this patient with HOCM. However, it resulted severe arrthymias such as complete AV-block and ventricular electrical storm. In this case we found that esmolol is rather effective than lidocaine and amiodarone in the treatment of ventricular electrical storm.

Influence of Peritoneal Transport Characteristics on Nutritional status and Clinical Outcome in Chinese Diabetic Nephropathy Patients on Peritoneal Dialysis

Background： High peritoneal transport status was previously thought to be a poor prognostic factor in peritoneal dialysis （PD） patients. However, its effect on diabetic nephropathy PD patients is unclear in consideration of the adverse impact of diabetes itself. The purpose of this study was to investigate the influence of peritoneal transport characteristics on nutritional status and clinical outcome in diabetic nephropathy patients on PD. Methods： One hundred and two diabetic nephropathy patients on PD were enrolled in this observational cohort study. According to the initial peritoneal equilibration test result, patients were divided into two groups： Higher transport group （HT, including high and high average transport） and lower transport group （LT, including low and low-average transport）. Demographic characteristics, biochemical data, dialysis adequacy, and nutritional status were evaluated. Clinical outcomes were compared. Risk factors for death-censored technique failure and mortality were analyzed. Results： Compared with LT group （n = 37）, serum albumin was significantly lower and the incidence of malnutrition by subjective global assessment was significantly higher in HT group （n = 65）（P 〈 0.05）. Kaplan-Meier analyses showed that death-censored technique failure and mortality were significantly increased in HT group compared with that in LT group. On multivariate Cox analyses, higher peritoneal transport status and lower residual renal function （RRF） were independent predictors of death-censored technique failure when adjusted for serum albumin and total weekly urea clearance （Kt/V）. Independent predictors of mortality were advanced age, anemia, hypoalbuminemia, and lower RRF, but not higher peritoneal transport status. Conclusions： Higher peritoneal transport status has an adverse influence on nutrition for diabetic nephropathy patients on PD. Higher peritoneal transport status is a significant independent risk factor for death-censored technique failure

Over the past decade,heat shock protein 90(Hsp90) has emerged as a potential therapeutic target for ***,the molecular mechanisms of down-regulation Hsp90 expression in osteosarcoma are incompletely *** this study,shRNA plasmid vectors against HsP90 were constructed and transfected into MG63 ***-regulation of HsP70 expressions were detected by RT-PCR and western blotting *** of cell proliferation was determined using the MTT ***-cycle distribution and apoptosis were examined by flow ***3K,p-Akt,Akt,mTOR,Bcl-2 and Bid were also detected by western blotting *** a mouse xenograft model of human MG63 tumors was constructed,tumor growth,microvessel density and proliferation in each group was *** shRNA vector targeted against HsP90 efficiently suppressed the expression of ***-mediated gene silencing of HsP90 resulted in significant inhibition of cell growth on culture plates and in soft ***,cancer cells showed significant G0/G1 arrest and increased apoptosis due to gene *** levels of PI3k,pAkt, mTOR,and Bcl-2 were decreased,whereas Bid levels were *** density as assessed by CD34 levels and cell growth by PCNA levels decreased according to immunohistochemical analyses. These findings suggest that downregulation of HsP90 promotes cell apoptosis and inhibits cell growth possibly through the PI3K/Akt/mTOR pathway and that HsP90 has a crucial role in the pathogenesis of osteosarcoma.

Objsctive:To describe the presentation of LF buckling into spinal canal as an iatrogenic spinal complication after anterior cervical decompression and reconstruction procedure. summary of Background Data:Buckling of the cervical ligamentum flavum(LF) commonly accompanies the loss of intervertebral disk height or facet joint ***,in the present case,LF buckling into spinal canal after anterior cervical reconstruction procedure,according to the literature available,has not been reported. Methods.A 59-year-old male underwent a cervical corpectomy of C5 followed by anterior reconstruction for the treatment of cervical spondylotic myelopathy(CsM).Numbness and weakness of upper extremities were relieved 2 weeks after decompression,but the gait imbalance was persisted and worsened at 10 months *** MRI at 12 months postoperatively showed LF buckling into spinal canal at the C5/6 level. *** secondary surgery,laminaplasty of C5 and C6,was conducted at 13 months postoperatively. The postoperative MRI showed that the spinal canal increased at C5 and C6 levels,and the spinal cord was decompressed without buckling of the LF. *** of the cervical LF should be considered among the complications of anterior cervical reconstruction surgeries.

Backgroud Acording an investigaion of 417 Grade A hospitals in China of recent 10 years,pacemaker implantation has been raising at an average of 11%per *** majority were received rightside DDD *** those patients there are a great part need cardiac resynchronization therapy(CRT),especially when they had got cardiomyopathy and/or heart *** definite guidelines were for how to update the resynchronization system in these *** Two CRT upgrading cases were done from right-side DDD *** case one,the patient’s left subclavian vein was chosen for the convenience of inserting the left ventricular *** CRT pacemaker was still put at the primary *** left lead was connected to the pacemaker through a small subcutanous *** case two,the left ventricular lead was inserted through the right subclavian *** DDD pacemaker was upgraded in the primary *** Two procedures were done successfully,differently and individually for the maximizing ***-operation follow-up showed the obvious parameters’improvement,including ECG/LVEDD/EF and *** CRT upgrading is a new path for those who had got heart failure with DDD *** really do improve heart function through increasing the synchrony of ventricular *** clinical cases and practice need for confirming this benefit.

We describe two cases of Duchenne’s cardiomyopathy with severe cardiac dysfunction, sporadic episodes of myoglobinuria induced by effort and increased levels of serum creatine kinase. Very mild signs of skeletal myopathy were clinically evident. Left ventriculography showed diffuse severe hypokinesia. skeletal muscle biopsy demonstrated a dystrophic process. The patients had no familial background of the disease. These 2 patients might have a sporadic inheritance pattern with severe cardiac involvement.
